PROGRAM COMPONENTS & ELIGIBILITY

What is Go365? Go365 is a wellness program offered at no additional cost to you that helps you understand the status of your health and adopt a healthier lifestyle. It encourages you to take steps to improve your health by helping you set and achieve your goals such as eating better and increasing your physical activity. By participating in health-related activities that can be tracked and measured, such as taking wellness classes, exercising and getting regular medical check-ups and screenings, members earn Points which increase their Go365 status level. Members also earn a Buck for every Point earned, which they can redeem for products, services and discounts. Go365 is available to employees (and their covered dependents) enrolled in a medical plan offered through the Indiana State Personnel Department.

How do I access Go365? You can access Go365 at Go365.com or by searching for the Go365 mobile app in your mobile device app store.

How do I find out if I’m eligible to join Go365? Go365 is available to employees (and their covered dependents) enrolled in a medical plan offered through the Indiana State Personnel Department and does not apply to conservation officers, excise officers or Indiana State Police plan participants. Eligible employees will receive a Go365 identification card in the mail with additional information for logging in. If you have misplaced or not yet received your identification card, you can log in at Go365.com using your social security number, birthday and ZIP code.

I am a new employee. Can I participate? Any employee enrolled in a medical plan offered through the Indiana State Personnel Department is eligible to participate in Go365 and has the opportunity to achieve an Earned Status of Silver by August 31, 2018, to qualify for the Wellness CDHP Upgrade during Open Enrollment for 2018.

What if I’m hired after August 31, 2018? Employees hired and covered by a state medical plan after August 31, 2018, cannot qualify for the Wellness CDHP Upgrade for 2019. Employees are still encouraged to participate in Go365 and other Invest In Your Health programs, which can be found at investinyourhealthindiana.com.

Can I still participate in Go365 if I am not eligible for Upgrade this year? If you are enrolled in a medical plan offered by the Indiana State Personnel Department, you are encouraged to participate in Go365 throughout the year.

Why do I have to be a member of the health plan to participate? Go365 is a coordinated benefit within our current health plans; therefore, it is only available to employees (and covered dependents) who are enrolled in one of the four medical plans offered through the Indiana State Personnel Department. We encourage every employee to seek a healthy lifestyle and there are other resources available to employees who are not covered by one of our health plans. You can find these resources at www.investinyourhealthindiana.com.

Does my family have to participate in Go365 if I carry family medical coverage? While there is no requirement for your family to participate, it is recommended that they do. The Points needed to reach Silver Status are determined by how many 18 and older dependents you have on your plan. Each 18 or older dependent on your plan adds an additional 3,000 Points (in addition to the 5,000 required for the subscriber) in order for your family to reach an Earned Status of Silver. Having your family members participate helps you reach an Earned Status of Silver more easily.

Can my spouse and/or family participate? Yes, we encourage your spouse and covered dependents to participate in the program. Everyone helps the account holder achieve an Earned Status of Silver within Go365 to qualify for the 2018 Wellness CDHP Upgrade. This is a great way to get to Silver Status faster! Each dependent age of 18 and older has a separate account that is linked to the account holder.

How do I activate my Go365 membership? Activating your membership is simple. Once you receive your Go365 ID card in the mail, visit Go365.com to register. You must have your Go365 ID number or provide your birth date and social security number. Upon registration, you are prompted to complete the Go365 Health Assessment the first time you log in to your account. Complete the Health Assessment and you’re ready to begin!

How is my Go365 age calculated? The Health Assessment asks you questions about your current daily routines. How you answer these questions, along with the results of your biometric screening, factor into your Go365 age.

My Go365 age will not drop every time I achieve a goal or complete an activity, correct? Correct. Although your Go365 age is constantly recalculating as new health information is received, you may not see a change immediately when you complete a Recommended Activity. Don’t worry, as little improvements can lead to changes in Go365 age over time.

