In 2011, CJE SeniorLife faced many challenges. First and foremost, the fragile economy continues to impact every aspect of our community. We have all had to adjust to a “new normal.” Many of our clients and residents are entering CJE’s comprehensive network of services with a significant decrease in their financial resources coupled with an increase in their frailty. And while we are committed to meeting their mental, physical, and emotional needs, CJE is also experiencing a decrease in support from many funding sources, including traditional governmental reimbursement programs like Medicare and Medicaid.

We are also preparing for a tidal wave of new clients as the Baby Boomer generation begins turning 65 in 2011. A new population study, commissioned by the Jewish Federation of Metropolitan Chicago, reports that locally, more than 20% of Jews are 65 or older. Clearly, while we are facing stiffer challenges, CJE’s mission is more relevant than ever.

In striving to meet the increasing needs of our community, we also have to be accountable to our stakeholders. In 2011, due to the troubled economy, our Board and staff leadership made the very difficult decision to not move forward with Karmel, our vision for the first Jewish-sponsored Continuing Care Residential Community in the Chicago area.

On the other end of the spectrum, our extraordinarily committed 700-plus employees served close to 19,000 individuals and family members in over 40 program areas. In addition, we initiated a beautiful renovation of Robineau Residence and Lieberman Center for Health and Rehabilitation which is celebrating its 30th anniversary. We also launched a Parkinson’s Disease Management Program which includes an extensive staff training initiative in order to provide clients with advanced and specialized Parkinson’s care. Finally, we submitted an innovative funding proposal, in alignment with the national Health Care Reform Act, which would allow CJE to provide select hospitals and their patients with cost-effective Transitional Care services.

In these tough times, we are fortunate to have an engaged Board of Directors who, through hard working committees, provide outstanding stewardship of our core mission, values and vision. We are also grateful for the hundreds of government programs, corporate and private foundations and individual donors who collectively provide the financial support to advance CJE’s mission. As we move into our 40th year of service to older adults, we are especially appreciative of our longstanding partnership with the Jewish Federation of Metropolitan Chicago.

And finally, our heartfelt thanks to all the clients, residents and family members who put their trust in us. It is truly a privilege to serve the Jewish and larger community with our unparalleled constellation of care to enhance the lives of older adults.

Board and Executive Leadership

The world is aging with lightning speed. Baby Boomers began turning 65 this year and by 2030, more than one billion people worldwide will be 65 years or older; 66 million of them will be Americans. A recent population study by the Jewish Federation of Metropolitan Chicago shows that more than 20 percent of Jews in Chicago are already 65 or older and that number will only increase in the coming years.

CJE SeniorLife has planned for the transformation that is occurring throughout the world and, as a result, remains flexible, forward-thinking and responsive to the complexities of aging, while still adhering to the values on which the organization was founded.

To achieve all of this in the face of profound changes to the economic, demographic and healthcare landscapes requires a vision that is dynamic and adaptable.

A Vision for the Future

CJE SeniorLife’s Constellation of Care...

What sets CJE SeniorLife apart from other organizations is how its programs and services complement one another, making it possible for CJE to provide transitional care that is comprehensive, seamless and customized to each individual’s unique circumstances.

Older adults and their families turn to CJE SeniorLife at different stages in their lives. No matter what needs they might have, they benefit from CJE SeniorLife’s invaluable network of support, guidance and resources. Sometimes they participate in a single program, other times they create combinations of services that evolve as they age.

• One individual connects to our organization at age 65 by attending a diabetes screening and educational seminar presented by CJE’s Center for Healthy Living. A year later he hires CJE’s Home Safety Assessment service to recommend modifications he can make to his home to continue living in it for as long as possible. When he finally needs supportive care, he chooses Gidwitz Place for Assisted Living.

• Another person supports CJE SeniorLife’s mission for years through annual donations and volunteering and then decides to enroll her 75-year-old mother who has Alzheimer’s in CJE’s award-winning Culture Bus program. As her mother’s condition progresses, she finds a home for her at Friend Center, CJE’s residence that is specially designed to address the unique needs of those with dementia.

• A 78-year-old military veteran receives help from CJE’s Resource Specialists in applying for government benefits and discovers that CJE could also provide him with home-delivered meals and transportation.

