Jacqueline Abt RN, MS

Centers for Medicare and Medicaid Services
Jacqueline (Jackie) Abt is a nursing information specialist with clinical nursing and
management expertise in emergency care, home health, hospice, discharge
planning, utilization review, quality assurance and care management. As a
Medicare contractor, Jackie led a Medicare systems Y2K testing team, trained
CMS employees in business requirements writing, and wrote business
requirements for CMS’ Common Working File redesign.
As a CMS central office employee, Jackie serves as an informatics liaison to the
ESRD project officers/other CMS divisions on both core contract issues and the
CROWNWeb data system. She is also the program lead for CROWNWeb
development, the GTL for NW 7’s Renal – Requirements, Communications and
Training (Renal RCT) contract, GTL for the Electronic Lab Data project, and
Medicare Improvement for Patients and Providers projects..

Candy Margiera
Administrator and CEO
Sarasota (FL) Physician’s Dialysis
Since 1988, Candy Margiera has served as the
Administrator and CEO of Sarasota Physicians Dialysis
Center, Inc. (SPDC) in Sarasota, FL. In addition to her work with SDPC,
she has served on the Medical Review Board for ESRD Network 7, on
the Board of Directors for the Florida Renal Administrators Association,
and as the Southeast Regional Director for the National Renal
Administrators Association.
As a key participant in both Phase 1 and Phase 2 of CROWNWeb,
Candy will be sharing tips and tricks learned at SPDC as an independent
facility user of CROWNWeb to meet the requirements of data entry in the
system.

Organization of TeamBefore using CROWNWeb, it’s important to prepare the team of users:
• Identify user roles (data entry, home dialysis, patient care coordinator)
• Communication across different roles
• Business office coordinates all activity
• Business office does final submission

Composition of TeamSarasota Physicians Dialysis team is made up of the following staff:
• Clinical and vascular access data entry – 1 PCT
• 2728 entry – 1 RN
• PD coordinator – 1 RN
• Administrator – Supervision (1)
• NOT FTE positions

Vascular Access Data EntryVascular Access data entry (in order):
• Print out from in-house EMR a report
• Contains access information for all patients
• Data entry clerk enters access info – including surveillance and pressure measurements
• Performed on the last day of each month
• 200 patients in 1 – 1.5 hours

Managing Transfer PatientsTransfer patients are handled individually through the business office:
• Transfer patient admitted in CROWNWeb
• Once admitted, business office pulls patients record from CROWNWeb
• Transfers out handled individually as well.
• Approximately 5% of population is transfer
