Please join us for the 4th Annual Golf Outing to raise money for Mission Trips, Youth Programming, and

Fellowship Athletics. The particulars are below. Have a blessed summer and hope to see you there!

Format: Four Person Scramble

Location: Fairways Golf Club

Address: 750 Country Club Lane, Warrington, PA

Registration: Thru September 4, 2015 (Email Steve for the registration form)

Cost: $380/new foursome or $360/returning foursome

Sign In: Starting at 7:30 am

(Putting green open at sign in time, no driving range at course)

Shotgun Start: 8:30 am

Payment Types: Cash, Check, Money Order, or PayPal. All entrance fees are due by the registration date listed above (no excep-

tions). The first three types of payments can be mailed to 1289 Morgan Horse Dr, Warrington, PA 18976 and the email address

above is the listing for all PayPal payments. Be sure to fill out the registration form on the back/also attached and either email it to

Steve or attach it in the envelope/as a message at the bottom of your Paypal order. Steve will need an email address for at least one

member of each team so please be sure to include it with your registration.

Includes: Greens Fees, Golf Cart, St. Andrew’s UMC Golf Umbrella to first 32 members signed up and paid, Lunch/Dinner after the

outing, Water & soda, Hot Dog at the turn, Trophies for 1st, 2nd, & 3rd Place as well as charitable donations to the team’s choice of

church or charity, Prizes for Longest Drive, Closest to the Pin, Hole-In-One Challenge, and Door.

Extras: Skins Challenge for $20/team (all money will be distributed day of/none of this will be kept by us), Purchase of team Mulli-

gan Package ($20), and Silent Auction Items (Cash or check with valid PA license only)
