Dear Colleagues,
Welcome to Monaco!
It is with great pleasure that I welcome you today to the Thom-
men Medical symposium here at the EAO 2009 meeting in
the beautiful Principality of Monaco. I thank you for joining us
for this special event.
We have excellent speakers and an exciting program which
presents the latest research on implant surfaces along with
new information on implant design and technology.
Strategies to modify titanium surfaces, based on topographical,
chemical and biological principles will be highlighted.
The results of an animal study examining a possible method
to influence the osseointegration of dental implants by condi-
tioning the implant surface will be presented.
Last but not least, challenging clinical cases will be presented
using new diagnostic-therapeutic procedures and the use of
ct-scan based software.
We hope you’ll join us for this informational and enjoyable
event.

Bienvenue à Monaco,
C’est un grand plaisir pour moi de vous souhaiter la bien-
venue aujourd’hui au symposium Thommen Medical, ici, à
l’édition 2009 de l’EAO dans la superbe Principauté de
Monaco. Je vous remercie de participer à cet évènement.
Nous avons d’excellents conférenciers et un programme
enthousiasmant qui présente les dernières avancées en
matière de surfaces implantaires ainsi que de nouvelles infor-
mations sur le design et la technologie implantaires.
Les stratégies de modification des surfaces en titane, fondées
sur les principes topographiques, chimiques et biologiques
seront mises en évidence.
Les résultats d’une étude animale étudiant la possibilité
d’influencer l’osséointégration des implants dentaires par
conditionnement de la surface seront présentés.
En dernier lieu mais non des moindres, des cas cliniques
difficiles seront présentés en utilisant des procédures diagnos-
tico-thérapeutiques et l’utilisation de logiciels travaillant sur
les scanners.
Nous espérons que vous nos rejoindrez lors de cet évène-

Concepts scientifiques des surfaces
Le titane est devenu le matériau de référence du fait de ses
résistance mécanique, stabilité chimique et excellente bio-
compatiilité. Ces propriétés des implants en titane assurent
un bon ancrage dans l’os mandibulaire ou maxillaire. Des
tentatives sont faites dans le but d’obtenir des temps de
cicatrisation plus courts pour les implants de façon à les met-
tre en charge aussi rapidement que possible après la chirurgie
tout en permettant une prompte osséointégration. Mis à part
un design performant de l’implant pour un bon ancrage
mécanique, ces objectifs peuvent être atteints en modifiant
la surface implantaire tout en gardant à l’esprit que les pro-
priétés de surface les plus importantes pour des implants
(métalliques) sont topographie, chimie,charge électrique et
mouillabilité.
Les deux approches pour améliorer les propriétés de surface
sont orientées vers l’optimisation de la micro-rugosité et
l’application de différents revêtements bioactifs. La mouillabi-
lité des surfaces micro-rugueuses a été optimisée pour une
augmentation potentielle de l’interaction implant-tissus. Une
autre approche consiste à ajouter des composants bioactifs.
La bioactivité des surfaces peut aussi se faire par des revê-
tements diffusant des principes actifs. Nous supposons que
les modifications chimiques et pharmacologiques de la surface
du titane initient une réponse osseuse plus forte que la seule
surface de pointe sablée et mordancée.
Les opinions et points de vue exprimés dans cette présentation sont uni-

