Introduction Dear Class of 2020, Congratulations! You have successfully completed half of your third year clerkships. It is now time to start planning for your fourth and final year of medical school. The fourth year is very different from third year. You will have the opportunity to finalize your choice of specialty and do some electives that are of particular interest to you. This ability to build your own schedule combined with the residency application process can seem quite overwhelming at times. Rest assured that everyone in the Office of Medical Education is here to help you through these stressful times. Over the next few months, you will begin to build your plan of study for next year. The materials provided in this document will provide all the important resources, deadlines, and procedures that you will need to complete your schedule. Please keep in mind that there is no “right way” to do this. You will find that you have a very different schedule than your classmates, even those classmates going into the same specialty. The increasing competitiveness of the match will make it important for you to be thoughtful and plan strategically. I encourage you to utilize all the available resources including faculty, mentors, the Office of Medical Education, including the Office of Student Affairs, and the Careers in Medicine tool to guide you through this process. As always, we are here to meet with you to address individual issues as they arise. Do not hesitate to contact us. Lastly, try to enjoy your fourth year of medical school while it will be stressful at times, it is a great year to just enjoy what you are doing and prepare for residency. Fourth year students and their advisers work to tailor each student’s fourth year experience to prepare them for residency. Students get the clinical and academic experience from the third year, but they have more freedom in choosing their schedules. The scheduling freedom of the fourth year gives students the opportunity to schedule time to prepare for the USMLE Step 2 exams, Clinical Sciences (CS) and Clinical Knowledge (CK) in addition to preparing for residency interviews. Students must also participate in the Phase Three OSCE during the final year of medical school.

Graduation Requirements Checklist Below is a list of all requirements that must be met to be eligible for graduation. (Draft in Progress)

(Draft in Progress) From the Progression Policy:

A. A maximum of four years are required to complete Phase A which includes passing USMLE Step 1.

B. Students must complete a minimum of 150 credit hours in the medical program, including all Phase A courses, all required clerkships in Phase B, and necessary Phase C sub-I and electives. No more than 27 credit hours of electives can count towards the 150 credit hours for graduation. Phase B and C must be completed within a maximum of four years.

C. Students must pass USMLE Step 1 and present supporting documentation by Feb 1st in the winter of

their third year of training.

D. Total time from matriculation to graduation cannot exceed 7 years.

E. Students must pass USMLE Step 2 CK and Step 2 CS exams to qualify for graduation. It is strongly recommended but not required that Step 2 CK is passed by the end of the 1st semester of Phase C.

F. Failure to meet these requirements will result in review by the Student Promotions Committee.

G. Students must attain the knowledge and skills, and develop capacity and behaviors required of a

physician.

H. Students must attain a level of clinical judgment which warrants entrustment by the Faculty as required for entry to residency.

I. Students must demonstrate a sense of responsibility and social accountability to patients and the

community.

Scheduling USMLE Step 2 CS Because there are only 5 test centers (in the country), and also because many other med schools have similar “sit by” deadlines, there are not enough scheduling appointments available for every medical student in the nation. To gain the advantage you need to start scheduling as soon as you get your financial aid for the spring semester. Approximately 1 month prior to your scheduled Step 2 CS exam, you are required to meet and review your MS3 Final CCE performances. These meetings will be set up by an administrator of the Clinical Skills Center.

“SIT BY” Deadline: You must pass both the USMLE Step 2 CS and Step 2 CK in order to graduate. The “SIT BY” DEADLINE for both of these exams is November 1st. Any student who is delayed 2 rotations may sit for CS, prior approval must be obtained by Dr. Vijay Khatri, Dr. Peter Yip, and Dr. Darilyn Falck. This sit by date is REQUIRED to ensure that we get exam results reported in time to certify you on the Official University Graduation Report with the main campus Registrar’s office. You should also have your scores in time to be reported MATCH.

