Message from the Chief As the Chief of the Kettering Fire Department, I present to you thisreport as a summary of the performance and accomplishments of your Fire Department for the year 2014. This report refl ects the hard workand dedication of the men and women of your fi re department that continue to ensure the citizens and visitors of Kettering receive thehighest quality of emergency services.

By any measure, this has been a year of great positive change and innovation within our emergency services delivery system. Duringthis past year we broke ground on the fi rst of four new fi re stations,located at 3484 Far Hills Avenue. Given the federal and state mandates for preparedness, these new fi re houses will provide the infrastructure necessary to deliver a contemporary, all-hazards emergency response model that will serve you now and for many years into the future.

In addition to our infrastructure improvements, we continually seek new ways to anticipate andrespond to the needs of our customers — the citizens of Kettering. The bulk of this is accomplished through our strategic planning process where best management practices are identifi ed. For instance, with the ever-increasing demand for service and the advent of new technologies,we continually update our equipment and training and implement new processes and policiesspecifi cally designed to meet your future emergency service needs.

While your fi refi ghters take great pride in delivering an excellent system of services, they arealways looking for new ways to serve our community through their charitable efforts. In additionto their annual adopt-a-family program and MDA efforts, this year they participated in the national“Firefi ghters for Operation Warm” in which they donated 42 winter coats to children at the KetteringCity Schools. In all, your fi refi ghters distributed over $5,000 to local charities continuing their long standing partnership in keeping Kettering the community of choice to live, work and play.

And fi nally, your Fire Department responded to over 7,800 calls for service last year and in doing soconsistently received high marks on our citizen surveys. We remain committed to our communityby actively engaging with our schools, neighborhood groups, and community events. We realize our continued success relies on our strong community partnership and our relentless pursuit of excellence in all we do. Department OverviewToday, the Kettering Fire

Department embraces thephilosophy of providing a comprehensive all-hazardsmitigation approach toour emergency deliverysystem. During 2014we responded to 7,856emergency incidents,including a wide varietyof emergencies rangingfrom medical incidentsand auto extrication tohazardous chemical spills,technical rescue scenes,and structure fi res. Themembers of the KetteringF i re Department arehonored to serve thiscommunity each and everyday. We value the rich

history of our profession a n d r e c o g n i z e t h e awesome responsibility tobe ready to respond to the community’s emergency needs. Our organization is committed to providingthe best level of customer service and the most efficient and effective system of emergency services possible. This past year was yet another year of progressive change and overall improvement to our emergency services delivery model. Our members work diligently throughout the year to improve our overall operational effectiveness

to better meet the needsof this community.

new staffi ng model and to serve in both our full timeand part-paid positions.It is remarkable to notethat the staffi ng modelchanges, which greatlyenhance our emergencyresponse, continue tobe implemented withino u r e x i s t i n g f i s c a lresources and without

services for the communityw h i l e m a i n t a i n i n gf i s c a l r e s p o n s i b i l i t y.Implementation of themodernization plan toimprove quality serviced e l i v e r y c o n t i n u e dthroughout the year andwas manifest in a varietyof aspects inc lud ing:apparatus replacement,new facility construction,personnel recruitment, andprofessional training.

The S t ra teg ic F i reStation Planning Programt o p r o v i d e i n d u s t r ycompliant fire facilitiesfor safe and ef f ic ientemergency response movedinto the implementationand construction phasein 2014 . L and waspurchased, architecturaldesigns were approvedand construction began.Numerous public meetingswere held to educate thecommunity on the needfor these new facilities aswell as to seek communityfeedback regarding howbest to incorporate the newfacilities into the existingsurrounding. City staff fromall departments participatedin a cooperative planningsession to ensure all aspectsof the plan were covered.The city began constructionon Station 32 in September a n d p l a n s t o b e g i nconstruction on Station 36in the spring of 2015. Thisoverall project will spanthe next several years andwill provide outstandingfacilities to meet the needsof a modern day emergencyservices organization.

to replace aging apparatusand to ensure safe ande f f e c t i v e eme rgencyresponse by providingoutstanding equipmentand apparatus in a fi scally responsible manner. Thenew fire apparatus werepurchased through EMSbilling revenues.