Dr Lorenz Meinel a obtenu sont diplôme de pharmacie à l’ETH de Zürich en
Suisse et celui de médecine à l’Université de Francfort en Allemagne. Il est
actuellement conférencier à l’ETH de Zürich dans le département de chimie
et bioscience appliquées. il est également professeur adjoint associé à la
Tufts University et membre du Centre de Recherche en Ingénierie Tissulaire,
un consortium des MIT, Harvard, Tufts et Columbia.
Dr Meinel est un membre du board de la Société Suisse des Pharmaciens de
l’Industrie et auteur et coauteur de nombreuses publications. Il a tenu des
conférences dans le monde entier et a reçu de nombreux prix
et récompenses.
Dans son poste actuel, Dr Meinel est un Senior Translational
Medicien Expert chez Novartis AG. Les opinions et points de
vue exprimés par le Dr Meinel dans ses présentations sont seu-
lement les siennes et pas nécessairement celles de Novartis
AG ou ses affiliés.
Scientific Surface Concepts
Titanium has become a gold standard, because of the mate-
rials mechanical strength, chemical stability and excellent
biocompatibility. These properties of titanium implants ensure
good anchorage within the mandible or maxillary bone.
Attempts are made for achieving shorter healing periods for
implants in order to load the implants as early as possible
after surgery while still allowing expedient osseointegration.
Apart from good implant design related to mechanical
anchorage, these requirements may be met by modifying
the implant surface keeping in mind that the most important
surface properties for (metallic) implants are topography,
chemistry, surface charge and wettability.
Two main approaches attempting to improve surface pro-
perties were directed at optimizing the microroughness and
applying different bioactive coatings. Microrough surfaces
were optimized for their wettability for potentially enhanced
implant-tissue interaction and better osseointegration. Ano-
ther approach adds bioactive components to titanium surfa-
ces. Bioactivity of surfaces can also work via drug eluting
coatings. We hypothesize that chemical and pharmacologi-
cal surface modifications on titanium initiate stronger bone
response than an advanced sandblasted and acid etched
surface alone.

Dr. Dr. Lorenz Meinel received his degree in Pharmaceutics at the ETH Zurich,
Switzerland and his medical degree at the University of Frankfurt, Germany.
Currently, he is lecturer at ETH Zurich at the Department of Material Sciences
and the Department of Chemistry and Applied Biosciences. Furthermore, he is
Adjunct Associate Professor, at the Department of Biomedical Engineering, at
Tufts University and Fellow of the Tissue Engineering Research Center, a
Consortium of M.I.T., Harvard University, Tufts University and Columbia Univer-
sity.
Dr. Dr. Meinel is a Board Member of “Gesellschaft Schweizeris-
cher Industrieapotheker – GSIA (Swiss equivalent to the German
APV) and author and co-author of numerous publications. He has
lectured worldwide and has received several prices and awards.
In his current position, he is Senior Translational Medicine Expert
at Novartis Pharma AG, Translational Medicine. The views and
opinions which will be expressed by Dr Meinel in his presentations

Evaluation of surface conditioned
dental implants – An animal study
Objectives: A possible method to influence the osseointe-
gration of dental implants is the conditioning of implant surface
coatings in order to improve hydrophilicity. The aim of this
study is to assay how bone formation is influenced by two
different surface coatings.
Methods: Two different implant surfaces were applied: sand-
blasted, acid-etched titanium (SPI®ELEMENT, Thommen, Swit-
zerland) and conditioned titanium. Prior to placement implants
were either surface conditioned in a solution containing
hydroxide ions or assigned to controls. 144 implants were
inserted on either side of the mandible of 18 minipigs. The
animals were sacrificed after 2, 4 and 8 weeks of submerged
healing. For the 8 weeks group, a sequential polyfluorochrome
labelling was performed. After sacrifice, one side of each
jaw was selected for histological and histomorphometrical
evaluation of bone implant contact (BIC), osteoid and bone
volume formation.
Results: Polyfluorochrome labelling indicated bone growth
on the implant surface towards the host bone for both surfaces.
After 2 weeks BIC reached 66% for conditioned surfaces
compared to 48% for controls, p=0,270. Differences levelled
out after 4 and 8 weeks. Osteoid levels were initially compara-
ble, showing a slower decline for conditioned surfaces after 4
weeks. Bone volume was higher for conditioned surfaces at all
times. No significant differences could be measured.
Conclusion: Osseointegration was achieved for both surfa-
ces. A tendency for increased BIC seems to be present for
conditioned surfaces after short term healing.
Dr. Dr. Bernd Stadlinger received a degree in medicine at the University of
Vienna (Austria) with a thesis at the Department of Plastic and Reconstruc-
tive Surgery. He also received a degree in dental medicine at the University
of Dresden (Germany) with a thesis at the Department of Oral & Maxillofacial
Surgery. Dr. Dr. Stadlinger is member of faculty at the Department of Maxillo-
facial Surgery in Dresden and is involved in various research projects on
implantology. He especially focuses on the evaluation of different implant
surface structures in animal experiments. In 2006 he was
awarded the Dentsply Research Prize for his studies on biolo-
gical implant coatings. In 2008 he received the Implantology
Grant of the European Association for Cranio-Maxillofacial Sur-
gery.

