Join Us for Our Annual OctoberfestIt’s that time of year again so mark your calendars! Experience the annual Octo-berfest craft show at Active Aging, Inc. located at 1034 Park Avenue in Mead-ville, PA on Friday, September 29th from 9am to 5 pm and Saturday, Sep-tember 30th from 9am to 3pm. Come and enjoy live demonstrations, beer and wine tastings, wonderful art displays, an all-new lunch menu and see everything from woodcrafts, metal-working, handmade jewelry and cloth-ing along with much more from local vendors. Your Crawford County Se-niors will also be displaying their hard work with beautiful tiles and hand-made quilts. Lunch will be held each day from 10am to 2pm with a take-out option. There will be so many choices that you won’t know what to choose! This year we will be serving sausage with peppers and onions, chili, broccoli cheddar soup and everyone’s favorite, Smith’s hot-dogs with all the toppings. We will also have our cabbage rolls again this year. New and exciting on our menu will be hot baked potatoes with all the trimmings. Fresh fruit will be available and crisp garden salads will be made fresh on site not to mention our Ba-varian pretzels with hot cheese dip. Of course we couldn’t forget those with a sweet tooth so expect fresh homemade pies, turnovers, apple dumplings and many other baked goods. If your mouth is watering already, make sure to stop by and experience the fun and festive atmosphere!We hope to see you participate in this fun! MEADVILLE LIONS CLUB WILL BE ACCEPTING
APPLICATIONS FOR GLASSESThe Meadville Lions Club will be accepting applications from Meadville area individuals of any age that are in need of glasses but are unable to pay for them. Individuals need to complete an application (available at the front desk of Active Aging) and come to be inter-viewed on the second Tuesday of each month from 5:30 – 6:30 p.m. at the Lew Davies Com-munity Building, 1034 Park Avenue, Mead-ville. People who live in outlying areas from Meadville can be referred to a Lions Club in their area. Questions – call 382-1905. You do not need an appointment.
September means Labor Day. Labor Day means the end of summer! Good grief that time went quickly. It seems like I say that every month. Maybe I do, but I always feel that way. Last month I promised stories from our family vacation, and boy am I going to deliver. However, I’m going to use all my space this time to tell this story. Please check out the rest of the Echo for exciting news on all the other things Active Aging has going on. We went to Glacier National Park in Montana in the middle of July. Most of our family vacations involve hiking in our National Parks, it is one of our favorite things to do, and Glacier is my favorite one. This was my third trip there, but the first time for Luke and Tucker, so I was excited to show them some of my favorite places. The first one of those was Iceberg Lake. It is a 5 mile hike into a beautiful lake that literally has icebergs in it year round. I think it is one of the most amazing sights I’ve ever seen. We hiked that trail on our very first day. We were probably about half way to the lake, walking single file because the trails are relatively narrow, when from behind me I hear Tucker say “hey, guys”. I turn around to see him looking into the bushes on the side of the trail, so I look there too. All of a sudden I see a huge brown head come up out of the bushes and hear a huffing sound. It didn’t take me any time at all to figure out we were standing within 15 feet of a grizzly bear. Tucker was reaching for his bear spray and I started saying “bear, bear, bear” over and over so the people behind us would know. Luke and I immediately moved toward Tucker until we were all in one huddle, bear spray in hand, backing calmly away from the bear. She never made a move toward us, or away from us. As we continued along the trail, we let fellow hikers know that we saw a bear, so they could be prepared. On our way back down the trail we stopped to talk to one of the park rangers who told us that the bear we saw was a momma grizzly with 3 cubs. I’m so glad I didn’t know that at the time! We were definitely closer to that bear than any of us would have ever gone willingly, and certainly close enough that if she meant to do us harm, there wouldn’t be much we could do about it. Thankfully, that wasn’t the case. Another ranger informed us further down the trail that another male grizzly was hanging out close to the trail and we should keep an eye out. We came around a bend in the trail to find a ranger holding up the trail traffic because that bear was