In 2014, the departmentcontinued to enhance our technology infrastructureto meet the needs of a m o d e r n e m e r g e n c yserv ice organ izat ion .This collaborative effortincluded a team of citystaff from AdministrativeSystems, GIS, Dispatch,IT, KPD, and the FireDepartment who workeddi l igent ly throughoutthe year to expand our capabilities within the newAlert Public Safety System,which is a comprehensiverecords management andcomputer-aided dispatchsystem. This new systemcontinues to enhance theeffi ciency of our emergencyoperations by providingan integrated dispatchsystem for rapid ca l lprocessing and dispatchof emergency crews,comprehensive fi eld datamanagement, and effectiverecord management tomeet state and federalrequirements. The system allows access for essentialfi eld data to enhance crew

safety and emergency s c e n e m a n a g e m e n t . Over the course of 2014,numerous upgrades ande n h a n c e m e n t s w e r ecompleted to improve theAlert system and expandits capabilities to includethe use of pre-plans andinspection data in the fi eld.

Throughout 2014, themembers of the KetteringFire Department wereactively engaged in servingthe community. The department implemented anew fi re safety educationalprogram throughout allk indergarten throughthird grade classrooms in2014. In addition, Station36 personnel initiated aweekly reading programwithin several Kettering

elementary schools. KFDcrews were active in thecommunity checking firehydrants, teaching CPR,conducting f ire safetyprograms, visiting pre-schools, installing childsafety seats, speaking to civicgroups, and participating incommunity events. T h e m e n a n d women of the Kettering Fire Department (KFD) answered 6,192 emergencymedical service (EMS) calls from our residentsin 2014. Our paramedicsand EMTs handled thesealarms with great pride

and professionalism. EMS makes up a large portionof the emergency responseserv ices prov ided byKFD in the agency’s all-hazard mitigation system.However, these servicesadjoin rather than precludethe organization in other areas, such as fi refi ghting, technical rescue, andHazMat operations. TheKettering Fire Department

Emergency Medical Servicestakes great effort in theservices delivered to thepub l i c in emergencymedical services. Many of our members receivedrecogni t ion for the i r outstanding actions in 2014.

In our ongoing effortsto strive for excellence,

t h e K e t t e r i n gF i r e D e p a r t m e n t experienced many changes and updatesi n o u r m e d i c a lprogram for 2014.Ear ly in the year,KFD welcomed Dr.Brian Springer as thedepartment’s newMedical Director toprovide guidance andmedical oversight. Dr. Springer is currentlyemployed by WrightState University andserves as AssociateProfessor, Director,Division of TacticalEmergency Medicine.Dr. Springer previouslyserved on the State of

Ohio EMS Board whichoperates at the state levelto determine the specifi cfunctions performed by our paramedics and medicalpersonnel . Under thedirection of Dr. Springer,KFD personnel were trainedto perform an advanceda i r w a y m a n a g e m e n tprocedure called surgicalcricothyrotomy in the fi eld enhancing our capabilities

d u r i n g c r i t i c a l l i f e - threatening emergencies. Th i s s k i l l a l l ows ou r paramedics to oxygenateour patients in the most difficult and complicated cases.

In order to stay profi cient in all aspects of emergencymedicine, the membersof KFD attend ongoing training as part of their professional development. These trainings include, but are not limited to: advanced a i r way ma in tenance ,cardiac care and cardiac a l e r t s , a c u t e s t r o k e care, traumatic injuries, pediatric emergencies, and pharmacology. Our members received over 2,300 hours in EMS trainingin 2014. Comprehensivetraining was provided t h r o u g h i n s t r u c t i o n from KFD experienced paramedics who are state of Ohio cert i f ied EMS instructors along with instructors from Kettering Medical Center, MiamiValley Hospital, Dayton Children’s Hospital and other outside agencies.

In order to ensure h igh qua l i t y med ica l care, the Kettering Fire Department implemented a comprehensive newQuality Assurance/ QualityImprovement (QA/QI)program for our EMS operations in 2014. This

The QA/QI program beginswith patient care reports,which are reviewed for documentation standardsand quality of patientc a r e . In addition,the department utilizes this information to createtraining opportunit iesto improve performanceand capabilities. The QA/QI program ensures our members are deliveringoptimal service delivery to the customers we proudlyserve.