Know The Rules: Before applying for the USMLE Step 2 exams you must become familiar with policies and procedures in the USMLE Bulletin of Information published for the specific year you will be taking the exam. This bulletin provides information on the following topics:

• Overview • Eligibility • Examination Content • Preparing for the Test • Applying for the Test and Scheduling Your Test Date • Testing • Scoring & Score Reporting • Indeterminate Scores and Irregular Behavior • Communicating With USMLE

Apply for a Scheduling Permit Once you have reviewed the Bulletin, you may apply for the USMLE Step 2 CS by logging into the NBME website for Licensing Examination Services. The permit eligibility period for the USMLE Step 2 CS is 12 months. Per the USMLE Bulletin, there is no fee to reschedule within your eligibility period if you cancel and reschedule more than 14 calendar days before (but not including) your scheduled test date. So scheduling an exam early will not cause you to incur any additional costs should it become absolutely necessary to reschedule (provided you give appropriate cancellation notice). The scheduling permit includes instructions on how to contact Prometric to schedule a test date appointment.

Selecting a Test Date You cannot take the Step 2 CS during your third year, but you may schedule the exam for a date occurring as soon as your third year ends. There is NO advantage to selecting a LATE test date (for example, after the deadline in November or December). Graduates have consistently reported that readiness for this exam is at its optimal level upon completion of the third year and diminishes as you get further into the fourth year. If you take the exam at a later date and do not pass, you may not be able to reschedule it in time to graduate in May. In the event, you should need to reschedule and the calendar shows no openings, applicants who are registered for the exam can

participate in an email notification system. You are notified by email when a testing appointment becomes available due to cancelation. Be advised that there is a lot of competition for the cancelled slots; and there is no guarantee you will get an exam appointment.

USMLE Step 2 CS Score Reporting: USMLE exam scores are always reported on Wednesdays. Results for the USMLE Step 2 CS are reported in batches for examinees testing over a defined period of time. As shown in the table below the USMLE Step 2 CS reporting schedule for 2018 includes test dates through December 31st, however the Deadline for May Graduates is November 1st, 2018.

For each “Testing Period” in the above schedule, Step 2 CS scores are released every Wednesday over a corresponding four-to five-week “Reporting Period.” It is expected that results for the vast majority of examinees who take the exam during the testing period will be reported on the first Wednesday of the Reporting Period. Results for 98%-99% of examinees who take the exam during the testing period are reported by the third Wednesday in a Reporting Period. For a small percentage of examinees (1%-2%), scoring and quality assurance may be not completed in time for these examinees to be reported by the first three reporting dates; these will typically be examinees who took the exam in the latter part of the testing period. Results for these examinees will be reported each week throughout the reporting period, and should be reported no later than the last day of the score reporting period. This schedule allows USMLE staff to enhance the quality assurance and data collection/scoring procedures performed prior to score reporting. Additionally, it provides examinees, as well as others who rely on Step 2 CS results, with guidelines regarding when a result will be reported for a given exam date. These guidelines allow examinees to plan their exam registration and scheduling in order to have their results in time to meet specific deadlines, such as those related to graduation or participation in the National Resident Matching Program (NRMP), or “the Match.”

Scheduling Courses in the Fourth Year

Student Scheduler (Draft in Progress) In the NOTES section you can specify if you are seeking a certain preceptor for a letter of recommendation or other notes you want the coordinator to take into consideration for processing your request. A structured process is being established for the M4 electives/sub-internship which is as follows:

A 12-block system has been created for the next academic year 2019-2020. Each block is comprised of 4 weeks.

When the M4 Elective schedule is opened for enrollment, student will be allowed to select 4 home rotations. These rotations will be on a “first come first basis.” Waiting list will be established.

Time for Interviews There are 44 calendar weeks available during the fourth year. You are required to schedule weeks’ worth to 31 credits. It is expected that you will utilize the 8-13 extra weeks for board exams and interviews. Use the chart below to determine when your peak interview season will be and avoid scheduling electives during that period. Hopefully, Required Courses will not conflict with your peak interview period. If that happens, you will likely need to reschedule those courses. Should that be necessary, contact the coordinator directly since the Sub-I and ICU courses will no longer be available on the Student Scheduler. Interviews are not counted as excused absences. If you will need to miss more than three or four days of a rotation, you must reschedule that rotation or work out a makeup schedule with the department.