Are there other ways to engage in Go365 if a medical condition or disability prohibits me from participating in certain activities or completing Recommended Activities in the program? If it is unreasonably difficult for you to achieve the standards for the rewards under the Go365 program due to a medical condition, or if it is medically inadvisable for you to attempt to achieve the standards for the rewards under this program, please call Go365’s Customer Care Team at 1-800-708-1105 and they will work with you to develop another way to qualify for the reward.

I sent Go365 an email through the website but have not yet received a reply. Why is that? We advise that members call the Customer Care Team. Your email is sent to an open inbox that all Go365 members send to as well. Since there are millions of users, the inbox is constantly in flux in regards to response time. We apologize for this delay, but ask that you contact the Go365 Customer Care Team at 1-800-708-1105 to get the answer to your question. That is the quickest option, as we have a dedicated Customer Care Team for the state of Indiana.

What does ‘Earned Status of Silver’ mean? Earned Status of Silver means all points must be processed and posted to your Go365 account by the August 31, 2018, deadline. Don’t wait until the last minute to submit information, since processing of activities can take up to 45 days to be reflected in your account.

What is my incentive for upgrading to the 2019 Wellness CDHP? The 2019 Wellness CDHP will be similar in design and structure to the 2017 Wellness CDHP. More information will be released later in the year once rates have been determined.

Can smokers qualify for the 2019 Wellness CDHP Upgrade? Yes, employees using tobacco can qualify for the Wellness CDHP, as long as they meet all eligibility and qualification requirements. While we encourage all employees to abstain from tobacco products, the Non-Tobacco Use Agreement is not required for the 2019 Wellness CDHP eligibility. You can also earn Points by participating in Go365’s smoking cessation program. Additional assistance is also available through 1-800-QUIT-NOW.

What about pre-existing conditions? There has not been, nor will there be, a pre-existing condition clause in any of our health plan offerings, including the 2018 Wellness CDHP.

If I earned eligibility for the 2018 Wellness CDHP, do I need to do anything to keep it? Yes, you must achieve an Earned Status of Silver in Go365 by August 31, 2018, in order to qualify for the Wellness CDHP in 2019.

I’m a state employee. My spouse/parent is the medical plan holder, but will not be next year. Can I still qualify for the 2019 Wellness CDHP? If your spouse or parent is currently carrying family coverage through a medical plan offered by the Indiana State Personnel Department and you are a covered dependent, you may qualify for the Wellness CDHP for the 2019 plan year if you:

Go365 POINTS AND REWARDS

How do I earn Points? Points are earned by completing activities. Examples of activities include reaching or maintaining a healthy weight, exercising regularly, educating yourself about certain medical conditions or risks, and making smart lifestyle choices such as staying or becoming tobacco-free. To learn more about activities that can earn Points, click here.

How many Points can I earn per plan year? There is no limit to the number of Points you can earn during a plan year.

What is the difference between Points and Bucks? Points are earned when completing an activity. You are awarded one Buck for each Point earned. You spend Bucks in the Go365 Mall on exciting rewards.

I just completed an activity. I don’t see any Go365 Points. Why not? It may take up to 45 days after your activities are completed for points to be awarded and displayed on the website.

What is the benefit of earning more Points and improving my status? As you earn more points, you achieve higher status levels. By doing so, you receive Bucks that you can spend in the Go365 Mall. To qualify for the Wellness CDHP Upgrade for 2019, you must attain an Earned Status of Silver in Go365 by August 31, 2018.

Do Go365 Points expire at any time throughout the year? Points do not expire throughout the year. Points can be affected by family status changes. See the end of the document for examples of this.

Do my Go365 Points empty at the end of the Go365 calendar year? 10 percent of your earned Go365 Points carry over from the previous plan year as long as you end the Go365 plan year in Bronze status or higher and stay enrolled in the same plan and/or sponsor’s plan. Since each Point is worth one Buck, the equivalent number of Bucks are awarded for the next year and carry over, along with all of your other unspent Bucks earned within the last three (3) years.