Along with the regionalparticipation in the QA/QI program, Kettering FireDepartment participatesin numerous initiativesfor emergency medicalresponse, HazMat and rescue, as well as OhioTask Force One. Ketteringr e g u l a r l y s e r v e s o nnumerous committees and participates in theRegional Physician AdvisoryBoard (RPAB). In 2014, KFDengaged in the regionalplan for EMS response toactive shooter incidents.This program is endorsedby the In te rna t iona l

Association of Firefi ghterss, the International Association of Fire Chiefs, and the United States Fire Administration to provide effective medicaal c a r e u n d e r d y n a m i c conditions. The goal of the program is to allow for a better prepared response to incidents such as thosse at Aurora, CO; Columbine High School; Virginia Techh; and the Washington Navvy Yard.

2014 brought about the awareness and preparednesss for Ebola type incidentss. Ebola, also known as Ebola Hemorrhagic Fever, is a rarre and deadly disease caused by one of the Ebola viruus strains. To date the Ebola virus is carrying roughly a 50% mortality rate. KFDD, along with the regionn, prepared for a collaborativve approach to respond to these types of incidents. The Kettering Fire Departmennt worked with Dayton MMRSS, Public Health of Dayton and Montgomery Countyy, M o n t g o m e r y C o u n t y Human Services, and the Greater Miami Valley EMS Council for an appropriatte and expedient responsee. Kettering Fire Departmennt continues to pursue the most h ighly e f fect ivee, efficient, and professionaal approach to emergency medical services. In the realm of modern emergency management and effective public safety response,

it is paramount that emergency responders have the appropriate knowledge, skills, and abilities to effect positive outcomes. The Training Bureau is tasked with providing the necessary resources and educational opportunities to ensure our staff are well prepared to respond to the community’s emergencies. KFD embraces an all-hazards mitigation approach to provide extensive training for members in a variety of fi re, medical, hazardous materials, and technical rescue disciplines. In 2014, training topics to include new recruit training, fire suppression and tactics,

emergency medical services, and specifi c technical rescue disciplines were offered to

challenge all members of the Fire Department and enhance professional development. The Training Bureau continues to expand in our efforts to train our members to the highest level possible in order to ensure that the Kettering Fire Department d e l i v e r s s u p e r i o r emergency services to this community. In 2014, the department completed over 11,000 hours of training and professional development! Training was conducted on a variety of common and special topics to ensure that the department is prepared for any non-criminal

emergency that confronts

this community. Included in the 2014 Training Curriculum were: Core Competency Training / Check-offs on all basic job functions, ice/water rescue, Ebola preparedness and infection control, extensive medical training, and advanced auto extrication to include school bus extrication training. Our Command Staff continued to participate in the nationally recognized Blue Card Incident Command Certif ication Program. This program provides critical information to assist the department’s Fireground Offi cers with tools to effectively lead and manage critical emergency scenes in a safe, consistent, and effective manner. Every day is a training opportunity where we teach, mentor, and challenge our personnel to answer the question, “Are you prepared?”

Technical Rescue

Te c h n i c a l r e s c u eis defined as a rescuesituation that presentsm o r e c o m p l e x a n ddangerous circumstances than what fi re departmentsnormally encounter. Thesetypes of incidents require ahigher degree of trainingand specialized equipmentto conduct safe operations.Several types of incidentsare categorized as technical rescue incidents including:water rescue, confinedspace rescue, trench andexcavation rescue, high and low angle rope rescue,structural collapse rescueand veh ic l e /mach inerescue. Although this listin not all inclusive in thecontext of technical rescue,these disciplines have been identifi ed through a needsanalysis within the City of Kettering as a job functionrequirement. As such, theKettering Fire Department has created a TechnicalRescue Team of 16 rescue specialists who are trainedat a higher level for this typeof emergency response.This team provides serviceto both our community aswell as the region.

