Infecties met ESBL-producerende bacteriën bij de mens
Marc Bonten, UMC Utrecht
94% of 98 fresh chicken meat samples ESBL positive
79% of 89 fresh chicken meat samples ESBL positive
What is similarity?
“Similarity”
• In human E. coli: 35% of ESBLs are poultry associated ESBL types
• 19% of human ESBLs also on the same plasmid and plasmid subtype as in poultry (IncI1 STC 7 or STC5)
• 11% of humans ESBLs share ESBL, plasmid, plasmid subtype and bacterial strain with poultry
Leverstein et al CMI 2011, NTVG 2011
Extrapolation of cases
• Study period: • Feb-March-April 2009
• Coverage: • +50% of all Dutch hospital beds
• Number of patients with ESBL E. coli bacteremia: • 21
• Number with poultry-associated ESBL genes: • 1 (5%) - 8 (38%)
• Extrapolated number in the Netherlands: • 2-16 in 3 months
The adjusted mortality of ESBL-producing Enterobacteriaceae bacteraemia: a meta-analysis
Wouter Rottier, Heidi Ammerlaan, Marc Bonten
JAC 2012; 67:1311-1320
Introduction
• ESBL+ Enterobacteriaceae bacteraemia → higher mortality
• Inadequate empirical therapy• Increased virulence• Effectiveness of antibiotics
• Meta-analysis Schwaber & Carmeli 2007• ESBL → higher unadjusted mortality

Aims of the Meta-Analysis
• Higher mortality in case of ESBL+ bacteraemia after adjustment for confounders?
• Effects of adjustment procedures applied

Conclusions
• Based on this meta-analysis: •
• ESBL bacteremia is, as compared to non-ESBL bacteremia, associated with higher mortality, even after adjustment for confounders
• Adjustment for intermediates reduces the association
• Inadequate therapy may explanation the association

Doel ESBL-bacteriëmiestudie
1. Beschrijven kenmerken patiënten met ESBL-bacteriëmie
2. Beschrijven therapie bij ESBL-bacteriëmie
Opzet studie
• Retrospectief cohort van ESBL+ bacteriëmieën• Alleen eerste episode
• Species: E. coli, K. pneumoniae, E. cloacae• Inclusieperiode: 1-1-2008 t/m 31-12-2010• 8 Nederlandse ziekenhuizen:
• Universitair: UMCU, Erasmus, AMC• Perifeer: Diak, Antonius, Amphia, Tergooi, Gelre
• Gegevens verzameld mbt:• Patiënt, bacteriëmie, therapie, uitkomsten
• Primaire uitkomst: sterfte binnen 30 dagen

• Veel urineweginfecties in cohort

Waarvan:
•Alle infecties met gedocumenteerde verwekker
of
• 1,465 (12.9%) > 1 isolaat• Cef-R Enterobacteriaceae (0.9%)
• Work in progress
Besproken
