Padma BhushanDr. P. K. WARRIER

Padma Bhushan Dr P. K. Warrier, Doyen of Ayurveda passed away on July 10, 2021 one month after celebrating his 100th birthday (Satapoornima) on June 8th 2021. He assumed charge as Managing Trustee of Arya Vaidya Sala Kottakkal in 1954 and was leading the institution for the last 67 years. He was one of the most senior practising Physicians of the country and has played a historical role in invigorating the traditional practices of Ayurveda by his significant contributions to research and innovative experiments. He took this holistic science of health care to distant shores of the world.

He was a much sought after clinician of international repute and a large number of patients with debilitating ailments from every walk of life and from every corner of the world have benefited from his most compassion-ate healing touch. He was a recipient of many national and international awards including Padma Bhushan and Padma Shri from President of India in 2010 and 1999 respectively. Dr P. K. Warrier was leading a simple life according to the principles of Ayurveda, and strictly followed dinacharya, natural diet, yoga and exercise. Arya Vaidya Sala Arya Vaidya Sala is a 119-year-old charitable trust established at Kottakkal, Malappuram Dist. Kerala in 1902 by illustrious visionary Vaidyaratnam P S Varier, who has started it initially as a village clinic. Arya Vaidya Sala has now grown into a multi-unit, multidisciplinary and multi-crore organization with a significant presence in clinical service, medicine manufacture, drug distribution, research, education, medicinal plant cultivation and book publication.

AVS have 26 branches all over India and well-established hospitals with all facilities at Kottakkal, Kochi and Delhi where all classical Panchakarma therapies and reputed Kerala special therapies are made available. AVS also provides free of cost treatment to deserving patients at 160 bedded charitable hospital at Kottakkal, Kerala.

Choorna pinda sveda, Jambira pinda sveda, Patra pinda sveda, Shashtika Sali Pinda Sveda, Upanaha sveda, Choorna pinda sveda, Jambira pinda sveda, Patra pinda sveda, Shashtika Sali Pinda Sveda, Pizhi-chil (Kayasekam) etc. are to be carried out sequen-tially. Most of the time sudation is done as prime treatment. It cures many diseases.

Sneha and Sveda are carried out just before vamana, virechana, vasti and nasya. By snehana, doshas are attaining KLINNA (slime stage) stage. Klinna do-shas are liquefied by Sveda. Doshas are brought to koshta(Gastro Intestinal Tract) and then eliminated by judicious eliminative therapy.

Merits of Vamana and VirechanaVamana and Virechana have to be done after prop-er snehana and Svedana. The purification therapy (Vamana and virechana) provide clarity of intelli-gence, strength to sense organs, stability to body tis-sues, good digestive fire and slow ageing.

Vamana is mainly carried out in psoriasis and other skin diseases. Virechana is effective in piles (Arsha), Jaundice, worm trouble, constipation etc.

Merits of Vasti – Enema TherapyVasti is mainly indicated in vata predominant diseas-es. There are 3 types of Vasti as follow: 1. Nirooha (Kashaya Vasti – with medicated de-

coction)2. Anuvasana (Sneha Vasti – with medicated oil) 3. Uttara Vasthi

Vasti is described as one half of the whole treatment. Vasti is apt in the diseases which are dependent on body tissues, alimentary tract, vital parts, head, neck and all other body parts. Vata is responsible for the transit of feces, kapha, pitta and accumulated malas. Therefore vasti is the best therapy in increased vata. Vasti is good for delaying ageing. It will provide hap-py life, strength, digestive fire, intellect, sound and colour. It can be performed among children, youth and old aged persons. Vasti is not creating any com-plications. By doing anuvasana vasti, all the body tis-sues are getting nourished.

Merits of NasyaNasya can be performed daily from birth to death in

small quantity. By performing nasya, the following benefits are achieved. 1. Consistent shoulders, neck, face and chest. 2. Clarity of skin. 3. Strong sense or-gans 4. Prevention of greying of hair.

Merits of DhumapanaMedicated smoke (Dhumapana) is done after nasya. It also cures cough, dyspnoea, rhinorrhoea, hoarse-ness of voice, halitosis (Bad breath), anaemia, diseas-es of hair, discharges, itching and pain of ear, mouth and eyes, lethargy and hiccup.

Merits of Buccal Retention etc.Gandusha (Buccal retension), Kabala (Gargling), Mukhalepa (Herbal facial) and Moordha Taila (ap-plication of oil on the head) are associated treat-ments in Panchakarma. Gandusha and kabala are always good for teeth. It cures the diseases of oral cavity. It also alleviates dis-taste, removes sediments, tongue coatings and hali-tosis.

In persons habituated with Mukhalepa, the vision becomes perfect. The face will never be gloomy. The face will become smooth and charming.

Murdha taila cures falling, graying, bronzing and splitting of hair. It cures diseases of head with vata origin. It provides strength to the voice, lower jaw and head. Clarity of sense organs is also attained by proper usage of Murdha Taila.

In this manner panchakarma and allied procedures are helping human being to prevent diseases. The knowledge regarding the effects of panchakarma is to be generated among public. Then most of the pub-lic will be attracted towards Ayurveda. Panchakarma Department is functioning in all Ayurveda colleges. Most of the leading Ayurveda hospitals are having attractive Panchakarma Department. Trained phy-sicians and technicians in the field of Panchakarma can bring back the glory of Ayurveda.

CREATING PUBLIC AWARENESS ON PANCHAKARMA

The cleansing procedures for the body are generally called as panchakarma. If doshas are vitiated highly, then panchakarma is to be carried out. The root lev-el cleansing will bring complete cure of the disease. Normal person can also do panchakarma to prevent the occurrence of diseases.

We usually carry out Servicing of vehicles (car/ scooter/ bus etc) in regular intervals. The mechanic will check all the parts of vehicle and carry out ser-vice thereby the life of vehicle will be increased and it makes smooth functioning of vehicle possible. Same manner, if our body is serviced in time, we can keep free from diseases. The servicing of our body is con-sidered as Panchakarma.

Vamana (medicated vomiting), virechana (medicat-ed purgation), Snehavasti (Enema with oil), Kashaya vasti (Enema with decoction) and nasya (nasal med-ication) are coming under panchakarma. Vagbhata has included Raktamoksha (Blood Letting) in place of snehavasti and renamed as Panchasodhana (five purificatory measures).

The pre operative procedures also play important role in the efficacy of treatment. Before vamana and

virechana, snehapana (intake of ghee or oil) and sve-dana (sudation) are usually done to loosen the body and to remove the rigidity.

The Merits of SnehanaThe person, who consumes sneha (medicated ghee) judiciously, can live hundred years with good diges-tive fire, clear gut, physical strength, good complex-ion, stable sense organs and slow aging.

Achapana (intake of sneha alone), vicharana (intake or external application of sneha combined with food, enema, nasal application, gargling, retention of oil in eyes, head, ear etc.) and sadya sneha (mixing with soup, gruel etc.) are various types of sneha.

