This report is a summary of the results of aquestionnaire survey of locally developed health education materials.This guide has been prepared to assist school districts in theplanning and initiation or expansion of local health programs. Itreports on the current status of locally developed programs in thecounty. Hopefully, this information will simplify the task of thosewho seek sources of health education programs in Suffolk County.Listed are the name and title of the person completing thequestionnaire and the number code assigned each district. The guideis partitioned into four grade level sections, kindergarten throughthird grade, fourth through sixth grade, seventh through ninth grade,and tenth through twelfth grade. Each section is preceded by the listof district names and their code numbers. The numbers in each cell ofthe topic, materials matrix, refer to the assigned district codenumbers. The research reported herein was funded under Title III ofthe Elementary and Secondary Education Act. (author)

SUBJECT: Survey of Locally Developed Health Education Materials
As a service to the school districts of Suffolk County, theRegional Center has conducted this survey of locally developed healtheducation materials. The enclosed report was made possible by thecooperation of chief school officers and members of their staff.
This guide has been prepared to assist school districts inthe planning and initiation or expansion of local health programs.It reports on the current status of locally developed programs inthe county. Hopefully, this information will simplify the task ofthose who seek sources of health education programs in Suffolk County.

INTRODUCTION
This booklet is a summary list of the results of the surveyof Locally Developed Health Education Materials. The questionnairewas sent to all districts in Suffolk County. Returns were receivedfrom fifty-six districts.
Following this section is a list of the twenty major contri-buting districts. This list includes the name and title of theperson completing the questionnaire and the number code assignedeach district.
The guide is partitioned into four grade level sections, K-3,4-6, 7-9, 10-12. Each section is preceded by the list of districtnames and their assigned code numbers. The numbers in each cell ofthe topic - materials matrix refer to the assigned district codenumbers.
The summary requires the following additions. The ThirdSupervisory District of Suffolk County has developed teaching guidesin all of the main topic areas for use with children who have avariety of disabilities. For further information concerning theseguides, which cover all grade levels, contact Mr. George Quinlan,Assistant District Superintendent for Special Education.
Babylon U.F.S.D. #1 has developed teaching guides, curriculumguides, overhead projectuals and bibliographies in all of the majortopic areas for grades seven through twelve. They also have de-veloped models for the same grade levels in the areas of safety andfirst aid, personal hygiene, community health, mental health andsex education. Smithtown C.S.D. #1 has developed teaching guides and bibli-ographies in the topic areas of narcotics, tobacco, and alcohol forgrade levels four through twelve. For further information contactMr. Rocco Tangredi, Pupil Personnel Supervisor.
The American Educational Research Association's, Review ofEducational Research, December 1968 is titled "Mental and PhysicalHealth." It reviews and discusses the vast amount of research forthe six-year period December 1962 to December 1968. The table ofcontents reads as follows; Mental Health in Education, Innovationsin School Mental Health Programs, Measurement in Relation to MentalHealth Programs, Utilization of Mental Health Professionals in the
School, Health Education, and Sex Education Research. For furtherinformation contact, AERA Executive Officer, 1126 Sixteenth Street,N. W., Washington D.C. 20036.
New York State Education Department's Curriculum DevelopmentCenter has produced a course of study for grades seven throughtwelve in the area of sociological health problems, alcohol, smokingand drugs.