Method of Communication The point of contact for all M4 related questions will be the clerkship coordinators. All communication and inquiries about M4 matters must include the special email: [email protected]. This email is always checked by one of the coordinators or administrative assistants and therefore ensures that your queries are addressed in a timely fashion. We would like queries not to be sent to individual faculty or admin just in case if they are not available in the office. It can delay the response and resolution of any issues. Hence the reason why we have established this process. During the M4 year we will arrange monthly/bi-monthly (according to the need) M4Connect Webinar to communicate back any of your rotation concerns. Dr. Vijay Khatri will be present at these sessions. If you need to have a specific clerkship director present at this webinar please inform the clerkship coordinators.

Frequently Asked Questions: (Draft in Progress)

Q: What if don’t get an interview at a program that I’ve applied to for a subinternship? A: Once you’ve accepted a subinternship, you should attend. A school has offered you a position that could have otherwise gone to another student. Dropping an elective when you find out that an interview is not going to be offered is considered unprofessional, and is strongly discouraged. Q: What if I am scheduled for a required course during the appointment for a USMLE Board exam? It is important to understand the types of residency training available as you choose a specialty and begin applying for residency positions. There are different types of training programs depending on your specialty choice. Some of the positions offered through the NRMP Main Residency Match include:

• Categorical-C: programs that begin in the PGY-1 year and provide the full training required for specialty board certification.

• Primary-M: categorical programs in primary care medicine and primary care pediatrics that begin in the PGY-1 year and provide the full training required for specialty board certification.

• Advanced-A: programs that begin in the PGY-2 year after a year of prerequisite training. • Preliminary-P: one-year programs that begin in the PGY-1 year and provide prerequisite training for

Step 2 Policy Statement MS-IV students MUST sit for both the USMLE Step 2 CS and Step 2 CK no later than the Sit By Date designated on the Academic Calendar for the year they complete their 4th year requirements in order to be eligible for graduation in May of their designated class year. Passing both Step 2 CS and Step 2 CK is required in order to graduate. Students who delay taking the exam after the Sit By Date, whether compelling reasons were granted or not, will not be guaranteed eligibility for graduation in May of their designated class year. It is the student’s responsibility to register to take the exams and to make any necessary transportation and housing arrangements. The earlier a student registers for the Step 2 exams, the more choices a student will have in terms of testing location and dates. There are limited test sites where the Step 2 CS exam is administered and students are encouraged to register for the examination as soon as possible due to limited space availability. Only those students who can demonstrate compelling reasons (such as personal or family illness, tragedy) for not sitting for the exam by the Sit By Date will be allowed to register after the Sit By Date. Students requesting a delay must submit a written statement to the Office of Student Affairs explaining the reason(s) along with any requested documentation no later than August 15th to be considered. Emergent compelling conditions will be considered on a case-by-case basis. Unacceptable compelling reasons include lack of exam preparation, testing anxiety or difficulty in scheduling a preferred testing site or date. Students have three attempts to pass Step 2 CS and three attempts to pass Step 2 CK. Failure to successfully complete either of these two exams on the third attempt will result in automatic dismissal from the School of Medicine. PROCESS FOR REQUESTING ABSENCES (Draft in Progress) Students must request advance approval for ANY ABSENCE (INCLUDING RELEASE TIME FOR STEP 2 CK and CS AND RESIDENCY INTERVIEWING) from their course director at least four weeks before the start of the rotation. If properly arranged and approved by the Course Director in advance, students will be excused one day for Step 2 CK and up to three days for Step 2 CS. Students cannot schedule both examinations during the same required rotation or elective. Course Directors may determine that the multiple absences will be detrimental to students’ educational experiences and require that the course be rescheduled if openings are available. For elective courses, course directors will have the discretion to assigned make up days or supplemental readings or assignments to account for the missed days. Failure to properly arrange the excused absence before the start of the rotation, unless emergent due to a last minute scheduling opportunity as defined below, may result in the makeup of missed time with penalty or denial of request. Emergent Absences for Interviewing or Step 2 Scheduling Students may request an emergent absence for an interview invitation if the invitation is received less than four weeks in advance of the start of the rotation. WRITTEN Documentation of the invitation for the interview must be submitted to the Course Director and Clerkship Department prior to approval. Emergent scheduling of the Step 2 CS or Step 2 CK may also be approved as an excused absence if documentation of an exam opening of less than four weeks is provided to the Course Director. Failure to provide WRITTEN documentation will result in automatic denial. Unexcused Absence Students with unexcused absences (defined as any absence that is not approved by the Course Directors in advance) will be required to make up and may be subject to an incident or critical incident report. Nothing in this policy statement should be construed as students having an allotment of personal days in which absences can occur. The Course Directors will determine how the unexcused absence is to be made up. Reoccurring Patterns Questions or concerns regarding a student's attendance pattern are to be brought by the course director to the attention of the Clerkship Department and Office of Student Affairs. All absences are to be reported as part of the student’s evaluation and the Honor Council and Student Promotion Committee will discuss reoccurring absence patterns with the student. In the case of reoccurring patterns, the Honor Council and Student Promotion Committee will determine the appropriate course of action.