only a few feet from the trail, casually eating huckleberries without any concern or interest in the people nearby. We ended up waiting about 30 minutes for him to decide to move further away before we were escorted by a ranger out of harm’s way. The park service shut that trail down for a few days to allow the bears to move from the area. It was quite an experience, one I don’t hope to repeat! How’s that for an eventful family vacation? Active Aging, Inc. Services Available For Age 60 And OlderSeniors in Crawford County may be eligible for a variety of in-home services that are pro-vided through Active Aging, Inc. Most of the services below can be received on a no or low cost share basis or through the Medicaid funded PDA Waiver Program. The services listed below may be obtained through a re-ferral made to our office. Intake and Referral is a free service provid-ed to anyone who calls into the Active Ag-ing, Inc. with questions. Specially trained employees can answer questions regarding in-home services, community services, in-surance and benefits. They may also take referrals for anyone interested in in-home services. Care Management/Service Coordination- Specially trained care managers and service coordinators are available to assess each con-sumer to determine their level of care and needs in the community. Care Managers/Service Coordinators (CM/SC) develop care plans with the individuals to ensure that their needs are being met. Once services are in place, the Care Manager or Service Co-ordinator will work to ensure that the con-sumer is getting all appropriate services to manage their needs. CM/SC are available to consumers during the week via phone and home visits as needed. CM/SC can answer questions regarding services and other com-munity services consumers may be eligible for.Personal Emergency Response Systems (PERS) provide safety and security for those who may live alone or are alone long periods throughout the day. Emergency buttons can be worn around the neck or on the wrist and
can be through a land line and even through a cellular network. If consumers have fall-en, are having a medical emergency, or feel threatened, they can push the button and have emergency contacts or medical person-nel respond 24/7.Personal Assistance and Personal Care with home support provide consumers with needed assistance for bathing, grooming, dressing, transferring, ambulation and meal preparation. Along with this assistance, con-sumers may also receive light housekeeping and home support for cleaning, laundry, errands, transportation to medical appoint-ments and in some cases, medication man-agement and reminding.Home Delivered Meals can provide assis-tance with prepared meals for those who are unable to cook on their own, or for those who do not have anyone that can assist them. Meals are frozen and are delivered on a weekly basis to the consumer’s home. Meals include an entrée with sides, bread, milk and fresh fruit. The Family Caregiver Support Program (FCSP) provides reimbursement to unpaid caregivers who are caring for an older rel-ative or friend. Caregivers may be reim-bursed for a variety of services and supplies, including respite care, incontinent supplies, home modifications, home adaptations or other medical supplies. The FCSP also pro-vides education to caregivers regarding care and managing dementia and Alzheimer’s disease. Reimbursement for this program is cost shared based on yearly total household income.The Nursing Home Transition program
(NHT) assists seniors residing in nursing facilities with the ability to return to the community with in-home services when ap-propriate. The NHT coordinator will make referrals for in-home services, set up housing if necessary and assist the senior with over-coming any barriers that are preventing them from returning to a home in the community. The assessment unit provides another ser-vice to seniors and individual with disabil-ities in the community. For those entering a nursing facility or wishing to receive un-der 60 waiver services, the assessment unit will schedule an assessment to determine the consumer’s level of care. Information regarding the assessment will be provided to the facility, enrollment agency, consumer and the department of human services to de-termine eligibility.If you or a family member would like to be assessed for any of the services listed, please call our office at (814)336-1792 or 1-800-321-7705. Please ask for Intake and Referral. Once a referral is made, care managers or service coordinators will set up an appoint-ment with the older adult to conduct an as-sessment.

