A3: As the holder of a current RPA Operator’s Certificate (ReOC), you will need to provide the details in this section as they appear on your current certificate.
If your legal name has changed, you will need to vary your ReOC by completing Form 101-02.

Confirmation the chief remote pilot has certified the operation.
Details of who is the RPA pilot responsible for carrying out the flight safety from start to finish.
Confirmation the members of the safety crew have completed the RPA induction training and certified ascompetent for the operation.
Details of the safety crew and the tasks they will be responsible for during the operation e.g. monitoringYTWB CTAF frequency, watching for aircraft and other hazards and restricting access to the area duringthe activity when RPA is in flight.
A6: Outline the RPA used during the operation. The RPA outlined here must exist on your remote operator’s certificate (ReOC). If the RPA details on your ReOC require updating, you will need to vary your ReOC by completing Form 101-02. List the RPA Type if the RPA Category is Medium size (or above). Include any distinguishing marks that identifies the RPA in operation e.g. colour, striping etc.
Part B –Area Approval / Permission Details
B4: Details of Operating Location
a. Provide a written outline (in a separate document) of the perimeter of the overall area of operation usingLatitude and Longitude coordinates to identify point-to-point edges of the perimeter or outline a single pointwith a radius.
For example:Point 1: S 27° 33.3, E 151° 55.1 Point 2: S 27° 33.4, E 151° 55.6
b. Provide an aerial view map e.g. google maps or aeronautical chart (in a separate document), and mark thepoint-to-point locations as well as the nearest aerodrome and any nearby helicopter landing sites.
c. The Aerodrome Reference Point can be found in ERSA and is indicated by the symbol.
B5: Flight Profile:
Provide an outline of the specific sortie within the operating location (as specified in B4, in a separate document). This includes the point-to-point intended routes within specified areas, the intended altitude at the points and when the RPA will be stopping to conduct activities e.g. to capture images. The outline can be either written or in the form of a diagram. This is to include (but is not limited to) extracts, flight profile of orbits (illustration), flight plan tracks, waypoints, transects etc...