• After recuperating from knee-replacement surgery at CJE’s Haag Pavilion for Short-term Rehabilitation, an 85-year-old woman receives at-home care from one of CJE’s devoted Home Health nurses. Later, she recommends Lieberman Center to a friend who is searching for a skilled nursing residence for his father.

To call CJE’s comprehensive network a “continuum of care” implies there is a single point of entry with a linear progression through its services. While CJE does offer a continuum within many specialties, such as dementia care, “continuum” simply does not adequately express the overlapping, interconnected options CJE offers the nearly 19,000 people it serves. Rather, CJE’s model of service delivery is far more complex and fluid – a “constellation of care.”

Independence and Social EngagementCJE SeniorLife helps older adults pursue lives of meaning, independence and good health and the organization’s efforts in 2011 were no exception.

CJE’s Center for Healthy Living (CHL) delivered on its promise of offering older adults ways to achieve positive aging through innovative programs that took participants to the Chicago Botanic Gardens, the Art Institute of Chicago and the Kohl Children’s Museum, among other destinations. CJE presented educational seminars, health screenings and disease-management workshops to combat the growing epidemic of diabetes with help from grants from the U.S. Administration on Aging via the Illinois Department of Public Health, Office of Women’s Health and the Alvin H. Baum Fund. CHL also introduced Home Safety Assessments. Designed to help keep older adults living in their homes and communities longer, this new service is staffed by CJE’s own Certified Aging-in-Place (CAPS) Specialist. In addition, CHL launched MyPainManagement.net, a website dedicated to the needs of those with chronic pain.

Promise and HopeNot everyone comes to CJE SeniorLife in moments of crisis, but those who do find life-saving resources in its Consumer Assistance department which in 2011 received a total of 17,000 inquiries for assistance and referrals to CJE’s network of programs and services.

CJE’s Resource Specialists continued to serve those hardest hit by changes in Medicare, Medicaid and the housing crash. In addition, CJE reached out to even more people through judicious partnerships with local banks, businesses and other community organizations.

Board and executive leadership traveled to Springfield, Illinois, and Washington, D.C., to give a unified and much-needed voice to Illinois seniors. They advocated for the very programs that help the thousands of older adults CJE serves, including those most vulnerable to the effects of a devastated economy. Among other successes, their efforts resulted in the continuation of CJE’s Managed Community Care Program (MCCP), now in its 17th year. The only program of its kind in the state of Illinois, MCCP provided 680 low-income, frail, elderly clients with a full range of home- and community-based services, including Adult Day Services and personal care. Seniors who might otherwise need nursing home care can continue to live independently in their own homes.

Investing in the Future Part of being a dynamic and adaptive organization means investing in existing infrastructure and keeping up with new technology and medical innovations. In 2011, improvements to CJE’s buildings included the completion of a major renovation to Lieberman Center and the Haag Pavilion for Short-term Rehabilitation,

and more than $2.5 million in improvements and renovations to CJE’s independent housing residences.

On the technology front, in order to move toward being “paper-free,” CJE implemented an electronic medical record (EMR) system, which saves time, streamlines communications between physicians, hospital discharge planners and CJE’s direct care staff and increases accessibility to patient medical information.

To address the growing number of clients with Parkinson’s, CJE developed an agency-wide Parkinson’s Disease training program in collaboration with Northwestern University Parkinson’s Disease and Movement Disorders Center and the Rehabilitation Institute of Chicago. Funded by a grant from the Braun Foundation, the program covers a wide range of disciplines throughout CJE’s constellation of care, including nursing, rehabilitation and social work. CJE’s Leonard Schanfield Research Institute will conduct a formal evaluation, including the measurement of relevant clinical and other health-related outcomes.

CJE’s network of support and services allows older adults to remain in their homes and communities and have their evolving and individual needs met.

Adult Day Services offers supervised, structured programs for people with Alzheimer’s and other cognitive impairments, as well as those who are frail or socially isolated. A group setting maximizes each participant’s potential by promoting autonomy, creativity and self-esteem. CJE’s ADS programs, including Culture Bus, are offered in downtown Chicago, Evanston and the northern suburbs.

