I. Educational Program Curricula 1. Halifax Community College (HCC) will offer initial Emergency Medical Services (EMS) classes from first responder to EMT-Paramedic. HCC will also offer all continuing education required by agencies in their service counties for these levels. In addition HCC will offer as needed and able, classes for MICN, EMS PA, EMS NP, Level I & II EMS Instructor. HCC will offer EMD continuing education programs as needed. If needed and able, HCC will offer initial EMD classes under the requirements of the National Academy of Emergency Dispatchers (NAED) or comparable national certification. All of HCC programs will fall under certificate programs (continuing education). HCC will offer the classes as indicated on the NCOEMS Institution Application form attached as attachment A. 2. HCC currently offers most of its EMS classes in a traditional classroom method. Classroom teaching methods will include the use of lectures, labs, multimedia and other classroom techniques. As needed some classes maybe taught in nontraditional methods including distance education, information highway, hybrid course and any other format that would benefit the student without compromising the quality of the course. Any distance education class offered for initial certification by NCOEMS will follow the “Online Learning Course Readiness Checklist” as outlined by NCOEMS (attachment H) and provide for appropriate review by NCOEMS personnel. The only Certification program currently being offered in a distance education format is the instructor’s methodology course. This class has gone before the NCOEMS and NCOSFM twice and has been approved both times including the latest updated version required by the state using the 7th edition IFSTA text. Sample syllabus for the instructor course is provided in attachment G. 3. Program Descriptions initial and continuing education. All initial classes will meet the minimum hour requirements as set forth by NCOESM. Hours will be disseminated as the lead instructor deems necessary based on the student’s overall performance in class with each topic. Note, requirements for each of these classes can be found at ncdhhs.gov/dhsr/ems/curricul.htm. A. Medical Responder Initial Course ID: EMS3025 Title: MEDICAL RESPONDER INITIAL Recommended Hours: 69 Maximum Hours: 83 Certification: This course must follow the guidelines established by the NC Office of EMS. The Office of EMS must approve courses that include portions delivered via distance education. The curriculum for the MR educational program shall be the 1995 United States Department of Transportation (US DOT) National Highway Traffic Safety Administration (NHTSA) First

Responder Course: National Standard Curriculum. The MR program must be conducted by an approved Basic or Advanced Educational Institution as defined in the rules of the NC Medical Care Commission. The lead instructor for the MR educational program must be a NC credentialed Level I EMS Instructor at the EMT, EMT-Intermediate or EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. Student Prerequisites: High School diploma or general education development (GED); or successful completion of an entrance exam assessing basic reading comprehension skills at a minimum at the tenth grade level. B. EMT-B Course ID: EMS3044 Title: EMERGENCY MEDICAL TECHNICIAN BASIC INITIAL Recommended Hours: 169 Maximum Hours: 203 Certification: This course must follow the guidelines established by the NC Office of EMS. The Office of EMS must approve courses that include portions delivered via distance education. The curriculum for the EMT educational program shall be the 1994 release of the United States Department of Transportation (US DOT) National Highway Traffic Safety Administration (NHTSA) EMT-Basic Course: National Standard Curriculum and the Supplemental Airways Modules for EMT-Basic; NSC. The EMT program must be conducted by an approved Basic or Advanced Educational Institution as defined in the rules of the North Carolina Medical Care Commission. The lead instructor for the EMT educational program must be a NC credentialed Level I EMS Instructor at the EMT, EMT-Intermediate or EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. C. EMT-I Course ID: EMS3045 Title: EMERGENCY MEDICAL TECHNICIAN - INTERMEDIATE INITIAL Recommended Hours: 256 Maximum Hours: 307 Certification: This course must follow the guidelines established by the NC Office of EMS. The Office of EMS must approve courses that include portions delivered via distance education. The curriculum for the EMT-Intermediate educational program shall be objectives identified from the 1998 Release of the US Department of Transportation (US DOT) National Highway Traffic Safety Administration (NHTSA) EMT-Paramedic Course: National Standard Curriculum. The EMT-Intermediate program must be conducted by an approved Advanced Educational Institution