The Technical Rescue Program is guided by twoNational Fire ProtectionA s s o c i a t i o n ( N F PA ) standards. NFPA 1670Standard on Operations

In the continuousprogression of implementinga detailed and documentedTechnical Rescue Program,Kettering Fire Departmentc o n d u c t e d s e v e r a lt ra in ings in 2014 toenhance knowledge and

capabilities. KFD membersattended a NFPA 1006/NFPA 1670 compliantheavy vehicle and busextrication training. Thistraining was conducted incooperation with the City of Kettering School’s - Bus

Transportation Services and Sandy’s Towing and Heavy Rigging. KFD instructors researched and developed this training to meet our community’sneeds. The Heavy Vehicle Extrication Training was

of fered to KFDmembers as wellas departmentsthroughout ther e g i o n . K F Dalso conductedconf ined spacerescue trainingsutilizing the Miami Valley Fire Allianceregional simulator.In our efforts todevelop the RescueProgram and ensureadequate resourcesare available tomitigate technicalrescue incidents,the departmentp u r c h a s e d a

Technical Rescue Trailer in 2014 to be utilized for trench rescue and structural collapse rescue. This trailer will allow the department to respond more effi ciently to these types of incidents and willcarry special equipment needed to safely operate i n t h e s e d a n g e r o u s environments.

In 2014, the City of Kettering entered intothe implementation andconstruction phase of the Fire Station StrategicPlan which was initiatedin 2006 in response toa g i n g i n f r a s t r u c t u r eand inefficient facilities.The analysis found that Kettering’s current firestations fail to meetindustry standards,lack the physical space and designfor 24-hour crews, are undersized for modern day f ire apparatus, and lackavailable land to properly allow for the improvements n e c e s s a r y t o upgrade to current s t anda rds . Th i slong term facilitys u s t a i n a b i l i t y plan calls for the construction of four new fi re facilities the next several years.

The new stationswere recommended asa result of an extensiveplanning process which tookplace over the past eightyears and was incorporatedinto the comprehensive modernization strategyof the Ketter ing F i reDepartment. The cityhe ld mul t ip le pub l i c meetings and met with key

stakeholders to gain inputon the design of the newstations. These new stationswill provide appropriatefacilities for the storageand safety requirementsof modern apparatusin addition to modern facilities to accommodate24/7 operations. Thefacilities will be located

scheduled to be complete by the end of 2015.

Starting in early 2015the City of Kettering willbe breaking ground onits second of four newfi re stations. This stationwill be located at 4745Hempstead Station Road. This facility will replace FireStation 36 and will include

In 2014, the FirePrevention Bureau workeddiligently to ensure thatKettering remains a safe place to live, work, andplay. Fire Marshal Bill Fordcontinued to provideessential support to thePlanning and DevelopmentDepartment by conductingover 161 plan reviews,construction inspections,acceptance tests of fi re protection systems, andfi nal occupancy requests.Unlike many communitieswhere the fi re department and building departmentsduplicate efforts ininspecting and conductingacceptance testingactivities, Fire Marshal Fordis cross certifi ed as a FireProtection Inspector andspends approximately60 percent of his timein support of planningand development whichpromotes effi ciency for both departments.

Construction-related activities are given a highpriority because they aretypically time sensitive andhelp to promote additionaljobs within the communityand ensure businessretention. Through thisrelationship many businessescompleted or beganconstruction and renovationprojects on or ahead of schedule in 2014. Listed hereare just a few examples of these projects:

Multiple renovationprojects continuedthroughout theKettering MedicalCenter.

In addition to the construction and renovation projects, the Fire Prevention Bureau worked diligentlyto ensure that existingcommercial propertiesremain compliant with theOhio Fire Code. To ensure this compliance, morethan 145 inspections wereconducted at commercialproperties throughoutthe city. These propertiesincluded preschools,daycares, restaurants, bars,healthcare facilities, andmany others. Throughthese inspections morethan 800 fi re code violations were discovered,and nearly all of theseviolations have beenresolved or are in theprocess of being resolved.

An area of great focus in 2014 for the Fire Marshal was fi re code compliance within all of the public and private schools in the community.

These schools consist of 18facilities totaling more than one million square feet of building space! In additionto inspecting the schools,the Fire Prevention Bureau completed inspections for 20 child daycare centers. These inspections helpthese centers maintain their state licensure whilealso ensuring these facilities remain a safe place for thechildren.

Annual fi re safety inspections wereconducted for the entireKettering Medical Center Complex, including theKettering College of Medical Arts facility. Thecomprehensive inspectionof this ever-increasingcomplex hospital facilityis required by the U.S.Department of Healthand Human Services.These inspections are alsonecessary to ensure that the

facility is eligible to receiveMedicare and Medicaidfunding which is criticalto the hospital’s economicvitality. The KetteringMedical Center is thelargest employer withinthe City of Kettering.

