May I introduce myself and at thesame time welcome you to Drivenby health magazine.

My name is Stephen Brunt and Iam a director of Blue Crane Limitedwhich is our publishing companybehind this new venture.

Let me start by very briefly tellingyou where this has come from…I was an accountant and pensionconsultant and about 6 months ago Igot chatting to a family friend ShaunStaniforth about an idea he had tobring a magazine model from hisnative South Africa, where he hadworked within the magazinepublishing sector.

Since that moment, I was hooked,hooked to Shaun’s vision, hooked toShaun’s passion and hooked to thegoal of making this magazine thereality that presents itself to you right now!

From creating a database fromscratch, punching away email addressesinto Microso excel in the early hoursof the morning, fuelled by Nescafe. Tochugging up the M40 to theBirmingham Care show in my wife’sHyundai Getz. Shaun and I haveworked endlessly to get this magazineright for you. You being the very peoplewho will benefit from its unique size,relevant suppliers and fresh content.

We are also very proud to havemade partnerships with the twolargest Health and Care home eventsorganisers, being Naidex and eCare Show. Not only has this createda solid platform for Driven by Healthto establish itself on a national levelwithin a short space of time but alsoincreases the traffic and awareness ofour website, which synergises withthe magazine itself.

As the Client Manager for AonCare Insurance, I regularly speak toprospective clients face to face orover the telephone. One of the mostimportant topics of conversationsurrounds risk management; I amoen asked what a care home can doto prevent claims or reduce the costof claims.

Slips, trips and back injuriesconstitute a high proportion of claimsmade under a typical care homeinsurance policy. e insurancecompany would typically settle theseclaims either under the EmployersLiability or Public Liability section.One of the most efficient ways inwhich a care home can minimiseliability claims is to ensure staff areadequately trained and their trainingand development is recorded in a

durable format, either electronicallyor in a filing system.

Another way to minimise the cost ofclaims in general is to ensure remedialaction is taken as soon as possible. Youwill read examples below of what you,as a care home owner, can do tominimise the risk you are exposed towhen managing your business.

Example 1A prospective client I met in Surreyhad two claims for unfair dismissal.e correct risk prevention measuresfor this client would be to contact theinsurer’s legal advice helpline beforetaking any action against anemployee to eliminate the potentialfor a claim. Prior to taking any actionagainst an employee it is alwaysprudent to speak with the legaladvice helpline, firstly to ensurelegislation is adhered to and secondlyto make sure a care home is not thesubject of an unfair dismissal claim.

Example 2A care home owner I met in CentralLondon was concerned about severalclaims he had made with his previousinsurer for injuries to employees. To ensure this type of risk wascorrectly managed the correctiveaction would involve having up todate training logs for each employeeand maintaining these on a regularbasis. If a claim was then to ariseunder the Employers Liability section of a policy, a care home canprove its employees are adequatelytrained in the duties they areexpected to perform.

Training logs may not necessarilyavoid a claim being made altogetherbut they could limit the settlementamount.

Example 3I was in conversation with an existingclient at a care forum earlier thismonth and I was asked the question,‘Do Insurance companies take intoaccount CQC reports whengenerating premium?’

e simplest answer is; aninsurance underwriter will consultCQC reports to ascertain whether ornot a care home is considered a ‘goodrisk’. Care homes with high qualityCQC reports will have a higherchance of receiving favourableinsurance premiums.

If measures such as the above aretaken, insurers are more likely towrite a risk at a competitive price.

Spare a Thought

Her life has been very active, havingenjoyed crewing yachts in her sparetime as well as entertaining forfriends. In her latter years she wasrenowned for lavish dinner partiesand putting on fundraising eventsfor her favourite charity.

Her working life was in Londonwhere, whatever she did, she wasrelied upon for being a goodorganiser, confident and kind.

During a fundraising event, shetook a step to the le aer introducingthe ‘guest of honour’ and fell off thestage. Several bones were broken andduring a spell in the rehabilitationunit, she was told by variousprofessionals that it was unlikely shewould ever return to being hersprightly self. It was suggested shemove into a care home while they triedto put in a care package, includingadaptations for her to return home.

Dorothy had found the experienceof hospital and all that had happenedaerwards as shocking. From anindependent, active, formidablewoman, she had become dependent,inactive and trapped.

At first, she tried to assert herself.Letting people know what she wanted.She tried to make suggestions of howto improve the chef ’s cooking and toldthe carers and kitchen staff the correctway of serving and removing crockeryfrom the tables. She bought her ownbone china, trays, doilies and suchlike, hoping to receive her meals in anattractive manner.

Within time, she resented the waythe carers brushed her hair, her teethand how her stockings were put on –or not on occasions.

At first, she had lots of visitors, allrallying around to cheer her up; theybought her flowers, fruit andchocolates and spent many hoursreminiscing with her. Most of thetales she played a dominant role inand she loved the attention. Withinweeks, these visits waned and soonthe regular visits were from herchildren and grandchildren, whoremained upset to see her in that way.