The lead instructor for the EMT-Intermediate educational program must be a NC credentialed Level II EMS Instructor at the EMT-Intermediate or EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. Student prerequisites: successful completion of an EMT-Basic course; high school diploma or general education development (GED); successful completion of an entrance exam assessing basic reading comprehension and English skills at a minimum at the post-secondary grade level; demonstration of math skills at a minimum at the high school grade level. Recommended hours: 160 didactic; 48 clinical; 48 field internship. D. EMT-P Course ID: EMS3031 Title: EMERGENCY MEDICAL TECHNICIAN - PARAMEDIC INITIAL Recommended Hours: 1096 Maximum Hours: 1315 Certification: This course must follow the guidelines established by the NC Office of EMS. The Office of EMS must approve courses that include portions delivered via distance education. The curriculum for the EMT-Paramedic educational program shall be the 1998 Release of the United States Department of Transportation (US DOT) National Highway Traffic Safety Administration (NHTSA) EMT-Paramedic Course: National Standard Curriculum. The EMT-Paramedic program must be conducted by an approved Advanced Educational Institution as defined in the rules of the NC Medical Commission. The lead instructor for the EMT-Paramedic educational program must be a NC credentialed Level II EMS Instructor at the EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. Student prerequisites: successful completion of an EMT-Basic course; high school diploma or general education development (GED); successful completion of an entrance exam assessing basic reading comprehension and English skills at a minimum at the post-secondary grade level; demonstration of math skills at a minimum the high school grade level. Recommended hours: 596 didactic; 200 clinical; 300 field internship. Pre- or Co-requisite course EMS 3000 Anatomy and Physiology. E. EMD Course ID: EMS3091 Title: EMERGENCY MEDICAL DISPATCHER Recommended Hours: 24 Maximum Hours: 29 Certification: Y This course must follow the guidelines established by the NC OEMS. The OEMS must approve courses that include portions delivered via distance education. The curriculum for the EMD program shall be the 1996 Release of the US DOT National Highway Traffic Safety Admin. Emergency Medical Dispatcher: NSC. The EMD program must be conducted by an approved

Basic or Advanced Educational Institution as defined in the rules of the NC Medical Care Commission. The lead instructor for EMD educational programs must be a NC credentialed Level I Instructor at the EMD level as defined by the NC Medical Care Comm. Student prereq: high school diploma or GED; or successful completion of an exam assessing basic reading comprehension at a minimum 10th grade level. Although not required, a minimum of one year of experience in an emergency communication center or completion of a recognized basic telecommunications educational program consisting of a minimum of 40 hours is preferred. Although not required, it is recommended that students have completed a CPR course meeting the requirements of the American Heart Association, American Red Cross or equivalent Basic Life Support professional rescuer level. This requirement is mandatory for EMD credentialing and would be helpful to the student in completing the program. F. Level I EMS Instructor Course ID: FIP3805 Title: EDUCATION METHODOLOGY Recommended Hours: 63 Maximum Hours: 76 Certification: Y This course will provide instruction for personnel who wish to teach fire and EMS subjects. The course material is the IFSTA Fire and Emergency Service Instructor Program. Instructors for this class must be qualified by a committee of three representatives from OSFM, OEMS and NCCCS. This course meets the objectives of NFPA 1041 Level II Standard. G. EMS Con-Ed EMS Con-Ed will be offered as requested by the county or local EMS systems. The counties will set the schedules and class topics per NCOEMS, DOT and county policy objectives. The county having jurisdiction will have an individual(s) sanctioned by HCC to sign for the re-credentialing of personnel practicing in their jurisdiction. The county having jurisdiction will maintain training records for re-credentialing purposes of those individuals practicing medicine in their system. A verification form (attachment O) of all persons re-credentialed by the county sanctioned representative will be sent to HCC and kept on file detailing the physical location of re-credentialing paperwork. HCC will be the only authorized agent to sign for re-credentialing of persons not affiliated with either Halifax or Northampton county EMS systems. 1. The EMT program must be conducted by an approved Continuing Education, Basic or Advanced Educational Institution as defined in the rules of the NC Medical Care Commission. The lead instructor for the EMT educational program must be a NC credentialed Level I EMS Instructor at the EMT, EMT-Intermediate, or EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. 2. EMT INTERMEDIATE Course ID: EMS3055 Title: EMT INTERMEDIATE - CONTINUING EDUCATION Recommended Hours: 36 Maximum Hours: 43 Certification: This course must follow the guidelines established by the NC Office of EMS. The objectives that are to be used in developing each lesson must come from the 2001 Release of the US DOT National Highway Traffic Safety Administration (NHTSA) EMT-Intermediate: NSC Refresher Course. The EMT-Intermediate program must be conducted by an approved Continuing Education, Basic or Advanced Educational Institution as defined in the rules of the NC Medical Care Commission. The lead instructor for the EMT-Intermediate educational program must be a NC credentialed Level I EMS instructor at the EMT-Intermediate or EMT-Paramedic level as defined in the rules of the NC Medical Care Commission. 3. EMS-P Course ID: EMS3060 Title: EMS-P - CONTINUING EDUCATION Recommended Hours: 36 Maximum Hours: 43 Certification: This course must follow the guidelines established by the NC Office of EMS. The objectives that are to be used in developing each lesson must come from the 2001 Release of the US DOT National Highway Traffic Safety Administration (NHTSA) EMT-Paramedic: NSC Refresher Course. The EMT-Paramedic program must be conducted by an approved Continuing Education, Basic, or Advanced Educational Institution as defined in the rules of the NC Medical Care Commission. The lead instructor for the EMT-Paramedic educational program must be a NC credentialed Level I EMS Instructor at the EMT-Paramedic level as defined in the rules of the NC Medical Care Commission.

