The Federal Trade Commission (FTC), Washington, D.C., requires that all funeral homes throughout the United States provide a General Price List to anyone who inquires about funeral arrangements. In compliance with Federal Law, we provide the General Price List to you. The prices listed are subject to change without notice.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

At the funeral home, you will be provided with a complete Merchandise Price List along with your personal copy of our Merchandise Compendium, in which the products available are pictured and fully described. Together, they represent our Casket Price List, Outer Burial Container Price List and price list of other items available as pictured and described in the Compendium. You will find the following range of prices for merchandise offered.

Alternative Containers and Caskets $ 30 to $14,995 Includes burial caskets, cremation caskets and ceremonial rental casket 11 Outer Burial Containers $ 1,095 to $12,495

Cremation Merchandise $ 49 to $ 8,495 Includes urns, keepsake urns, jewelry & urn vaults for burial.

Merchandise Warranties The only warranty on the merchandise sold by us is the express written warranty, if any, provided by the manufacturer of such merchandise. We make no warranty, expressed or implied, with respect to merchandise. A manufacturer's warranty, if any, will be provided to you.

Payment Policy To lose someone you love is a devastating human experience. It can be traumatic

and seriously impact one's judgment. Therefore, it is important to be realistic with respect to funeral expenditures.

We do not want your choices to be more than is affordable nor less than you would like to arrange. What is important is that arrangements be in keeping with your wishes and budget.

We believe in the recognition and meaningful celebration of a life that has been lived and will assist in arranging an appropriate service to meet your individual needs.

Benefits Assistance We will assist in the application for and notification of Social Security, Veterans Administration, Veterans Affairs, Personnel Departments and Fraternal organizations for any available benefits that you may be entitled to receive. At your request, we will even handle all life insurance claims on your behalf.

Cash Advances Services and merchandise provided as cash advance items will be billed at the same amount paid by the funeral firm, this includes cemetery charge, cemetery equipment supplied by vault company, death certificates, clergy honorarium, public transportation, newspaper charges, etc.

Basic Services of Funeral Director & Staff..................................................$ 2,065 Our charge includes, but is not limited to, 24 hour availability of staff to respond to initial request for service; arrangement conference; securing, preparing and filing of necessary authorizations, permits and certificates; preparation and placement of necessary notices; coordination of service plans with parties involved in the final disposition of the deceased and staff availability and assistance during and following the funeral. This charge also includes basic overhead expenses. This fee for our basic services and overhead will be added to the total cost of the funer- al arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. )

Transfer of Remains to Funeral Home.........................................................$ 475 This rate covers a 25 mile radius from the funeral home. This distance is considered local. Unless stated otherwise, all transfers referenced are considered local. Add $ 1.75 per loaded mile thereafter.

Embalming......................................................................................................$ 925 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

Embalming is required if you: 1) Select a service with a viewing

2) Arranged for shipment by common carrier or interstate transportation

3) Select arrangements that require us to hold the remains for more than 24 hours provided no refrigeration is available or a hermetically sealed container is not used; and provided that embalming does not conflict with religious beliefs or medical examination.

Direct Cremation (range of prices starting at).............................$ 3,465 to $ 11,270 Our charge for a direct cremation includes basic services of funeral director and staff; transfer of remains from place of death (local); sanitary care, dressing and placement in a cremation casket or alternative container; positive identification by next of kin or authorized representative prior to cremation; transfer to crematory if applicable; the cremation process and return of the cremated remains to the next of kin. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering) The containers we use are heavy cardboard with a lid, treated to resist leakage.

Prices for our services, facilities and motor equipment listed on our General Price List are provided at a reduced rate for infants and young children. This compassionate allowance is expressed as a percentage of the regular price. 10% under one year of age and 50% up to age 14. For a selection of Children’s Caskets see our Merchandise Price.
