This document will be reviewed at least annually by the Libraries Emergency Preparedness Committee and revised as needed.

** In an emergency situation, persons in the library are of first concern. ** In all cases when the University Police Department (UPD) is called, UPD will assume control of the situation and determine whether additional emergency services are needed. All University Libraries employees are encouraged to sign up for the Eagle Alert emergency notification system. For registration information, go to www.usm.edu/safety/eagle-alert. All University Libraries employees (including student employees) will view the “Shots Fired!” video available on the University Police Department’s website at www.usm.edu/police/training-video-active-shooter-situation. Active Shooter training required by Executive Order 1411: • Required by all employees including student workers • Access the training portal at https://www.mspb.ms.gov/active-shooter-situations.aspx.

• The preferred browser for accessing this content is Internet Explorer. • Click on Active Shooter Situations Webcast and complete the training. • Print the certificate of completion, fill in all necessary information, and sign the form. • Follow instructions provided by your division’s vice president or executive officer to submit

your signed certificate.

ASSAULT, RAPE, or HARRASSMENT of PATRON or EMPLOYEE

1. Call University Police (601-266-6911 or 601-266-4986) and explain the situation. 2. Notify circulation desk personnel (601-266-4250) so they can direct police officers to the

appropriate location. 3. A library employee will stay with the individual(s) and attempt to keep the person(s)

calm until University Police arrive. 4. Only 8 a.m.-5 p.m. Monday-Friday, notify Libraries Administrative Office (601-266-

4241). 5. If unable to notify the Libraries Administrative office, notify the Dean of Libraries,

Dr. John Eye (mobile: 601-325-4518). 6. Notify Title IX Office (601-266-6804).

Incident Report Form

1. Library employee responding to the incident will complete an incident report form (Appendix A) and submit to Circulation.

2. The original incident report will be kept on file by the Dean’s Office/Libraries Administrative Office.

3. The University Police Department maintains its own record of incidents and copies will be provided to University Libraries upon request.

BOMB THREAT Bomb threats may be received via telephone, written message, in-person, or by first-hand observation of a suspicious object. The following instructions assume a telephone threat, but may be adapted to respond to threats received by other means. If you receive a telephone bomb threat:

1) Obtain as much information as possible. 2) Keep the caller on the line as long as possible. 3) Don’t hang up the phone unless the caller has disconnected first.

Take notes while you are on the phone and try to write down everything the caller says. Ask the caller:

• When will the bomb explode? • Where is the bomb located? • What kind of bomb is it and what does it look like? • Why did you place the bomb?

Signal someone nearby to alert University Police. The panic button may be used if you are alone and a panic button is within your reach.

1. Call University Police (601-266-6911 or 601-266-4986) and report the exact words of the threat or give a detailed description and location of the suspicious object.

2. Record the following information while it is fresh in your mind: • Time of call / Time suspicious object was first observed • Estimated age and sex of caller / Description of suspicious object • Caller’s tone of voice, speech pattern, accent, etc. / Location of the suspicious

object • Background noise audible from the caller’s location / Who first reported sighting

the suspicious object • Caller’s familiarity with the library and/or the university

3. Notify circulation desk personnel (601-266-4250) so they can direct police officers to the appropriate location.

4. Only 8 a.m.-5 p.m. Monday-Friday, notify Libraries Administrative Office (601-266-4241).

5. If unable to notify the Libraries Administrative Office, notify the Dean of Libraries, Dr. John Eye (mobile: 601-325-4518).

Do not evacuate the building unless instructed to do so by University Police. (Scripts for public address announcements for emergency evacuation and closing the building are included in Appendix B.)

Bomb Threat & Incident Report Forms 1. Library employee receiving the bomb threat will complete an incident/bomb threat report

form (Appendix A) and submit to Circulation. 2. The original incident/bomb threat report will be kept on file by the Dean’s

ELECTRICAL FAILURE Each department will maintain one or more flashlights and check the batteries monthly. A list of flashlight locations is included in Appendix H. Reporting Procedures Monday-Friday

8 a.m.-5 p.m. Libraries Administrative Office will contact Physical Plant (601-266-4144). Noon-1 p.m. (i.e., if Libraries Administrative Office is closed) Circulation desk personnel will contact Physical Plant (601-266-4144) and then notify the Libraries Administrative Office (601-266-4241).

At any time other than 8 a.m.-5 p.m. Monday-Friday or during University holidays

Circulation desk personnel will phone: 1. University Police (601-266-6911 or 601-266-4986), who will call the Physical Plant

emergency contact 2. Access Services Librarian, Sarah Mangrum (mobile: 601-810-5001) 3. Dean of Libraries, Dr. John Eye (mobile: 601-325-4518) 4. Head of Library Technology, Lisa Jones (mobile: 601-467-8054). If unavailable, use

the separate “Call List for Emergencies” in Appendix G to phone other library technology staff in case servers or other computer equipment requires attention.