conditionnée – Une étude animale.
Objectifs: Une méthode possible qui permet d’influencer l’osséoin-
tégration des implants dentaires est le conditionnement de surface
pour en augmenter l’hydrophilie. Le but de l’étude est d’évaluer
comment la formation osseuse est influencée par deux différents
traitements de surface différents.
Méthode: deux surfaces différentes d’implant ont été utilisées: l’une
sablée-mordancée (SPI®ELEMENT, Thommen Medical) l’autre
conditionnée en plus. Préalablement à la pose chaque surface a été
soit conditionnée dans une solution contenant des ions hydroxydes,
soit attribuée au groupe contrôle.
144 implants ont été insérés de chaque côté de la mandibule de 18
cochons d’Inde. Les animaux ont été sacrifiés après deux, quatre et
huit semaines de guérison enfouie.Pour le groupe à 8 semaines un
étiquetage séquentiel au polyfluorochrome a été réalisé. Après sa-
crifice, un côté de chaque mandibule a été sélectionné pour tester
le couple de desserrage et l’autre pour l’évaluation histologique et
histomorphométrique du contact os-implant (BIC), des ostéoïdes et
de la formation de volume osseux.
Résultats: L’étiquetage au polyfluorochrome a montré une crois-
sance osseuse vers l’hôte pour les deux surfaces. Après 2 semaines
le BIC a atteint 66% pour les surfaces conditionnées cà comparer à
44% pour le groupe contrôle, p=0,270. Les différences s’estompent
après 4 et 8 semaines. Le volume osseux était plus important pour
les surfaces conditionnées durant toute la période d’observation.
Pas de différences significatives ont pu être détectées.
Conclusion: les deux surfaces ont permis l’ostéointégration. Une
tendance à un plus fort BIC semble présent sur les surfaces condi-
tionnées après une période courte de guérison.
Dr Bernd Stadlinger a obtenu son diplôme de médecine à l’Université de
Vienne (Autriche) avec une thèse au Département de Chirurgie Reconstruc-
tive et Plastique. Il a également obtenu le diplôme de médecine de l’Univer-
sité de Dresde en Allemagne avec une thèse dans le Département de
Chirurgie orale et Maxillo- Faciale. Dr Stadlinger fait partie du Département de
Chirurgie Maxillo-Faciale de Dresde et participe à différents projets de re-
cherche en implantologie. Il se consacre particulièrement à l’évaluation de
différentes surfaces implantaires en expérimentation animale.
En 2006 il a obtenu le Prix de la Recherche Dentsply pour ses
études sur les revêtements de surface des implants. En 2008
il a obtenu l’ Implantology Grant de l’Association Européenne
de Chirurgie Cranio-Maxillo-Faciale.