The Fire Marshal provided guidanceand expertise as a member of the TechnicalWorking Group for the proposed County/CityMigrated Radio SystemImplementation Project.The Fire Marshal’s offi ce further assisted with theprogramming of essentialvolunteer pagers andengine house alertingsystems which resultedin signifi cant savings tothe City. During 2014 the Fire PreventionBureau conducted severalinvestigations of fi res within the community toprovide cause and origin

services to fi re crews. The Fire Marshal is assisted by apart-time investigator.

Hydrant ProgramIn 2014, Montgomery

County Water Department corrected many of thedefi ciencies found throughthe hydrant function testsconducted in 2013. Inaddition, our summer hydrant crews cleareddebris and obstructionswhile painting over 1,000hydrants throughout thecity. This maintenance isdone to increase the lifeexpectancy of the fi re hydrants while maintainingthe esthetic appearance of the hydrants. During thesummer months of 2015,KFD will be conductinghydrant function testsagain to ensure hydrantsare kept in a state of readiness for use in theevent of a fi re.

Public Education In 2014, the KetteringFire Department continuedto educate residents throughtraditional methods whileexperimenting with newtechniques to advanceits f ire and l ife safetymessage. Fire departmentcrews engaged with thecommunity in a varietyof traditional educationalmethods. During 2014,Kettering Fire Department

took new approaches in aneffort to better reach our residents of all ages.

K e t t e r i n g F i r eDepartment was active in 2014 facilitating morethan 90 American HeartAssociation CPR and First

Aid Courses! These coursesresulted in the issuance of over 1,000 certificationsand prepared both our residents as well as city staff with the skills to potentiallysave lives. Programs suchas these when paired withcommunity AED programss igni f icant ly enhancesurvivability in case of acardiac emergency.

with the Police Departmentand Safety Section hostedmonthly Child Passenger Safety Fitting Stations.Through this program,our city staff took theopportunity to educateparents in child safety

and to ensure the proper installation of more than120 child and infant car seats. This program provides essential information anda hands-on approach to keeping our community’s youngest residents safe as they travel on Kettering roadways and beyond.

T h e d e p a r t m e n t continued to host the Ke t te r ing Leader sh ip Academy for a day of practical hands-on exercises to allow community leaders the opportunity to experience firsthand what it’s like to use the “Jaws of Life” and other hydraulic tools to extricate trapped victims of an auto accident. The group gained valuable knowledge as to exactly what it takes to train and equip a modern day fi re service to be ready to answer the call and effectively mitigate citizen’s emergencies during their time of need. The group also gained perspective on the physical and mental challenges of the profession by simulating some of the activities that our personnel must perform on a dailybasis.

fi lmed an educational videoteaching the importanceof fire prevention as wellas preparedness. This fi lmwas distributed along witheducational literature toevery kindergarten throughthird grade class room in the city. We were gratefulto have teachers throughoutKettering embrace theprogram during NationalFire Prevention Week andhelp ensure that children know how to prevent fi resfrom occurring in the home and how to react if a fi redoes occur.

The past year alsopresented the departmentwith an opportunity topartner with the Dayton Metro Library. Through this partnership, departmentmembers were given theopportunity to deliver fire safety messages topreschoolers as well as adults. Preschool agedstudents were shown what a fi refi ghter would look and sound like if there was a fi re.This community experienceand training helps prevent children from hiding outof fear in the event of afi re. Presentations were also offered to adults through this partnership with theDayton Metro Library. These presentations gave adults anopportunity to learn about

historical context such asthe Great Chicago Fire andthe lessons learned in thewake of this tragic event.

Another newo p p o r t u n i t yoffered in 2014was a day campcalled “Firefi ghter For a Day.” Thisday camp wasoffered throught h e K e t t e r i n gR e c r e a t i o n Complex followingN a t i o n a l F i r e Prevention Week.This one-day campg a v e c h i l d r e nage 8 to 12 anopportunity tohave a hands-onexperience of thelife as a fi refi ghter.These studentswere given thechance to: operatea real fi re hose, tryon a firefighter’su n i f o r m , a n dc r e a t e f i r eevacuation plansfor their homes. These studentswere also giventhe opportunity toeat lunch with oneof our fi re crews and askquestions about what lifeis like as a fi refi ghter.