The Merits of SvedaSudation is to be selected according to the condition of disease and diseased person. The selection of var-ious sveda is to be done logically. Particular order is mentioned while describing types of sveda. Tapa, Upanaha, Ushma and Drava are to be performed in order. Generally various sveda is performed in vat-avyadhi (arthritis like conditions and pain in joints). If it is done from rooksha to snigdha order, will be found more effective. EXPLORING THE BASICS OF DRAVYAGUNAVIJNANA

Practicing herbs as the natural remedies for the dis-ease management and its prevention is dated back to the ancient period. Diverse arrays of healing sys-tems are grown up in the grounds of such practices around the globe. Among them, Ayurveda, the in-digenous medical system of India places an import-ant role in maintaining the health and prevention of diseases. Our ancestors keenly observed the nature and they turned this observational knowledge into a perceptional level. A plant grows on rocks by break-ing the stones using its roots may helpful in remov-ing the urinary stones formed inside the body. This insight made them to observe the nature more and thus developed the treatment modalities for various illnesses. This theory is known as Lokapurushasat-myavada explained in Charakasamhitha and is one of the fundamental principles of Ayurveda. A ratio-nal, well developed pharmacological and pharma-ceutical basis lays the foundation of therapeutics in Ayurveda. As per the shastra, four important fac-tors (Paadachathushtaya) are needed for a success-ful treatment. The factor which is important in the treatment right after the Vaidya is Dravya (Drug). Without the real knowledge in dravya, a Vaidya can-not make the treatment fruitful. Thus Dravyagunavi-jnana becomes an integral part of all the branches of Ayurveda.

Ayurveda is nothing but the science which imparts knowledge about life (Ayu) through the analysis of ayushyaani (substances which promotes longevity)

and anaayushyanidravyas (substances which ham-per the longevity). Dravyagunavijnana is the branch of Ayurveda which exclusively mentioned for the detailed study of such dravya and its attributes. It is a comprehensive discipline which embraces various aspects of knowledge about the identification, col-lection and classification of drugs; their physical and chemical properties; biotransformation and meta-bolic activities; uses, dosage and therapeutic actions; compatibility, synergism and vehicle; incompatibil-ity and toxicity, and their different pharmaceutical applications.

The word Dravyagunavijnana comprised of two words dravya and guna. Here, the latter covers gu-na(qualities) and karma (functions), which are the inherent parts reside in dravya. In Ayurveda, every dravya is considered as panchabhouthika. The pro-portion of panchamahabhootha determines the guna of a dravya. Based on that proportion, dravya does the karma, the therapeutic action in the body. The qualities present in the dravya are also present in the body. Because “deho hi aaharasambhava” (body is made up of the food we consumed). Hence, the sim-ilar (samanya) qualities of a dravya will increase the similar qualities in the body and dissimilar (vishe-sha) qualities will do the opposite. It is the dravya which responsible for the vrddhi(increase) and ksha-ya(decrease) of dosha, dhathu&mala inside the body and influence the srothamsi (channel in the body).

There are so many classifications of dravya are avail-able in the classics. The most fascinating one is based on its source.

• Oudbhida (Plant origin)• Jangama (Animal origin)• Parthiva (Mineral origin)

There are subdivisions under each category, which covers most of the natural things in the universe. There are around 1200 medicinal plants are men-tioned in the samhithas. Some more are added during the nighantu period and Ayurveda says, “Jagatyevam Anoushadam”. Every plant in the universe is having the medicinal properties when it is used by a Vaidya with proper yukthi. So one can identify such medic-inal plants, only with the thorough knowledge in assessing its physical and innate powers, here shows the importance of Dravyagunavijnana.

Ayurveda attributes the mode of action of the drug to the five principles (Rasapanchaka) namely Rasa, Guna, Virya, Vipaka and Prabhava. Majority of the times these gunas (attributes) may not be measur-able but inferred through their pharmacological actions. The study of the biological and therapeutic effects of the drugs (what drug does to the body) are well explained by these principles which can be iden-tified as pharmacodynamic principles of Ayurvedic pharmacology. These pharmacodynamic principles also influence the pharmaco-kinetics (study of the absorption, metabolism and excretion of drugs and their relationship to the pharmacological response) of the drugs. Routes of drug administration play an important role in the elucidation of drug action. A drug may exert different effects when given by dif-ferent routes. Madanaphala when given orally acts as Vamana dravya (emetic) and if administered with Vasti dravyas (ingredients of enema) facilitates for its quicker transportation in the colon and acts as a synergistic for toning up the nervous system or relieving intracranial tension. Yashtimadhu (Gly-cyrrhiza glabra) when used in an optimum dose act as shamanoushadhi and in higher doses, it acts as a vamanopaga (enhancing the vamana). The po-tent drug Lashuna (Allium sativum) is disagreeable for internal administration as kalka/ swarasa, when it made into a ksheerapaka, it becomes easy to con-sume and the potency will be optimised. So, Maatra,

Kalpana (Dosage forms), Sevanakaala (Time of in-take) and anupaana (vehicle/carriers) are important factors regarding the drug action.

In the modern pharmacology, the drug action is ex-plained through its bio molecules which are the sec-ondary plant metabolites. The following important pharmacologically active principles in medicinal plants include;

a. Alkaloids (Basic substances containing cy-clic nitrogen, which are insoluble in water but combine with acids to form water soluble salts) which is found in Guduchi (Tinospora cordi-folia), Kaalmegh (Andrographis paniculata) etc

b. Glycosides (Ether like combinations of sugars with other organic structures): Sathavari (As-paragun racemosa), Triphala

c. Oils (Fixed oils, volatile oils and mineral oil):Eranda (Ricinus communis), Tila (Sesa-mum indicum), Lavanga (Syzygium aromati-cum)

d. Resins (insoluble in water and soluble in alco-hol): Guggulu (Commiphora mukul) ,Asana (Pterocarpum marsupium)

e. Oleoresins (mixtures of volatile oils and res-ins): Hingu (Ferula asafoetida), Shallaki (Bo-swellia serrata)

f. Gums (secretory products of plants and are dispersible in water): Khadira (Acacia cate-chu), Snuhi (Euphorbia neriifolia)

g. Tannins (Non-nitrogenous plants constituents and precipitate proteins of cells: Asoka (Saraca asoca), Kutaja (Holarrhena antidysenterica)

h. Antibacterial substances fungi and moulds: Shilapushpa (Parmeliaperlata)

Every human being is unique in their phenotypical and genotypical manner, that’s why Ayurveda gives more importance to personalised cure for diseases and its prevention. Similarly, the dravya also works in a unique manner for each individual, that’s may be the reason why we may fail to establish the drug ac-tion of Ayurvedic herbs through scientific research-es. EXPLORING THE DOCTRINE OF TREATMENT - KAYACHIKITSA

Kaya chikitsa is the most important clinical branch among eight major branches of Ayurveda which are –known by the term “Ashtanga Ayurveda” col-lectively. Kaya Chikitsa is clinical skill oriented and most broad branch of Ayurveda system of medicine and may be called as backbone in clinical practice of Ayurveda. The nomenclature ‘Kaya Chikitsa” is composed of two words- “Kaya” and “Chikitsa”. The term Kaya refers to Deha (body) and Agni (Diges-tive fire of body) and chikitsa mean ‘treatment’. The literal meaning of the subject is the clinical specialty of Ayurveda dealing with the treatment of diseases involving whole body (sarvanga sanashrit vyadhi-naam) viz. skin diseases, metabolic disorders, fevers, psychiatric disorders, neurological disorders etc. : as per ayurveda, the root cause behind almost all these diseases is Malfunctioning of Agni ( force respon-sible for digestive processes and metabolism in the body). So, the main treatment approach (chikitsa) in Ayurveda targets at correction of abnormal state of Agni and hence the nomenclature of the subject.