Excused Absence Unexcused Absence

Definition Absences or lateness due to unexpected medical and/or personal issues are unavoidable. Examples of an emergency or unexpected absence include personal illness, family emergency, or death in the immediate family. Family obligation, employment, travel and previous plans are not generally accepted as excused absences. Excused absences not addressed specifically in this policy are considered and granted on a case-by-case scenario by the Deans of Student Affairs.

Absence and repeated tardiness to class and clinical rotations are considered unprofessional conduct and will be taken into consideration by the Honor Council and Student Promotion Committee. Repeated tardiness, excessive absences and unreported absences may result in make-up days and remediation.

Students’ responsibility Students must complete the Excused Absence Request Form and send it to the Course Instructor and Course Director no later than two business days following an unanticipated absence. Meeting with Course Director to review the missing content to come up a makeup plan

ONLY Exception: COM 901 Honors Medical Research Elective can be paired with the following electives: Mindfulness, Healer's Art, or Leadership, these are all part-time evening courses. Teaching Assistant Electives

To provide some flexibility in the schedule during the interview months, several Teaching Assistant (TA) M4 elective rotations have been established. Maximum credits that can be earned for TA electives: 4 credit units Credit calculation for TA electives are as follows:

Unit of Credit: 1 credit for every 15 lecture hours and 30 preparation/homework hours (LEC). 1 credit for every 25 workshop hours and 25 preparation/homework hours (AL).

On the spreadsheet if a student attended 3 OSCE sessions per day for 2 days, the formula would be: 8 hours for 3 sessions/day*2(preparation/study) =16*2(the total number of days) =32 hours/25(per credit) =1.30.

MED 810: OSCE Program Assistance: To sign up for the OSCE sessions, you must contact the clerkship coordinators. Do not approach the OSCE course directors to enroll or sign up. If any changes occur to your schedule that you cannot make it to the OSCE session, please contact the clerkship coordinators to cancel or reschedule. If there is no show, no call, the OSCE course directors will notify the coordinators of your absence and will be considered as an unexcused absence.

Sub-Internships (Acting Internship) Policy A Sub-internship also known as Acting Internship is a M4 clinical experience completed at CNUCOM-affiliated hospital systems or through VSLO in which the medical student is expected to demonstrate the knowledge, skills, and attitudes expected of a first-year resident. Any home, away or VSLO rotation that is labelled as a Sub-Internship/Acting Internship and the syllabus SHOULD reflect the higher level of clinical care responsibility.

Minimum Sub-internship: 1 core specialty (General Internal Medicine, General Surgery, General Pediatrics, Ob/Gyn, Family and Community Medicine, Emergency Medicine)

Sub-internship can be completed at an “away’ site. Due to the increased level of effort and responsibility required by the medical student during a sub-

internship, the credit units awarded are 1.5 units /week. Minimum number of sub-internship required to graduate= 1 Maximum number of sub-internship= 3. Hence the maximum credits that can be earned from sub-

internship is 18 credit units. Keep in mind that there is a specialty limit of 12 weeks, including electives and sub-internships.

Rotation requirements to classify an elective as Sub-Internship: Required Components

1. Contain an inpatient component 2. The rotation should be 3-6 weeks in length 3. Provide the student with primary responsibility for a panel of his/her own patients and direct

supervision by preceptor 4. Students should have a patient load comparable to the PGY-1 resident on service. 5. Students must take in-house call (daytime or nighttime)—and may be required to take call at the

current ACGME frequency of residents. Optional Component

1. Students have first call responsibility for patients, with adequate supervision 2. Students receive “ sign-out” from house-staff so they are involved with care transitions and cross-

coverage responsibilities 3. Students may be allowed to enter patient orders that are then signed off by the supervising preceptor

or resident.

