Placing and restoring implants in normal to narrow bone without bone grafting is an indispensable skill, which will increase your bottom line at least 20 to 30% each year.

You will learn the seven signs and symp-toms of Occlusal Disease, and how they impact the longevity of the denti-tion and restorative work. The Occlusal Disease Diagnosis System is a methodic and practical system to diagnose and treat Occlusal Disease, including occlusal equilibration and the "3 Golden Rules of Occlusion." Morning presentation In contrast to classical implant studies which show that bone loss around dental implants occurs primarilyduring the first 18 months after implant placement, peri-implant disease takes an average of three years before it is apparent. Every implant system has design features that can contribute to peri-implant disease. This course will expose design weaknesses, particularly around cemented implant components, and how to avoid problems that can occur. Many types of cement have inherent problems; some corrode titanium and most are invisible on radiographs. Others are deliberately colored to blend with soft tissue which makes them difficult to detect and remove. Participants will have a better understanding of which cements to use, when and why. You will learn how to minimize the detrimental effect of excess cement and prevent implant failure. You will also be given up-to-date information about supportive peri-implant maintenance protocols.

Cement extrusion into the peri-implant sulcus may result in inflammation (peri-implant mucositis) which could compromise the soft tissue complex and lead to loss of integration (peri-implantitis) and implant failure. To date, no published protocol exists for cementing implant superstructures. The goal of this course is to critically examine these issues and give up-to-date and as of yet unpublished data that will be used to suggest a suitable protocol for clinical practice. Excess cement induced peri-implantitis must be considered iatrogenic dentistry and, if not addressed, a looming malpractice issue.

Afternoon presentation MANAGEMENT OF PERI-IMPLANTITIS, OCCLUSION, AND IMPLANT ESTHETICS PROBLEMS: - A PERIDONTAL AND PROSTHETIC INTERDISCIPLINARY APPROACH

The afternoon session will begin with discussion of an interdisciplinary approach to resolve cement induced failure in selected cases. High visibility anterior cases will be presented in which periodontal grafting procedures and prosthetic tissue manipulations can achieve an esthetic solution. The role of occlusion in implant failure will be discussed as well as occlusal management of tooth mobility in controlled periodontal patients.

An interdisciplinary approach to esthetic implant dentistry will be discussed in the final segment. Topics will include preferred bone grafting techniques to achieve an esthetic result, and the management of patients with a high smile line or with disproportional tooth size. How to place immediate implant restorations, and how to use platform switching to manipulate the soft tissue will be demonstrated in clinical case reports.

Dr. Punjabi has completed training in two specialties: oral and max-illofacial surgery at the MetroHealth Medical Center Campus of the CWRU and plastic and reconstructivesurgery at the University of Miami/Jackson Memorial Hospital, Miami, Florida. He is board certified by theAmerican Board of Plastic Surgery and the American Board of Oral & Maxillofacial Surgery.

Dr. Punjabi is in private practice in Redlands, CA. He is a Clinical Associate Professor of Plastic Surgeryand Clinical Associate Professor of Oral & Maxillofacial Surgery at Loma Linda University and Chief ofPlastic Surgery at Riverside County Regional Medical Center. His experience also includes private practiceof dental/oral surgery in Jamaica, West Indies.
