CONSOLO REPORTS This guide will explain basic Consolo Reports functionality. Additional important information can be found in other user guides. Additionally, basic functionality may be enhanced by periodic updates to Consolo’s software. While we endeavor to keep these manuals up to date, in order to stay abreast of current functionality, you should always review the Release Notes, and attend pre-release training, whenever a new version of Consolo is released.

ACCESSING REPORTS All Reports are accessed through the “Admin” Tab:

REPORT MAILING Consolo can generate a daily Census Report by email. This HIPAA-Compliant Report provides a daily update on all census, referral, and admission activity. To control who receives this Report.

Go to the Admin Menu Select Report Mailing Select Recipients (The must have a Role with Permissions to receive the Report) Select the Person to Receive the Report Click Save Test by Clicking the Send Report to Myself link on the Change Recipients Page

NOTE: There must be an email address in the User profile of the person receiving the report. NOTE: Only users with an Admin role or Parent Role (with Permission included) can receive report.

BASIC REPORTS FUNCTIONALITY Reports include various "filtering" options. When present, Date Range is always required. Other filter options are generally optional. Leaving filter options blank will return results for ALL values of that filter.

“Reset Filter” at top right of screen will clear any data currently in the Filter Options. If the Filter Options have values selected that you don’t want to use, or if you get unexpected results when running a report, try “Reset Filter” and then re-enter Filter Options and re-run the report.

Date Range is always required. Select a specific Office(s), Area, Region, or Corporation. Complete other filters as desired. “Run Report” to generate results.

“Toggle Filter” hides filter options. “Reset Filter” clears all filter settings.

The Date Range Filter quickly accommodates various date range configurations:

Once “Run Report” is clicked, the following message appears:

You may now navigate about Consolo freely. You may navigate to other screens, close tabs, or even log out of Consolo. Your report will generate on our Servers. Once completed, you’ll see a Notification icon at lower right of your screen:

Select from the list of Date Range options, or manually enter dates in the Date Range fields. Dates must be entered in MM/DD/YYYY format.

Click on “Notifications”. You’ll see recent notifications, newest on top. Click on the result you wish to view:

Other recently-completed Reports will list in your Notifications list. You may retrieve them multiple times, within 24 hours, by clicking on them. Remember, though, that results may have changed since you last generated a report. Archived Reports will expire after 24 hours; you’ll receive a “404 Unavailable” error. Just run the report again to retrieve fresh results.

Use the “Search” box to search within the report for a specific word, date, number, or phrase.

Use the “Show/Hide columns” button to add/remove columns from the display

Click on Column Headers to re-sort the data by that column. For example, in the report above, clicking on “Medical Rec” will re-sort the list chronologically by MRN.

At the far right of the report, in the top row, you’ll find icons for exporting the report:

“Copy” will copy the report to your clipboard. From there, you may paste it into Excel, Word, or some other program.

“CSV” will save the report as a CSV file on your computer. CSV files can be opened with Excel.

Exporting reports allows you greater ability to format rows, columns, etc. for printing

Printing can also be done from your web browser, using your browser print function. Landscape mode works best.

When done with the report, simply close the Tab. You may access the report again, within re-running it, by clicking “Notifications” and then clicking the report you wish to re-view (see Page 3). Remember, however, that results may have changed since you last generated the report.

REPORTS ORGANIZATION All Reports are accessed via the “Admin” menu:

Clicking “Reports” opens the following screen:

Click the name of a Report to access it. Use the “Categories” at left side of screen to search within smaller

sets of Reports.

NOTE: HISTORICAL PAYROLL INFORMATION IS FOUND UNDER ADMIN/ BILLABLE

TIME LOCKS:

Billable time locks (old, locked payroll information) can be Filtered by Date Range. The resulting information can be sorted by column and exported, just like other Reports.