Center for Healthy Living programs and services help older adults link to community resources, make informed choices about aging issues and maintain their physical, mental and social well-being. Lifelong learning, health screenings and cultural and social events help them lead healthy, independent lives. CHL also administers two unique websites on caregiving and pain management:www.CaregivingHelp.org www.MyPainManagement.net

Consumer Assistance staff in Chicago and the northwest suburbs provide free information, resources and advocacy on issues that impact older adults, including consumer benefits, community services, public assistance, Social Security, housing referrals, home repair, crime victim assistance and transportation. Special features include lending library, computer lab and classes and Russian-speaking staff.

Counseling from CJE’s licensed clinical social workers is available to older adults and their families, along with a variety of support groups.

Community Services

We’re here for you.CJE SeniorLife helps older adults pursue lives of meaning, independence and good health.

Geriatric Care Management professionals assess and consult with older adults and their families as they experience the transitions of aging. Acting as liaisons to family caregivers who live nearby or far away, they coordinate service delivery, guide decision-making and monitor care.

Home Health professionals from CJE’s Medicare-certified and state-licensed home health agency provide intermittent care to homebound older adults who are under a physician’s orders. Staff includes registered nurses, physical, occupational and speech therapists, social workers and home health aides.

Kosher Meals are delivered to older adults who are unable to prepare their own food. In addition to nutritious hot meals, CJE offers weekly delivery of fresh-frozen meals via Kosher to Go.

Personal Care Services provide assistance with activities of daily living and chore housekeeping through an arrangement with Gentle Home Services or through CJE’s personal care program for eligible low-income seniors.

Transportation on CJE’s Shalom buses offers scheduled, door-to-door trips to grocery stores, medical appointments and social events. Medicar Service provides comfortable and convenient transportation via paratransit vans to and from medical appointments or other destinations.

The Friend Center Assisted Living

Residential OptionsAssisted Living is offered at Weinberg Community for Senior Living, a state-licensed campus in Deerfield that embraces Jewish culture.

• GidwitzPlace features 125 one- and two-bedroom apartments for older adults who require a supportive environment yet still wish to live as independently as possible. Amenities include personal care services, housekeeping, kosher restaurant-style dining, a full schedule of activities, respite care and registered nurses on site 24 hours a day.

• TheFriendCenter for Alzheimer’s Care is home to 35 people with Alzheimer’s and other cognitive impairments. Residents enjoy spacious private rooms, assistance with bathing and grooming, stimulating activities and kosher meals. Respite care available.

Independent LivingCJE SeniorLife owns and operates Section 8 subsidized and affordable apartments throughout metropolitan Chicago. In addition, CJE offers Robineau Residence, a group-living residence for 24 older adults who qualify for government-subsidized housing, with amenities for residents that include activities, transportation, excursions and a central dining area.

Skilled Nursing & RehabilitationLiebermanCenterforHealthandRehabilitation, a residence in Skokie with 240 private rooms, provides long-term skilled nursing care, a short-term rehabilitation unit and a dementia special care unit. Hospice care is also available. Robert and Sylvia Haag Pavilion, the short-term rehabilitation in-patient care unit, serves patients after hospitalization, providing physical, occupational and speech therapy primarily to Medicare-certified patients. Creative arts, therapeutic, recreational and spiritual programs are offered daily to all Lieberman and Haag residents. Kosher dining at Lieberman is approved by the Chicago Rabbinical Council (CRC) and prepared under rabbinical supervision.

Applied ResearchThe Leonard Schanfield Research Institute (LSRI) conducts social and applied research in the field of aging that provides the catalyst for new and innovative program development.

Jewish LifeRecognizing the importance of a healthy spiritual life to the well-being of older adults,CJE SeniorLife offers residents Shabbat Services and pastoral care from a chaplain. Cultural and educational opportunities are offered to the public by The Lakeshore Mercaz Center for Older Jewish Adults, a collaborative effort of CJE and four synagogues.

Volunteer OpportunitiesMore than 5,500 volunteers provide vital services to clients including participating in activities, delivering meals to the homebound, visiting those who are isolated, assisting with Shabbat luncheons and more.
