Livingston County Residents The eligibility ceilings for the Senior Citizen and

Disability exemptions for Livingston County are $19,500 with a sliding scale up to $27,899.99 for the 2011 assessment roll. Several municipalities have the same limits. These exemptions must be applied for by March 1, 2011.

A property owner who becomes 65 anytime in the 2011 calendar year may now be eligible for the Enhanced STAR Exemption and must file by March 1, 2011 in order to receive it on the 2011 assessment roll.

Basic STAR is available to any homeowner on their primary residence whose federal gross income is less than $500,000. This exemption must be applied for by March 1st, and it does not have to be renewed every year.

Agricultural land exemptions must be filed by March 1, 2011.

All other exemptions, new and renewal, must be filed by March 1, 2011 with your Town Assessor.

FOR SALE OF REAL PROPERTY The County of Livingston is soliciting sealed proposals for the sale of real property located 6 Wadsworth Street, Geneseo, New York. (Tax Map No. 80.12-3-55.) Terms & Conditions: 1. Purchaser shall bear the cost of any studies,

surveys, plans and fees required by the Village of Geneseo, County of Livingston and/or State of New York.

It's a common problem: A driver goes to his or her car on a cold winter'smorning, puts the key in the ignition, only to discover the car engine won'tturn over.

Cold weather can wreak havoc on a car's performance. For an engine tostart, a complicated process involving several essential components mustgo off without a hitch. When the key is turned in a car, that turn causes aspark to take place in the spark plugs. This then ignites the fuel and oxygenmixture present in the engine.

When the air temperature is cold, the chemical combustion taking placein the engine may be compromised. That's because all chemical reactionsslow down when it is cold. Combine this with a cold battery that has alsoslowed down due to the temperature and, as a result, won't crank the enginevery efficiently, and starting the engine is suddenly very difficult.

One of the remedies to this situation is to keep the engine as warm aspossible. It can take 8 to 12 hours for an auto engine to cool down after thevehicle has been driven. So parking the car in a garage and out of the windcan help keep the heat in longer and prevent cold start problems. That'sbecause wind does not cause the heat from the engine to dissipate as quick-ly.

Another thing to try that can helpengines perform better in the coldweather is a lower viscosity motoroil, which will flow more easily.

last treatment for people suffering fromsevere emphysema. Only a few locationsin the U.S. offer the procedure.

In the surgery, part of the damaged lungis removed, which allows the remaining tis-sue and surrounding muscles to work moreefficiently. This makes breathing easier.

or 50 years, Betty Hutcherson, 73, of Springfield, Ill., smoked. She quit nine years ago,but the health effects linger.

In her case, one of those effects is emphysema, a lung disease with no cure thatmakes breathing difficult as the tiny air sacs in the lungs become damaged. But Hutch-erson wanted to go on a cruise without using an oxygen tank.

A procedure offered at Memorial Medical Center in Springfield — and a handful ofother hospitals — that removes the damaged parts of lungs improved her breathingand other aspects of her health.

"I have been able to lose 20 pounds, and I can ride my stationary bicycle for 25 min-utes every day," said Hutcherson, who had lung volume reduction surgery in August."Before, I could not walk more than a few feet, as I struggled with breathing." Hutch-erson still uses an oxygen tank but can walk more these days and still plans to take thatcruise.

The surgery is for patients who havesignificant emphysema and whose breath-ing capacity is less than 30 percent. Inthese cases, their lungs are hyperinflatedand push down on the diaphragm, mak-ing it hard to breathe, Hazelrigg said.

The surgery is done through thora-coscopy. A stapler is inserted through anincision, and it is used to seal off and re-move diseased air sacs in the lungs.Hutcherson had a third of each of herlungs removed.

"In the surgery, the bad areas are re-moved and the diaphragm comes back upand improves the functioning of thelungs," Hazelrigg said.

Candidate screeningsCandidates for the surgery go through a

number of tests beforehand, said TheresaBoley, an assistant professor at SIUSchool of Medicine, a Memorial MedicalCenter nurse practitioner and coordinatorof the lung volume reduction surgery pro-gram.

"They do breathing tests and a varietyof exercises including walking 30 minuteson a treadmill," she said.

This also helps them be healthier forthe surgery, Boley said.

History of the surgeryThe first lung reduction surgeries were

done in the 1950s. A handful of patientsdid well, but there were also a fewdeaths.

The program went dormant until the1990s, when laser surgeries and im-proved technologies made lung reduction

surgeries viable. The mortality rate whenthe program at Memorial Medical Centerstarted was about 4 percent in 1994-95.Doctors have cut that to less than 2 per-cent of people who have the surgery.