Career Track - Specialty of Interest Rotation Policy Specialty of interest refers to the specialty the student is interested in pursuing as a career and securing a residency in that field. It is very important that the residency does not begin in the M4 year. You will gain in depth education for this field of choice during your residency and if necessary during the fellowship.

1. Maximum weeks for a specialty: 12 weeks. The AAMC Medical Specialties categories will assist the student in understanding how specialty limit is defined. Hence if a student selects rotation in Cardiology, Pulmonary and Infectious disease this will be considered 12 weeks in the Internal Medicine and thus reach the maximum allowed.

2. “Non-specialty of interest” rotation tracks given by Dr. Khatri and Peter, requirements will provide a guide of recommended exposure of clinical rotations that will help and support your career, 8 weeks.

Submission of Elective, Preceptor, Institution Evaluations (Draft in Progress)

Students must complete Preceptor and Institution evaluation within 1 week from the last day of the rotation. Once you have completed your evaluations, the school will be able to release your evaluation from the Preceptor by request through the clerkship coordinators. Grades will not be released until the evaluations from both the student and preceptor have been completed. In order for the student to receive credit for the rotation, an evaluation will need to be completed by the attending(s). If an evaluation is not received for the rotation, credit is not awarded to the student.

Creation of New Electives or Changes of Existing Electives Any changes to an “existing elective” or to “create a new elective” must be performed by the Preceptors. Students cannot make any changes to “existing electives or sub-internships,” such as length of rotation, location, preceptor. All new electives are established through the office of the Chair of Clinical Medicine, Dr. Peter Yip, and the Director of M4 electives/sub-internships, Dr. Vijay Khatri. This process allows the clerkship coordinators and the Registrar to ensure that the students are registered for a CNUCOM-approved course. The process of establishing an elective is performed by the faculty and is as follows:

1. A standardized syllabus has been established so that the necessary CLO/PLO are addressed for mapping. Similarly, for sub-internship’s we have to ensure that they meet the specific requirements to deem a rotation as a sub-internship.

2. Once syllabus is provided, it is review by the M4 Director for completion of content and appraisal for mapping language by Office of Assessment.

3. Administration forwards the syllabus to the Phase B+C committee for voting and thereafter to the Curriculum Committee.

4. At the same time we request a detailed CV from the preceptor to rank them at CNUCOM. Preceptors are required to complete a Faculty Assessment Form (teaching, clinical, research, service components) to allow the Rank and Promotion Committee to judge the teaching and clinical experience.

5. Rank and Promotion Committee then reviews and submit their recommendation to Dr. Gordon Wong and Dean Joseph Silva.

6. An institutional or individual physician affiliation agreement process is initiated. 7. Our coordinators then inquire about the on boarding process and requirements. 8. Finally the Registrar assigns the CNUCOM-approved course a course number and we add it to the M4

Catalogue. 9. We ask the Preceptor which BLOCKs they wish to host our students and how many student/month.

This entire process can take up to 120 days (4 months) for completion. If students or faculty are aware of any community clinicians who may be interested in serving as a preceptor they should send the contact information to the Chair of Clinical Medicine (Dr. Peter Yip) as this office begins the process, stated above. CNUCOM will not approve special ONE TIME ONLY electives.

Credit Allocation by the Registrar

Students must be enrolled in an approved elective found in the M4 Elective Catalogue or through VSAS/VSLO to receive credits.

Any electives or sub-internships that are not approved by the registrar and M4 committee, will not receive credits.

Students may choose to do electives and sub-internships that exceeds the maximum amount of specialty and away limits.

Any electives or sub-internships that exceed the limit will not count towards graduation credit requirements but will be shown on your transcript. This will not count against you.

M4 Flight Plan, which lays out your planned rotations and showing you’ll receive 31 credits for graduation, has to be submitted and finalized by July 1st. This ensures ample time for the M4 committee to review and authorize clinical rotations that meet the requirements for graduation and allow the Registrar to conduct a preliminary degree audit.

Once your M4 Flight Plan is approved and finalized, the Registrar will be able to register you to the electives, sub-internships, and rotations.

Students who are not enrolled or registered for electives for certain amount of time and credits will impact the student’s financial aid and student loans.

