Inspection 1. Those attending the Final Inspection were Mr. Richard Danial Webster - U.S. EPA, Mr. Raymond Hamwey - C. J. Mabardy, Thomas Riley, and William Trimbath of NUS. The Final Inspection occured on December 11, 1984. 2. The contractor has completed most of the items which were indicated on the punchlist prepared during the prefinal inspection. 3. The only item which remains to be completed is the installation of a silt barrier around the circular concrete inlet and the mounding of soil around the two existing monitoring wells. 4. Rick Leighton presented a brief summary of the project at the conclusion of the Final Inspection. He said that NUS has performed their services in a correct, technical, and professional manner. The contractor, C. J. Mabardy, in his opinion, did not perform ' their services in a quick and timely manner. 5. Since the seeding of the clay cap was performed at a very late date, NUS requested that Mr. Ray Hamwey send a letter to NUS stating that the subcontractor, which performed the hydroseeding, return to the site to reseed and locations which may not take. Mr. Hamwey mentioned that he would this. 6. Tom Riley cleaned the job trailer of remaining NUS equipment and gave instructions to the contractor that he could remove his own equipment from the trailer and also disconnect power and telephone service. · 7. It was difficult to observe the condition of the cap since about 1" to 2" of ice covered the cap. 8. Rick Leighton asked Ray Hamwey if all of the subcontractors have been paid. Mr. Hamwey responded affirmitively that the subcontractors have been paid. This memorandum contains observations obtained during the Final of the Silresim IRM.

Inspection

1. Those attending the Final Inspection were Mr. Richard ~eighton, ~fr. Danial Webster - U.S. EPA, Mr. Raymond Hamwey - C. J. Mabardy, r~r. Thomas Riley, and William Trimbath of NUS. The Final Inspectionoccured on December 11, 1984.

2. The contractor has completed most of the items which were indicated on the punchlist prepared during the prefinal inspection.

Professional Paving, the finn which had performed the highway repairwork, and Mr. Charles Mutrie, who supplied the clay to the Silresim ... .· 'J project, have not been paid.

9. Tom Riley is currently preparing for submission to EPA a group of contract documents which will include the following:

a. A photographic record of the work performed at the site.

b. Copies of the daily reports which indicate the progress of the project as recorded by the on-site inspectors.

c. The material slips which accompanied construction productsbrought to the site.

d. Results of the soil density tests performed at the site.

e. Correspondence from the contractors, EPA, state and federal authorities which was received during the project .

10. NUS will prepare a contractor performance evaluation presenting our opinion of the work performed by the various contractors at the site.

11. NUS will monitor the progress of paying the invoices.
