INTRODUCTION
The term family as defined by AdellePelliteri is the basic unit of the community. All
members should have to maintain a healthy lifestyle status. Each family member must have a
disease free body. The health of the family is considered as a whole based on public health
perspective.
Family health office is tasked to monitor health programs which would help each family
attain the proper knowledge concerning their health. It is concerned with the health of the mother
to the unborn, the newborn, the infant, the adolescent and the youth, the adult men and women,
and also to the elders.
Salvacion G. Bailon-Reyes stated that Community Health Nursing is a field of Nursing
Practice delivered outside of purely curative institutions like hospitals but in community settings
such as the home, the school, and place of work, health centers and clinics.
As student nurses, our goal is to enhance, improve and promote health care. We focused
to our respective families and widened our scope towards the whole community to bring out their
major concerns.
The community involved in this study consists of one hundred thirty five (135)
households and a total of five hundred ninety six (596) residents of Barangay SitioGulod inMayamot Antipolo City.

ACKNOWLEDGEMENT
Many people contributed in the creation of this case study. We are grateful to the families
who allowed us to use them as our correspondents and to be part of our ideas especially to our
individual families that we immersed to and welcomed us as part of their families.
We would also like to express our gratitude to our clinical instructors, Mr.
FloranteArtienda, Mr. AvelinoBalagbis, Ms. Madonna Dapeg and Ms. JesusaGrimalt for guiding
us and giving us their thoughtful ideas and comments regarding our stay in the community.
Ultimately, we look up to you as our great mentor and educator. Thank you so much from
the bottom of our hearts. Thank you for stimulating our minds and guiding us for a brighter and
better future ahead in our field.
In addition, we would like to thank the barangay officials and staffs headed by
Mr.PabloOldan and Mr. Arthur Salvana for allowing us to interact and conduct surveys in certain
families in the community.
To our loved ones, who are always there to inspire us, to help us persevere, we dedicate
this to all of you. With the support that you had given us, you inspire us to become more diligent
in our chosen field and make us more determined to become better nurses in the future. Again, from all of us, thank you very much.