How can I earn Go365 Points? All Go365 members can complete more than 30 activities to earn Points. If applicable, children under 18 can help the family earn up to 1,000 Points per child per year for various activities. Click here for a list of ways to earn Points.

What type of Recommended Activities does Go365 help me set? Go365 can help you create and achieve an array of important Recommended Activities such as reaching and maintaining a healthy weight, quitting smoking, exercising regularly, getting your cholesterol or blood pressure measured, and more. You’ll see that little changes in your day-to-day life can add up to huge rewards.

Do fitness trackers interact with Go365? Yes, certain fitness trackers interact with Go365. You must sync your device to your account in order to earn Points. View this chart for information on what type of information is collected from each device. If you encounter issues, please contact the Go365 Customer Care Team at 1-800-708-1105.

How do I connect a fitness device to my Go365 account? Click here for instructions on how to connect a fitness device.

Is it possible to achieve Silver Status without using a fitness device or having a gym membership? Yes, there are a variety of ways to earn Points in the Go365 program. Completing Recommended Activities, calculators, courses, conversations, and utilizing your health benefits (yearly health exams, dentist/vision appoints, yearly wellness exam, etc.). Each person/family’s path to an Earned Status of Silver is unique to them. For assistance with finding out how to earn Points, click here.

Is Health Coaching available through Go365? Yes, log into Go365 and click on “Quick Links” at the top of the screen, and then “Health Coaching”.

I’m having difficulty understanding how to earn Points. Where can I turn to for help? For assistance with finding out how you can earn Points, visit the “Need Help” page on the Invest In Your Health website: http://www.investinyourhealthindiana.com/upgrade/help/. If you still are encountering issues, contact the Go365 Customer Care Team at 1-800-708-1105.

How do I self-submit proof for various activities I complete? You can submit proof for activities that you complete on the same page that you began the activity. A step-by-step process can be found here. You can also submit photo proof through the Go365 app. To do this, just log in and go to the activity that you completed. You will be prompted to take or find a photo in your photo library. You must submit the forms or other kind of proof within 90 days of service or final day of activity.

What types of perks can I get with my Bucks? Use your Bucks at the Go365 Mall for rewards like movie tickets, name-brand electronics, travel gift cards, fitness devices, and more.

What is the Go365 Mall? The Go365 Mall is where you redeem your Bucks for exciting items like gift cards, fitness devices, electronics, and more.

Do my Bucks expire at the end of the plan year? Your unspent Bucks carry over year-to-year, but you must redeem them within 3 plan years. Your Bucks expire if you are no longer covered by your plan, or if you end the plan year in Blue Status.

How many Bucks can I be awarded per year? The maximum Bucks that you can earn per plan year is 30,000 per adult. At the end of the plan year, any unspent Bucks will carry over to the next year.

If I spend my Bucks do I lose my Points? No, when you spend your Bucks it does not reduce your Points.

Why is my Bucks purchase being taxed? In accordance with IRS Publication 15-B, Bucks purchases in the Go365 Mall are considered taxable fringe benefits that are included on the employee’s W-2 and subject to federal tax withholdings, as well as Social Security and Medicare taxes. After you redeem your reward from the Go365 Mall, the retail/taxable value of that reward is added to a subsequent paycheck as additional gross income. Payroll will withhold the appropriate amount of tax for that reward corresponding with your personal tax elections, which will result in a net reduction in your pay.

Go365 HEALTH ASSESSMENT

What is the Go365 Health Assessment? The Health Assessment is an online questionnaire that helps you evaluate how healthy you are now, determine future health risks, and help you decide where and how to make improvements. Based on your answers, along with the results of your biometric screening, Go365 calculates your Go365 age and gives you access to your health results.