SPECIFIC REPORT INFORMATION ACCOUNTS RECEIVABLE REPORT Lists claim totals by patient according to Month End Close recording. Shows outstanding AR balances as of the date specified. Filter Options:

End Date – Last Date of results you wish to view

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Payer – Only results for a specific Payer

Patient – Only results for a specific patient. Type patient last name in field; select match

County – Only results for a specific County

ACTIVE PATIENT SUMMARY REPORT This report is designed to list each patient multiple times; one entry for each state of the patient. If there are multiple rows per patient, this does not mean the patient is duplicated, but rather it means that something changed in the patient's charts. Do not be alarmed. Please verify the data in each row. If the data is expected- e.g. a patient transferred from home to facility- that is normal. If a column shows blank or unexpected information- e.g. no diagnosis or MRN- that is a cue to investigate that patient’s chart and make corrections. Shows all active patients and daily level of care for a given time period. At the bottom of the Report, view data for number of days per level of care, unique patients, and current days of care. Allows for many different filtering options. A general-purpose Report capable of compiling data about a wide range of patient attributes, including:

Start of Care and Discharge dates Demographics (age, gender, race, veteran) Location (home or facility, facility type, death location) Hospice assignments (MRN, nurse, physician, team) Services (DME, pharmacy, transportation providers) Code Status & Hospital Avoidance Level of Care Primary Payer, Secondary Payer and Payer IDs Changes in any of these (1 entry per change) Number of Active days since admission & number of Active days per report date.

Filter Options:

Show Level of Care Details – Adds columns to right of each patient row showing level of care on each day of the Date Range

Group by Payer – Organize results in groups, by Payer Physician – Only results for patients assigned to specific Physician (type last name, select match) Medical Director – Only results for patients assigned to specific MD (type last name, select match) County – Only results for patients residing in a specific County Disaster Acuity – Only results for patients with a specific Disaster Acuity Team – Only results for patients assigned to a specific Team DME Provider – Only results for patients assigned to a specific DME Provider Pharmacy – Only results for patients assigned to a specific Pharmacy Hospital Avoidance – Only results for patients with a specific Hospital Avoidance preference

ACTIVITY REPORT Shows all claim and payment activity for a patient or office for a given Date Range. This report lists all claims or back balances during a particular Date of Service. If filtered by one patient and for a large date range, you can see all claims for a patient and any payments associated to those claims will be displayed under the claim highlighted in GREEN. It displays the specific name of the payer and not just "Medicaid" or "Private Insurance" as a category. It displays payment amount, date and check number so this report is often used to see exactly when a particular claim was paid. This report can also be filtered by Month with no other filters to see all claims generated with that date of service. This filtering option is often used just after claim generation to see that all patients have a claim. To do additional sorting of data Export to CSV Filter Options:

ADMISSION PAIN TRACKING REPORT Shows results for Pain Observations completed at 24, 48, and 72 hours post-admission. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Team – Only results for patients assigned to a specific Team

ADMISSIONS SUMMARY Summarizes all admissions in Date Range. Include/exclude re-admits from the Report. Total Admissions and Average Disposition Days (days from referral to admission) display at the bottom of the Report. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Facility – Search only for patients who were admitted at a specific Facility Payer – Search only for patients admitted with a specific Payer Indicator – Search only for patients admitted with a specific ICD Code Diagnosis – Search only for patients admitted with a specific Diagnosis Only Re-Admits? – only search for patients who were Re-Admitted in the Date Range of the report Not Re-Admits – Exclude Re-Admissions from the Report

AGING REPORT Shows outstanding balances by claim with billed age. This report contains list of claims including date range, claim number and view. It displays the age of AR balance based upon month when claim was closed. If system is not Hard Closed each month, then the AR will be displayed in the “Current” column. Filter Options:

End Date – The last dates for which results are desired Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Payer – Only results for a specific Payer Patient – Only results for a specific Patient (type last name, select match) County – Only results for patients residing in a specific County

AR ACTIVITY REPORT Shows beginning balance, revenue activity, payment activity and ending balance by patient for a particular period. This report displays revenue by level of care, payment amount, adjustment amount, and write-off amount by patient. It does not tell you the date of the payment or the check number, but the payment will only show up on this report if it falls during the date range of the report. This report is Not Focused on Date of Service like the Activity report is. Revenue and Payments on this report would be revenue and payments recorded within the date range of the report. So if a Sept claim was generated in December after Sept, Oct and Nov were closed, that revenue would be on the December report and not the Sept report. If months are not closed then the revenue will fall into the month of service. The AR Activity report displays beginning and ending balances. If a patient has new revenue or payment items during the period they will be listed. Patients with no activity during the period and multiple payers or locations may not be listed, so the Grand Total ending balance for the previous month may not match the Grand Total beginning balance for the next month unless the patients in both months are the same. Filter Options:

BEREAVEMENT CARE PLAN STATUS Shows the activity and survivors for all unique care plans for all deceased patients being served during the Date Range of the Report. Shows dates for all assigned and completed Bereavement Care Plan interventions. Clicking on patient name will open patient's homepage in a new Tab. Clicking on Survivor name will open Survivor's Bereavement Care Plan in a new Tab. Filter Options:

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Patient – Only results for a specific patient (type last name, select match)

BEREAVEMENT CONTACT SUMMARY Shows a summary of all contact information for Survivors, or bereaved Family/Contacts. This Report returns address, phone, and map information for Family/Contacts associated with deceased patients who were included in the Report filters. This Report produces a table suitable for exporting to create mailing labels. Clicking on a Patient Name opens the patient homepage in a new Tab. Clicking on the Family/Contact name opens the summary information screen for that Family/Contact. Filter Options:

Date Range – Only results for patients who died during date range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

BEREAVEMENT INTERVENTION SUMMARY Shows current, expiring, or future bereavement interventions for the Date Range and filter options selected. Shows assigned staff members, interventions, their expiration dates and resolution statuses. Data is compiled from each patient's Bereavement Plan of Care. Clicking on patient names opens the patient's homepage in a new Tab. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

BILLING VERIFICATION Compares what claims have recorded to what was calculated to be billed. Filter Options:

CALL REPORT This report is different than the On-Call Tracking Report. This report facilitates communication between shifts. This Report compiles Visit Notes that have the "Call Report?" Checkbox selected. Click on patient name to open the patient homepage in a new Tab. Click on "Date of Note” to open the specified Note. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Patient – Only results for a specific Patient (type last name, select match) Include Discharged? – Include results for patients discharged during Date Range User – Only results created by a specific User Team – Only results created for patients assigned to a specific Team

CAP REPORT Shows insurance and transfer info on patients. Please review the CMS-related Reference Documents located in the Training Tools Tab for additional instruction. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Facility – Only results for patients located at a specific Facility during the Date Range (type facility

name, select match) County – Only results for patients located in a specific County during the Date Range Disaster Acuity – Only results for patients with a specific Disaster Acuity

CARE PLAN EXPIRATION REPORT Shows current, expired and expiring plans of care for the Date Range and filter options selected. Shows assigned staff members, interventions, their expiration dates and resolution statuses. POC dates and data are compiled from each patient's Hospice Plan of Care. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Patient – Only results for a specific Patient (type last name, select match) Include Discharged? – Include results for patients discharged during Date Range User – Only results for care plans created by a specific User

CERTIFICATION SUMMARY Shows active, unsigned, expired and expiring patient certifications for the filter options selected. Will also show certifications with a Late (more than 2 days after benefit period begins) Face to Face Visits. Report displays in up to four sections.

EXPIRED Certifications – Active patients with no current certification

UPCOMING Certifications – Active patients with a current certification that expires within 3 weeks

Unsigned Active Certifications – Patients who have a current Certification, but the Certification has not been signed

Recently Discharged patients – to verify all certifications are signed).

Clicking on patient names opens the patient's CERTIFICATION in a new Tab. Filter Options:

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Include Active, Unsigned Certifications? – Include results for patients who are Active, and have an unsigned current certification

Include last 90 Days of Discharged Patients? – Include results for patients discharged within past 90 days

CHART AUDIT REPORT Shows every clinical chart entry available for the filter options selected. This Report has utility for chart auditing, visit tracking, and productivity evaluation. Clicking on patient names will open the patient's homepge in a new Tab. Clicking on the Effective Date will open the specified chart entry in a new Tab. Filter Options:

Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Payer – Only results for a specific Payer

Patient – Only results for a specific patient (type last name, select match)

Diagnosis – Only results for patients with a specific diagnosis

Include Discharged? – Include results for patients discharged during the Date Range