Implant Therapy:
New Solutions for Challenging
Clinical Cases
In unfavourable dental and anatomic conditions, perio-
implant-prosthetic therapy may prove to be difficult, if not
impossible, unless adequate reconstructive procedures are
applied.
Functional, aesthetic, phonetic and prognostic requisites will
be evaluated from an interdisciplinary perspective when
establishing a diagnostic-therapeutic plan.
A personalized approach to bone volume augmentation will
be illustrated and the evolution of new diagnostic-therapeutic
procedures (CAD-CAM) and their influence on a less invasive
surgical alternative will be discussed.
Utilizing both static and dynamic images, an analysis of the
most up-to-date scientific literature will be presented in order
to determine and apply the most rational choices based on
pre-defined criteria, within the range of the various recons-
tructive options.
Dr. Mauro Merli earned his degree in Medicine and Surgery at the University
of Bologna in 1986, and in 1989 specialized in Odontostomatology. He dedi-
cates his clinical activity to treating complex cases from an interdisciplinary
perspective.
He is an active member of the Italian Society of Periodontology (S.l.d.P), The
Italian Academy of Prosthetic Dentistry (A.l.O.P ) and of the European Asso-
ciation for Cranio-Maxillofacial Surgery (EACMFS).
Dr. Merli held the position of Secretary of the Italian Society of Periodontology
from 1995-1997, treasurer for the 1997 - 2001 four year term, vice-president
from 2001-2003 and is President for the 2008-2009 two year term.
Author of several scientific articles, Dr. Merli lectures in National and Interna-
tional conferences and courses. Together with his brother Aldo and sister
Monica, Dr. Merli runs a private practice founded by his father Mario, which
has recently expanded to a large scale multidisciplinary clinic.

voire impossible, à moins de mettre en oeuvre des procédu-
res restauratrices adaptées.
Les exigences de fonction, esthétique, phonétique, et établis-
sement du pronostic seront évaluées dans une perspective
interdisciplinaire lors de l’établissement du plan de traitement.
Une approche personnalisée de l’augmentation osseuse en
volume sera exposée et l’évolution de nouveaux procédés
de diagnostic (CFAO) et leur influence seront discutés.
Une analyse de la bibliographie scientifique à jour en utilisant
des images à la fois statiques et dynamiques sera présentée
afin de déterminer et appliquer les choix les plus rationnels
fondés sur des critères pré-définis dans l’ensemble des op-
tions restauratrices.
Dr Mauro Merli a obtenu son diplôme de Médecine et de Chirurgie à Bologne
en 1986 et s’est spécialisé en 1989 en dentisterie. Il consacre son activité
clinique au traitement de cas complexes d’un point de vue interdisciplinaire.
Il est un membre actif de la Société Italienne de Parodontologie (Sidp),
l’Académie Italienne de Prothèse (AIOP) et de l’Association Européenne de
Cranio-maxillo-faciale (EACMFS).
Dr Merli a été Secrétaire de la Société Italienne de Parodontologie de 1995 à
1997, trésorier de 1999 à 2001, vice-président de 2001 à 2003 et à présent
président pour le mandat 2008-2009.
Dr Merli est l’auteur de plusieurs articles scientifiques et donnent des confé-
rences et cours nationaux et internationaux. Avec son frère ALdo et sa soeur
Monica, Dr Merli dirige un cabinet privé fondé par son père Mario, cabinet qui
s’est récemment développé en une grande clinique interdisciplinaire.

Why Thommen?
Full maxillary implant-based bridgeworks are the some of the
most challenging restorative procedures to perform. The use
of ct-scan based software helps treatment when used to its
optimal capacity by securing radiographic and surgical
templates to fixed reference points.

Dr Jean-Nicolas HASSON, received his Doctorate in Dental Surgery at the
University Louis Pasteur in Strasbourg (France) in 1976. He earned his
specialty degree in Periodontology in Paris (Université Paris VII) in 1979 and
graduated in Periodontics in 1981 at the University of Southern California (Los
Angeles, U.S.A). He founded in 1989 a large scale clinic in Mulhouse (France)
and practices Periodontics and Implant Dentistry.

d’Etudes Cliniques Spéciales en Parodontie à Paris (Université Paris VII) en
1979. Il a suivi le cursus de spécialité en Parodontie à l’Université de Californie
du Sud (Los Angeles, USA) où il a été diplômé en 1981. En 1989 , il a créé à
Mulhouse un cabinet de grande envergure où il pratique l’implantologie et la
parodontie clinique.
