Forum & Keynote DinnerThe day's activities include an interactive afternoon educational forum, led by LLI members and invited guests, followed by an evening reception, dinner, and keynote address.

Keynote Dinner Speaker: Uma R. Kotagal, MBBS, MSc, Senior Executive Leader, Population and Community Health, and Senior Fellow, Cincinnati Children's Hospital.

Dr. Kotagal was director of the neonatal intensive care units at the University Hospital and at Cincinnati Children’s for several years. While practicing, she recognized that care and outcomes improvement were a system property. She completed additional training, receiving her Master of Science in clinical epidemiology and clinical effectiveness from the Harvard School of Public Health, and refocused her clinical efforts on quality transformation at a systems level. She was also a visiting scholar at the Center for Risk Analysis at the Harvard School of Public Health and a visiting professor at Tufts New England Medical Center, completing further training in the field of decision and cost effectiveness analyses.

Dr. Kotagal was born in Bombay, India, where she received her undergraduate and her MBBS from the University of Bombay. She did a rotating internships at the University of Bombay and Detroit General Hospital. She completed a pediatric residency and fellowship in neonatology at Children’s Hospital of Michigan, and went on to complete a fellowship in neonatal physiology at the University of Cincinnati.

Dr. Kotagal also serves as chair of the quality steering team of the Ohio Children’s Hospital Association, as a member of the advisory committee of the Toronto Patient Safety Center, as an associate editor of BMJ Quality & Safety, and as a member of the Institute of Medicine.

The Lucian Leape Institute (LLI) was established in 2007 by the National Patient Safety Foundation (NPSF, which has since merged with IHI) to provide a strategic vision for improving patient safety. Composed of nationally known thought leaders with a common interest in patient safety, the LLI functions as a think tank to identify new approaches to improving patient safety, call for the innovation necessary to expedite the work, create significant, sustainable improvements in culture, process, and outcomes, and encourage key stakeholders to assume significant roles in advancing patient safety.

Recent focus areas include a public opinion poll on error and harm, educating and engaging trustees in safety and quality, and developing and implementing a measurement framework for assessing safety.

The LLI is named for Lucian Leape, MD, who served as the founding chairman and remains an active member of LLI. Dr. Leape was a member of the Institute of Medicine’s Quality of Health Care in America Committee, which published the following seminal works in patient safety: To Err is Human:Building a Safer Health

System (1999) and Crossing the Quality Chasm (2001). He is often referred to as the father of the patient safety movement.

Today, the LLI is chaired by Gary S. Kaplan, MD, FACMPE, Chairman and CEO of Virginia Mason Health System. Dr. Kaplan is a long-standing member of the LLI and is an established advocate for patient and workforce safety. The LLI continues to aggressively pursue work with health care leaders, law makers, key stakeholders, and patients and families, with the sole purpose of creating significant, sustainable improvements in patient safety.