She had been promised a returnhome but she still had difficulty with hermobility. As each room in her househad stairs leading into them it wastaking time to set up. ey were waitingfor ramps and other mobility aids.

Dorothy’s assertiveness reduced toaggressive snipes and things did notimprove with the catering at thehome. She spent hours sitting in herroom ruminating about the thingsthat were done wrong there. To an

outsider, or perhaps a carer, theyseemed miniscule but to her they hadbecome her main focus.

Staff who used to spend a littleextra time with her were only enteringher room when they needed to. Shebecame more and more isolated.

So what went wrong?I wonder, if she’d been a permanent

Care Home resident would thingshave been different for her?

Would the staff have made anextra effort to get to know her storyor not?

Would she have had more of herown possessions around her to helpher settle in?

Would there have been more effortto include her in any home activitiesor would she have been introduced toother residents?

Do we get to really know who ourresidents/patients/clients are? Whatare their values, beliefs, their proudestmoments, what will they beremembered for? What makes themsmile and what inspires them? If not,what are we protecting ourselves orthem from? Is our world becoming soregulated with CQC/Safeguarding/Health and Safety and SupportPlanning that we lose the person andwho they really are?

Do we create the “difficultbehaviour” by our actions?

Next time you see aclient/resident/patient, spare a thought.How could you be instrumental inmaking their day more pleasurable?

Hygiene – 10/10Wow, the place was clean. ebathrooms sparkled and the dustnever stood a chance.

Décor – 6/10Ok the Décor. Hmmm, well to me itlooked a little old fashioned and ratherin need of a visit from LaurenceLlewelyn-Bowen. However CareHomes are for the elderly and the placedid look like my Grandparents housealbeit a super clean version. I guess thisgives the residents the homely feel thatmany of them are used too. Anyway Ididn’t like it but that doesn’t mean myGrandma wouldn’t!

Client Interaction – 9/10I was very impressed with the waythe Care Home staff encouragedresidents to take part in activities

with each other within the CareHome. During my visit I witnessed aquiz in session in the communallounge and it was lovely to see the old folk laughing and sharing a jokewith each other over the answersthey had given.

e staff were all very courteousand respectful to the residents butalso clearly had a personalrelationship with each of them thatcouldn’t be artificial.

Overall Experience – 44 /50 (88%)

It’s the thumbs up for e MaplesCare Home. I would definitely sendmy Grandma here. Nice friendly andcaring staff and a clean and tidy placewith lots going on.

In my line of work I see it everyday, clients not wanting to waste theirvaluable time waiting on the NHSwaiting lists, so they go out andpurchase either a cheap wheelchair orsomething extortionately priced.Something they then oen regrethaving spent their money on becauseit’s not suitable for their needs. eythen inevitably end up back on thewaiting list for months beforesomething suitable is given to them.

Buying a piece of equipment tomeet mobility needs is not easy, thereare a lot of questions that need to beasked before you purchase. Don’t justhead down to your local mobilityshop, research their reputation beforeyou go, my advice to you, look for ashop that will assess your needs andbe honest, the more honest you arethe greater the chance of satisfyingyour needs.

Ask the four W’s and H, What,Where, When, Why and How? eseare the simple questions that need tobe answered to ensure the correctequipment is purchased. Make sureyou receive a full assessment. eyneed to measure your width andlength whilst seated. Let the assessorsee you seated naturally, don’t situpright if you don’t normally sit like

that, there is equipment that can beput in place to help support yourposture. If you have severe posturalneeds you need a postural assessment,these can be done privately by variouscompanies or by your localWheelchair Service, most mobilityshops won’t be able to advise correctlyon these issues. You are not being apest by asking these questions, wherewheelchairs are involved there are alot of ways to adapt the wheelchair tosuit any client, of course that doesmean spending more money or wouldyou rather have a wheelchair that isuncomfortable. Try the wheelchairbefore you buy.

So many of my clients want awheelchair to push their partnersaround, the common problem I hearis the wheelchair is too heavy topush, although there are light weightwheelchairs, more oen than not, it’snot the chair that’s too heavy. It’s thecombined weight of the client andthe chair. For example the lightestweight chair that I know of weighs12.7kg, add an occupant who weighs67kgs the person pushing now has topush 79.7kg. at’s not easy for a 60something year old frail lady to push,there is one suggestion I can makeand that’s buy a Self propelled chair.e larger wheels give a greatercircumference creating more frictionfor the propellant and it also allowsan easier ability to turn. It’s not goingto solve all of your problems howeverit may make it easier.