Why should I complete the Health Assessment? To take advantage of all that Go365 offers and create a personalized experience in the program, you need to complete your Health Assessment. In addition, you earn Points for completing the Health Assessment. You can earn up to 1,250 Points for completing your Health Assessment, which translates to 1,250 Bucks that you can begin spending in the Go365 Mall. NOTE: Due to 2016 federal EEOC regulations, wellness programs (including Go365 and Invest In Your Health) cannot incentivize children to provide medical information. This means children including adult dependents (ages 18 and older) will not have Points or Bucks awarded for completing their Health Assessments and biometric screenings. It is recommended they still complete these activities, because both the Health Assessment and biometric screening as these activities help create a personalized experience in the program. Please keep in mind, spouses continue to be awarded Points and Bucks for completing their Health Assessment and biometric screenings as the 2016 federal EEOC regulation only impacts children including adult dependents (age 18 and older).

Is the information I provide to Go365 secure? Data collected by Go365 is strictly protected by state and federal privacy laws.

How long does the Health Assessment take to complete? The Health Assessment is web-based and takes about 15-20 minutes to complete if done in one session. It is also broken into segments that can be completed in about five (5) minutes per segment. You can also complete this on the Go365 mobile app.

What if I don’t have my lab results yet to enter into the Health Assessment? All you need to get started in the Health Assessment is your height and weight. You can update the report once you have your biometric screening results.

Can I complete the Health Assessment on my mobile phone? Yes. Download the free Go365 app from either the iPhone or Android stores and log in with your registered username and password.

How does the biometric screening work in conjunction with the Go365 Health Assessment? You might not know the answers to every Health Assessment question, so having a biometric screening provides Go365 with your body measurements and blood test results. Your biometric screening and Go365 Health Assessment results are summarized in your personal Health Report that provides the current state of your health, and these factors combine to determine your Go365 age. NOTE: Because of 2016 federal EEOC regulations, wellness programs (including Go365 and Invest In Your Health) cannot incentivize children to provide medical information. This means children including adult

dependents (ages 18 and older) will not have Points or Bucks awarded for completing their Health Assessments and biometric screenings. They can still earn Points in other ways, but because of these regulations, they cannot receive Points for those two activities. It is recommended that adult dependents still complete the Health Assessment and biometric screening, as these are essential tools used to understand their health status. As a reminder, each additional adult dependent on your plan (age 18 and older) adds 3,000 Points to the amount you need to reach to qualify for the 2018 Wellness Consumer Driven Health Plan (CDHP). If you are the only person on your plan, you need 5,000 Points to attain Silver Status. If the plan includes just you and an adult dependent, you need to earn 8,000 Points. If the plan includes you, a spouse and an adult dependent, you need to earn 11,000 Points. Please keep in mind, spouses continue to be awarded Points and Bucks for completing their Health Assessment and biometric screenings. The 2016 federal EEOC regulation only impacts children including adult dependents (age 18 and older). BIOMETRIC SCREENING

What is a biometric screening? A biometric screening is similar to a yearly wellness exam that gives you a better understanding of your health by measuring your blood glucose (sugar), blood pressure, Body Mass Index (BMI) and cholesterol. It provides an assessment of an individual's basic health status and offers valuable insight into possible health risks.

Why should I have a biometric screening? Living a long and healthy life is nothing to take for granted. National research shows that many Americans are unaware of their health status. When individuals reported their level of risk for certain types of disease—like hypertension and diabetes—only 11 percent believed they were at risk. But after a health screening, nearly 45 percent learned they were at risk. Knowing a few simple numbers that represent your disease risk is the first step toward living a longer, healthier life.

Where can I schedule my biometric screening? Biometric screenings are scheduled at state facilities throughout Indiana. At these clinics, there is no charge to employees, spouses and their covered dependents age 18 or older, who carry a medical plan through the Indiana State Personnel Department. If you attend one of these clinics, your results are sent automatically to Go365 for Points. If you are unable to attend a state clinic or a clinic is not scheduled for your work location, a biometric screening can also be completed for free at partnered locations around the state or at your primary care physician’s office. You must bring your Go365 ID card regardless of where you get your biometric screening. Visit the biometric screening page on Invest In Your Health for more information on scheduling your biometric screening. At the link you can learn how to find partnered locations near you and the appropriate voucher that you must take with you to the screening. How much does the biometric screening cost at state-sponsored events? At the state-sponsored biometric screening events, there is no charge to employees with a medical plan provided by the Indiana State Personnel Department or their covered dependents age 18 or older.

