The program is located at the Texas Diabetes Institute, 701 S Zarzamora, 78207. A physician referral is required. Children 8-17 years and at least one parent must participate with the child. The child must have at least one risk factor: obesity, heart condition, diabetes, acanthosis nigricans, high cholesterol, or hypertension. phone no.