4. EMD EMD continuing education will be offered by HCC as requested by the county systems having jurisdiction. The county(s) will be responsible for setting class schedule and topic list. Topics will be set according to NCOEMS and county policies as well as any other legally mandating authority. The county having jurisdiction will have an individual(s) sanctioned by HCC to sign for the re-credentialing of personnel practicing in their jurisdiction. The county having jurisdiction will maintain training records for re-credentialing purposes of those individuals practicing medicine in their system. A verification form (attachment O) of all persons re-credentialed by the county sanctioned representative will be sent to HCC and kept on file detailing the physical location of re-credentialing paperwork. 4. Content for each initial course will come from the above listed guidelines. Enhancement and enrichment of content will vary from class to class and will be decided by the lead instructor and program coordinator(s) based on student’s past successes and failures and local needs. 5. All continuing education programs will be requested by the county system having jurisdiction and delivered through HCC to meet the requirements of the county and of their individual system plan. All continuing education will be done with input from the county system and representing agencies. 6. Structure of classes will be done to meet the minimum requirements of all regulating agencies. A. Didactic sessions will cover all of the required objectives and will meet at least the minimum time requirements of governing agencies. This most often will be regulated by NCOEMS documents or referred to DOT curriculum guides. B. Skills for all levels will be done through scope of practice evaluations on a pass or fail basis. Evaluators will conduct these skills. Skills will be assessed on a regular basis so that the students will be able to pass their final skill scope of practice evaluation either for initial certification or recertification by NCOEMS. Skills will be presented in didactic or lab settings of class. Skills will be taught in an expanding methodology consist with teaching of psychomotor skills from demonstration phase to testing phase. Skills will be evaluated by using check off sheets provided by textbook publishers. If the publisher does not provide necessary skill check off sheets the instructor may use comparable sheets such as National registry. Skills will be taught and evaluated by instructors approved by the lead instructor for the course. The end goal of the skills is for the student to be able to pass TSOPs and be able to function in the field appropriately. Sample TSOP forms are provided (attachment D). C. Clinical instruction for all levels will be conducted by proper preceptors. All skills/hours will be on a pass or fail basis. Hours and skills required will meet the minimum requirements of all governing agencies.

D. Field instruction for all levels will be conducted by proper preceptors. All skills/hours will be on a pass or fail basis. Hours and skills required will meet the minimum requirements of all governing agencies II. Organization 1. Institution Application form is attached as attachment A. 2. General Institutional Information A. Halifax Community College is a two-year college accredited by the Southern Association of Schools and Colleges. HCC provides EMS education through the Continuing Education Department and meets the guidelines specified by NCOEMS. HCC is a NCOEMS approved Advanced Delivery Agency. Areas of EMS interest might include our affiliation with American Safety and Health; we are a training center for their BLS programs. We have instructor affiliations, which allow us to offer American Heart Association ACLS and PALS courses as well as affiliations to offer PEPP, BTLS/ITLS, GEMS courses and other EMS related courses. B. HCC’s program area consists of all of Halifax County and the western part of Northampton County. We also have a collaborative agreement with Roanoke Chowan Community College to offer EMS classes at the 911 Center in Jackson, Northampton County. Note that classes held in both counties by HCC follow the same schedule. C. HCC has an open door admissions policy; we serve anyone who comes to us in pursuit of education. We serve two county EMT-P agencies (Halifax County EMS & Northampton EMS), one EMT-I (Gaston EMS) and several BLS agencies (Roanoke Valley Rescue Squad, Eastern Medical Transport, Warren County Volunteer Rescue at Littleton, and first responders operating with Halifax County). D. The mission of HCC is: Halifax Community College is a two-year educational institution with an open door admissions policy. The purpose of the college is to improve the quality of life and the educational and economic base of the service area. The college prepares adults for the workforce through certificate, diploma and associate degree programs as well as lifelong learning opportunities. The college strives to meet the needs of the student population, business community, area industries, and public agencies by providing low cost, geographically accessible education at convenient times. E. We feel that HCC is qualified to offer EMS classes, including Paramedic and Emergency Medical Dispatcher based on our staff’s, resources and abilities as detailed in this plan. F. HCC has been and will continue to meet the educational needs of our community, including the field of EMS. Currently we are the only EMS delivery agency within our service area. Within this service area, we provide EMS training at various levels for twenty different departments plus local industry.