Partners in Protecting our Community

T h e K e t t e r i n g Professional Firefighters -IAFF Local 2150 engagedthe community, both while

on duty as members of theKettering Fire Department,as well as off duty asconcerned citizens and wereable to provide assistance tomany local charities during2014. The firefighters’

group continued its longstanding tradition of helpingfamilies in need through the26th annual Adopt-a-Family program. With assistancefrom the Kettering CitySchools, Meijer and other local businesses: 7 familiesand 22 children wereadopted and provided withpresents, utility assistance,and a nice holiday meal.

2014 marked the firstyear that our firefightersjoined with the nationalprogram “Firef ighters for Operation Warm” to provide 42 winter coats to the Kettering City School children who didn’t have an adequate winter coat. It is their intent to grow this program in 2015 through fund-raising to be able to double the number of coats provided to children in need within the Kettering community. The Kettering Professional Firefighters collectively were able to distribute over $5,000 to local charities during 2014 and hope to continue to be “Partners in Protecting our Community” through engaged commun i ty service in assisting as many local charities as possible.

in each sleeping room.Having detectors in theappropriate locationsprovides occupants withthe early notif icationessential to survival in caseof a fi re. The Kettering FireDepartment operates a Smoke Detector Program to assist residents in need by making smoke detectors available for their use. Thisprogram is made possiblethrough both corporate and private donations. Thisprogram serves to educateresidents on the dangersof fire and the need for working smoke detectorsto provide early notifi cation that a fi re hazard exists.

For a res ident to be eligible to participateand receive one of thesedetectors , they must provide proof of residencyalong with a written fireevacuation plan for their home. The requirementof the evacuation plan isan essential component to ensure that families take time to create an evacuationplan that meets the needof their specific home aswell as the specific needsof each person living in thehome. Numerous smoke detectors were distributed in 2014, and as a result of thisprogram, the communityrelations team has had the opportunity to assist familiesin creating an effective

evacuation plan for their homes and improving thesafety of their families.

Knox Box Program A Knox Box is a securelock box that a l lowsemergency crew to accessan unoccupied structure,eliminating the need for forced entry, ultimatelycausing less damage to the

structure. The Fire PreventionBureau continues to educateresidents and businesses onthe importance of havinga Knox Box to allow safeand effi cient access duringan emergency. ThroughoutKettering, there are nearly3 0 0 b u s i n e s s e s a n dresidences equipped witha Knox Box. This is one of many methods used by theKettering Fire Departmentto keep property loss to aminimum while ensuringquick access can be made toa structure in the event of anemergency. Battalion Chief Yike began his serviceto this community in 1976 as a volunteer fi refi ghter and was hired full-time in 1981.Ed was promoted to a Captain in 2001 andthen to Battalion Chief in 2008. He alsoserved as a Fire Instructor and Dispatcher for many years. During his tenure, Chief Yikeachieved an extensive resume of fi re servicecertifi cations and experience, which made him invaluable as an instructor and a leader for the department. In 2002, Ed was awardedthe Greater Miami Valley EMS Council’sOutstanding EMS Professional Award, whichrecognized his outstanding work as anEmergency Medical Dispatcher, EmergencyMedical Technician, and his critical incidentdecision making. Life Saving AwardsThe Kettering Fire Department’s Life Saving Award is a prestigious and incredible honor awarded to active duty personnel whose direct actions save the life of person whose heart has stopped beating and shows no signs of life. This award is only achieved when the patient who was clinically deceased is not only resuscitated and regains life sustaining functions, but moreover is able to make a full recovery and resume normal life activities. It is one of the most rewarding accomplishments any member of this organization can make in the course of their career and to know that their actions contributed to saving the life of another human being. To understand the signifi cance of the last statement, the individual that is saved must be discharged from the hospital and resume normal rrlife activities.