Ahoy ye’ mateys! I hope all of you had a chance to stop by and say hello at our booth at the Crawford County Fair. September is one of my favorite months of the year for many reasons, not the least of which is the weather. Who doesn’t like warm days and cool nights?There is a lot going on in the Foundation offices these days. Coming up on Saturday, September 23rd, the Active Aging Foundation will be holding its fourth annual Big Cash Treasure hunt. There will be $1,600.00 in cold, hard cash treasure to be won! $1,000.00 for the 1st place team in treasure points then $300.00, $200.00 and $100.00 for teams in 2nd, 3rd and 4th place in treasure points, respectively.Think of this as Escape Room meets Geocaching meets PokemonGo, with the chance to win big money at the end! Be sure to start making your teams now, as this should be yet another fantastic, fun event for everybody. Here’s a quick tip: for bonus treasure points, make sure your teams are all dressed in a team theme attire. We’ve just launched our first GoFundMe campaign to meet an im-portant need here in Crawford County. With the advent of new state regulations on the meals provided to senior citizens through Active Aging’s Home-Delivered Meals program, seniors now receive meals with reduced calorie counts. We know that many seniors will share their meals with their pets for a variety of reasons, including their lack of funds to buy an adequate amount of pet food each month. By supporting these seniors with a supplemental supply of pet food to help them maintain their pet’s nutritional needs, we have found that the senior’s nutritional needs are also being met because they are now eating all of their Home Delivered Meals without feeling the need to share their meals with their pet. Started in 1984, the Pharmaceutical Assis-tance Contract for the Elderly (PACE) Pro-gram helps older persons living in Penn-sylvania to pay their prescription expenses. PACENET is the Pharmaceutical Assistance Contract for the Elderly Needs Enhance-ment Tier. This program has been created to assist older Pennsylvanians, whose incomes are slightly higher than the PACE income limits, with the cost of their prescription drugs.Eligibility Information:PACE requirements:• 65 years of age or older.• A resident of Pennsylvania for at least 90 days prior to the date of application.• Ineligible for pharmaceutical benefits un-der medical assistance.• Total income for last year for a single per-son must be less than $14,500.• Total income for last year for a married couple must be less than $17,700.Once an application is approved, an enrollee is responsible for $6 copayment for generic drugs or a $9 copayment for brand name drugs for each prescription reimbursed by PACE.PACENET requirements:• 65 years of age or older.• A resident of Pennsylvania for at least 90 days prior to the date of application.• Ineligible for pharmaceutical benefits un-der medical assistance.• Total income for a single person from last year must be between $14,501 to $23,500.• Total income for a married couple last year must be between $17,701 to $31,500.PACENET may have a monthly drug plan premium to pay at the pharmacy, which will be cumulative if not met each month. Once that monthly drug premium has been met, PACENET has an $8 copayment for generic drugs and a $15 copayment for brand name drugs. Your out-of-pocket costs to you are computed by the PACE program.To use your PACE/PACENET card: Take your card and prescription to any drugstore that accepts PACE. Always keep your card with you. Do not leave your card at the drugstore. Do not mail your card to a mail-order pharmacy.You may not use your card to purchase drugs out of the state or to mail drugs out of the state. You can use your card ONLY while you are in Pennsylvania.Can I have other medical insurance or pre-scription coverage and still apply for PACE/PACENET?Yes, However, PACE is the payor of last re-sort and will only cover prescription costs not covered by other medical insurance. You will be asked to provide information about your insurance coverage when you apply for PACE/PACENET, and you should in-form your pharmacist of all your medical and prescription plans.Is it possible to get help with completing the PACE/PACENET application?Yes! Medicare Prescription Drug
Plans Begins October 15th and
Ends December 7thOpen enrollment for your Medicare pre-scription drug coverage begins October 15th and ends December 7th for coverage effective January 1, 2018. After December 7th you will not be able to choose a different plan for 2018 unless you have a reason that allows for a special enrollment period. It is very important that you have your cur-rent plan checked during the open enroll-ment period even though you may be hap-py with the current coverage of your plan. Plans will change the formulary lists and you may not be aware that one, or more, of your current medications will not be cov-ered by your plan in 2018. Premiums, co-pays and deductibles also may change on your current plan for next year. The APPRISE Program at Active Aging, Inc. in Meadville can assist Medi-care beneficiaries of any age understand any changes your current prescription drug plan may be making for 2018 and to help you choose and enroll into a plan for 2018. APPRISE is the free State Health Insurance Counseling Program for all Medicare bene-ficiaries. APPRISE Counselors are trained and certified by the PA Department of Ag-ing. What is the PA Link?Aging and Disability Resource Centers (ADRC) exist nationwide. In Pennsylvania, ADRC is known as the Pennsylvania Link to Aging and Disability Resources, commonly known as the PA Link. It is designed to be a cross-age, cross-system network of local, regional, and state partner agencies that collaborate to help individuals who need home and commu-nity based services and supports. This coordinated network of partners allows a consumer to initiate contact with any PA Link partner and ac-cess information, assistance, and resources through a “no wrong door” ap-
proach, thereby minimizing consumer and provider duplication, reducing confusion, streamlining access, and creating much needed “safety nets”.
