Program Purpose

• To assist health centers located in areas impacted by disasters declared by FEMA in calendar years 2018 and 2019 -- Hurricanes Florence and Michael, Typhoon Mangkhut, Super Typhoon Yutu, and wildfires and earthquakes occurring in calendar year 2018; and tornadoes and floods occurring in calendar year 2019

• FEMA-declared disaster events available in Appendix A

Funding Purpose

• CADRE funding supports eligible health centers to:

Respond and recover from emergency and/or disaster impacts and/or;

Increase their capacity and capability to respond to and/or recover from future emergencies and/or disasters; and

Support the continuity of access to high quality primary care services for underserved and vulnerable populations

Summary of Funding

• Approximately $79 million to fund an estimated 225 awards across three funding tracks

• You can apply for funding for only one funding track:

Track 1 - Equipment-Only

Track 2 - Minor Construction-Related

Track 3 - Major Construction-Related

• 3-year period of performance: September 1, 2020 through August 31, 2023

Eligibility Requirements

• To be eligible to apply for and receive CADRE funding, organizations must:

Receive Health Center Program operational grant funding at the time of the release of this funding opportunity and at the time of award;

Have at least one service delivery site located in a FEMA declared emergency and/or disaster-impacted area within your approved health center scope of project (as indicated on your Form 5B: Service Sites)

Proposed Service Delivery Sites

• Proposed projects must be located at a service-delivery site that is in an area designated by FEMA with an Emergency or Disaster Declaration as listed in Appendix A in the CADRE NOFO.

• Proposed projects may add a new service delivery site within the FEMA declared disaster-impacted area as listed in Appendix A in the CADRE NOFO.

• Any site additions for applications submitted for the Equipment-Only track will be limited to proposing only new mobile medical van site(s).

Track 1 - Equipment-Only

• Purchase of moveable equipment with a useful life of more than one year. Examples of moveable equipment include, but are not limited to:

Telehealth equipment (e.g., laptops, servers, videoconferencing equipment)

Medical equipment (e.g., scales, stethoscopes)

Purchase of new mobile medical vans or the replacement of an existing mobile medical vans

• Applicants can request up to $100,000

• Equipment can be purchased at any number of eligible service delivery sites

Track 2 – Minor Construction Related

• Propose up to 3 site-specific projects

• May include site-specific moveable equipment

Alteration/Renovation (A/R) activities

• Activities to modernize, improve, and/or reconfigure the interior arrangements of an existing facility;

• Permanently affixing equipment (e.g., heating, ventilation, and air-conditioning (HVAC) units; exterior generators; lighting);

• Work to improve accessibility (such as sidewalks and ramps) and/or life safety requirements in an existing facility;

• Work outside of the building is limited to improvements to the building entry for handicapped accessibility, generator concrete pads, and other minor ground disturbance.

Construction/Expansion (C/E) activities

• The construction of a new stand-alone structure and/or associated work required to expand a structure to increase the total square feet of a facility;

• The installation of a permanently affixed modular or prefabricated building and/or significant site work such as new parking lots or storm water structures.

Unallowable Costs

• Key reminders that CADRE funds MAY NOT be used for:

Provision of services or operational costs.

Costs related to EHR ongoing operation or maintenance.

Pre-Construction costs incurred prior to 90 days before the award date.

Creation of shell space for future use.

Facility, land, or passenger vehicles.

Application and Submission Information

• Phase 1: Grants.gov

• Phase 2: HRSA Electronic Handbooks (EHB)

• All applications must be submitted in Grants.gov and HRSA Electronic Handbooks (EHB) by the deadline

• Eligible health centers will receive an email through the HRSA Electronic Handbooks (EHB). This email will provide instructions on how to access the application module in EHB.

Equipment List(Project Specific Forms)

• Required for all projects with equipment purchases

• Provide a complete and reasonable equipment list (clinical and non-clinical) that aligns with the goals and objectives of the proposed project

• Include moveable equipment with a useful life of more than one year purchased with funding

Environmental and Historic Preservation Conditions

• Conditions for environmental and/or historic preservation reviews and compliance with Federal laws.

DO NOT begin activities until conditions have been approved and lifted.

Plan project schedules accordingly. Consultation under Section 106 takes a minimum of a month to prepare, while an Environmental Assessment may take several additional months to prepare, review, and distribute for public comment.
