Background and Governance Arrangements

The public Bodies (Joint Working) (Scotland) Act 2014 received Royal Assent on 1 April 2014, with implementation in East Ayrshire from that date. East Ayrshire Integration Board was formally established on 1 April 2015 creating the Health and Social Care Partnership and bringing together the management and delivery of health and social care services.

The Scottish Government intends that the Act will provide the framework to support improvement in the quality and consistency of both health and social care services through the integration of these services. The vision for integration is to produce better outcomes for people through services that are planned and delivered seamlessly from the perspective of the patient, service user or carer.

At a local level integration is seen as an opportunity to further progress health improvement and support and nurture of children in their early years as well as addressing health inequalities and the impact of alcohol and drugs on our community.

The Integration Joint Board (IJB) is responsible for overseeing the delivery of delegated NHS Community Based Services and the delegated Council Services that include all social work services for adults and children. The IJB was formed following the approval of the Integration Scheme by Scottish Government on 1 April 2015. The Strategic Plan was also approved by the IJB at the first meeting on the 2 April 2015, allowing for the transfer of management and delivery of services to the new Partnership.

The Council and NHS Board agreed the appointment of Eddie Fraser as the Director of Health and Social Care Services in February 2014 and the final configuration of the services to be included within the scope of the new Partnership in March 2014. In addition, there has been further consideration of the need for, and the appropriateness of, lead partnership arrangements in line with service or clinical effectiveness with one of the Ayrshire partnerships providing leadership for services across all 3 Ayrshire Partnerships.

In East Ayrshire this means assuming the management responsibility for all Ayrshire Primary Care Services and many Out of Hours Health and Social Care Services.

The IJB is a separate legal entity as a corporate body, with equal voting membership from NHS Board and Council. In addition there are a number of appointments in respect of stakeholder representatives and also professional advisors to the Board.

The Council and the NHS Board will continue to work on Ayrshire-wide cross-cutting issues through the Strategic Alliance, a joint senior officers’ advisory forum which also includes senior representatives from North and South Ayrshire Partnerships. Business agreed through this route – relevant to the work of the East Ayrshire Partnership – will be signed off by the East Ayrshire IJB, with recommendations being made to the Council and NHS Board as required.

The Partnership has a lead responsibility for the co-ordination and delivery of the Community Plan Wellbeing Delivery Plan. This allows the Partnership to work closely with Council, NHS, Community Planning Partners and our local communities.

An East Ayrshire Strategic Planning Group has been established with the purpose of developing the Strategic Plan on behalf of the Integration Joint Board. The membership

of the Strategic Planning Group is wider than the Board and includes representation from clinical health professionals, social work services, housing services, care home and home care providers as well as voluntary and community services and people who use health and social care services.

The Strategic Plan is a key mechanism by which the Integration Board will provide detail on the priorities for the Partnership.

The Strategic Planning Group provides an ongoing role to support the further development of the Strategic Plan in localities. Workshops and care group sessions, will continue to be organised to support the development of the priorities as outlined in the Strategic Plan. Locality planning groups are being developed to ensure that the Strategic Plan priorities are in line with locality priorities. The IJB will seek to ensure the implementation of the Strategic Plan priorities and they are aligned with the Community Planning Partnership Wellbeing Delivery Plan.

2. JOB PURPOSE

Support the Head of Service in the strategic management, development and performance of services taking a lead role in delivering organisational change that seeks to embed a culture and practice of the Health and Social Care Partnership in order to ensure the delivery of the Partnership’s key objectives, specifically in relation to the Community Plan; H&SCP Strategic Plan, Health Board Local Delivery Plan, Single Outcome Agreement and Community Health Care Plans.

Community Response Services and will assist the Head of Service in achieving service objectives and supporting operational delivery working within the East Ayrshire Health & Social Care Partnership’s purpose, values and commitments.

The post holder is key to ensuring the Service meets its commitment to the delivery of the quality strategy and world class services. As such the post holder will accept delegated accountability for the delivery of safe standards of health and social care ensuring a focus on service user safety, effective management of resources, continuous improvement, staff training and development and education and research.