III. Financial Resources Halifax Community College is a state funded institution and will abide by rules set forth by the Department of Community Colleges. Students who are affiliated with a fire, EMS or law enforcement agency will be fee exempt according to the rules of the North Carolina Community College System (NCCCS) for classes through our Continuing Education’s occupational educational department. Non-affiliated students will be charged for classes according to the Halifax Community College fee schedule as set by the NCCCS. IV. Facilities 1. Halifax Community College will use adequate training facilities for each class and activities it sponsors. Classes will be held both on campus and off campus. On campus consists of multiple, adequate classrooms and training environments ranging in size from 15 students to over 200 students. There are physical labs and computer labs available as well. Halifax Community College does have satellite campuses in Enfield, Scotland Neck and Littleton that are available for classes. Many classes are taught off campus at local fire or EMS stations. These settings are always checked for adequacy by the program coordinator(s) and lead instructor prior to initiating class. If a sight is deemed inappropriate, then a more adequate location for class will be used. A list of normal sites is listed in attachment M. A. Halifax Community College complies with all applicable fire, safety and health requirements and undergoes periodic inspections by authorities having jurisdiction as well as in house evaluations on a minimum of quarterly. Student’s health and safety at these sites are also continuously evaluated by the instructor during classes. B. All settings used by Halifax Community College for classes will be appropriate for the class to be offered along with appropriate lab areas or other needed support areas. C. Classroom and lab assignments will be made by the program coordinator(s) and cleared through Halifax Community College’s business office where they are tracked electronically. V. Equipment and Supplies 1. Halifax Community College will provide or make available all equipment needed for classes being offered by the institution. The college will keep at minimum the equipment required by NCOEMS for teaching institutions on hand or available through contract with HCEMS. Other equipment will be purchased or made available as requested by lead instructor as budget allows. If the proper equipment is not available for a particular class, then Halifax Community College will either sub-contract the class with an entity able to provide equipment needed or will not

offer the class until such time appropriate equipment can be arranged. For sample list of equipment see attachment L. A. Halifax Community College will provide all appropriate equipment needs for classes offered through the institution as stated above. Program coordinators and instructors will review equipment for appropriateness on a regular basis. This will be determined based on its condition, teaching merit and use by local responders in the field. B. Equipment can be checked out from the program coordinator(s) or their representative. At least a three working day notice prior to the need is expected for all requests from the instructor to the school. This will help to ensure that the needed equipment is available for each class need regardless of how many classes are being taught simultaneously. Instructors will be responsible for the equipment while it is checked out to them and is to be returned in a timely manner. Instructors will be responsible for cleaning and up keep of equipment used or contacting the program coordinator(s) with issues in this area. The equipment checkout log (See attachment P) will be reviewed by the program coordinator(s) on a regular basis to keep track of equipment whereabouts. Audio-visual equipment as well as reference material can be checked out through the college library system. Instructors must inform program coordinator(s) of special equipment needs in a timely manner to allow for procuring of special equipment. This system will help with coordination issues when multiple classes are in progress ensuring all classes will have access to the equipment they need. C. Equipment will be made available in appropriate numbers for proper class participation either through procurement of extra equipment, limited class size or the use of team teaching techniques. Equipment will also be made available for open labs and remediation when needed. Notice of equipment needs should be sent to the college at least three working days prior to the date of the need. D. Halifax Community College owns the majority of equipment that is listed on the educational equipment and supply list provided by NCOEMS. Equipment that HCC doesn’t own will be accessible from Halifax County EMS as needed. Equipment use will be coordinated through any of the county EMS ambulance supervisors. Borrowed equipment will include: wheeled cot, stair chair, pediatric immobilization devices, Braslow tape, pediatric immobilization devise, and car seat (See attachment L). VI. Educational Medical Advisor 1. Dr. Westover will serve as HCC’s educational medical advisor. (see Attachment C) Dr. Westover already serves as the county Medical Director for both of the counties we service. A. Appointment and selection of this appointment will be based on applicants’ qualifications as required by NCAP and applicants familiarity with the local system. This appointment will be a part-time as needed position until a time that either party decides a change is needed.

B. The medical advisor will advise HCC’s EMS program for scope of practice and quality assurance of our students. This may include but is not limited to field and clinical internships, didactic sessions, student issues or other pertinent issues within the HCC’s program. While the medical advisors opinions will be highly revered, HCC does not expect the medical advisor to be involved in course development or the day to day operations of the program unless he/she would like to function in these capacities in any way. This person will have authority as directed in 10A NCAC 13P.0106. VII. Program Coordinator 1. The lead program coordinator will be HCC’s EMS Coordinator, Kevin Kupietz. Kevin is an EMT-P and Level II EMT-B instructor with more than ten years experience in the position. (See attachment Q) Intermediate and Paramedic programs will be co-coordinated by a Halifax County EMS Level II Paramedic Instructor; at this time this person will be Phil Ricks. (See attachment B) A. The above positions will be recruited and selected based on standard hiring process of the representing agency. The personnel or combination of personnel will meet the qualification requirements set forth by NCOEMS. These appointments will be made on part or full time basis as deemed necessary to meet NCOEMS requirements and program demands. These appointments will remain intact for the duration of that person’s employment or until job responsibilities have been reassigned to other qualified person(s). B. The Dean of Continuing Education will supervise the Program coordinator(s). The program coordinator(s) will have the supervision responsibility of all instructional staff and all courses within the EMS field. C. The program coordinator(s) will be responsible for: overseeing all program development, course offerings, record keeping, student development and staff development. The program coordinator(s) will also be responsible for all staff hiring, program evaluation and administration. D. The program coordinator(s) will also be responsible for the 15 required responsibilities of a program coordinator. These include: Ensuring OEMS requirements are met, maintaining educational plan, maintaining facilities and equipment, maintaining educational programs, remediation and student development services, ensuring up to date curriculums, maintaining proper field experiences, maintaining appropriate agreements and contracts, ensuring a quality assurance program in place, ensuring for instructor and preceptor qualifications, evaluations and upgrades, reviewing critical success factors and applying to the program, evaluating student performance and developing standardized student evaluation processes. Note, the lead program coordinator will be overall responsible for these 15 responsibilities. The intermediate/paramedic coordinator will only be responsible for overseeing the ALS aspects of the 15 responsibilities listed above. This will include ensuring ALS instructor and preceptor qualifications and evaluating ALS instructors.

VIII. Instructional Faculty 1. Faculty shall be determined by the program coordinator(s) as needed for courses offered. E. Level I instructors will be used to teach a variety of classes including continuing education. Depending on the course being taught, the Level I instructor will be supervised either by a level II instructor and/or program Coordinator(s). Level I instructors must meet NCOEMS qualifications. All instructors will undergo orientation with one of the program coordinators prior to teaching on their own. Orientation will minimally consist of: college policies, student relations, paperwork, instructor expectations, problem resolution. (See attachment N) F. Level II instructors will be able to instruct any EMS related class that they are individually qualified to instruct. The program coordinator(s) will supervise the level II instructor. Level II instructors must meet NCOEMS requirements. All TSOPS must be under the direction of a level II instructor at or above the rate of those being tested. It is preferred that the level II be on site but if this is not possible the level II must be able to be contacted by phone during the TSOPS. All instructors will undergo orientation with one of the program coordinators prior to teaching on their own. Orientation will minimally consist of: college policies, student relations, paperwork, instructor expectations, problem resolution, supervising others. (See attachment N) G. Skill Evaluators will be used from qualified EMS personnel as decided by the program coordinator(s). Skill evaluators will be required to have an orientation supervised by the program coordinator(s) prior to being able to sign off on students. The evaluator will be at or above the level of the student being evaluated. The evaluator will have at least two years EMS experience. Orientation will minimally consist of: college policies, student relations, paperwork, instructor expectations, problem resolution, Evaluation techniques, and communications with lead instructor. (See attachment N) H. Clinical/Field Preceptors will be named based on their seniority, competency level and willingness to work with students. Preceptors will maintain positive control of their students at all time, log the students’ time and allow the student only to perform the skills which the student is eligible to perform. All clinical and field preceptors will be approved by the authority having jurisdiction. They must have a minimum of two years experience in their field. All preceptors must be orientated before being allowed to function as a preceptor. Orientation will minimally consist of: college policies, student relations, paperwork, instructor expectations, problem resolution, proper evaluation techniques, patient control, student control and communications with lead instructor. (See attachment N) I. Specialty course instructors will be hired on an as-needed basis. These instructors will be expected to be course material experts and will be supervised by the program coordinator(s). These instructors must meet the requirements of NCOEMS for the classes that they are going to instruct.

J. Adjunct instructors may be used for EMS classes but will be supervised by appropriate instructor or program coordinator(s). K. A lead instructor will be placed in charge of each credentialed EMS initial course and permanent continuing education site. The lead instructor will be selected based on qualifications and past evaluations. The lead instructor must meet all applicable NCOEMS requirements. The lead instructor will keep the program coordinator(s) informed of class progress and any problems that may arise. The lead instructor will also be responsible for the 9 NCOEMS expectations for a lead instructor which includes using current educational objectives, ensuring students meet all applicable requirements, maintain involvement in the class, monitor other instructors in the class, resource coordination, evaluate student progress including remediation, and provide student support and maintain course program and records. 2. Appointment of all instructional staff will be on a part time, contractual basis with HCC. The appointment will be based on qualifications, teaching ability, recommendations etc. Appointment will be for the time of contract only. 3. All instructional staff will be responsible for the following: Maintaining their own teaching credentials, researching and providing topic outline for topic/class they will teach, arranging for all needed materials for topic to be taught, Cleaning and returning all equipment used for presentation, holding class or being available for contracted time, maintaining professional atmosphere in class at all times, maintaining all required records and are responsible for student safety at all times. Faculty behavior will be positive and professional at all times. The instructor will always set the good example for the students to follow. Instructor appearance will be neat, clean and dressed appropriately for the topic to be taught. All lead instructors must provide students with numbers where he/she can be reached for extra help or notices. All instructors are responsible for maintaining their own teaching credentials and are required to notify the program coordinator(s) of any change in those credentials as soon as possible. 4. Appointment qualifications for faculty will be determined by the program coordinator(s) on a case-by-case basis based on the topic to be taught, instructor evaluations, skills and other factors that will lead to the success of the class. Any class offered falling under the realm of NCOEMS or any other certifying body will have an instructor staff that meets the requirements of that particular governing body. This institution will endeavor to contract with the best instructor for the class possible. It will be the instructor’s responsibility to provide all appropriate proof of qualifications to be maintained in their personnel file. 5. Faculty to student ratios will be looked at as needed for individual classes. Some factors that will be used to determine ratio will be regulating agencies (i.e. AHA or PADI), strengths or weaknesses of the instructor or of the students, time frame to cover topic, complexity or danger of the topic or skill, class room size or environmental concerns. Consideration will be given to instructors, who express concern about the need for multiple instructors. In a preceptor environment no one preceptor can be responsible for more than two students at a time. General rule will be no more than 15 students per instructor for skills and no more than 50 students in didactic sessions without approval from one of the program coordinators. TSOPs will be done on

a one to one ratio. There may be up to 3 students in the room participating in the scenario but only one student is being evaluated at a time. 6. The program coordinator(s) should monitor all instructors at least on an annual basis. From these evaluations, student evaluations and test results it will be decided if the instructor is performing to college expectations. If he/she will be considered for contracting with other classes. If he/she had areas of opportunity for improvement, he/she will be worked with on these areas and allowed to teach on a limited basis until he/she is to the level of expectation. If after providing remedial assistance, the program coordinators agree that the instructor does meet HCC’s standards for instructors, then we will not re-contract with that instructor. According to NC Department of community College rules a minimum of 50% of all classes > 8 hours should be monitored by a school director. Students will have the option and are encouraged at every class to fill out a standard evaluation form for the class. The instructor has the option and is encouraged to submit an evaluation form on the college for every class they teach. Note: any instructor doing student teaching hours will be under the direct supervision of a qualified instructor for the entire student teaching time. (See attachment R) 7. Faculty Orientation and Development A. All initial instructors will undergo an orientation session with one of the program coordinators. This session will describe the roles and responsibilities of the instructor, show the instructor how to maintain the paperwork and give any special information the instructor may need for the topic or group he/she is teaching. The instructor is free to ask any questions at this session. B. The individual instructor will be required to meet NCOEMS instructional recertification requirements. Program coordinator(s) will provide periodically instructor upgrade opportunities locally. C. Preceptors will be assigned by the by the field/clinical site. Lead instructors for any class needing preceptors will notify the site contact person(s) in advance of any precept work being done. This letter will outline what is expected of the students and particularly what skills the student will be able to perform in the preceptors presents. D. HCC will provide at minimum an annual instructor update for its instructors. IX. Support Staff Support staff for the HCC EMS program will consist of college employees and Halifax County employees. HCC administrative staff will maintain class rosters/records and instructor contracts. Initial class records will include exam grades, clinical/field sheets, performance and skill evaluations and instructor recommendation for taking the state exam. Copies of administered exams will be kept at HCC. Sandra Hammack will head up this function of recordkeeping with the assistance of the registration and business offices. After hour issues can be dealt with by the evening assistant for Continuing Education. If the college program coordinator cannot be reached, the Dean of Continuing Education will be contacted. Phil Ricks at Halifax County EMS will maintain individual topic rosters for Halifax and Warren county rescue students. If Phil Ricks is unavailable then Warner Ferguson will be contacted. Paul Young of Northampton EMS will maintain Northampton county student individual class rosters. If Paul Young is

unavailable, Chuck Joyner can be contacted. The county of the individual student will maintain continuing education records. HCC will maintain a backup set of records of classes taken through HCC in the form of transcripts. Students will be able to access their own records through policies in place through the registrar’s office. Students wanting to have credits transferred into or out of HCC’s system must follow the policies of the admissions office. All records will be maintained according to rules set forth from the Department of Community Colleges. All HCC student roster sheets will show the class title, location and instructors name. Instructors will keep up with class meeting dates and student attendance on the roster. In tested classes, the instructor will also provide a grade sheet showing student’s grades for graded materials. (See attachment I) IX. Clinical and Field Internship 1. The lead instructor, overseen by the program coordinator(s), will conduct clinical placement for HCC. Standard clinical site will be with Halifax Regional Medical Center emergency Department in Roanoke Rapids. Standard field internship sites will be Primarily Halifax County EMS. Northampton EMS and Gaston EMS may be used in special cases after approval by the lead instructor. Other sites may be approved with approval from the medical director and program coordinators as well as any and all paper work is in order prior to the start of clinical/internship. These sites have been approved based on call volume, patient type, availability and quality of preceptors. All preceptors will ensure OEMS standards are followed. The lead instructor will follow up to ensure clinical/field internships are being completed properly. Clinical times will be done in line to meet goals, objectives and practices of NCOEMS, DOT, AHJ, etc. as determined for the individual course or student, if sent as part of remediation. The lead instructor will ensure that the clinical/ field sites have a letter from him/her stating the students by name that are allowed to do clinical, a list of what the students are allowed to do, contact numbers for the lead instructor and any other information the instructor deems pertinent. (See attachment K) A. For a student to conduct clinical/internship at any of the above sites the student must be released by the class’s lead instructor, there may be no more than two students at any one site at the same time and there must be a pre-approved preceptor with the student when any skills are to be performed. Students will sign up for clinical time on calendars provided at each site. All students will follow the clinical/internship rules and regulations (see attachment E) at all times. Any deviation from these rules may result in the student being asked to leave and may jeopardize the students ability to pass the class. The student will be responsible to ensure clinical/field time and procedures are properly documented and turned into the lead instructor (see attachment E&S). The preceptor on the form will document all skills attempted. All skills are either successful or unsuccessful, if the preceptor feels that there is a general weakness on the student’s part a remediation plan will be worked out with the lead instructor to strengthen the problem. (1) The standard for clinical setting will be Halifax Regional Medical Centers ED, 250 Smith Church Rd, Roanoke Rapids, NC. Charlotte Ferguson, nurse manager for the ED (252-535-8000), will be in charge of clinical students. In her absence the ED Clinical Supervisor will be responsible for the students. The HCC medical advisor and program coordinators may procure

other sites on a case-by-case basis only after acceptance of site and preceptors. Then only after all appropriate paperwork has been completed and on file. (See Attachment J). (2) Standard field internship sites will be primarily Halifax County EMS (contact Phil Ricks @ 252-583-2088). Northampton EMS (contact Chuck Joyner @ 252-534-6811) and Gaston EMS, 244 Craig St, Gaston, NC (contact Robert Byrum @ 252-535-2173) may be used with permission by the lead instructor. Other sites may be used with approval from the medical director and program coordinators. All paper work must be in order prior to the start of the clinical/internship. Field sites will have designated preceptors. Students must be with a designated preceptor to conduct a skill. (See attachment K) B. Experiential objectives will be setup according to the certification level of the class and will follow NCOEMS recommendations. HCC has no added objectives to the NSOEMS curriculum. (See attachment S). C. Supervision of the student will be the responsibility of the designated preceptor. The preceptor will ensure that the student’s performance is satisfactory or will note otherwise on the clinical/internship form. The preceptor will not give credit for skills or time for unsatisfactory work. Any unsatisfactory work of the students should be documented and reported to the lead instructor. HCC program coordinator(s) and lead instructor will periodically evaluate the clinical/internship program by talking with preceptors, students and visiting clinical/field sites periodically. D. Evaluation and performance standards will be done for each credentialing level offered by HCC and will follow the requirements set forth by NCOEMS. (1) For EMT-Basic students HCC will require each student to participate in at least 16 hours of scenario based training. Clinical/field internship will be voluntary and will only be allowed as directed by the lead instructor. All other levels will follow the minimum required skill performances as outlined by NCOEMS. (2) Students performing skills in a clinical/field setting will be graded on a successful or unsuccessful basis. Students will only be given credit for successful skills. If the preceptor notes weaknesses, the lead instructor will be notified and remediation will be offered as needed. (3) Performance standards as stated above insure that no student will be allowed to advance without successful mastery of all skills required by the NCOEMS for their credentialing level. E. No student may begin clinical/field internship without lead instructor’s permission. Each student must either purchase liability and accident insurance from the college or provide a letter from their squad commander that the student’s squad insurance will fully cover him/her in the clinical/field environment. All students will conduct themselves by the rules and regulations for clinical/field internships (see attachment E).

X. Student 1. HCC is an open door college; students in good standing with the college will be accepted into class. Some classes will have prerequisites to enter. MR & EMT-Basic students will have to pass a reading placement test at the 10th grade level or prove equivalent. EMT-I students will have to pass a reading placement exam at the post secondary level and a math entrance exam at 10th grade level or prove equivalent. EMT-P students will need to meet the same requirements as the EMT-I student plus have an accepted A&P class. TABE A version testing method will be used for all entrance exams. At this time HCC has no provisions to give credit for work done outside the scheduled EMS program. (i.e. challenging through exam or experience for EMT-B). If this condition should come up, it will be done in specialized classes on a case by case basis as determined by the school director and lead instructor within the guidelines allowed by NCOEMS. 2. Students will be required to attend at least 90% of the class unless suitable allowances have been made and accepted by the lead instructor and program coordinators for unusual circumstances. All course work must be completed 100% before a student will be allowed to take the state exam; this will be ensured by homework assignments and exams. 3. All students at every level will be evaluated by written exams and scope of practice evaluations appropriate to the student’s credentialing level. All students in every level must maintain an 80% cumulative average on module exams and an 80% on the final exam. Homework assignments will be on a pass or fail scale and weighted in the student’s final grade. Exams will be made from objective referenced test banks for validity. Exams will be monitored and changed as required to maintain a reliable bank of exam. Scope of Practice exams will be pass or fail and carry no other weight on the student’s final score. Students must pass the scope of practice exam to be allowed to take the state exam. Clinical/field internships are graded on a pass or fail; this grade is not weighted into the student’s final score. Students must pass all required clinical/field internship requirements to be allowed to sit for the state exam. Students who fall below the 80% cumulative grade will be referred to remediation to help bring the score back up. We will not use an academic probation system; if a student does not meet the requirements listed above he/she will not be allowed to sit for the state exam. 4. Remediation opportunities will be available to any student who feels he/she needs extra help with class work. Instructors will be available prior to each class session to help students. Students can be referred to mentors at local squads or departments to help them out. HCC has additional test bank and homework material for each level to help students out. Computerized study guides and training aides will be available at the college Learning Assistance Center for student use. The student services division and learning lab are also available for any specialized help that may be needed. Students will be informed of all these options. It is the students’ decision to use these aides. The lead instructor will refer students who fall below an 80% cumulative or on a single assignment or fail a clinical/field assignment or scope of practice to an appropriate remediation as needed. It is the students’ responsibility to follow through with the remediation options. Failure to complete remediation could cause the student to fail the class.

5. HCC does not have an academic probation policy for EMS students. At the beginning of class, the instructor will go over the requirements for being allowed to take the state exam with the students and have students sign a form stating these requirements. If these requirements are not met the student will not be allowed to take the state exam. If a student falls below 80 % on written exams or fails a scope of practice or clinical/field assignment the student will be referred to remediation. Students will be apprised of all grades and individual progress in a timely manner. (Attachment F) 6. For a student to satisfactorily complete any EMS level course, he/she must meet all written, scope of practice and clinical/field internships requirements prior to the end of the class period. 7. Halifax Community College has guidance and professional development opportunities through our Student Services division as well as through the program coordinators, lead instructors and supplemental instructors. Students in initial class are helped to become members of a local squad and are introduced to people that can help guide them (mentors). 8. If a student should feel there is a problem or feels that he/she is being treated unfairly, the student can present it to the lead instructor or program coordinator(s). If the problem is not resolved to the student’s satisfaction then the student can take the problem to the Dean of Continuing Education. If the student still does not feel satisfied there is a formal grievance procedure the student can follow that is outlined in the student handbook. Students can obtain a copy of this procedure or a copy of the student handbook at any time upon request. It is also on the HCC website for viewing any time. XI. Learning Resources HCC has access to state of the art multimedia centers and learning labs. These resources will be available to EMS students as needed. Remediation will be made available to all students. Students of every class will be offered extra time with instructors, use of self study materials, mentoring system or other appropriate remediation measures to offer the highest degree of success to each individual student regardless of his/her particular learning style. HCC has a fully staffed basic skills lab that will be made available to all students should they need help or do not score adequately on general skill exams. This lab will be free of charge to the student and available to individuals at any point of their education process. HCC also has a very competent Student Support Service Division that can help students deal with other problems as they become apparent. XII. Record Keeping HCC maintains all class records as required by the NCCCS and under goes a yearly audit of record keeping abilities through NCCCS. Normal class records maintain vital information such as the student’s attendances, dates classes held, student’s final grade instructors name and signature verifying information on the roll sheet is correct as well as other information. HCC follows all Privacy Act requirements with student records. Student records are kept in both electronic and hard copy files with the hard copies from each class kept in the vault room. Records are available to students upon written request to the registrar’s office as stated in the

Halifax Community College student handbook. This includes transcript requests for job placement or re-credentialing. A student will need to request the transcripts in writing to the registrar’s office. Other entities such as NCOEMS will have access to student records as allowed by privacy act laws. All student records are maintained longer than the four years recommended by NCOEMS. For initial EMS testable classes, HCC will also maintain files concerning things such as: - Record of all student grades throughout the course. - A copy of any required skills tests. - A copy of any clinical sheets along with preceptor sheets. - A master copy of exams given through the course. - Copies of CIS roster or other proof of instructor’s recommendation for state testing. - Proof of pre/co-requisites. For further information see provided check off sheet provided in attachment I. For all other classes, HCC will maintain the normal college associated records as noted above. All paperwork associated with recertification requirements will be maintained by the county as indicated in their individual county plans. The county having jurisdiction will have an individual(s) sanctioned by HCC to sign for the re-credentialing of personnel practicing in their jurisdiction. The county having jurisdiction will maintain training records for re-credentialing purposes of those individuals practicing medicine in their system. A verification form (attachment O) of all persons re-credentialed by the county sanctioned representative will be sent to HCC and kept on file detailing the physical location of re-credentialing paperwork. HCC will be the only authorized agent to sign for re-credentialing of persons not affiliated with either Halifax or Northampton county EMS systems. XIII. Critical Success Factors 1. The purpose of measuring critical success factors is to give HCC a tool for gauging the success of its programs. Critical success factors are a combination of yearly state test results, overall pass rate of students, employer satisfaction (done formally every three years), student course surveys (done every class) and monitoring and evaluations done by administrative staff members (performed several times per class). The program is under continuous evaluation by the program coordinators and college administrators. 2. The method of gathering information will be formally by written surveys and informally with talks and interviews and monitoring. This includes looking at student and employer satisfaction ratings as well as looking at state pass rates as well as drop out and retention rates. 3. Program outcomes will be measured for quality and quantity based on the requirements of the North Carolina Community College System, NCOEMS and any other regulating agency. Outcomes will also be measured against the need of the community to put quality students to work in the field.

4. The overall program evaluation will be on-going as outlined above. Individual classes will be monitored in progress by comparing the classes’ results to previous classes by using standardized controlled exams, set scope of practice grading guidelines and experienced instructors with program coordinator(s) involvement. By doing this, when weaknesses are identified, they can be corrected before the end of the class. 5. HCC had met all of their critical success factors in its past report. HCC is hoping to continue to meet all of the critical success factors in the future while improving on the numbers for each monitored point.