As per Ayurveda science, the living body is not only a physical body but Mana or satva (mind), Atma

(soul) and Indriya (sensory organs) all together. So for attaining healthy state and also for treatment purposes, ayurveda medicine focus on measures and drugs which not only balances dosha, dhatu and mala of the body but also maintain the proper func-tioning and coordination of mind with body and soul along with proper functioning of sense organs. That is why; the treatment in Ayurveda has Holistic approach and hence called as “Holistic Medicine”.

Preventive medicine is also recognized as an import-ant subject which also come under Kayachikitsa . As for the proper treatment of diseases, proper diagno-sis of the condition is mandatory, so diagnostic pro-cedures (Rogi pariksha) also forms important part of it and taught as clinical/ Bedside medicine. Kaya chikitsa deals with the general and specific therapeu-tic principles of a vast range of disorders affecting almost each and every organ system like Gastroin-testinal, cardiovascular system, respiratory system, Genitourinary, Endocrine system, Nervous system etc. Some important fundamental and specific ther-apeutic principles applicable in treatment of diseases are discussed below-

Kayachikitsa

1. Prevention from diseases (Swasthsya Swathya rakshanam) has given more importance in Ayurveda than cure.

2. Everything in this universe is made up of five basic elements viz. Akaash, Vayu, Agni, jala and prithvi called as “Panchmahabhoota” (Yat pinde tat brahmande)- it is most important fundamen-tal principle of treatment.

3. The human body should be treated not only physically, but also panchmahabhoot (five basic elements), mind, soul and senses – so treatment aims at treating all together for complete relief from ailments.

4. Nidana parivarjan (abstinence/ avoiding of caus-ative factors) is basic factor in prevention and cure of all diseases.

5. “Naanaushdhibhootajagate kinchitah”- means everything in this universe has some medicinal value.

6. There are three basic treatment approaches- drugs (Pharmacological), spiritual and psycho-logical.

7. Body is made up of Dosha (three basic humours in body), Dhatu (tissues) and Mala (waste prod-ucts of digestion and metabolism) which are Panchbhootatamaka (means physiological and anatomical forms of panchmahabhoota in body). Equilibrium and imbalance in these is responsi-ble for normal functioning of body and occur-rence of diseases respectively.

8. The prime and most important pathological fac-tor behind every disease is derangement or weak Agni (digestive fire).

9. Four pillars of treatment (Chikitsa Chatushpada) – Very unique principle of ayurvedic medicine. They include efficient physician, potent drugs, good attendants and obedient and strong patient – all perfect fours’ are mandatory for perfect & successful treatment.

10. The Ayurveda treatment strategy is to bring do-sha, dhatu, Mala back in balanced physiological condition and in the meantime psychological and spiritual happiness.

11. Perfect treatment (Visshudh Chikitsa) - A unique principle of Ayurveda system of medicine which make it holistic and best therapy is -“a perfect treatment is that which cures one ailment with-out vitiating body’s normal physiological func-tions and producing other new disease or side effect”.

The preventive medicine is an important branch of Kaya chikitsa which focuses on development of im-munity in the people by suggesting various measures related to diet and routine life style, use of Rasay-an (immunity enhancing) drugs and ethical codes& conduct (behavioural modifications and spiritual upliftment) called as Sadvritta or Aachar rasaya-na. Ayurveda also accepts the theory of Karmaja Roga (diseases caused by evil deeds). So, the branch Aachar Rasayana is very important which deals with spiritual upliftment.

The importance of Yoga and Pranayama is also well known for their role in prevention and cure of dis-eases along with getting supreme objective of human life as Moksha.

We can say that Kaya chikitsa subject is a very vast, skill-based, practical, patient-oriented / clinical sub-ject which may be called as the backbone of Ayurve-da system of medicine. There is nothing greater than contributing to society by playing role in the preven-tion and cure of diseases.

The assessment of proper maintenance of Dosha, Dhatu, Agni, Mala is not so easy as they are not mea-surable. Hence one should be keen on observing the Prasannata of Atma, Indriya and Manas as they are the indicators of happenings inside the body.

Tridosha (Vata, Pitta and Kapha) are considered as main pillars of body whereas Ahara, Nidra and Brahmacharya are considered as supporting pil-lars. Hence any hitch in these supporting pillars will shake the balance of Tridosha and lead to disease manifestation.

Importance of Ahara in Health MaintenanceIn order to maintain the balance of Tridosha, it is im-portant for an individual to consume balanced diet throughout his life. Careful selection of food items belonging to various food categories (Ahara Varga) is essential to meet the energy requirements and the requirements of macronutrients and micronutrients.

The selection of food items is also influenced by few other factors like Prakriti (basic quality of food), Ka-rana (resultant quality of food processing), Samyoga (different combinations), Rashi (Total quantity and proportion of individual component of meal), Desha (place of origin and consumption), Kala (seasonal food), Upayoktr (personalized selection) and Upayo-ga Samstha (guidelines for food intake).

Food consumed should be fresh and warm, added with little amount of ghee/oil, including all 6 Rasa (sweet, sour, salt, pungent, bitter, astringent) with predominance of sweet taste. One should consume food only after digestion of previous food and quan-tity should support easy digestion within 3 to 6 hours.

Importance of Nidra in Health MaintenanceWhen body and mind is tired of various activities throughout the day, the body demands rest in the form of sleep. Natural sound sleep nourishes and strengthens the body, rejuvenates and promotes health. Improper sleep patterns like day sleep, ex-cessive sleep and reduced sleep can result in imbal-ance of Dosha. Abhyanga (application of oil to whole body), taking bath, intake of rice with curds, intake of milk, meat soups, application of cooling Lepa on scalp, pleasant state of mind are few factors which help to impart sound sleep. Disturbed state of mind such as fear, stress, anxiety and anger, tired body due to excess exercise, fasting, uncomfortable bed are some of the factors responsible for insomnia or dis-turbed sleep.

Importance of Lifestyle in Health MaintenanceThe state of Dosha fluctuates during different phase of the day & night, and also in accordance with season-al variations. To combat these fluctuations Ayurveda has specified some regimen to be followed daily (Di-nacharya) and seasonally (Ritucharya). Apart from these, timely attending natural urges such as faeces, urine, hunger, thirst etc influence the health.

Routine of an individual starts with waking up in Brahma Muhurta (early morning in between 45 to 90 minutes before sunrise), the most suitable time to keep the mind calm and enthusiastic throughout the day. It is followed by regimens like Dantadhava-na (brushing), Jihva Nirlekhana (scraping tongue), Snana (bath), wearing clean cloths etc for maintain-ing personal hygiene. Certain regimens such as An-jana (applying collyrium), Nasya (instilling medicat-ed oil to nostrils), Dhoomapana (inhaling medicated fumes), Abhyanga (application of oil to whole body), Karnapoorana (instilling medicated oil into ears), Kavala-Gandusha (gargling medicated decoctions or oil) are aimed at strengthening the sensory organs. Daily regular exercise is also essential to strengthen the body and keep an individual active.

Environmental changes according to the seasons are also having influence on the health. Food and activ-ities (seasonal regimen) should be adopted to over-come these changes and influences. For example, consumption of liquid predominant diet in summer to overcome dehydration caused by excessive heat, wearing thick woollen cloth in winter to maintain body temperature etc. Regimen of the upcoming season should be gradually adopted. Sudden Shift in the regimen may end up in health issues. Seasonal purificatory procedures are explained like Basti Kar-ma in rainy season, Virechana Karma in autumn and Vamana Karma in spring to pacify aggravated Vata, Pitta and Kapha Dosha respectively.

Flatus & Faeces are the by-products/ end-products of digestion which are to be attended on time. Regular suppression of urges may end up in health problems like heaviness of the body, indigestion, constipation, headache, body ache etc. Suppression of Urine may end up in hampering the urinary system causing uri-nary calculi, pain in urinary bladder, groins etc and

also body ache. Sneezing, coughing and vomiting are the protective primary response for any exter-nal stimulant and are not to be suppressed. Hunger, thirst, sleep are the natural physiological demand of the body which are to be fulfilled to remain healthy.

Importance of Mental StabilityNot only Tridosha are mere responsible factors for disease formation but also imbalanced manodosha namely Satva, Rajas and Tamas also contribute caus-ing various diseases. Hence proper management of Manodosha is also important. In this aspect Sadvrit-ta (social and behavioural conducts), Achara Rasay-ana, Manovega Dharana etc are explained.

Sadvritta emphasizes about speaking truth, showing empathy towards others, helping needy people, being calm, avoiding sinful activities, respecting elders etc which also open a healthy atmosphere for communi-cation with the society. It also emphasizes the social responsibilities like covering mouth while cough-ing, sneezing, keeping distance with the people who are having communicable disease etc. Swasthavritta also emphasizes the control over certain feelings like being greedy, jealous, enmity, intolerance to others good works, excessive desires etc.

Keeping self fit and fine in different aspects like phys-ical, mental and social planes also satisfy the WHO definition of Health as “Health is a state of complete physical, mental and social wellbeing and not merely the absence of disease or infirmity.”

Environmental responsibilities:Swasthavritta explains about the importance of en-vironmental factors in upholding the health of the society. Causes for environmental pollution, resul-tant changes in the environment, and their impact on health status as well as measures of management

are well described. To keep the air purified, different fumigatory drugs and techniques are explained. Wa-ter can be kept pure and drinkable by purificatory procedures like boiling, dipping heated iron ball etc. Measures to enhance the odour and appearance of water are well described. Preventive and protective measures while building house, kitchen, or even in Sutikagara, Kumaragara etc are explained. Measures such as sweeping/wiping, sprinkling medicated wa-ter (for sanitizing), smearing cow-dung are described to keep the land clean.

CONTEMPORARY RELEVANCE OF TOXIC CONCEPTS OF

AYURVEDAA medical speciality is a branch of medical practice, which is focused on procuring skills to treat a de-fined group of patients and their diseases. The sys-tem of modern medical specialities evolved gradual-ly during the 19th century. Throughout history, the most important division was into surgical and inter-nal medicine. Like any other science, Ayurveda also contains speciality branches through which it offers the utmost benefit for diseased persons and helps to maintain a healthy body and mind. Ayurveda consists of Ashtanga or eight speciality branches. The classi-fication of these eight branches is based on Nidana and Samprapti of a disease. When we go through the texts, the signs and symptoms of many diseases are similar, may it be any clinical condition seen through the perspective of any branch. But, when the treat-ment approach for those diseases is observed, they differ to one another and are primarily based on the respective Nidana and Samprapti. It is the root cause of the disease and the course of its progress which decides to which speciality care the patient needs to consult. Such methods of interventions yield more fruitful results, which can only be cherished through Ayurveda.

Agadatantra, is one among Ashtanga, which is com-monly called ‘Visha Chikitsa. According to Chakra-pani Datta’s commentary on the statement of Chara-ka Acharya in Chikitsa Sthaana 23rd chapter, the spectrum of diseases caused by Visha can be broadly

classified into those symptoms produced by external exposure of toxins (dehasambandhi visha) and those produced after the intake of food materials(annadi-gata visha). Acharya has given equal importance to both exposures which signify the role of contaminat-ed food intake in disease causation. Thus, it includes all diseases as explained in Nidana Sthaana where the toxin is one of the important causative factors (either from external exposure to a toxin or through the food) for disease manifestation. The modern system of medicine has started to quantify the sig-nificance of various kinds of toxin exposure (envi-ronmental and through food) and their relation to disease causation. Here more than the bacterial tox-in load of food, other food additives and adulterants which gets mixed with food (colors, preservatives, sweeteners etc.) is more important. Various forms of exposure to toxins occur through different adhista-na of visha (plant origin - Sthavara, animal origin – Jangama and mineral origin – dhatu) and artificial combination (Kritrimavisha). The importance of Agadatantra treatment principles in snakebite man-agement and other insect bites which is effectively practices in many regions are beyond explanation. The areas least focused by Ayurveda physicians in-clude the concept of Gara visha (Kritrima/ artificial), Dushivisha and Viruddha ahara in disease manifes-tation. Here Gara visha and Dushivisha can enter the body through both ways, through food and by other external sources.

Sthavara and Jangama VishaBased on their origin, they have been classified into two categories viz. Jangamavisha (animal poison) and Sthavaravisha (plant and mineral poisons). The sites of Sthavaravisha and Jangamavisha are narrated as ten and sixteen respectively.

Poisons, which originate from plants and minerals, are classified into ten types depending on the part of the plant. These are kanda (type of root), sara (ex-udate), niryasa (oleoresin), pushpa (flower), mula (root), phala (fruit), patra (leaf), twacha (skin), dug-dha (milky exudates from the plants) and khanija (minerals). Poisons which originate from animals like snake venom, scorpion sting, rodents, dog, leech, and insect secretions and sting come under this type. Symptoms like dizziness, burning, oede-ma and loose motions are the generally developed clinical features. The role of treatment principles of Agadatantra in this area is very familiar and in use from time to time.

Gara VishaThis complex concept explains the method of ac-cidental or deliberate poisoning by preparing dif-ferent combinations which can either be with two poisonous compounds or by two non-poisonous compounds. Different modalities of administration were explained, of which, one is Ahara. It is said that such Gara visha causes varied clinical presentations. Since it is having the character kalanthara vipakit-wam, it resides inside the body for an indefinite peri-od and produces cumulative toxicity.

In the present era, Gara visha includes, wrong com-bination of medicines (adverse drug reaction), pres-ence of harmful chemicals like pesticides in food materials, chemicals added in food processing and packing (artificial sweeteners, colors, preservatives) exposure to chemicals by the usage of cosmetics, footwears, ornaments etc. indicating the burden of toxin exposure. It also includes a few systemic ill-nesses which have a definite history of exposure to Gara. When we consider the disease manifestation in a person presented, Gara is not a diagnosis, but a cluster of symptoms which mimic an individual dis-ease.

Viruddha AharaThe concept of Viruddhaahara signifies the impor-

tance of wrong combination of food materials which itself is not a diagnosis, but only a cause for disease formation. This concept comes under the non – poi-sonous combination of Gara visha. It is very import-ant to correlate the mechanism as to how Viruddha Ahara is a cause of many metabolic disorders. It is also essential to know how certain food combina-tions interact with each other and create disease. Acharya Charaka clearly says that certain diet and its combinations, which interrupts the metabolism of tissue, which inhibits the process of formation of tissue, and which have the opposite property to the tissue are called as Viruddha Anna or incompatible diet.