5. iTech Coordinator, Brian Soule (mobile: 601-329-5967) Decision to Evacuate the Library If it is dark outside and natural light is not available inside the building:

Evacuate immediately following the procedures outlined in Appendix D. If it is daylight outside:

• Wait 15 minutes before evacuating the building. If power is not restored after 15 minutes, evacuate all patrons from the building following the steps outlined in Appendix D.

• Library employees will gather in the lobby to await further instruction. • Circulation personnel will:

1. Assemble a flashlight, master key (located in the safe in room 101B), walkie-talkie, and bullhorn.

2. Go up the northeast stairs and make announcements on the 3rd, 4th, and 5th floors following the script attached to the bullhorn. The announcement will explain:

• the power is out and elevators are not functional • building is being evacuated • anyone requiring assistance to exit should come to the stairwell and, if

necessary, use the red emergency phone to summon University Police

• Research Services personnel will: 1. Make an announcement in the 1st floor Learning Commons explaining that the

power is out and the building is being evacuated. 2. Get a flashlight and bullhorn, go up the 2nd floor, and make announcements in the

following locations: a. 2nd Floor South (room 209 & 242) b. research carrels corridor (room 211) c. stacks (room 201) d. de Grummond exhibit room (room 203)

The announcement will follow the script attached to the bullhorn and explain: • the power is out and the elevators are not functional • the building is being evacuated • anyone requiring assistance to exit should go to the northeast or northwest

stairwell and, if necessary, use the red emergency phone to summon University Police

(Scripts for bullhorn announcements for emergency evacuation and closing the building are included in Appendix C.) Decision to Close the Library A decision to close Cook Library and release employees will be made by the Dean, in consultation with Physical Plant and University Police, and dependent upon 1) anticipated length of the outage and 2) environmental conditions, such as, excessive heat or cold. Safety of persons in the building must be taken into consideration. If the Dean is unavailable, a decision will be made by the Access Services Librarian. If a power outage occurs after 10 p.m. and lasts longer than 30 minutes, the library will be automatically closed for the remainder of the day. A post-10 p.m. closure will occur according to these procedures and does not require further authorization from the Dean. The Dean should be notified of the power failure and library closure.

FIRE If you see a fire, activate the nearest fire alarm. When the full building alarm is engaged, University Police are automatically notified and will determine whether additional emergency services are needed. Use your best judgment whether to attempt to use a fire extinguisher for a small fire. If a minor fire appears controllable, operate the fire extinguisher using the PASS method:

1. Pull the pin from the handle. 2. Aim the nozzle at the base of the fire. 3. Squeeze the handle. 4. Sweep back and forth until the fire is extinguished.

If a fire alarm sounds, all library patrons and employees are to evacuate the building through the nearest exit. Do not use the elevators. Close doors to confine the fire and limit oxygen, but do not lock doors. Circulation employee in charge will make an announcement on the public address system to evacuate the building and request anyone needing assistance to go to the northeast or northwest stairwell and then use the emergency phone to contact University Police. (For a script, see Appendix B.) Circulation personnel will be stationed at the north entrance to prevent patrons from entering the ramps and stairway to the library or re-entering the building. Technical Services personnel will be stationed at south entrance to prevent anyone from entering the building. All employees, except those in Technical Services, will gather in the Powerhouse courtyard for a head count. Technical Services personnel should gather in the courtyard at the south entrance. Each department or unit is responsible for the head count of its employees. Head count numbers will be reported, in person, to the circulation employee in charge. The circulation employee in charge will be stationed near the main (north) entrance of the building. Using the head count checklist in Appendix F, the circulation employee in charge will notify University Police if any employees working in the Cook Library building are unaccounted for. When the building has been released by University Police, the circulation employee in charge will first call for employees (library and auxiliary units) to enter the building, and then call for patrons to enter. A circulation employee will be posted at the security gates in case the security alarm is engaged. The circulation employee in charge will send someone to the south courtyard to notify Technical Services personnel that it is safe to re-enter the building.

Custodial Services “Hurl Kits” (containing water-absorbing beads and disinfectant cleaner) are available at the Circulation Desk (LIB 101) and the Research Assistance Desk (LIB 109), if library personnel choose to assist in the cleanup. Custodians’ normal working hours are 4:00 a.m.-12:30 p.m. Request custodial services using the following protocols. Monday-Friday

8 a.m.-5 p.m. Library employee will notify the Libraries Administrative Office (601-266-4241), who will contact 1) Custodial Services (601-266-4437) or 2) Physical Plant (601-266-4414). Noon-1 p.m. (i.e., if Lib. Admin. Office is closed) Library employee will notify circulation desk personnel (601-266-4250) and circulation personnel will contact Physical Plant (601-266-4414) and then notify the Libraries Administrative Office (601-266-4241).