Visit Type – Only results that have the specified Visit Type (from Visit Notes)

CLAIMS STATUS SUMMARY REPORT Shows an overview of payments, AR and revenue for a given time frame. This report pulls together data from several other financial reports. On the left-hand side of the equation it displays Cash collected, Change in Accounts Receivables, Adjustments and Write-offs. On the right-hand side it displays revenue by level of care or type of charge broken out by payer. Filter Options:

Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

CLAIMS STATUS SUMMARY REPORT Calculates Data used for the Annual CMS cost reporting Tool. Filter Options:

Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

CONTINUOUS CARE REPORT Shows continuous care time for the Date Range selected. At the bottom of the Report is a toggle button that allows you to view Subtotal information for continuous care. A "Unit" of time = 15 minutes, per Medicare guidelines. Filter Options:

Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Facility – Only results for patients located at a specific Facility during Date Range (type facility name, select match)

Facility Type – Only results for patients located at a specific Type of Facility

Payer – Only results for patients assigned a specific Payer

Patient – Only results for a specific patient (type last name, select match)

Team – Only results for patients assigned to a specific Team during Date Range

DEYTA REPORTS These Reports are only available if your agency has a contract with Deyta. These Reports allow you to download (as excel or CSV files) and export data to Deyta (via their internet portal) for Bereavement, Family Survey, Employee Satisfaction, or Referral Follow-Up purposes.

EMPLOYEE TIME / PHYSICIAN CARE PLAN OVERSIGHT REPORT This Report is suitable for tracking employee/clinician time and productivity, and for verifying Time Sheet and Payroll information. Shows employee time details over a given Date Range. Time Totals for the Date Range and filter options selected display at the bottom of the Report. Time entries are taken from the “Patient Time” section of clinical charting, and from Users’ Time Sheets (non-clinical time). Checking the "only physician care plan oversight box" will produce a Report that captures physician care plan oversight time information from physician orders. This information may be useful to physicians who want to track this time. Clicking on patient names will open the patient's homepage in a new Tab. Filter Options:

Date Range Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation Time Type – Search only for entries that have a specific Time Type:

o Patient Time (Time included on Clinical Charts as Time In / Time Out for Visits) o Volunteer Time (Time completed by Users whose Role is Volunteer) o Non-Clinical Time (From Users’ Time Sheets) o On Call Time (From Users’ Time Sheets) o Paid Time Off (From Users’ Time Sheets) o Sick Time (From Users’ Time Sheets)

Patient – Only results for a specific patient (type last name, select match) Role – Only results for Employee Time completed by Users with that Role User – Only results for Employee Time completed by that User Only Physician Care Plan Oversight? – Check this box to search only for Time included on

FACE TO FACE VISIT SUMMARY REPORT Shows upcoming Face-to-Face visits due for Medicare patients entering their third or higher benefit period. Includes links to patient homepages and to new face-to-face visit notes for each patient. Also includes patient / primary caregiver contact information and links to Google Maps and MapQuest. Report displays in two sections, one for CURRENT Certifications that need a face-to-face visit, and one for UPCOMING Certifications that need a face-to-face visit. Filter Options:

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Physician – Only results for patients assigned to a specific Physician

Team – Only results for patients assigned to a specific Team

Shows room and board revenue and patient liability by patient and grouped by facility. This report is for Room and Board billing. Run the report prior to billing to make sure all room and board patients are listed with the correct rate and liability. If a patient is not on this report, they are not properly set up for R&B billing and will not get a claim. Prior to generating claims the 2 columns on the far right will display as $0.00. Then after generating claims re-run report to see the amounts displayed in the 2 columns on the right entitled: Actual Charge and Actual Liability. These are the actual amounts displayed on the claims. Filter Options:

Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Facility – Only results for patients located at a specific Facility during Date Range (type facility name, select match)

HOSPICE CAP ANALYSIS REPORT Shows an approximation of Medicare CAP analysis. Please review the CMS-related Reference Documents located in the Training Tools Tab for additional instruction. Filter Options:

Start Month

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

HUMAN RESOURCE DETAILS REPORT Employees may be tracked in Consolo, in the Admin/Users area, by Hire Date, with associated details relating to each Period of Employment. Specific Human Resource Related Details (such as: Licensure,TB Tests, Insurance Information, etc.) may be tracked by Effective Date and Expiration Date. This report summarizes this information. Filter Options:

Date Range

Filter Date On – Only results for items Effective or Expired during the Date Range

Offices – Only results for a specific Office(s), Area(s), Region(s) or entire Corporation

Human Resource Topic – Only Results for a specific Topic

IDG AND HOSPICE ASSIGNMENTS SUMMARY REPORT This Report produces a three-sectioned list of all patients Active, Admitted, and Discharged within the Date Range of the Report. The Report itself is useful for facilitating an IDG meeting. To produce an IDG Report for all patients in the report, or for a specific number of patients, a basic printable report using the "print options" can be generated. To generate an interactive IDG Report for any patient, an interactive "wizard" can be utilized. The Wizard produces an IDG Report/Ongoing comprehensive assessment that can be electronically signed and stored in Consolo, and e-faxed from a patient’s homepage. See the IDG Report Reference Document in Training Tools for more information. This Report also summarizes staff Assignments. Filter Options:

LENGTH OF STAY REPORT Displays the Length of time a patient is on hospice. The report includes Discharged Patients Only by default but also may be filtered to include Admitted or Active Only. Data will display with one patient line item per patient however, multiple office details display. Please keep in mind the following:

Patient Counts - This section will display one line item per patient so if the totals in the other sections do not appear to match, remember the Patient may be listed in more than one Office Column in the Patient Counts.

Diagnosis Breakdown - This section only counts when an Diagnosis (LCD Worksheet) is entered in the patient’s Clinical Indicators & Diagnosis (if Count appears low, all patients may not have an LCD - Find details in the Patient Count Section).

The bottom of the report includes summary information for the Office, Area, Region, and Corporate levels of your agency.

MEDICARE VISIT SUMMARY REPORT Shows all Clinical visits for Medicare Patients which are required to be reported on claims. This report is used to review Medicare clinical visits prior to billing. Run the report for a particular date range with no other filters, then check to see if all visits are listed. If not listed then perhaps the user forgot to choose a Visit Type with an asterisk. Also check to see that all visits on this report have the proper units. If the units are larger than they should be, check the start time/day and stop time/day. Sometimes users accidentally enter the wrong date which inflates the units significantly. Click a couple of times on the column header entitled "Actual Units" to sort and reverse sort by that column. This will cause all visits with zero units to be grouped together and then all visits with large units to be grouped together as well. Filter Options:

PAYMENTS POSTED REPORT Summary of all payments posted in a time frame. After payments are posted you can filter this report by payer and date range to see the payments displayed with posting date, check amount, and check number, as well as a detailed listing of all claims posted within that payment. At the bottom of the report you will see a total of cash collected for a time frame as well as the total adjustments, write-offs and their reasons. If multiple offices are included in a payment, and you filter by office, you will see claims and totals associated to that one office only. Filter Options:

Report displays all time, by type, for all users in the Date Range/Office(s) selected. If a given time type was not used by anyone in the Date Range/Office(s) specified, that time type will not have a column in the Payroll Report. Columns can be shown/hidden. Payroll Report can be "locked", which prevents users from modifying the time entries contained within the Report. The Payroll Report can also be named and stored for Historical reference. SUPERVISORY VISIT EXPIRATION REPORT Shows supervisory visits that have expired and visits that are due to expire. This data is calculated based on "Supervisory Visits", not from supervision sections of specific Nursing or other assessments. Clicking on Patient Name will open the patient's homepage in a new Tab. Clicking on "Last Visit Date" will open the last Supervisory Visit for that patient in a new Tab. From there, a new Supervisory Visit can be created/completed. The supervisory visit expiration report shows data for ALL patients, even those that don't have a current Hospice Aide. LPN/LVN's also require supervisory visits, and some locations also do supervisory visits for volunteers. The NHPCO Regulatory Committee suggests- and we agree- that Hospices simply chart a supervisory visit every 14 days for all patients- this will ensure compliance with CMS Regulations. In the event no supervision is needed, the note could simply say "no supervision needed." Filter Options:
