To achieve this, we worked to ensure our new moms in San Luis Obispo County knew getting postpartum care was still safe and recommended during the COVID-19 Crisis through comprehensive case management. Three Schedule Once scheduled, the members are sent CenCal' s custom postpartum health promotion materials including an award winning appointment reminder card, "Baby Cal'' •• --------------- ••••• Two Case Management team contacts the new mom, provides them with information and helps them schedule their postpartum appointment.