The food, which is wrong in combination, prepared through wrong processing, consumed in inappropri-ate quantity, and/or consumed during the incorrect time of a day and in the wrong season for a consider-able period can lead to the diseases due to Viruddha Ahara.

According to Arunadatta’s commentary on the state-ment of Acharya Vagbhata in the 7th chapter of Sutra Sthaana, the diseases manifesting due to Viruddha ahara can present in an acute or chronic patterns like that of the intensity of Visha or Gara respective-ly. Here we can see the acute clinical conditions like Amavisha (Alasaka, Visoochika), Moorcha and vari-ous surgical emergencies that may arise due to them (peritonitis and septicaemia can be considered). At times the condition can be fatal. It can also be pre-sented as chronic clinical conditions like Kushta, Grahani and Sotha.

DushivishaThe concept covers the area of cumulative toxicity. Day to day exposures to these chemicals (dust, gas-es, metals and other chemicals) accumulates in the body and act as Dushivisha. It signifies the regular exposure to these chemicals (gradually increases the toxic concentration in blood). The role of environ-mental toxicants is yet to be identified. Dushivisha is any intrinsic toxin (a Sthavara, Jangama or Samyoga/Kritrima visha) which had already entered the body and has been manifesting a recurrent symptom com-plex.

AgadatantraA clinical condition is only fit to be called a Dushi visha origin, if it has not been eliminated by prop-er treatment, resides inside the body in deeper tis-sues in dormancy (Kaphavrtatva) for years together (Varsha gananubhandhi) and manifests as a disease when suitable conditions arise. Thus, the symptoms produced by Gara visha and Dushivisha will be sim-ilar, the only difference being, Gara visha lakshana are the manifestation on the first exposure to a tox-in and Dushivisha requires repeated exposures and manifestations. It is to be noted that all the Acharyas have not enlisted the diseases caused by Dushivisha. The manifestations have been brought under the umbrella of the site of affliction and Dhatu pradosha-ja Vikara (according to the Sthaana samshraya of Dushi visha).

At present time Chronic neurotoxicant exposure (lead, opioid analgesic drugs, aluminium, copper, industrial gases, organophosphates, insecticides, well water drinking and pesticides) requires weeks to years and in some cases, decades to produce cell dysfunction and cell death that result in detectable neurological alterations and may result in neurode-generation, meaning the loss of structure or func-tion of neurons. Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, and Amyotrophic Lateral Sclerosis are some of the diseases identified by mod-ern scientists with a positive correlation of disease manifestation and environmental toxin exposure. Likewise human exposure to certain chemicals (ben-zene, halocarbons, ketones, nitrosamines) induc-es inflammatory states that may end up in diseases like nephritis, hepatitis, scleroderma etc due to the production of reactive oxygen species. Even during the pregnancy period, exposures to harmful envi-ronmental contaminants can cause permanent and irreversible anatomical, functional and metabolic changes in the body of the mother and fetus. Chron-ic, non-communicable diseases (NCDs) linked to early environmental exposures include disorders of neurobehavioral development, adult and pediatric asthma, hypertension, obesity, diabetes, cardiovas-cular disease and cancer. At present time we can ex-plain these factors under the term Dushivisha.

The importance of dushivisha, is that when a disease has Dushi visha as nidana the treatment should in-variably include removal of toxicity through shodha-na followed by samana medicines and then by rasay-

ana for a better longevity. Here lies the importance of the concepts of Agadatantra. Since the diseases presented by the patients will be similar, but it can easily misguide the physician and the content of vi-sha will get unnoted. So, whenever a disease does not respond to conventional treatments, an element of Visha can be considered.

A few clinical conditions which have Visha as a ni-dana include:1. Agantuja Jwara (presently Covid-19 and includes

fever in all kinds of infective conditions) and the cases with recurrent fever (Punaravartaka Jwara)

2. Pandu- Gara is mentioned as a nidana and while explaining the treatment Acharya has mentioned Dadimadi Ghritam which is having Vishaghna property also.

3. Udavarta- one of the nidana is Gara.4. Udara- In ascitis like conditions, one of the ni-

dana is Gara.

In the present era, Chronic neurotoxicant exposure, food, which are wrong in combination, presence of harmful chemicals like pesticides in food materials hold a major share in the manifestation of diseases. If we consider the above concepts of Agadatantra while eliciting and evaluating the history most of the eti-ological factors and pathogenesis of diseases which visit our hospitals have relevance. With all these fac-tors, Agadatantra is of paramount importance in to-day's world. This branch of Ayurveda is important in all the areas of human life beyond just being consid-ered as the treatment of bites and stings.

SHALYATANTRA - A PEDAGOGY OF AYURVEDIC

SURGICAL PRINCIPLESImagine a time when there are no doctors and the world is suffering from a lot of diseases. A group of noble thinkers decides to meet up and find a solution for the same. They travel to Kashi, a place that is the abode of King Dhanwantari. They have heard about the King who has been doing miracles and helping people in need. His abilities were said to be nothing short of Godly.

This group of people approach the great King and request him to teach them this noble art of healing. It would have been approximately 5000 years and above since that event conspired and the legacy of Lord Dhanwantari still carries on through genera-tion to generation of Shalyatantra practitioners un-til today. The science of Ayurveda is a supreme one since its ultimate aim is to eradicate diseases rather than to treat them alone.

In Ayurveda, there are two main schools of thought. The Bharadwaja and Dhanwantari school of thought. Bharadwaja school, has been mainly inspired from

Kayachikitsa (conventional medicine) as their treat-ment methodology and the Dhanwantari school holds Shalyatantra supreme.

Ayurveda was divided into eight branches. This was to facilitate the study of Ayurveda as well as to make it easier to practice. The eight branches thus divid-ed were Shalyatantra, Shalakyatantra, Kayachikitsa, Kaumarabhritya, Bhootavidhya, Agadatantra, Rasay-anatantra and Vajeekarana.

Diseases of each branch and its particulars are de-scribed in the Sushruta Samhita, which is a part of the Brihatrayi (Charaka Samhita, Sushruta Samhita and Ashtanga Hridaya) in Ayurveda.

Acharya Sushrutha explains the branches and the diseases that come in their respective preview in the first chapter of the sutrastana itself because a student can choose his line of expertise and study accord-ingly.

It is interesting to note that the basic principles of Sushruth samhitha consists of all the details the stu-dent needs to assist in the procedures that the OPD of Acharya Sushrutha might have dealt with at the time. This would have been an orientation course for the student to swiftly get acquainted with the proce-dures, instruments and other aspects of the Shaly-atantra.

Shalyatantra is one of the branches which deal with surgery, marma chikitsa (vital points related proto-cols), orthopaedics etc. The Sushruta Samhita opines that Shlayatantra is the most important treatment because of its many utilities. The application of its protocols and methods can be used in many diseases and conditions.

Even though there is a good deal of reference to Shalyatantra in the Charaka Samhita and Ashtanga Hrudaya, they seem to have given more importance to the Kayachikitsa division of Ayurveda.