"What we have been able to do is im-prove upon the technology and screeningsto improve outcomes," Hazelrigg said.

Recently, there was a very disturbing

report that you may have read con-

cerning the unreliability of the

measuring devices used in over-the-

counter liquid medications for children.

Also, the directions on the labels of the

pediatric medications were not always

clear or consistent.

There were 200 over-the-counter liquid

pediatric products evaluated that were

used to treat pain, cold symptoms, gas-

trointestinal symptoms and allergies. Re-

sults of the study showed that 98.6 per-

cent of the time there were inconsisten-

cies between the dosing directions and

the markings on the dosing device.

For example, the dosing directions used

tablespoons as a unit of measurement, but

on the dosing device, only ounces were

used. About a quarter of the medications

did not include any type of measuring de-

vice. Plus, when the measuring device was

included, 24 percent lacked the necessary

markings on the device.

Prior studies have shown that even

when measuring devices are accurate, the

dose given by caregivers is frequently in-

accurate. In addition, the child's unwill-

ingness to swallow the medication results

in much of the liquid never being ingest-

ed.

Because improper dosages may have

significant consequences, the Food and

Drug Administration has made recom-

mendations concerning over-the-counter

liquid products. For example, they should

all include a measuring device.

The directions that are provided should

contain the same abbreviations and units

of measurements that are present on the

measuring device. Extraneous markings

should not be on the device because they

can cause confusion. Plus, the use of deci-

mals or fractions should be avoided.

Not mentioned in most of the discus-

sions on the subject of medications for

young children is, "What should you do

when the child refuses to take the medica-

tion?"

In these situations, babies or even 2-

year-olds can keep their mouths closed so

tightly that you just can't get the liquid in.

Accomplishing this may take some inge-

nuity on your part.

Mixing the medication with something

the child likes sometimes works. There

are now many medications that have a va-

riety of flavors, making them more palat-

able.

THE GREAT EQUALIZER Sometime in the future, historians will look back upon the pre -

sent, and compare the technological advancements made over the last twenty years to the great cultural Renaissance that began in the 14th century. I know for many people, myself included, the mere mention of the word “Renaissance” causes eyelids to droop and day - dreams to form. ( Just like in 7th grade.)

However, the breadth and depth of the effect recent technologi - cal advances have made on our lives cannot be overstated. This is particularly true as it concerns the practice of law, and in a big way.

Nearly twenty years ago, when I was working in a large law firm in Albany, it was not uncommon for one of the senior attorneys to assign three or four people to research a single issue. We would trudge off to the library and spend countless hours flipping through dusty old law books, looking for just the right case to support our position. The work hours expended were tremendous, and, accord - ingly, so was the cost to the client. As a consequence, it gave the well-heeled client a significant advantage over his adversary of more modest means. It also gave the large law firm a substantial practical advantage over the smaller firm, where the smaller firm would not have the available labor hours to expend.

Not so today. What formerly was accomplished in twenty hours of research the old-fashioned way, I can now accomplish in minutes without ever getting up from my desk. This is no exaggeration. With the advent of very powerful Internet legal search engines that accept “natural language” search requests (for example, “Does suing for “anxiety” suffered by a plaintiff in a personal injury case allow the defense to investigate the plaintiff’s pre-accident mental health history?”), the advantage that large law firms had over smaller offices has completely vanished. The playing field has been leveled. And I couldn’t be happier about it.

And by the way, the answer to the question above, is “yes.” If the lawyer for the plaintiff (with a pre-accident mental health history) makes the mistake of suing for anxiety or other emotional injuries suffered by the client, the defense can absolutely get all of the records of the client’s prior mental health treatment. This under - scores the importance of being truthful with your lawyer and the importance of hiring the right lawyer.

Candidate should also have experience with diagnosis and procedure coding, HIPAA regulations, Joint Commission and Electronic Medical Record regulations and compliance.

Health Information Management: Clinical Documentation Specialist: Full time days (7AM-3:30PM). Candidate must be an RN with 5 yrs clinical experience, and have knowledge of NYS, Federal, and HIPAA laws. Strong customer service background with ability to multi-task in busy work environment. RHIT or RHIA credential preferred.

Vent Unit: RN Part time nights, (56 hrs or 30 hrs per pay). Valid NYS RN license, and current CPR required; prior vent experience preferred.

Respiratory Therapist: Part time (alternate weekends). Current NYS RTlicense required; prior experience preferred.

Nurse Practitioner or Physician Assistant: Part time days/evenings at Mercycare Addiction Treatment Center of Hornell. Valid NYS license required; general practice with mental health and addiction treatment experience preferred.
