Definition of Herpetic stomatitis

     Herpetic stomatitis is a viral infection of

the mouth that causes ulcers andinflammation.

     These mouth ulcers are not the same ascanker sores, which are caused by adifferent virus.

Causes, incidence, and risk 

factors

     Herpetic stomatitis is a contagious viralillness caused by Herpes virus hominis (alsoherpes simplex virus, HSV). It is seen mainlyin young children. This condition isprobably a child's first exposure to the

herpes virus.

Symptoms

     Blisters in the mouth, often on the tongue or 

cheeks     Decrease in food intake, even if the patient is

hungry     Difficulty swallowing (Dysphagia)     Drooling     Fever (often as high as 40° c) may occur 1 -2 days

before blisters and ulcers appear      Irritability     Pain in mouth     Swollen gums

Signs and tests:

     Herpetic stomatitis is normally diagnosedbased on its very typical appearance.

     Laboratory studies are seldom done.Sometimes viral culture and special stainscan help with the diagnosis.

Treatment:

     Herpetic stomatitis can be treated withthe acyclovir family of antiviral

medications.

     An oral topical anesthetic (viscouslidocaine) is available for severe pain, butit must be used with care because the

anesthetic deadens all feeling.

Expectations (prognosis):

     The child should recover completelywithin 10 days without medical treatment.

     Oral acyclovir may speed up recovery.

Complications:

     Herpetic keratoconjunctivitis, a secondaryherpes infection in the eye, may develop.

     This is an emergency and can lead toblindness.

     Dehydration may develop if the child

refuses to eat and drink enough because

of a sore mouth.

Cystitis - acute

     Acute cystitis is a bacterial infection of thebladder or lower urinary tract. Acute means

sudden or severe. Most of the time, your bodycan get rid of these bacteria when youurinate. However, sometimes the bacteriacan stick to the wall of the urethra or bladder,or grow so fast that some bacteria stay in the

bladder.

Symptoms

The symptoms of a bladder infection include:

     Cloudy or bloody urine, which may have afoul or strong odor 

     Low fever (not everyone will have a fever )

     Pain or burning with urination

     Pressure or cramping in the lower abdomen

(usually middle) or back      Strong need to urinate often, even right after 

the bladder has been emptied

Exams and Tests

     A urine sample is usually collected to perform thefollowing tests:

     Urinalysis is done to look for white blood cells, redblood cells, bacteria, and to test for certainchemicals, such as nitrites in the urine. Most of thetime, your doctor or nurse can diagnose aninfection using a urinalysis.

     Urine culture - clean catch may be done toidentify the bacteria in the urine to make sure thecorrect antibiotic is being used for treatment.

Treatment

     Antibiotics taken by mouth are usuallyrecommended because there is a risk that

the infection can spread to the kidneys.

     For a simple bladder infection, you will takeantibiotics for 3 days (women) or 7 - 14 days(men). For a bladder infection with

complications such as pregnancy or diabetes, OR a mild kidney infection, you willusually take antibiotics for 7 - 14 days.