In a very simple language, the word “Shalya” means something that is not needed in the body. For exam-ple, thorns, nails, arrow heads, metal bits, stone, pus, body waste etc. The treatment protocol for removing it from the body comes under Shalyatantra (Shalya nirharana vidhi). There are two kinds of shalya- that which affects the body and that which affects the mind. There are four main protocols for the same in Shalyatantra- Yantra, Shastra, Kshara and Agni.

Sushrutha samhita tells us that if you practice Shaly-atantra, the practitioners will become prosperous, fame will come to him, it would become a worthy profession for livelihood and he will surely be in the good book of Chitraguptha.

Shouchyam (Cleanliness), Ashukriya (fast and effi-cient), Shastra Thaikshneeyam (Proper Instruments/ sharp instruments/ calibrated etc.), Asweda (un-flinching under pressure), Vebhathu (Steady Hands), Asamoha (The surgeon who is mentally strong/ does not faint), Vaidyascha (Physician), Shastra Karma-ni Sashsyathe (is qualified for surgical procedure). These are the qualities required by the vaidya who is to take this branch as a speciality.

It is important to note here that Acharya used the

word Vaidya here, which means the student after the completion of his first part of the study of Ayurve-da, can choose this branch of Shalyatantra. This also means that this branch was the first Super speciality in the history of Medical science.

Another question that might arise in the mind of a student would be if Shalyatantra contains only sur-gery. A Shalyatantra practitioner should adopt the protocols in Shalyatantra only if the other treatment protocols fail to give any results to the patient, which clearly means that he or she should be an expert in Kayachikitsa also. Some experts do go straight to Shalyatantra procedures in certain cases. This is their experience and their reasoning (yukthi).

There are various other treatment protocols in Shaly-atantra like Rakthamokshana, Agnikarma, Marma chikitsa which are used frequently in various diseas-es and conditions. They are very effective when used properly by a skilled practitioner.

In Raktamokshana, blood is drained using various methods. Here the concept of a fourth dosha is ap-plied. This is a revolutionary idea if you think about it. It is sad to see that it is usually used as an insult in the western world. Any procedure if used improper-ly can fall out of favour. This procedure is a fascinat-ing one which gives fast and sure results if used wise-ly. One of the methods in Rakthamokshana therapy known as Jalaukavacharana (Application of medical leeches) is catching on in the name of Hirudotherapy in the modern surgical field and it is used in plastic surgery. It is very common for the western scientific world to take ideas from our science, put a new name for it in Latin and baptise them “new”.

Ksharakarma is the method of removing the root of the disease using alkali preparations. This is a won-derful method that ensures results for the patient with ease. It is widely used in the proctological field which is one of the most valuable contributions that Shalyatantra is making nowadays. The procedures like Ksharakarma, Ksharsutra etc. are well practiced and are considered best in proctological practice in the world. It has inspired the modern practitioners to adopt similar medicated seat on methods and pro-cedures that have given excellent results.

ShalyatantraOf these, Ksharasutra procedure is the most well-known procedure. It is a medicated set on that gives the best results which have been duplicated by sur-geons from other countries like Japan. The recur-rence rate of Fistula in ano is tackled only by Kshar Sutra therapy.

Ksharasutra procedure is a method of ligating in cas-es such as a Fistula in ano, hemorrhoids etc. It is in-deed a versatile method and is widely practiced and researched in our field. A multitude of papers have been published on it in universities at the top of our educational system.

Agnikarma is the procedure of cauterising and stim-ulating specific points on the body to get immediate relief from certain diseases. There are many aspects of this amazing therapy which a trained Shalyatantra practitioner can work magic with. This is the most important because the recurrence rate after this pro-cedure is nil.

The diseases that come under the Shalyatantra spe-ciality are fracture and dislocation Management, or-thopaedic conditions, various surgical procedures, ulcer management and basically any disease that cannot be handled by conventional medical proto-cols alone.

When it comes to orthopaedics, fracture and dis-location management has long been practiced by doctors who practice Shalyatantra. The fracture and dislocation management in Ayurveda is what mod-ern science follows in their treatment principles. The basics are the same but they have introduced many new innovative surgical methods for the same. Yet the Ayurvedic procedures manage the conditions without any surgery and complications. Only very complicated fractures and dislocations are advised to undergo surgery. Most of the time it is managed through closed reduction methods perfected with time. The three Rs which form the basis of fracture management are derived from Ayurveda. The chap-ters in Sushrutha samhitha on Bhagna chikitsa are a worthy read and are so thorough that till now noth-ing needs to be added to it and nothing could be dis-carded.

Another aspect of the Shalyatantra is marma chikit-

sa which is a branch that deals with vital points of the body and the management of injuries to those points. These vital points are also used to bring relief to certain conditions. Various marma manipulative techniques are also used in Shalyatantra for condi-tions like back pain, neck pain etc. which in turn have inspired the Chiropractic system in the western world.

The method of haemostasis (Raktha sthambana) is described in Sushrutha samhitha and it is the same that is being used nowadays in the most modern hospitals in the world using sophisticated machines. Suture materials that are absorbable and otherwise were first explained in Sushrutha samhitha. When the majority of people from our nation saw the mov-ie “Apocalypto”, where ants were used as sutures, I am sure they did not know that it was the contribu-tion of Ayurveda.

The management of ulcers is another very important treatment protocol. There are 60 methods that have been explained that can be used to manage even the most difficult of ulcers. Diabetic ulcers, varicose ul-cers etc. can be cured without the need of any surgical intervention in most of the cases. Fascinatingly sim-ilar to the Jwara chikitsa in Charaka samhita, Vruna chikitsa in Sushrutha samhitha explains the basics of Ayurvedic management of almost all diseases.

All the tools we use in Shalyatantra can be divided into two- Yantra and Shastra. Yantras are simply in-struments that are not sharp like forceps, probes, re-tractors etc. The shastras are sharp instruments that are used for the eight surgical procedures (Astavidha shastra karma). It is to be noted that the modern sur-gical instruments have their roots in Ayurveda and the instruments that we currently use are the updat-ed versions of the same.

Today Shalyatantra practitioners mostly deal with anorectal conditions -hemorrhoids, fistula in ano, fissure in ano, rectal prolapse, pilonidal sinuses etc. Fracture and dislocation management, orthopae-dic disorders, cancer and palliative care etc. are also practiced by some.

Outside Kerala, Ayurvedic doctors who practice Shalyatantra perform a variety of surgeries like

Gastric Surgery, Neuro Surgery, Laparoscopic proce-dures, etc. They are well trained and they keep up with excellent standards required.

Shalyatantra practitioners must be very active. If you look at the rest of the branches they can prescribe and treat with helpers. They do not have to be involved directly. But the Shalyatantra practitioner is active-ly involved in the procedures. My father Late Prof. Dr. P N Mohan Kumar who was also a Shalyatantra practitioner advised his students to be physically ac-tive and fit to perform these procedures themselves. He also advised his students that the method of re-ducing the pain of the patient is less important than the result.

There have been a number of setbacks in the field of surgery for Ayurveda. In western world, Barbers used to do these Shalyatantra procedures. It was done in a very unhygienic environment and there have been a lot of infections and deaths associated with such pro-cedures. Those made the physicians of the time look down upon them and judge them barbaric. When the European influence came here, people began to look down on it too.

