K-HEN recommended measures• Outcome: Number of surgical patients that

develop a post-operative PE or DVT (#104) OR Potentially preventable VTE (#103)

• Process: VTE patients receiving unfractionated heparin with dosages/platelet count monitoring by nomogram or protocol (#100) OR VTE Discharge Instructions for patients with confirmed VTE (#99) 4