The post holder will provide leadership to ensure the delivery of service user care and will assume a leadership role across the multi-disciplinary team for a variety of issues as agreed with the Head of Service.

Responsible for a specific delegated span of control, the Senior Manager will manage the Health and Community Social Work resource and secure operational delivery of the range of services, ensuring optimum use of available resources in achieving safe standards of care.

Provide operational leadership to Health and Social Work staff and independent contractors and professional leadership for Social Work or Nursing depending on the professional background, provide professional leadership for the Health & Social Care Governance arrangements and provide professional advice to the Head of Service and other staff groups.

3. DIMENSIONS

As a member of the Senior management team, leading service provision and transformational change you will work alongside the Clinical Director of Ayrshire Urgent Care Services, Clinical Nurse Manager for Advanced Nurse Practitioners, other managers, leaders for Social Work, nursing, allied health professionals, finance, O&HRD and Business Support services colleagues to develop and deliver high quality Service User care.

Reporting to the Head of Primary Care and Out of Hours Community Response Services, the post holder will be employed by EAC/NHS Ayrshire and Arran. 5. ROLE OF PRIMARY CARE AND OUT OF HOURS COMMUNITY RESPONSE SERVICES

The Department of Primary Care and Out of Hours Response Services is responsible for the management and delivery of all primary care and out of hours response services across Ayrshire and Arran. The purpose of the service is to:

Manage the strategic delivery of all services provided by independent contractor groups including GPs, Dentists, Opticians and Community Pharmacists;

Deliver an integrated health and social care out of hours service (Ayrshire Urgent Care Service) including: medical, nursing, social work and social care response;

Ensure the appropriate use of contractual and management mechanisms to achieve health and care governance, probity and discipline, alongside service and infrastructure development;

Ensure that the persons who provide Primary Care and Urgent Care services are suitably qualified and eligible to do so, in accordance with all relevant legislation and clinical and social work professional regulation, including by establishing and maintaining lists of eligible medical practitioners, dental practitioners, pharmacists, optometrists and ophthalmic medical practitioners;

Ensure the effective management of all contracts with independent contractors who provide primary care services in accordance with all relevant Regulations and Directions issued by Scottish Ministers;

Commission services from primary care providers in line with national and local strategies;

Manage a budget in respect of Primary Medical Services, General Pharmaceutical Services, the Managed Dental Service and the Hospital Eye Service (dispensing) and the non-cash limited budgets in respect of General Dental,

Manage a budget in respect of Pharmaceutical Services and Ophthalmic Services and implement the payment verification processes which apply to payments to all primary care service providers;

Manage a budget in respect of Ayrshire Urgent Care Service

Ensure the effective monitoring, management and authorisation of payments for fees, grants and allowances to the providers of primary care services;

Lead the implementation and delivery of the development plan of GP and Dental Premises ensuring expert advice and guidance on the application of the Primary Medical Services (Premises Development Grants, Improvement Grants and Costs) Directions and the Statement of Dental Remuneration;

Lead and co-ordinate education and learning programme for GPs, their staff and the wider primary care teams;

Working closely with Directors of Health and Social Care, other Heads of Service and Corporate Heads on critical projects and strategic aims of partners;

Achieve strong local accountability through involvement of the community, people who use services, family carers, community organisations and local Councillors; and

Contribute to relevant HSCP, NHS and local authority planning processes

Provide leadership across span of control ensuring safe, effective Service user and family centred care through the delivery of the best possible standards of service.

Lead the delivery of the arrangements for working in partnership with all key stakeholders. This includes:

Promote and ensure a corporate approach to Ayrshire Urgent Care Service across the Health & Social Care Partnerships, promoting a person centred focus at all times and focusing on outcomes.

Working with the Professional Leads for Primary Care, Social Work and Urgent Care to promote and ensure a person centred approach to the delivery of safe, effective and efficient urgent care services to the population of Ayrshire.

