INSTRUCTIONS FOR BENEFITS OPEN ENROLLMENT

NOTE: You may go back at anytime in this enrollment process, to any step, or start over from the beginning at any point in these instructions.

When logging on to Employee Web Services – EWS, during Benefits Open Enrollment period, the below Pop-Up message appears. This message will appear each time you log in to EWS, until you complete the enrollment process.

The message instructs you to click the “Action” tab. The dropdown list from the “Action” tab will show “Benefit Enrollment” as an option.

Click on the “ Benefit Enrollment” option.

The “Benefit Enrollment” tabs come up.

The “Employee Info.” tab shows all of the employees current benefit plans.

NOTE: Big Five EWS Open Enrollment is ONLY for Health, Dental & Vision enrollment or changes to the employees current plans. Changes to Life Insurance, Retirement or Aflac plans can not be made through the EWS module. Contact the plan representative or HR for assistance with these types of plans.

The “Health” tab shows the current available benefit plans for health insurance.

Click “Decline” in the Action box next to the Health Plan that you do not want. Click “OK” when the message box pops up or click “Cancel” if you have clicked “Decline” by mistake.

Once you click the “OK” to decline the health plan $0.00 amount will automatically populate in the Employee Cost box.

Click “Select” in the Action box next to the Health Plan that you want.

$0.00 amount will show when a plan is declined.

When “Select” is choosen from the Action box the “Plan Editor” box comes up. A list of your dependents will also show on the right side of this box.

In the “Plan Option” drop down box, select the coverage you want. Only select the type of coverage that corresponds with your employment status. Example: Twelve month employees may only select coverage that starts with 12 Month. Ten month employees may only select coverage that starts with 10 Month.

Scroll for plan information.

Select the coverage that corresponds with your employment status and the type of coverage you want.

Below, the “Plan Option” for a 12 Month Employee & Spouse has been selected, as well as the corresponding dependent. NOTE: Make sure you select the correct “Plan Option” for your corresponding employee status. (Do you work 12 months or do you work 10 months)

Click the “Save” button when plan option and dependent selection is complete.

Select the corresponding dependent if applicable for the type of plan option selected.

Send HR a note if needed.

When the “Save” button is clicked, the plan editor screen will close. The “Health” tab will show a dollar amount in the Employee Cost box. This is total monthly cost to the employee.

Click the “Next” button. No Flex option is currently offered.

Example: Cost Per Pay Period for 12 Month Employee and Spouse is 183.20 x 2 = 366.40 monthly.

Click the “Next” button. Retirement (403B) is offered. You must contact Human Resouces for information on enrolling in Big Five Retirment Plan.

Click the “Next” button, Life Insurance. You must contact Human Resouces for information or changes to your Life Insurance or Dependent Life Insurance Plan(s).

Click the “Next” button.

The “Other” tab shows the current available benefit plans for Dental and Vision Insurance.

If you do not want “Dental or Vision”, “Dental” but not “Vision”, or “Vision” but not “Dental” Coverage, click “Decline next to the coverage you do not want.” Then click “OK” when the message box pops up or click “Cancel” if you have clicked “Decline” by mistake. Once you click the “OK” to decline, either or both plans, $0.00 amount will automatically populate in the Employee Cost box. (see page 4 of instructions)

The “Summary/Submit” tab shows the “Benefit Plan” selected, “Plan Option”, “Employer Cost” (This is what Big Five pays each month for your selected “Health Insurance Coverage”), “Employee Cost” (This is what you the employee pays per month for the coverage you have selected) and “Status” showing if coverage was “Elected” or “Declinded”.

Click on the “Print Summary” to print out the summary of benefits you have enrolled in or declined.

Status Box = If coverage was elected or declined.

Plan Option (who is covered)

Employee Cost Box = What you the employee will pay per month.

Amount Big Five pays per month for employee and dependent coverage.

When you have completed your “Open Enrollment”, click the “Submit” tab. The “Submit Enrollment” box will populate. Click the “OK” button. This will send your enrollment information to HR.
