Kamiak National Honor Society Application Full Legal Name Preferred Name Graduation Year Email Congratulations, you have met the standard of scholarship for the National Honor Society. Each member of the KHS Faculty Council will rate each student on their Leadership, Service, and Personal Statement using the Application Rubric (see attached). The Character score will be averaged from student’s submitted Character Recommendations. Rubrics will be totaled and scores will be averaged to determine the applicant’s score. A score of 13 out of 16 (with no “1” scores) will serve as a guideline for selection and non-selection, but the final selection must be made by the majority vote of the Faculty Council. Completion and submission of this application does not guarantee selection. Membership in the National Honor Society is one of the highest honors that can be awarded to a high school student and requires continued responsibility and obligations of demonstrating standards in all four areas of criteria. Criteria for National Honor Society Scholarship (3.5 G.P.A.) Students who earn (and maintain) a cumulative grade point equivalent standard of excellence of 3.5 or above meet the scholarship requirement for membership. These students are then eligible for consideration on the basis of service, leadership, and character. *Candidates must be members of the sophomore, junior, or senior class. Service This quality is defined through the voluntary contributions made by a student to the school or community, done without compensation and with a positive, courteous, and enthusiastic spirit. Leadership Student leaders are those who are resourceful, good problem solvers, promoters of school activities, idea-contributors, dependable, and persons who exemplify positive attitudes about life. Leadership experiences can be drawn from school or community activities while working with or for others. Character The student of good character upholds principles of morality and ethics, is cooperative, demonstrates high standards of honesty and reliability, shows courtesy, concern, and respect for others, and generally maintains a good and clean lifestyle. Grounds for Dismissal Students may be removed from National Honor Society for any of the following: Academic Dishonesty (plagiarism, cheating, etc.) Volunteer hours not completed and/or turned in on time (one warning before dismissal) Cumulative GPA dropping below 3.5 for two consecutive semesters Suspension or expulsion Procedures Complete the application form (continued on the back of this sheet). Write neatly and legibly. Personal Statement: Type your personal statement and attach it to your application. Prompt: What does honor mean to you and how have you demonstrated it? (300-350 words). Character recommendation forms: give to three people to whom you are not related to complete (they will turn them in). At least two must be teachers at Kamiak (but not necessarily your current teachers). You and your parent/guardian read, sign and turn in the entire completed packet by March 29, 2019 to the basket in the library or room 221.