On January 26, 2014, TheKettering Fire Public SafetyDispatch center received a911 call for a male patientwho was experiencingdifficulty breathing. Thefi re dispatcher gathered thenecessary information andimmediately dispatched Quint and Medic 36’semergency crews to assist.While enroute, the caller stated the patient was nowunconscious. Upon arrival,emergency crews enteredthe home and found their patient lying on his bedwith no heartbeat and norespirations. This alarm had quicklyescalated in severity fromdifficulty breathing topatient now in cardiac arrest.KFD personnel, throughtraining and fi eld experience,prepare for the worst casescenario, and for this alarm,

The crews hadtaken all appropriate medicalequipment into the home and quickly took action providingAdvanced Life Support care.Members of the crews wereable to simultaneously initiatecritical care while gatheringimportant informationregarding the patient’smedical history. Personnelbegan life sustaining CPR,initiated advanced care andused the Life Pack 15 todeliver the life-saving shock.The patient regained a pulseand spontaneous respirations

after approximately 8 minutesof critical interventionsand effective AdvancedLife Support. The patientwas transported to Miami Valley Hospital South whileproviding oxygen andsupportive care. This alarm shows howthe importance of earlynotification of 911 whenthe patient first beganto experience difficultybreathing, quick emergencyresponse, and the importanceof how highly trained medical professionals working in thefi eld can save lives.

staged for KPD to secure the scene. On arrival, crewsfound the patient lying ona couch with very shallowrespirations, no palpable pulse and s igni f icanttrauma to the abdomen.The crew rapidly assessedthe patient and determinedthe p re sence o f l i f e threatening injuries. Basedon the primary medical assessment, the crewswiftly moved the patientto their cot and then to themedic for rapid treatmentand transport. For severe traumatic injuries, time is

of the essence to providethe best chance of survival.The crews recognizing thisspent less than 5 minutes onscene which was in itself alife saving measure. Duringthe rapid transport to Miami Valley Hospital South, thecrew provided advancedlife support techniquesfor f lu id replacementand effective CPR. Theeff ic ient and effectiveactions taken by thesemedical professionals wereessential to this patient’ssurvival. The excellentfi eld care, combined witha rapid transport to theclosest hospital, and theoutstanding trauma careat the hospital to stabilizethe patient were essentialactions in order to save thepatient’s life.

O n M a rc h 2 7 ,2 0 1 4 , t h e K e t t e r i n gCommunications Center received a 911 call for anunresponsive female andrapidly dispatched thecrews from Engine andMedic 36 to assist. While enroute to the scene,responding crews wereadvised that the patienthad been exhibiting classicsigns of a heart attack prior to becoming unresponsiveand that CPR had beeninitiated by the patient’sfamily member MarylinBerardi, who is also a nurse. In less than 5 minutesfrom the time of the call,KFD crews arrived on sceneand found their patientto be in Cardiac Arrest.Kettering Fire Departmentm e m b e r s a s s u m e d patient care and beganresuscitation efforts. Thecrews resumed high quality

CPR and simultaneouslyprepared Advanced LifeSupport interventions.

KFD c r ews workeddi l igently to continueadministering advanced lifesaving interventions anddrug therapy in an attemptto stabilize the patientand regain a functionalheartbeat. During thisincident the patient’s heartexperienced six changesin cardiac rhythms vastlycomplicating her care. KFD’s paramedics expertlyinterpreted these changesand delivered multipledefibrillations with theLife Pack 15 as well asnumerous pharmacologicalinterventions in an attemptto restart the patient’sheart. This patient wentin and out of cardiacarrest multiple times andour crews constantly re-

assessed her condition andresponded with appropriate care. Shortly after moving the patient to the medic, crews evaluated that the patient now showed signsof purposeful movements,and eventually regained consciousness. The crew evaluated the patient’s heart rhythm through 12-lead monitoring equipment and alerted Ketter ing Medical Center to initiate a cardiac alert and prepare an interventional team to takethe patient immediately to the cath lab. This alarm is a truetestament of the AmericanHeart Association’s Chain of Survival and why early recognition of the signs and symptoms of a heart attack, bystander CPR, and rapid response from emergency crews is so important.Recent studies have proventhat early and effective CPR is critical to a patient’s survival in a cardiac arrest. On August 7, 2014,Ket te r ing emergency crews were called to abeauty salon where abystander witnessed aworker collapse. The seriesof events that happenednext are a testament to Kettering’s Public SafetyDispatch Center and our emergency r e sponsesystem. If we break thiscall down to each of the emergency services, youwill get a look into allthe pieces of an actual emergency medical alarm.

