Understanding the IRS errors
1. There can be 1094 errors that need to be corrected, as well as 1095 errors.
2. The most common 1095 error that clients get from the IRS is a mismatch between the individual’s
name and SSN.

3. If the field under “Covered Individual Name” is blank, and only a name appears under “Name”, then
the SSN error is with the Employee. If there is a name under “Covered Individual Name”, then the
SSN error is with the Covered Individual
4. To correct the error for an employee, click on the Edit button found on the far right. That will take
you directly to the Employee Details where you will make the correction.

reprint the employee’s 1095, click on “Save and Print”. A pdf will be generated, and you can access
it under “Documents” in the ALE Member Dashboard.
6. To correct a Covered Individual, click on the Covered Individual Details tab. Locate the Covered
Individual you wish to correct and click “Edit”.

location where you’ll make the correction.

9. Example: for Error AIRBR835: Total Employee Count for ALE Member (monthly) is less than Full-
Time Employee Count for the same month – you would correct the FTE month that is larger than the

10. After completing the corrections and clicking on Save or Save Changes, you will be taken back to
the IRS Corrections page where you will see the Edit with the author and a Date and Time stamp –
this will help you keep track of your corrections/edits.
11. All correctable errors for employee records should be done in the IRS Corrections module with 2
(two) exceptions:
• Adding an employee and
• Deleting an employee.
To add an employee during the Correction process, use the Reporting Set Editor. Access Reporting
Set Editor via the Reporting Set and click “Review”:

Changes.

12. Adding or Deleting an employee from the IRS submission will require you to resubmit the 1094
because adding or deleting an employee changes the total number of forms 1095-C that are sent in
the transmittal (see below). The number is automatically calculated by ACA-Track, but you will need
to resubmit the 1094. Directions are found on Item # 14 below.
13. Once you have made all 1095 corrections, you are ready to click on “Submit 1095 Corrections”.
You will be asked to enter your login credentials again and “SUBMIT” to the IRS as a final step.

14. Once you have made all 1094 corrections, you’ll want to generate a pdf of the corrected 1094,
review it and make sure it’s accurate. On the IRS Corrections page, click on “Generate 1094 PDF”.
A pdf of the new, corrected 1094 will appear under “Documents” on the ALE Member Dashboard.
Review the 1094 to ensure all information is correct.

15. Now you are ready to resubmit the 1094 to the IRS. Click on “Submit 1094 Corrections”. You will
be asked to enter your credentials, then Submit to IRS.
16. After resubmitting to the IRS, check back later to see what the IRS response is to your
resubmission(s). If the response is “Accepted without Errors”, you are DONE! If the IRS response is
“Accepted with Errors”, you will need to continue the correction process until you have determined
that the errors are not correctable.