M3 Clerkship Deferrals There shall be a fair and equitable process for students to defer clerkships to allow pursuit of other educational activities in the M3 year. Policy: Voluntary deferral of M3 core clerkships to M4 is not allowed (OB/GYN, Pediatrics, Surgery, Internal Medicine, Psychiatry, and Family Medicine). Deferral of M3 core clerkships in the M4 year will occur for remediation purposes (students MUST complete the remediation clerkship before beginning M4 rotations). VSLO (formerly VSAS) away electives require that the OB/GYN, Pediatrics, Surgery, Internal Medicine, Psychiatry, and Family Medicine clerkships be completed. Deferral of Neurology and/or Emergency Medicine (CNUCOM non-core clerkship) requires submission of the Deferral Request Form and completion of a DEFERRAL PROCEDURE outlined below. Procedure: As deferrals of clerkships can impact various aspects of scheduling, it is imperative that the students follow the Deferral Policy and Process.

1. Students who wish to postpone M3 clerkship time should make initial inquiry with Linda Inthongpradith.

2. The student should complete and submit a Deferral Request Form. 3. Requests are reviewed by the DEFERRAL EVALUATION COMMITTEE. Requests will be ranked by the

following priority list: a. Students deferring clerkships for academic, medical, or administrative reasons b. Students deferring clerkships for sub-internships in early match residencies (urology,

ophthalmology) c. Students deferring clerkships for sub-internships in small, competitive fields (dermatology,

ENT, neurosurgery, orthopedic surgery, radiation oncology, plastic surgery) d. Students deferring clerkships for other reasons

4. Within these groups, student requests will then be ranked in the order that the students have submitted the Deferral Request Form.

5. The coordinator will contact the students by email in order of rank to offer options for deferral. Each student will have 72 hours to respond with their selection, and changes can only be made after all other students’ initial deferral requests have been addressed. The Curriculum Office reserves the right to deny a student’s request at any time based on scheduling constraints or other logistical issues.

STATEMENT OF UNDERSTANDING (Draft in Progress) As a Senior Medical Student, I understand that it is my responsibility to ensure that my schedule is requested and approved on by the Clerkship Department at least 45 days in advance of my elective start date. Failure to maintain a correct schedule will result in no credit for the elective even if the elective was completed. I understand that once submitted and approved with the Registrar’s Office, it is my responsibility to obtain reporting information at least one week in advance and that I must complete the elective. If the elective is not completed, a grade of “F” may be recorded on my transcript. Cancellations of the elective are my responsibility and must be at least 45 days before the elective start date. Failure to notify the school about dropping electives will result in a “W” grade and will be not be removed from the transcript. I further understand that the 45 day rule will be strictly enforced starting June 10, 2019 and the Office of Medical Education will formally register me for the courses with the Registrar’s Office at the end of the month for that month’s courses. Honors Medical Research Request

COM 901 (1 cr; P/F) 9700 West Taron Drive, Elk Grove, CA 95757

REQUIREMENTS AND ELIGIBILTY

COM 901 provides students with an opportunity to collaborate

with faculty on medical research.

Requirements & Responsibilities

The student must have a minimum of one COM faculty advisor, but

can also have a non-COM faculty/community physician supervisor

with whom the research is conducted.

The supervisor should be familiar with research in the relevant

discipline (s), to ensure that the direction and monitoring of the

student's progress is informed by up to date subject knowledge

and research developments. Breadth of experience and knowledge

of the supervisor will mean that the student always has access to

someone with experience of supporting research students through

to successful completion of their scholarly project.

The student and supervisor must arrive to an agreement

regarding the research topic, the timeline, and the amount of

contact time for the progress of the project.

The student is responsible for taking the CITI courses (a basic

course for all students, and a more oriented course for those

students that need IRB approval).

The student is responsible for other aspects of the project,

including enlisting the aid of any required supervisor, coordinating

communication with the supervisor and the faculty advisor,

helping the supervisor in obtaining the necessary protocol

approvals (IRB, IACUCC, IBC) if needed, developing the hypothesis

of work, performing the planned study, preparing a document

with the results, data analysis, and conclusions, and preparing the

poster for the Research Day (check COM academic calendar for

event date).

The student is required to meet with the faculty advisor, weekly.
