The goods and services shown below are those we can provide to our customers. You may choose only

the items you desire. However, any funeral arrangements you select will inc lude a charge for our

basic services and overhead. I f legal or other requirements mean you must buy any items you

did not specifically ask for, we will explain the reason in writing on the statement we provide describing

the funeral goods and services you selected.

Direct Cremation is the disposition of human remains by cremation, without the added cost of

embalming, formal viewing, or ceremony. Our charges include all of the following items:

Basic service of the funeral director and staff,

Professional Services Basic Services of Funeral Director and Staff $695

This fee for our basic services and overhead will be added to the total cost of the funeral arrangements

you select. This fee is already included in our charges for direct cremations, forwarding of remains, and for receiving

of remains. This fee includes, but is not limited to, SimpleCremation.org online arrangement platform, availability of

staff 24 hours a day by phone, staff and facilities to respond to initial request for services, use of our positive

identification system, consulting with the crematory, filing of Texas death certificate and obtaining local permits, and

providing assistance with social security notification.

Transfer of Remains to Funeral Home (within the Texas counties listed below) $195

Tarrant, Dallas, Bell, Williamson, and McLennan Counties

Contiguous County Additional Transfer Fee (within the Texas counties listed below) $95

Contiguous counties include: Collin, Denton, Ellis, Hood, Johnson, Kaufman, Parker, Rockwall, Somervell, and Wise

Additional Mileage Transfer Fee (outside of the service areas mentioned above) $2.95

Per mile, one way. Miles are calculated from the location of death to Simple Cremation Fort Worth.

Refrigeration (for the first 7 days) $175

Shelter and care of Deceased in our licensed, climate controlled holding facility

This fee is added when the deceased weighs more than 300 lbs (maximum weight of 450 lbs)

Embalming $600

Use of preparation room, equipment, professional staff and materials necessary to perform the embalming

procedure. Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral

arrangements, such as a viewing. If you do not want embalming, you usually have the right to choose an

arrangement, such as direct creation or immediate burial, that does not require you to pay for it.

Minimum Preparation of Remains $195

Minimum preparation may be necessary for private identification viewing or witnessing of cremation when

embalming is not selected and could include one or more of the following professional services: bathing, sanitary

care, dressing of unembalmed remains, or setting the facial features of unembalmed remains.

Specialized Care of the Deceased $195

For trauma, autopsy, or organ donor restorative repair.

Basic Hairdressing and Cosmetology $65

Hair styling, make-up, and/or tending to facial hair as needed.

Packaging and Shipping of Cremated Remains (within the United States via USPS) $150

Delivery of Cremated Remains (within 35 miles) $150

Obituary Submission Fee (per newspaper) $20

Filing Insurance Policies for Advanced Funding $150 + 3%

A filing fee of $150 will be added to the funeral purchase agreement to allow for advance funding. To that, 3% of

the funeral purchase agreement amount is charged to allow a third party to complete the insurance claim.

Individuals wishing to use the proceeds of a life insurance policy to pay for funeral goods and services, in lieu of

paying cash, may elect to have a third party assist you in the filing of a claim on behalf of the beneficiary(ies) of the

policy. Please note that if you utilize a third party to file a claim seeking expedited receipt of life insurance proceeds

on behalf of a beneficiary, there will be a fee to be paid associated with the filing of such a claim. Any such fees will

be deducted from the proceeds of the life insurance policy.

Facilities and Services Memorial Service $1700

Coordination and direction of funeral arrangements and service. Use of facilities and staff for a memorial service at

one of our Our Memorial Chapels. ** (per 2 hours)

Coordination of Veteran’s Honors Service (veteran’s DD214 will likely be required) $275

Includes coordination with VA National Scheduling for a committal service at a national cemetery, scheduling of

active duty military honors, and retrieval of the veteran burial flag.

Staff and Equipment for Graveside or Committal Service $275

Use of equipment and staff for a service at the place of final disposition within Dallas and Tarrant Counties

Facilities and Staff for Visitation or Viewing $275

Use of equipment and staff for a visitation or viewing at one of our Our Memorial Chapels. (per 2 hours)

Facilities and Staff for Identification Viewing at Crematory $275

Availability is subject to crematory’s schedule and staff. An ID Viewing must be done within 96 hours of death, is

available only to the legal next-of-kin, and is limited to 1 hour. This service may not be available after an autopsy,

organ donation, trauma, or for other reason which your funeral director can help you to understand.

Facilities and Staff for Witnessing Cremation at Crematory $375

Availability is subject to crematory’s schedule and staff. A Witnessing Service can only be performed after all

mandatory permits have been received by the funeral home, is limited to 12 guests (with all minors accompanied by

their legal guardian), and may include a brief viewing, if possible, prior to cremation.

Cremation Options If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be

made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are

made of cardboard and corrugated fiberboard.

Simple Cremation Package is available at our Fort Worth location only and we require consent from both parents for

Travel Protection Plan (for pre-paid contracts; special conditions may apply) $399 and up

When death occurs anywhere in the world more than 75 miles from one’s residence, the Travel Protection Plan will

facilitate return of the decedent’s remains; including transportation and arrangement of payment to an IEA qualified

service provider for transport to SimpleCremation.org. While our Travel Protection Plan provides Peace of Mind when

traveling on vacation, it also covers members when visiting family and friends or even for unforeseen medical care

away from home. This service is for pre-need (death has not occurred) clients. Special conditions may apply.

Services, merchandise, and third party charges provided as cash advance items will be billed at the same amount paid by the

funeral firm. They may include obituaries, permits, etc. In the event of the death, all of our fees, including services, merchandise

and cash advance fees, are due and payable at the time arrangements are made. State and local governments may charge fees or

taxes that are not included in the above quoted prices.

Forwarding of Remains $2040

This charge incudes our Basic Services of Funeral Director and Staff, receiving the deceased into our care within 35

miles, embalming, and professional care of the deceased, transfer of the deceased to a common carrier, and an air

tray or combination shipping container. This charge does not include airfare, required documents or fees, a casket,

merchandise, viewing, rites, or ceremony.

Receiving of Remains $2040

This charge includes our Basic Services of Funeral Director and Staff, receiving of the deceased from a common

carrier within 35 miles, temporary shelter of remains, and direct cremation. This charge does not include airfare,

required documents or fees, a casket, merchandise, viewing, rites, or ceremony.

Scattering of Cremated Remains $175

Non-recoverable scattering of cremated remains performed by SimpleCremation.org personnel in a designated area

permitted by local regulations. Transportation fees will apply if outside a 30-mile radius.

Expedited Cremation $200

Cremation process occurs 36 hours after permits and authorizations are secured

Situational Services SimpleCremation.org specializes in cremation only. Other funeral service items federally mandated.