Also don’t forget about a cushion!When I’m talking cushions I’m nottalking about one that can be puffedup and put back on the couch. esecushions are built for a reason; theyare built to stop pressure sores frombeing created, or to stop the canvasesfrom sagging. ere are a hugevariety of cushions that could besuitable, anything from a standardvinyl to a top of the range airflowcushion like Starlock and Cloudcushions. Cushions are not cheapand also are not an item that youwant to buy more than once if youget it wrong. ink about the lengthof time you will be sat in thewheelchair (if you are a full time user,once a day or even occasional) youwill need some form of cushion inplace. Consider your past andcurrent medical history, have youhad a pressure sore or do you haveone, if so you need to opt for a higherrelieving cushion, if not think aboutcomfort and what you prefer to siton. Again try before you buy, a lot ofcompanies have reps that can comedown and help you out, and you arenot obliged to buy the wheelchairand the cushion from the same store.

ese common rules don’t justapply for wheelchairs, ask questionsand make sure you know what youwant from what you are buying.Remember be realistic, with therecession the way it is, not everyonecan afford the Stephen Hawkins ofwheelchairs.

If you Google the word wheelchairyou will be provided with about26,600,000 results in a quarter of asecond. Admittedly, by page 28 thelistings relate to things like LadyGaga’s bejewelled 24 carat goldeffort, but the fact remains thatthere is a dizzying array of choice atthe fingertips of the majority ofpeople. In the time it takes to enjoya cup of tea you could have lookedat pictures and descriptions ofdozens of different types ofwheelchairs, or powerchairs, ormobility scooters. e amount ofchoice this can afford is amazing.

However, there is a drawback. Atthe risk of sounding like a Luddite,and while the internet can afford anabundance of different options, itcannot advise on which is mostsuited to the person viewing them.e majority of manufacturers dohave fantastically useful andinformative websites, but they cannotreplace the benefits of one-to-oneadvice. Of course, sometimes peopledo not feel the need to be advised.e American lecturer andhumourist Josh Billings said “Adviceis like castor oil – easy to give butdreadful to take.” at having beensaid, in an industry where the terms‘customer’ and ‘patient’ can oen beinterchangeable, getting the rightproduct is incredibly important.

We recently had a lady who cameinto one of our MyHealth Centreswho had purchased a mobility

scooter online. It had seemed like areal bargain at a couple of hundredpounds cheaper than we would havesold it. However, when it arrived itwas in a box, meaning that thecustomer had to wait for several daysuntil her son-in-law could comeround to put it together for her. Onceit was complete, the weatherprevented her from going out on it.When, eventually, she did get theopportunity, she found that her stiffknee – the reason for her needing thescooter in the first place – meant thatthere was insufficient legroom for herto sit on the machine comfortably.She finally got through to thecustomer services of the companywho had sold the scooter only to findout that she was outside of thetimescale for returning the item.

It may seem old-fashioned but Iam a firm advocate of people tryingbefore they buy. In this lady’s case theproblem with her knee would havebeen immediately apparent and wewould have been able to recommenda different type of scooter whichmight accommodate a straight leg,for example. We would also suggest ahome visit to ensure that the scooterwould fit in her shed, or house, orwherever she intended to keep it. Shewould also have advice onmaintenance and battery charging aswell as the knowledge that she couldspeak to someone should she everencounter a problem.

When that scenario is translated toeven more specialist equipment, suchas wheelchairs or pressure-relievingitems, the need for proper advice is

All of which is not to say thatonline shopping is inherently bad.We, for example, have an extensivewebsite which sells everything fromprofiling beds to cutlery, mobility

scooters to incontinence products.Indeed, the ability to shop fromhome is a huge benefit to manypeople who struggle to get out. Myargument, though, would be that agood website should be backed upwith the option of being able to askquestions about the products on offerand seek advice as to whether theyare the most suitable for the personin question. Sometimes people donot always need the potentiallyexpensive item they think they need:they may not require a £300bathlier when a £10 grab rail maybe enough! Similarly, some itemshave a much larger price ticket assoon as they are described as ‘dailyliving aids.’ One-touch electric tinopeners, for example, are readilyavailable on the High Street for far

less than their counterparts on somewebsites geared toward disabled andelderly people.

e Worldwide Web has made theworld a much smaller and more easilyaccessible place. It is arguably thegreatest invention of all time. But thehuman touch is invaluable. Personaland home assessments are, in manycases, not only desirable but necessary.Individual advice on the comparativebenefit of one product over anothermeans the customer can be certain thatthey make the right choice.Furthermore a real, physical showroomstaffed by real, accessible people givesthe peace of mind that they can becontacted aer any purchase has beenmade. Whether they want helpmanaging a physical problem likepain, breathlessness, tiredness or lossof appetite or support in dealing withtheir fears, family or financial worries,loss of purpose of the social isolationthat a serious illness can cause thewellbeing team are available to help.Each patient has a plannedprogramme tailored to address theirunique requirements. At St Wilfrid’sthey work closely with each individualto help them to address the symptomsand worries affecting them.

Weight control can oen be aproblem for people with spina bifida,and can start from childhood. Parentsoen overestimate the amount ofcalories children need to grow, andchildren with less muscle, and whoare less active, will need fewer caloriesthan active, mobile children.

can really help their children byproviding a diet full of goodness,which leaves out sugar and fatty foodslike sausage rolls where possible.
