Cannabis Employee Benefits Package
Enrollment Guide
1. Summary of the Programa. The Cannabis Employee Benefits Package is designed to combine best-in-class
carriers and products into one package. The plan has one flat rate and set productsso that you can easily calculate your costs and allows for simple ongoingadministration with one bill and one point of contact for all your benefits needs.
b. This package includes two options:i. Option 1: Standard Package – $125 per employee per monthii. Option 2: Enhanced Package – $235 per employee per month
2. Eligibility & Contributionsa. The Cannabis Employee Benefits Package is 100% employer-paid for all eligible
employees. It includes everyone working 30 hours per week, but may cover anyemployee working at least 15 hours per week. Spouses and dependents are noteligible for the plan.

3. Enrollment a. To enroll in the program, complete the online group master application at
cannabisbenefitspackage.com and the census, which can be downloaded at cannabisbenefitspackage.com. Both must be submitted through the online portal or via an email to [email protected]. Once the census and the master application have been received, you will receive a confirmation of receipt email from our team along with requests for any missing information. Application processing may take up to 10 business days. During this time, you will receive an email from LifeLock requesting electronic document signatures for the release and use of private individual information.
b. Once processed, you will receive an email with the following information: i. Your coverage effective date ii. Confirmation of enrollment details iii. Instructions on the employer portal to be used for adding and deleting employees from coverage throughout the year iv. Current billing information and request for the first month’s premium payment v. Log in for HR360
4. Billing a. The first month’s premium is due prior to the coverage effective date. Ongoing monthly billing
will occur on the first of the month. Premiums not paid within 10 days will be subject to retroactive termination. Account details, enrollment information, and monthly billing statements are available on the employer portal.
5. Renewal a. Your plans will renew annually one year from your effective date. You will receive new plan
information approximately 60 days prior to renewal.

a. Can I enroll a spouse, child, or domestic partner? i. Coverage is only available to employees.
b. Can I charge the employees anything for the Cannabis Employee Benefits Package?
i. The Cannabis Employee Benefits Package is set up for employer-paid plans only. Other employee-paid products may be available. Email [email protected] with questions.
c. Can I write off the expense for my business? i. A portion of the premium may be deductible. Please consult with your CPA or tax
advisor. Business expenses and IRC 280(E) are outside our scope of expertise.
d. Can I offer this to only some of my employees? i. All employees working 30 or more hours per week must be included in the plan.
You may offer the benefit to employees working 15 or more hours per week if you choose. Please contact [email protected] with questions.
e. How do I enroll an employee? How do I delete an employee? i. All changes to enrollment can be made through the employer portal, which can be
accessed at www.cannabisbenefitspackage.com. Refer to the employer portal instruction guide at www.cannabisbenefitspackage.com for instructions on adding or deleting employees from the program.
f. Can I buy only some of the products? i. No, the program is packaged together to consolidate billing and for
discounted pricing.

g. How do I file a claim or use my benefit? Do I have an ID card? i. There are no ID cards for this product. Each carrier has a different process.
1. To receive care with Redirect Health you must call 888-407-7928 or text using the Redirect Health App by downloading at redirecthealth.com/app. The care navigation team can take care of medical needs right over the phone without an in-office visit. If an in-office visit is necessary the care team will schedule for you. To learn more about the benefits of EverydayCARE view the informational video. You will receive a welcome packet and ID cards in the mail, which take 2 to 3 weeks to receive after the policy effective date.
2. To utilize your dental and vision program with LifeMap, give your provider your Social Security number. They will look you up in their system to verify your coverage. You have in-network and out-of-network coverage, but your benefit is better if you stay in-network. To see if your provider is in-network or to find a list of providers in your area in the LifeMap networks, visit the following: a. Dental b. Vision c. 3. To file a life insurance or AD&D claim, with LifeMap, complete the claim form at www.cannabisbenefitspackage.com. Instructions on submitting the form are on the claim form. You can also visit their website.
4. To utilize your critical illness and accident benefits with LifeMap, complete the claim form at www.cannabisbenefitspackage.com. Instructions on submitting the form are on the claim form. You can also visit their website.
5. To access your Employee Assistance Program (EAP) through Lifemap, you can call 866-750-1327 or visit www.MyRBH.com and enter “LifeMap” as your group code.
6. To utilize your LifeLock benefit, download the LifeLock App (available on Android and Apple phones) and follow instructions to enroll in online access. You can also call their customer service line at 800-607-9174.
