DIABETIC RETINOPATHY: An Avoidable Blinding Disorder

Diabetes is a group of diseases marked byhigh blood glucose (blood sugar) leveldue to defect in insulin production,

insulin action or both. If untreated, it leads to avariety of serious eye complications and evenpremature death. Control of diabetes lowers therisk of complications.

There are mainly two types of diabetes Type-1 & Type-2. Type 1, also called Insulin- depend-ent Diabetes Mellitus (IDDM), occurs in 5-10 % of cases and affects childrenand young adults.

Type-2, also called non-insulin-dependent dia-betes mellitus (NIIDM),constitutes 90 -95% ofcases and sufferers areolder people. This type ofdiabetes is associatedwith old age, obesity, fami-ly history of diabetes, raceand ethnicity.

Diabetes is often calledsilent killer because itcontinues to damage thebody without producingcomplications until veryadvanced stage. In eye, italmost affects every struc-ture; like it causes lid infection, recurrent con-junctivitis, corneal erosion, formation ofcataract at early stage. It also affects retina. Themost dreaded complication in eye is in the formof retinopathy called "diabetic retinopathy".

Diabetic retinopathy (damage to retina) iscaused due to involvement of retinal blood ves-sels that can eventually lead to blindness. It isan ocular manifestation of systemic diseasethat affects 80% of the patients who have hadthe disease for ten years or more. Intimidatingstatistics notwithstanding, research indicatesat least 90% of new cases would have beenavoided had there been proper and vigilanttreatment and monitoring of eye. At least 4% ofthe world population is affected by DiabetesMellitus, and 50% of them develop eye problem.The risk of blindness is 25 times higher in dia-betic than non- diabetic persons.

Diabetic retinopathy is a blinding disorder,

and only an ophthalmologist can detect earlysigns of diabetic retinopathy. All diabetic per-sons should, therefore, have their eye examinedat least once every year.

According to World Health Organization(WHO), the number of adults with diabetes inthe world would increase alarmingly from 135million in 1995 to 300 million by 2025. In India,this increase is expected to be from 18 million in1995 to 54 million in 2025.

In diabetes, retinal changes result from fivefundamental processes: weakness in

small caliber blood vesselsresulting in formation of

microaneurism (smalllocalized dilatation),

development ofexcessive leakageof blood contentinto the sur-

rounding reti-na, blockage ofblood vesselsdue to forma-

tion of smallclot, prolifera-tion of new blood

vessels which areleaky and have the

tendency to bleed andformation of fibrous tis-

sue and their subsequent contraction. All theseprocesses lead to diabetic retinopathy markedby a variety of changes in retina.

Vision loss occurs in all stages of diabeticretinopathy. Treatment is effective only beforethe loss of vision. If loss has already occurred,doctors cannot cure it but it can be haltedthrough Laser Treatment. Therefore, it is betterto go for regular checkup by an eye specialist soas to pick up the earliest signs of retinopathyand treat accordingly before the vision lossoccurs.

There are certain risk factors associatedwith diabetic retinopathy. It tends to acceler-ate its course and increase its severity.Males are twice likely to be affected byretinopathy than females. The strongest pre-dictor of diabetic retinopathy is the dura-tion of diabetes.
