Internet-delivered Cognitive Behaviour Therapy (ICBT) is a relatively new approach to treatment that is a convenient way for patients to access care. ICBT is highly standardized and involves patients reviewing weekly lessons over the Internet. Patients also receive brief weekly support from a therapist via secure emails or phone calls. Past research shows that ~75% of patients complete ICBT and report large symptom improvements. Although these results are very promising, research also suggests that ICBT could potentially be improved by being more personalized in terms of treatment duration and amount of therapist contact, especially for clients with more severe

symptoms. We will conduct three studies to evaluate personalized ICBT. The first trial will involve therapists who work in a clinic that specializes in ICBT. Over 18 months, we will randomize 440 patients with depression and/or anxiety to either receive standardized (5 lessons completed in 8 weeks; weekly therapist contact) or personalized ICBT (5 lessons; personalized duration and therapist contact). In the second study, 440 patients will be treated by therapists working in five community mental health clinics in Saskatchewan. We will compare outcomes of 220 patients treated with standardized ICBT in one year to outcomes of 220 patients treated with personalized ICBT the following year. In both trials, we will compare patients who receive personalized and standardized ICBT on outcome measures, intervention usage, satisfaction measures and costs. In the third study, we will interview therapists and managers to learn about strengths and challenges of delivering personalized ICBT. The research will ultimately advance how ICBT is used in clinical practice and is expected to lead to improvements in mental health outcomes and mental health care delivery. ‘Internet-delivered cognitive behaviour therapy in Saskatchewan 2017-2018 Budget Year’ $356,000 Ministry of Health PI: Heather Hadjistavropoulos

Depression and anxiety are highly prevalent and disabling conditions that are frequently undertreated. There is strong evidence that Internet Cognitive Behavior Therapy (ICBT) is effective for treating these conditions. ICBT involves patients reviewing strategies for coping with symptoms over the Internet along with patients communicating with a therapist over a secure email system. ICBT is advantageous for those who have difficulties accessing care. Our past research shows that it is feasible to train providers from multiple disciplines in ICBT and for them to deliver ICBT effectively and safely within the community. There are some barriers to delivering ICBT, however, that need to be overcome before this approach can be routinely incorporated into clinical practice. The current grant involves a partnership between researchers and decision makers responsible for mental health programming in rural and urban areas of Saskatchewan to improve access to ICBT. Specifically, we aim to: 1) develop and evaluate an online presentation to inform patients about ICBT; 2) develop and evaluate an online screening process to identify patients who are eligible for ICBT; 3) implement and evaluate a 5-module ICBT program that can be used for depression and or anxiety and can be offered either as a first step in care, as an alternative method of delivering care or as an adjunct to face-to-face care; 4) examine therapist perceptions of ICBT; and 5) explore decision-making related to ongoing use of ICBT in Saskatchewan resulting from this research. We are particularly interested in factors that will facilitate or constrain health regions adopting ICBT in the longer-term. Patient feedback will be sought throughout the project. The research has the potential to significantly influence how ICBT is managed and delivered not only in Saskatchewan, but in other areas of Canada. ‘Online Therapy Unit Participation in Mental Health Commission of Canada’s SPARK Training Institute Reaching Out: Effective Approaches to Sharing Knowledge about Mental Health’ Successfully submitted application for coordinator to participate in two day fully funded training institute.