At any time other than 8 a.m.-5 p.m. Monday-Friday or during University holidays Library employee will notify circulation desk personnel (601-266-4250) and circulation personnel will contact University Police (601-266-4986). University Police will contact Physical Plant.

SEVERE WEATHER The campus emergency warning siren is located on top of Owings-McQuagge Hall. When the emergency warning siren is activated, or as instructed by University Police or Libraries administration, circulation personnel will:

• Make an announcement on the public address system instructing library occupants to take shelter on the first floor, away from windows and glass doors. (For a script, see Appendix B.)

• Lock the elevators to restrict access to 3rd, 4th, and 5th floors. • Lock the northeast and northwest stairwells to restrict access to 3rd, 4th, and 5th floors. • Place stanchions and rope across the bottom of the lobby stairway to deter persons from

going to the upper floors. When the threat of severe weather has ended, campus alarm will sound an all clear. Circulation personnel will:

• Make an announcement on the public address system informing library occupants that the threat of severe weather has ended and they may return to the upper floors of the library. (For a script, see Appendix B.)

• Unlock the elevators to restore access to 3rd, 4th, and 5th floors. • Unlock the northeast and northwest stairwells to restore access to 3rd, 4th, and 5th floors. • Remove stanchions and rope from the bottom of the lobby.

Hurricane Preparation Library personnel will follow the university’s instructions if a hurricane is approaching. Detailed procedures are outside the scope of this document. General preparations during a hurricane warning include: back up computer files, protect and secure all confidential files, protect electronic equipment from water damage by covering with plastic, and close window blinds.

THREATENING or DISRUPTIVE BEHAVIOR This may include an active shooter, civil disorder, hostage situation, threat of violence, or actual violence against a library patron or employee.

1. Call University Police (601-266-6911 or 601-266-4986) and explain the situation. 2. Notify the circulation desk personnel (601-266-4250) so they can direct police officers to

the appropriate location. 3. Only 8 a.m.-5 p.m. Monday-Friday, notify Libraries Administrative Office (601-266-

4241). 4. If unable to notify the Libraries Administrative office, notify the Dean of Libraries, Dr.

John Eye (mobile: 601-325-4518). Library employees should remain calm, show concern, and try to keep the affected individual(s) as calm as possible. In the event of an active shooter, university police officers can be expected on-site within 90 seconds. Active shooter

1. GET OUT - Run away, if possible. Library employees may direct patrons to the nearest or safest exit, but are not expected to put their personal safety at greater risk. Statements such as, “Run!” or “Follow me!” or “This way!” may be appropriate.

2. HIDE OUT - Hide if you are unable to exit safely. Library employees may assist patrons in locating a safe hiding place, but are not expected to put their personal safety at greater risk.

3. SPREAD OUT - If you remain in place, spread out, i.e., don’t cluster with others. 4. TAKE OUT - As a last resort, cooperate with others to gang up on the shooter. Throw

things at the shooter to distract him/her. In preparation for a possible active shooter situation, all University Libraries employees will view the “Shots Fired!” video available on the University Police Department’s website at www.usm.edu/police/training-video-active-shooter-situation. Employees will also watch the active shooter video required by Executive Order 1411, which is available at: https://www.mspb.ms.gov/active-shooter-situations.aspx. Information regarding completion of this training is located at the front of this manual. Hostage – cooperate with the hostage taker and wait for authorities to resolve the situation Panic Buttons Several panic buttons are installed in Cook Library.

• Research Assistance Desk (LIB 109) in the Learning Commons - located under the north (front) center section of the desk, near the intercom handset (Aiphone TD-6H)

• Circulation Desk (LIB 101) – located on the island underneath the computer • Library Administrative Office (LIB 202) – located under the northwest corner of the desk

(near the phone) in the reception area

When pressed, the University Police dispatcher will see where the call is coming from. UPD will immediately dispatch an officer and will also call the phone located nearest the panic button. When the return call comes in, please answer the phone if you are able. The panic button will be used only in instances where you are unable to report the problem using normal telephone procedures. Note: If some or all of the library building becomes a crime scene, consider that access may be restricted for a period of weeks or months.

