The PALS will assist with the management, the “support materials” components of the project for participants.

The PALS may need to travel extensively throughout the tribal health service area to meet with participants to achieve the goals stated, as well as to participate in, plan, and implement community-based activities and events.

The PALS will document all relevant participant data for the study participants.

Assists in the development and monitoring of all performance improvement initiatives and reporting requirements of the prevention program.

Demonstrates positive attitude and professional manner, as well as appropriate problem solving and communications. Maintains cooperative and effective working relationship with diabetes program staff, other UIHS employees and supervisory staff, with tribal officials and visitors to UIHS.

Meets all requirements of UIHS policy & procedure.

Must have a willingness to learn on the job, employ cultural techniques, be trained in said techniques as indicated.

The PALS will agree to participate in recruitment activities and lifestyle sessions that may occur within or outside regular working hours or on weekends.

The PALS will be an integral part of a multi-disciplinary team (both the Diabetes Team and Prevention Team), and will work closely with the diabetes program staff to identify impediments to adherence and retention in the project. They may also be asked to participate in recruitment activities as needed.

Must be willing to participate in the lifestyle balance program activities in order to serve as a mentor and example to help facilitate relationship and behavioral change for participants in the intensive program.

Must be able to maintain strict confidence of all information received on the job.

Participates in mandatory initial training and annual update training as indicated as well as continuing education as required.

EDUCATION/EXPERIENCE:High School diploma or equivalent. Excellent interpersonal and communication skills. Experience in community organization or participation in community and/or cultural building projects. Knowledge of health, sanitation and safety codes. Understanding of the basic principles of nutrition, exercise, and weight loss would be additional benefit.Experience in working with a wide range of age groups is preferred.