Manage the analysis and review of research and collated analytical data used to inform key strategic decisions and develop options for future service delivery based on principles supported by the most recent research findings and other evidence.

Professional/Service Advise the Heads of Service of specific local need by interpreting data and monitoring of

Ensure all relevant processes are managed in accordance with the principles of the Quality Improvement and Total Project Management System (TPL) utilising PRINCE 2 methodology as appropriate including Planning, Control and Risk Management to ensure consistency of approach in project planning across the Partnership.

Ensuring the maintenance of systems, control processes and risk management arrangements to ensure compliance with internal/external governance and best practice requirements.

Contributing to the development of action and improvement plans to resolve incidents/issues and support processes for ongoing monitoring.

The post holder has delegated responsibility for decisions and judgments made regards financial and workforce management, including monitoring and control of Ayrshire Urgent Care Service budget within their span of control. The post holder will:

Manage contracts and contractual service level agreements for services ensuring all contract work is carried out within the terms and specifications agreed in the Partnership’s tendering and procurement processes.

Responsible for ensuring the recruitment of suitably qualified and experienced staff to maintain the delivery of quality standards of care within the group, regularly reviewing and implementing plans to effect any necessary changes or redistribution of resources.

Assignment

Objectives are set annually by the Head of Service Significant degree of autonomy to make professional/ operational decisions Work is subject to Head of Service overview. However much work is self-generated by the

professional agenda and needs of the organisation through discussions with multidisciplinary colleagues and reaction/ response to activity and developments.

Work is also generated by initiatives delegated by the Associate Medical Director, Primary Care and East Ayrshire Chief Social Work Officer.

Review

Review will be annually by formal appraisal, personal development plan and objective setting.

Formal Supervision will take place regularly as part of professional Social Work requirements.

10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB

Management of an Out of Hours Service with the attendant risks of operating a pan Ayrshire service when other services and supports are not available

Leading the transformation of the Ayrshire Urgent Care Service to achieve integration, respond to GP workforce gaps and moving to new service models such as utilising technology

Management of a diverse workforce (managed and contracted) over range of sites and in the community. This has added complexity of working across employee terms and conditions of both NHS and Council.

Leading the management and co-ordination of the Health and Social Care Partnership’s strategic direction to modernise Social Work and Primary Care services ensuring an integrated and consistent approach to the setting, delivery and achievement of objectives, standards and performance indicators.

Liaison at strategic and operational level within H&SCPs and across NHS and Council in key areas of Risk Management, Health & Safety, Civil contingencies.

Leading on the implementation of strategic plans for the NHS Board and Ayrshire HSCPs covering Ayrshire Urgent Care Service and ensuring key national and local priorities are reflected.

Contribute to community planning processes by attending consultation meetings and community events and engagement with a wide range of stakeholders with a varied agenda ensuring a consideration of health and social inequalities within the local population at all times.

The postholder is expected to provide and receive highly complex information that is made up of several components which can often be conflicting, where opinion differs and there is uncertainty. This involves the postholder analysing and interpreting situations where there are a number of options and the implications of each of these have to be considered. 12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB

Physical Demands: Required to work out of hours in order to provide supervision and support for teams and

staff. This might include team meetings, HR or personal development meetings, providing leadership when changes are being implemented, this should be kept to a minimum where at all possible

Light to moderate: includes moving light equipment, e.g., briefcases/lap-tops, projectors for presentations; using keyboards regularly/frequently for data analysis, spreadsheets, word processing etc and email.

Driving between sites and to various meetings.

Mental Demands: Intense concentration/in-depth mental attention frequently required, e.g. undertaking

complex analyses and performing complex calculations; analysing problems and proposing solutions, leading meetings, influencing partnership and partner agency staff and managers at all levels of seniority, public speaking, often working under pressure and balancing multiple demands in complex/changing environments. The Health and Social Care Partnership as a corporate body will not be the employer and once appointed to either the Council or NHS Ayrshire and Arran, the successful candidate will be seconded to the Health and Social Care Partnership.