Appendix A BOMB THREAT REPORT FORM

Cook Library If you receive a telephone bomb threat, try to obtain as much information as possible. Keep the caller on the phone as long as possible. Appendix B: Public Address Announcements

The public address system is to be used for emergency evacuation, hazardous weather, closing the building, and paging patrons for University police officers. Emergency Evacuation: “Please evacuate the building. Do NOT use elevators. Anyone needing assistance, proceed to the northeast or northwest stairwell and telephone University Police.” (Repeat the announcement.) Hazardous Weather, except tornado – use when the campus alarm sounds or instructed by University Police or Libraries administration: “May I have your attention? A hazardous weather warning has been issued for the Hattiesburg area. Please take shelter on the first floor away from windows and glass doors.” (Repeat the announcement.) Tornado – use when the campus alarm sounds or instructed by University Police or Libraries administration: “May I have your attention? A tornado warning has been issued for the Hattiesburg area. Please take shelter on the first floor away from windows and glass doors.” (Repeat the announcement.) Closing the building – use the recorded announcements or, when necessary, make the announcement live.

30 minutes: “May I have your attention please? Cook Library will be closing in 30 minutes.”

15 minutes: “May I have your attention please? Cook Library will be closing in 15 minutes. If you have materials to be checked out, please make your way to the Circulation Desk now.”

5 minutes: “May I have your attention please? Cook Library will be closing in 5 minutes. The lights will be turned off. Please make your way to the exit immediately”

Paging patrons - To locate persons in an emergency situation, patrons must contact the University Police Department (UPD). UPD must then come to the library, request that circulation personnel page someone, and wait at the circulation desk for that person. Announcement: “May I have your attention please? [State the person’s name], please come to the Circulation Desk.” (Repeat the announcement.)

Appendix C: Bullhorn Scripts

Bullhorns will be used by Circulation and Research Services personnel in the event of an electrical failure that renders the library’s public address system inoperable. Electrical Failure – building evacuation Circulation – 3rd, 4th, and 5th floors “May I have your attention please? An electrical failure has occurred and the building is being evacuated. Please exit the building immediately using the northeast or northwest stairs. Do NOT use the elevators. If you require assistance, proceed to the northeast or northwest stairwell and use the red emergency phone to contact University Police.” “I repeat, the building is being evacuated. Please exit immediately using the northeast or northwest stairs. Do NOT use the elevators. If you require assistance, proceed to the northeast or northwest stairwell and telephone University Police.” Research Services – Learning Commons “May I have your attention please? An electrical failure has occurred and the building is being evacuated. Please exit the building immediately using the main lobby doors.” “I repeat, the building is being evacuated. Appendix D: Evacuation and Closing Procedures

Follow normal closing procedures regarding locking doors, etc. (Closing procedures are available at the Circulation Desk and Research Assistance Desk.)

• Circulation personnel will: 1. Assemble a flashlight, walkie-talkie, and bullhorn. 2. Go up the northeast stairs, and make announcements on the 3rd, 4th, and 5th floors

following the script attached to the bullhorn. The announcement will explain that the building is being evacuated and all patrons are required to exit immediately using either the northeast or northwest stairs.

3. Walk each floor to ensure that all patrons have evacuated. 4. Take the front door key or master key (located in the safe in room 101B) and lock

the glass emergency exit doors at the east central, west central and south entrances. This is necessary because the magnetic locks do not function without electricity.

5. Tape pre-printed signs to the inside of the front entrance doors stating that the library is closed due to a power outage. Pre-printed signs are kept at the Circulation Desk because printing will not be possible without electricity.

6. Record an extended absence greeting for the 601-266-4250 voice mail to announce the unexpected closure and, if known, state when the library will re-open. An example of a generic announcement is, “Cook Library is closed due to unexpected circumstances. Please check the library’s website for more information.” Instructions for changing voice mail greeting are in Appendix F.

7. If necessary, notify University Police to inform them that a Campus Security Officer (CSO) will not be needed.

8. If necessary, contact any employees scheduled to report to work later in the day and tell them about the unexpected early closure.

• Research Services personnel will:

1. Assemble a flashlight and bullhorn. 2. Make an announcement in the Learning Commons explaining that the building is

being evacuated and all patrons are required to exit immediately using the main lobby doors.

3. Take a flashlight and check that patrons have evacuated the Curriculum Materials Center (109J). The Speaking Center (rooms 114, 115, 116, 117, 118, and 119) and The Writing Center (room 112) will be responsible for their own areas, as will Starbucks (room 105).

4. Go up to the 2nd floor, and make announcements that the building is being evacuated and all patrons are required to exit immediately. Explain that elevators are not functional and anyone requiring assistance to exit should go to the northeast or northwest stairwell and use the red emergency phone to summon University Police. Make announcements for the following locations:

a. 2nd Floor South (room 209) b. research carrels (room 211)

2. Enter the PIN, followed by the # key.

6. Listen to the new message for accuracy. Record again, if necessary
