PROGRAM Welcome Welcome to The Hospice of Windsor and Essex County’s conference entitled, Coming Together: Creating an Interprofessional Model in Hospice Care.

For the past 35 years, the Hospice has responded to the needs of the community through support, education and empowerment. When we developed our strategic directions, our Board of Direc-tors reached out to our friends in the community, asking how we could serve their needs. We heard you loudly and clearly when you

asked for more educational opportunities. With our new educational programming, tar-geting audiences from patients and caregivers to formal care providers, we are pleased to offer more opportunities for professional development and continuing education. We are particularly excited to offer this conference as a vehicle for interprofessional knowledge exchange.

According to the poet and playwright William Butler Yeats, “Education is not the filling of a pail, but the lighting of a fire”. May we all be ignited by our ongoing learning and be sources of warmth and light to excite and inspire our patients, families and col-leagues. Maurice T. O'Callaghan is one of Canada's finest and most popular professional speakers. Born in Toronto, educated at York University and the University of Western Ontario, Maurice brings a wealth of personal and managerial experience to the speaking platform.

Integrating Interprofessional Education and Research into Practice

Dr. Margaret Steele is currently the vice Dean, Hospital Interfaculty Relations, Schulich School of Medicine & Dentistry. Margaret is a Professor in the Departments of Psychiatry, Pediatrics and Family Medicine. She earned her Honours degree in Microbiology and Im-munology at Western University in 1983 and completed her MD in 1987.

She obtained her Fellowship in Psychiatry in 1992 and in Child and Adolescent Psychiatry in 2013, which was the first year of the Child and Adolescent Psychiatry Fellowship exam. Margaret earned a Masters of Higher Education from the Ontario Institute of Studies in Education/University of Toronto and became a Fellow of the Hedwig van Ameringen Ex-ecutive Leadership in Academic Medicine at Drexel University in Philadelphia, Pennsylvania in 2008. In 2012, Margaret became a Distinguished Fellow of the Canadian Psychiatric Association and in 2013 she became a Canadian Certified Physician Executive.

This workshop will outline the process that occurred to enhance palliative care at a community hospital. The team will outline key events/supports that were crucial for preparation and presenta-tion to key stakeholders and lessons learned will be reviewed. Finally, data from their first year will be presented with interpre-tation and their expectations for the next year. Participants will:

Gain an understanding of the key facilitators in developing or redeveloping palliative care

Gain an understanding of lessons learned in the develop-ment of a truly "interprofessional" team

Gain a better understanding of the patient populations and services that were required during the team’s first yearthrough presentation of their year one data.

Talking Together: Spiritual Care and Social Work As One

Shared Care Palliative Care in Ontario; The Interprofessional Way Forward!

This presentation will focus on the collaboration of spiritual care and social work in a hospice setting, and how the professions work together to provide best practice in a holistic way. This collaboration also reduces the amount of time that a client has to tell their story. Participants will learn about:

Providing a new perspective on bringing professionstogether to provide optimal care

Adapting services based on need - how we provide anticipa-tory grief support to children, adults and aging population

Relating information to the interdisciplinary team and re-porting back to ensure that the Hospice is providing holisticcare. The idea of what now? How do we take this model and implement it within all disciplines?

Providing Palliative Sedation at End-of-Life — A Collabo-rative Approach by a Multidisciplinary Team at End of Life

Jon Ann Meko, BA, BScN, RNEC,CHPCN(C), MN (C) Nurse Practitioner,

and Roslynn Walpole, RT, Erie St. Clair CCAC Palliative Care Consulta-

tion Team This presentation will discuss the challenges and successes of working collaboratively as an effective team to support patients’ wishes at End -Of-Life . The story of a patient (“Nancy”) living with ALS and receiving support from the Palliative Care Consultation Team (PCCT) program will be shared with a focus on what strategies worked and what didn’t . Participants will:

Deepen their understanding of patient-centered care through amultidisciplinary collaborative approach to support the patient’s(Nancy’s) wishes at End –Of-Life (EOL).

Have a discussion, re palliative sedation at home and reflect on outcomes and debrief on palliative sedation at home

Understand the value and importance of the role of the Respira-tory Therapist at EOL with an ALS patient.

This workshop will allow participants to understand the term and

evolution of “shared care” as applied to models of palliative care and

be able to apply this to their own systems and places of care. Goals of

this session are to:

Gain clarity on what was and is ‘shared care’ as applied to the palliative care population and systems of care

Understand how to use a ‘best practice’ approach and theory toassess for readiness for shared care, status of shared care in ones one workplace and community of care, and then set a course offuture development

Be positioned to lead and facilitate further understanding and applications of shared care models and methods after this day,back at ones own place of work.

Plenary Session Description

Interprofessional education can be moved into the mainstream of health professional education: It just takes patience and perseverance!

Carole Orchard, RN, BSN, Med, EdD., Associate Professor & Coordinator of Interprofessional Health Education and Research, Western University.

Dr. Carole Orchard is an internationally recognized scholar in the field of IPC. Her research focuses on development of health provider teams in practice settings and the role of the patient and their caregivers in team based care. She has created a conceptual framework to change the culture and measure-ment instruments to assess team collaboration and individuals' socialization into this new model of care.

In this workshop the Western University Interprofessional Education model will be presented including its evolution to the point of moving to a 'buy-in' by program faculty, the how to's in moving towards an inter-professional collaborative teamwork within health professional educational programs at the pre-licensure level. Participants will have opportunities to consider this model and how it might be applied in their own settings. Objectives: 1. To gain insight into the theoretical, competency, and proficiency approaches that can be used in developing interprofessional education programs

within educational institutions

2. To explore learning strategies that can be integrated into learning activities that are seen as relevant to practice by students, such as palliative care

3. To discuss how to evaluate the participants’ learning and the program effectiveness , to determine its effectiveness and impact on participants’learning.
