•  Most  common  reason:  clerical  error!  

•  Symptoms:  fever,  chest  pain,  hypotension.    

•  Hemoglobin  in  serum,  urine.  

•  Labs:  ↓  haptoglobin,  ↑  bilirubin,  DAT  posi0ve.  

•  Type  and  cross-­‐match  shows  ABO  mismatch.

•  Hemolysis  occurs  days  a1er  transfusion.  

•  Caused  by  an0bodies  to  non-­‐ABO  an0gens.  

•  Hemolysis  usually  extravascular.  

•  Presenta0on:  falling  Hgb  a1er  transfusion.    

•  Usually  not  severe.  

•  DAT  +.  An0body  screen  iden0fies  the  an0body.

•  Cytokines  →  fever,  headache,  nausea,  chest  pain.  

•  Diagnosis:  rule  out  everything  else  

•  Treatment:  Tylenol.  Leukocyte-­‐reduced  components.

•  Transfusion-­‐related  bacterial  infec0on  is  an  uncommon  but  serious  risk.  

•  Pa0ents  suddenly  develop  fever  and  shock.  

•  Pa0ent  –  and  blood  unit  –  must  be  tested.
