Electronic Authentication Steps for Accessing EvaluationWeb

Before EvaluationWeb access is granted, all persons requesting access must be e-authenticated (identity proofed) via the CDC’s Secure Access Management Services (SAMS) system. Once e-authentication is complete, Luther Consulting will provide instructions on accessing EvaluationWeb.

Below are the steps to become e-authenticated:

Step 1: Go to the EvaluationWeb login page at http://www.xpems.com and click on ‘Request CDC eAuthentication.’

Step 2: Complete the e-authentication request form, including project (HIV), jurisdiction (Minnesota), your legal first and last names, email, telephone number, and agency. Click ‘Go’ to submit the online form to CDC.

Step 3: Complete the Add/Edit Account Form (PDF: 179KB/ 2 pages) or (Word: 64KB/2 pages) to set up an EvaluationWeb account and the correct permissions for a new user. The form is also used to change permissions for an existing user according to changes in job responsibilities.

Step 4: The User Agreement Form (PDF: 152KB/1 page) must be signed and submitted to MDH along with the Add/Edit Account Form when a new user account is being established.

Step 5: Review the Rules of Behavior (PDF: 240KB/15 pages) and indicate acknowledgement by signing the User Agreement Form.

Step 7: You will receive your first invitation email from “Sams‐No‐Reply (CDC)” to register with the SAMS Partner Portal (see example in Attachment 1). SAMS is CDC’s secure website where public health partners can access sensitive information and applications, such as EvaluationWeb, that are not available to the public. The email will also have your SAMS user ID and a temporary password.

Step 8: Register with SAMS by logging into SAMS using the website link, user ID, and temporary password included in your first invitation email and completing the registration form. The password you set during the registration process will be your password for accessing SAMS in the future. You must register with SAMS within 30 days of receiving your invitation email. If you wait longer than 30 days, you will need to repeat Steps 1 and 2 again.

Be sure to enter your name and address as they appear on your government‐issued ID, unless you have legally changed your name or moved since the ID was issued.

If you legally changed your name since your government ID was issued, you will also need to provide supporting documentation, such as a legal name change certification or marriage certificate. If you have moved since your government ID was issued, you will also need to provide a current utilities bill. Step 9: After registration you will receive a second email from “Sams‐No‐Reply (CDC)” (see example in Attachment 2). This email contains the identity verification request form at the bottom.

Print this email and present it with a government‐issued identity document (and any supporting documentation, if needed) to a notary public or MDH designated proofing agent.

The notary or proofing agent will verify your identity document (and any supporting documentation, if needed) and complete their portion of the printed email. Specific instructions on verifying the identity document and completing the form are on the email form.

Acceptable identity documents include government‐issued photo ID cards that have an ID number (e.g., state‐issued driver’s license, U.S. military ID, or U.S. passport).

Step 10: Fax or mail the printed email form, along with a copy of the identity document(s), to:

Step 11: You will receive approval by a third email from SAMS (see example in Attachment 3). Ignore the instructions for logging into EvaluationWeb that are included in this email. You may also receive a letter through the U.S. Postal Service at the address you provided.

You will receive a fourth email from SAMS (see example in Attachment 3) saying that your account with the SAMS Partner Portal has been activated. Step 12:

Once approved by SAMS, you will receive two emails from Luther Consulting with your EvaluationWeb login instructions, including username and temporary password. You must log into EvaluationWeb within 24 hours of receiving this email or your temporary password will expire. If this occurs, contact Luther Consulting. Helpful Tips

Consistency Counts

The purpose of Identity Verification is to establish that you are who you claim to be. In order to complete this

process, the information you provide must be accurate and consistent. Here are some ways to make the process run

smoothly:

If the name on your ID does not match the name you used during registration (you got married; you had a

legal name change; etc.), you'll need to provide some supporting documentation with your request form ‐ for example: a copy of your marriage license or order of legal name change.

If the address on your ID does not match the address you used during registration (you moved), you'll need to

provide supporting documentation with your request form ‐ for example: a copy of your voter registration card or a utility bill in your name that shows the new address.

Make Legible Photocopies

In order to speed you through the Identity Verification process, please ensure that your documents copy and fax

cleanly. Specific things you can do include:

Select the ´Enlarge´ option when photocopying your IDs ‐ 50 or 100 percent enlargements help make your

documents more legible.

Ensure good contrast and clarity on your original photocopy before you fax.

Set your fax to a high dpi or ´fineness´ setting ‐ the bigger the better.