Buddhism is one of the reasons for its decline in In-dia. In Buddhism, Ahimsa (non violence) was one of its core virtues. It is not my intention to blame it on Lord Budha who was a noble soul, but it is the frenzy of the followers to do one better that caused the downfall of this branch of ayurveda inside India.

Shalyatantra was demonised as an Asurika chikitsa and that stigma is still prevalent in the field. But you need a sharp weapon to take out a thorn.

For the better part of Independent India, the Gov-ernment was not sure where Ayurvedic surgery should come in. The common public are yet not sure if Ayurveda has surgery. When a patient asks me if there is surgery in Ayurveda, I answer with a ques-tion. Who do you think invented surgery?

Even in the historical times, no student or Vaidya could step into the surgical theatre directly. They had vigorous training processes that may be con-sidered old school but effective as mentioned in the Yogyasutreeya chapter. That shows the level of pro-fessionalism and ethics at the time. Once the teacher was sure the student was capable, then and only then they were allowed into the theatre. Standards were kept in the field of medical science and were accord-ing to the limitations of the time they lived in.

For a student who wants to see if taking up this branch as a speciality is advisable or not, I say go for it. It is my opinion that the student should choose a branch which gives satisfaction at the end of a busy day. As for me….. no where else I would rather be.

Rasa Aushadhi

Rasasasthra though it was originated and developed as a separate Science in the beginning, during the due course it completely merged with Ayurveda. It is not considered as one of the eight main branch-es of Ayurveda, however it gained so much impor-tance that it has become an indispensable part of Ayurvedic treatment. The basic idea of Rasasasthra OR Indian Alchemy was to covert a lower metal in to a higher metal ie. Dhatuvadha and in due course the same concept of converting a weak body OR dis-eased body into a strong one has developed ie De-havadha. Rasasasthra was developed to prepare such medicines, which will increase longevity of life, keep the healthy status of the body constant so that a per-son can serve the god longer and attain salvation.

Rasaushadis possess some unique character than herbal medicines like it gets effective in less dose, it does not processes any taste, immediate action & more potent. Mainly metals and mineral drugs are used to prepare Rasaushadies. Apart from these the drugs with animal, aquatic & plant origin are also used.

The pharmaceutical procedures involved in the preparation of Rasaushadies are quite complex and lots of repetitions are done to get the final product. During the pharmaceutical procedures, the metallic OR mineral drugs are treated with herbal drugs in the form of swarasa, kalka etc. Therefore, the final Rasaushadies will be going to be a complex herbom-ineral compound.

Shodhana (purification) and marana (conversion in to absorbable form) are the two pharmaceutical procedures mandatory for Rasaushadies. Shodhana process is not merely a purification process; it also imparts some other medicinal qualities to the drugs. In the same way marana process, is also not mere conversion in to smaller and absorbable particles. It also imbibes some more qualities of the herbal & other drugs in which it has processed.

Basically Rasaushadies can be divided in to four cat-egories depending upon the effect on Agnisamskara.1. Khalvi Rasayana2. Parpati Rasayana3. Koopipakva Rasayana4. Potali RasayanaBhasma cannot fait in these categories since only a single metal OR mineral is converted in to absorb-able form of it (exception are there like Trivang-abhasma etc.)

1. Khalvi Rasayanam:- This is the simplest form of Rasaushadi. Where the metals or minerals are converted in to bhasma form first and the Bhas-mas are mixed with herbal powders in definite proportion and titrated in a mortal & pestle with specific liquids (Bhavana dravya) when it become a consistency it is rolled in to pills with specific size. About 80% of the Rasaushadies are khalvi rasayanas.Eg:- Arogyavardhini Rasa, Tribhuvana keerthi Rasa & Brihatvata Chinthamanirasa etc.

2. Parpati Rasayana:- Here the basic metallic & mineral bhasma are mixed with Khajjali & then melted. The molted khajjali is suddenly poured to an even surface and pressed to form a crisp wafer called parpati.Eg:- Rasaparpati, swarnaparpati

3. Koopipakwa Rasayana:- The basic raw ma-terials (Khajjali with other mineral drugs) are heated for a specific time in a glass bottle with kramagni(mridhu, madyama and threevragni) to get the final product. The pharmaceutical proce-dures in the preparations of koopipakva rasaya-na demands experience and expertisation in this field. The final product is usually of sindoora var-na and are commonly called as sindooram. Eg:- Rasasindooram, makaradwaja (chandro-daya Rasa) etc.

4. Potali Rasayana:- As per the classic the ingre-dients of the preparation are made in to a small pyramid like structure and heated in molted sulphur. Presently these types of preparations are prepared by experienced vaidhyas only. An-other procedure is also in practice were all the ingredients are grounded in khalvayanthra and rolled into pills. Still the name potali rasayana is retained.Eg:- Hemagarbha potali rasa, Hamsagarbha pa-tali rasa etc.

As Rasaushadies, are very effective in almost all the critical conditions& present day Rasaushadies are gaining much popularity among the Ayurvedic practitioners because of its efficacy & low cost. At the same time the question about the toxicity is also emerging out. The toxic effect of Rasaushadi is not a myth rather it is a reality. The classical Rasasushada books clearly explain about the toxic effect of im-properly prepared Rasaushadi.

In olden day the vaidhyas OR physicians were used to prepare the medicine by themselves OR under di-rect supervision with maximum care in shodhana & marana procedures and there by the safety of final product was assured. Present days the practitioners do not have direct observation on these preparations because it was prepared somewhere and the phy-sician’s duty is restricted only in prescribing these

medicines. Hence there arises the safety issue.

The toxic effects of Rasaushadies mainly happen in the following ways.1. Improper & incomplete shodhana & marana

procedure 2. Excessive dose3. Prolonged use.

Proper standardization techniques should be made mandatory for the Rasaushadies, so that the first problem can be handled properly for some extent. So many tests have been explained for Rasaushadies to know the state of a final product & most of them are subjective parameters like varitharam, Rekha-puranan etc. With the help of modern equipments these subjective parameters can be converted in to objective parameters like Rekhapooranam to particle size etc. Before prescribing a Rasaushady the reputa-tion of the pharmaceutical company is also be noted so that it can be assured that the patients gets safe and effective drugs.

Before prescribing any Rasaushady it will be noted that the Dose & durations of the treatment is fixed. The prescriptions should not be allowed to be re-peated without doctor’s consultation.

There are some simple Rasaushadies which did not demand elaborate procedures made with simple ingredients and it should have to be brought in to mainstream. For example, Laghusoothasekara Rasa, Kaphaketu rasa etc, the pharmaceutical procedures involved in the preparations are simple and effective therapeutically as well as cost wise.

There is no need to be panic about the toxicity of Ra-saushadies. Rasaushadies are going to be toxic only if proper pharmaceutical procedures are not followed carefully. As it is said, ‘A drug known in respect of its name, form & properties OR even if known improp-erly administered leads to bad consequences.’

Viddhakarma therapy is an ancient Ayurvedic para surgical procedure mentioned in Sushruta Samhi-ta. Acharya sushruta is the pioneer of surgery in the medical system hence known as the father of surgery. Viddhakarma is a quick result promising treatment modality and a blessing to whole living kingdom.

Ayurveda has blessed us with multiple, effective and harmless treatments and medications. Viddha Karma is one of the eight Shastrakarma as defined in Sushrut Samhita. Viddha Karma also known as “Vedhan chikitsa” is the treatment of puncturing or piercing certain points that reduces pain and also helps to reverse the mechanism of pathology in cer-tain possible way. It is also known as Suchivedhana.