The 911 cal l came into our Public SafetyDispatch Center where anemergency medical traineddispatcher answered thecal l and immediate ly started medical instruction for the caller to initiate CPR. Since all of the dispatchers, Police and Fire, are EMD(Emergency Med ica l Dispatching) tra ined, the cal l was handledby multiple dispatchersin the center. The firedispatcher dispatched

the closest fi re and medic units, while Dispatcher EvaConley initiated live savinginstructions, and the policedispatcher sent patrol units.This allowed fi re and policeunits to be dispatchedwithin 47 seconds fromtime of call! Dispatcher Conley, calmly took controlof the call and gave criticallifesaving instructions to thecaller. The fi re dispatcher supervisor stated, “It wasapparent that Eva’s calmand professional directionhelped save the patient’slife.”

On that afternoon,Aubrey Groh was in theprocess of getting her hair

done when she witnessedher hair dresser collapseto the f l oo r. Aubreyimmediately called our 911Emergency Dispatch Center to notify them of the medicalemergency. She quicklytried to gather as manybystanders as she could toaid in the overall efforts tosave this patient’s life. After recognizing that the patientwas not breathing, Aubreybegan life saving CPR togive the patient the bestpossible chance at survivalwith instructions given byour emergency medical dispatcher.

Shortly after dispatch, KPD Offi cer Jenn Smithhart arrived on scene and used her CPR training to assist the bystanders with CPR before KFD’s arrival.

Fire and EMS crews arrived within 4 minutes 12 seconds of dispatch and placedthe patient on the Life Pack 15 Cardiac Monitor which showed a shockable rhythm.With one defi brillating shock the patient regained a heartbeat and pulse. The quickreaction of the crews allowed them to deliver this life-saving shock, prepare the patientfor transport and arrive at the hospital within 15 minutes of their arrival on scene. Thetotal time of this call was 20 minutes from the time the 911 call was answered until thepatient was at the hospital.

This call shows the importance of every piece of our emergency services system.From the calm demeanor and professional instruction of Dispatcher Eva Conley, the cooperative response of the other dispatchers to get fi re and police crews enroute, toKPD Offi cer Smithhart’s effective CPR and the quick response of the fi re crews withlife-saving advanced cardiac care. The entire public safety services system proved its effectiveness in saving a member of our community.

Department’s Medic 36 andQuint 36 were dispatchedwithin 35 seconds of thecall and given updatedinformation while en-route.On arrival, the emergencycrews found a 52 year-old-male patient lyingon the living room floor unconscious. The patient’swife stated she witnessedher husband collapse andcalled 911 immediately.The emergency crewsidentifi ed the patient wasin cardiac arrest and rapidlyinitiated Advanced CardiacLife Support care in anattempt to resuscitate thepatient. After administering

fi ve defi brillations (shocks) and numerous rounds of advanced pharmacologictherapies, the patient had a return of spontaneous circulation (which means his heart starting beating again and he regained a pulse). The crew then prepared the patient for transport and provided advanced medical care enroute to Kettering Medical Center.Upon arrival to KMC, thehospital staff continued the life sustaining measures to allow the patient the opportunity to make a full recovery from this traumatic event.

In th i s ca se , fo r apatient to survive thistype of cardiac eventand make a full recoveryfrom cardiac arrest, theemergency management system worked flawlesslyfrom the first 911 phonecall, to rapid dispatch of appropriate resources,efficient response fromhighly trained medicalcrews, effective care onscene, and safe transportto a cardiac interventionalfacility. On November 18, 2014,Kettering Public SafetyCommunications Center received a 911 call for an unresponsive patient.Dispatchers rapidly sentQuint 36 and Medic 32crews while simultaneously in i t i a t ing emergencymedical dispatch questions and pre-arrival instructionto the caller. Upon arrival,Quint 36 found a malepatient was unresponsivewith a weak pulse andlabored breathing. Thecrew of Quint 36 quicklydetermined the patient to be in critical conditionand began emergency medical treatment. Whenthe medic crew arrived,the determination wasmade to rapidly move thepatient to the medic for transport.