Can my spouse/dependent attend the state-sponsored biometric screening events? Most free biometric screening events are open to state medical plan subscribers and their covered dependents age 18 or older. Some events are open only to employees due to security concerns. Sites are classified as “Open” or “Closed” on the event schedule.

Where can I find the biometric screening event schedule? Biometric screenings are March through June. More information can be found on our website: http://www.investinyourhealthindiana.com/upgrade/go365-biometric-screening/

Why is my office not having a state-sponsored biometric screening event? We are trying to reach the greatest number of employees, while also representing most agencies and geographical areas. If you are unable to attend a state clinic or a clinic is not scheduled for your work location, a biometric screening can also be completed for free at partnered locations around the state or at your primary care physician’s office. Visit the biometric screening page on Invest In Your Health for more information on scheduling your biometric screening. For more information about this process, see the following questions.

What if I can’t attend a state-sponsored biometric screening event? If you are unable to attend a screening event, there are several other partnered locations where you can get a biometric screening, or you can get a screening at your primary care physician’s office. Instructions for both of these scenarios can be found on the bottom of the biometric screening website.

Can I do my biometric screening on work time at a state-sponsored screening event? If a biometric screening event is occurring at your location, it can be scheduled during work time. You must receive supervisor approval to attend. You are allowed up to 30 minutes paid time to complete the test. Any additional time incurred is your responsibility. Supervisors are encouraged to accommodate these requests when possible.

Where is the biometric screening form to take to my doctor? The biometric screening form is available in your Go365 account. Specific instructions for locating this form can be found here.

What if the doctor charges me to complete the biometric screening form? If your physician charges you to complete the form, you can either pay the fee, visit a Go365 partnered clinic, or attend one of the onsite screenings held at a state facility. HSA funds cannot be used to cover the cost of the form being completed by the physician.

What if I have already been to my doctor this year? If you have completed biometric testing (height, weight, waist circumference and blood test including; total cholesterol, BMI, HDL, LDL, triglycerides, fasting blood glucose) at your doctor’s office within the last 90 days, you can upload the results of your biometric testing from your physician’s office on Go365.com.

Should I keep a copy of the biometric screening completed form? Yes! To ensure you receive your Points for a completed biometric screening, ask for a copy of the completed form. If I attend a state-sponsored biometric screening at a state location, do I need to submit any form to Go365? No. The state has arranged for the company completing the biometric screenings to submit your results directly to Go365 and not shared with the state. It can take up to 45 days for your points to be reflected in your account.

What happens if my biometric screening and/or nicotine test results were in-range one plan year and out of range the next plan year? If you received in-range test results in one plan year, but your results are out of range during the next plan year, you will still receive Points for Prevention and in-range Healthy Living as if you had your biometric screening and/or nicotine test in the next plan year. Points for in-range results (excluding BMI) carry over for one plan year. BMI must be checked annually. New activities are recommended when you submit updated biometric screening results. NOTE: Because of 2016 federal EEOC regulations, wellness programs (including Go365 and Invest In Your Health) cannot incentivize children to provide medical information. This means children including adult dependents (ages 18 and older) will not have Points or Bucks awarded for completing their Health Assessments and biometric screenings. What is a Wellness Champion? A Wellness Champion is a volunteer who serves as an ambassador of wellness to his/her respective agency/work location, through the promotion of, and active participation in, Invest in Your Health wellness programs. These individuals lead by example through practicing healthy behaviors and supporting the state’s wellness initiatives. For a more detailed description of the Wellness Champion’s responsibilities, please view the Wellness Champion Description.
