In Person: Short Test of Functional Health Literacy in Adults (STOFHLA) KSTOF Short Test of Functional Health Literacy in Adults (STOFHLA) Module OVERVIEW The Short Test of Functional Health Literacy in Adults (STOFHLA) Module is new to the 2011 collection wave. At its core is a reading comprehension assessment known as the "Short Test of Functional Health Literacy in Adults (STOFHLA)." Although an abridged version of the "Test of Functional Health Literacy in Adults (TOFHLA)," STOFHLA is likewise premised on the notion that successfully navigating the health care system requires a level of literacy greater than what is typically acquired in the conventional classroom. When being assessed, patients are asked to read and complete passages that might be found in health care settings by inserting missing words at selected intervals. More specifically, they are prompted to choose the correct word from a list of four possibilities at each place where a word has been removed from the text. Two generic passages are used: the first, which has sixteen missing words, is a set of instructions disseminated to patients prior to an upper GI series (X-ray examination) while the second, which has twenty missing words, comes from the patient rights and responsibilities section of a Medicaid application form. Possessing discrepant levels of intelligiblity, the first passage is at a 4th grade reading level (4.3 on the Gunning Fog index) while the second is at a 10th grade level (10.4 on the Gunning Fog index). Note that although the STOFHLA is a timed assessment to which seven minutes are allotted for completion, the patient is never told as much and simply asked to stop if they haven’t finished after seven minutes. Functional health literacy scores are computed as the sum of patients’ correct answers to the thirty-six total items comprising the STOFHLA. It is recommended that scores be interpreted as follows: STOFHLA Score Functional Health Literacy Level 0-16 Inadequate: unable to read and interpret health texts. 17-22 Marginal: has difficulty reading and interpreting health texts. 23-36 Adequate: can read and interpret most health texts. If the patient scores at the marginal or inadequate levels of functional health literacy, it is unlikely that they will be able to read and understand health care instructions. Such patients are at increased risk for improperly taking medications and failing to satisfy the tenets of prescribed diets or treatment regimens; health care providers must make modifications to normal operating procedures if they wish to accommodate the needs of patients such as these. OVERVIEW

The Short Test of Functional Health Literacy in Adults (STOFHLA) Module is new

to the 2011 collection wave. At its core is a reading comprehension assessment

known as the "Short Test of Functional Health Literacy in Adults (STOFHLA)."

Although an abridged version of the "Test of Functional Health Literacy in

Adults (TOFHLA)," STOFHLA is likewise premised on the notion that successfully

If the patient scores at the marginal or inadequate levels of functional health

literacy, it is unlikely that they will be able to read and understand health

care instructions. Such patients are at increased risk for improperly taking

Note: For many cases a participant’s spouse or other family member was present during the interview.

This sometimes resulted in the participant relying on the other person for help in answering some or many

of the questions. At the end of each module the interviewer indicated the amount of help the participant

received and who provided that help. That assessment is covered by codes 1 through 5. Additionally, a

code of 7 indicates that the entire interview was administered to a proxy while a code of 6 indicates that

this particular module was answered by a proxy but other modules were answered by the participant.

Note: The participant was considered to have completed the STOFHLA assessment if they responded to as

many items as they were able to within a validly administered seven-minute time limit. While many

participants marked an answer - whether correct or incorrect - at each of the thirty-six STOFHLA items,

others were unable to respond to all items within the time frame. If the participant left all of the selection

boxes alongside a missing word’s four answer possibilities unmarked (or if they marked multiple boxes),

they were deemed not to have attempted responding to that STOFHLA item regardless of whether they

simply ran out of time or were legitimately confounded by it. If we have no record of how the participant

responded to one or more STOFHLA items (e.g. we don’t even know if they attempted the item or not),

the assessment was deemed to have been administered or documented incorrectly and this variable has

been assigned a value of "5." If the participant was unable to see well enough to read the the STOFHLA

passages provided to them for the assessment, it was not administered and this variable has been assigned

a value of "6."

Note: If the participant had not finished the STOFHLA assessment after 420 seconds (seven minutes), the

interviewer was instructed not to inform them that time was up but to say, "Thank you. That gives us

exactly what we need. Now we will go to the next question." If the participant voiced a desire to quit the

assessment before seven minutes had elapsed and the interviewer suspected the involvement of a literacy

issue, the interviewer was instructed to say, "You can stop if you want to, but the information you can give

us is very special and valued highly by us. Would you like to keep trying?" All participants who opted to

quit the assessment before seven minutes had elapsed were assigned a value of "4" on this variable.