Now a day, Viddhakarma has come to major treat-ment stream and has become widely popular day by day in pain management and is used as main therapy in the management of Musculoskeletal disorders like Low Backache, Shoulder Joint pain, Cervical Pain etc. It is also used in some chronic non responding

ailments such as retinal disorders, abdominal diseas-es, ENT disorders, respiratory diseases, urinary sys-tem diseases and many more.

Pain is the common complaint in any musculo-skeletal disorder particularly, pain related to work. Considering the cost effectiveness, practice of vidha-karma related to Pain management is increasing day by day. Ayurvedic surgery in ancient India was extremely advanced today there was no doubts that ayurvedic Viddhakarma originated from practice of using needles in surgery and the knowledge of spe-cial points (marmashtana).

In time of Maharaja Ashoka near about 8th century BC, at the Indian Buddhist University, Takshashila, under the auspices of King Ashoka, the study of List-ed Science was conducted by a pioneer of Buddhist theology, 'Mosk'. Acharya Mosk was a staunch advo-cate of non-violence, urging any of his disciples to wield a weapon not allowed of course, he also insist-ed on not using the weapon for treatment.

This same religious significance lead to the birth of medical systems such as VIDDHAKARMA. Mar-tial arts, now considered one of the most important sports, was born as a form of therapy in which the opponent is attacked at certain points of the body without a weapon for self-defense, which is the basic principle of VIDDHAKARMA therapy. The spread of VIDDHAKARMA therapy, which originated on the basis of Buddhism and developed on the basis of ancient scriptures and mysticism, is due to the scien-tific and cultural heritage of India.

In ancient time, VIDDHAKARMA was frequently used by qualified vaidyas. Its quick responding and least time consuming treatment modality with nee-dles, which is indeed quite impressive. This method of treatment became prevalent all over India and de-veloped from India…. Yes you read it right… It’s not the modified form of acupuncture which originat-ed from china to India. It’s one of the purest form of Acharya Sushruta’s surgical expertise, knowledge and clinical implementation. After becoming preva-lent all over India, Viddhakarma, later on widely got spread in to China, Japan, Korea, and Sri Lanka. VID-DHAKARMA therapy also has a special place in the United States recently. Due to the predominance of Buddhism in China, there was a large class of people who believed in it, so a lot of research was done on it. That is why VIDDHAKARMA therapy is known as form of acupuncture and part of Traditional Chinese Medicine which is not at all true. These both treat-ment modalities are totally different depending on their basic principles and mode of action.

Many get confuse Viddhakarma as an Acupuncture method. The principles used in Viddhakarma are entirely different than Acupuncture. Viddhakarma points are in relation to Marma points and uses par-ticular Viddhakarma Shalaka for treatment. Viddha-karma points are based on cell morphology as de-fined in Vaisheshik Darshan, a branch of philosophy accepted in Ayurveda. In Acupuncture therapy, it involves the selection of specific points in the body, called meridians, and the flow of oxygen through the meridians throughout the body which is called "chi" in Chinese. The meridians connect the whole body to dynamic energy, harmonizing with the ecologi-

You get those butterflies, deep down your gut and you realize that you are breathing faster, your heart is pounding right in your throat and your mouth is dry, there is a kind of insecurity that’s burping from somewhere within and you hesitate to look around, especially to look back. There is some kind of a fear in the air,

a fear of the unknown. So you walk faster and faster, and there you are, right in front of your house. You open the gate and you don't even bother to close the gate behind you, then you literally run to the veran-da and even before you press that calling bell, your Mom stands right in front of you with opened doors and seeing her face, you feel at the height of security and hence, you breathe in life.

THE MAGNIFICIENT HUMAN! I guess it won’t be wrong, if I say that it’s quite like-ly that we do encounter incidents like the one men-tioned above at some or other point of time in our lives. Putting it a bit scientifically, what just happened to that girl is nothing but the ‘Primitive Human Re-sponse to Stress’. I address it as primitive because this is the mechanism which has helped the human race to survive through all the ages of early man era, when he was a hunter and gatherer in the deep dense forests on this planet, millions of years ago.

Where he had to hunt to feed his family, and mean-while an encounter with the wildest of the animals was literally inevitable. There exists a much discussed example of, the man hunting a rabbit for his family and a tiger trying to follow him either for the rabbit or for his life, and in both the cases, what he could do is to just run and escape, as fighting back with the beast is never a smarter option, and his children back at home sit hungry waiting desperately for the feed he brings and above all he must run for his survival.

So in short, this is a support mechanism of the hu-man body to safeguard itself from some kind of a danger. But to thrive in such a situation successfully, the human body needs abundant supply of energy, and a very crucial reaction time, wasting a second may be the soul reason for losing one’s life. Where does such ultra higher level of alertness, and deci-sion making and physical energy get sourced from? To be very precise the answer to this question is that, it all comes from your own body and we have a very perfect ‘Energy Storage Reservoir System’, of which we are actually unaware.

UNDERSTANDING THE HUMAN BODY Now, let me explain the mechanism of ‘Response to Acute stress’ in short. So, when there is a danger, technically addressed as encountering a stressor, this stressor could be either mechanical or psychological. The body reacts to this stress, and there are two kinds of responses that are possible. Either the body has to

defend it that is to fight against it or to escape away from it. The only condition that applies here is that, whatever it may be, it has to happen instantaneously. There is no fraction of time to be wasted. In both the cases the body has to do a lot of physical workout in order to maintain the homeostasis of the body. There is a superlative demand of extra energy, that too at a very short notice.

THE MECHANISMYour brain senses a danger. The nervous system takes up the action. The brain and the spinal cord create the ‘Structural- Central Nervous System’, but in this situation the show stealer is the ‘Functional Nervous System’ called the ‘Autonomous Nervous System’, which has a sub division called the ‘Sympathetic Nervous System’, which comes into action here and commands the Adrenal Glands to secrete hormones, like Adrenaline and Nor adrenaline immediately and Cortisol later.

You might have heard about the ‘Adrenaline Gush’ used commonly among us. This so called gush is known as the ‘FIGHT OR THE FLIGHT RE-SPONSE’ of the human body towards stress. When these bio chemicals or the hormones are released into the circulating blood certain changes occur in the body. Like –

• Increased respiratory rate and increased heart rate. This meets the extra energy demand by rapidly supplying high volumes of oxygenated blood to the tissues that are in constant work, say the skeletal muscles that play the major role in both ‘Fight’ and ‘Flight’. Also to collect the high amounts of carbon dioxide expelled by these cells in the course of this fast trade.

• The increased rate of metabolism acts on the store houses of fats and proteins. It’s always a wonder to note that how this body meets that high demand of extra energy for the instanta-neous transformation into the state of fight or flight. It’s these rich storage chambers of the body which sponsor the entire event.

• The main function of the hormone called the cortisol is to breakdown the fats and proteins like glycogen to establish that continuous supply of glucose to the vigorously working tissues.

• The blood vessels also attain a temporary change in the structure by becoming dilated and a lit-tle hardened to cope up that high volume, high speed blood flow.

• The peripheral blood vessels on the other hand become constricted, so that more blood is sup-plied to the core part of the body, which is the reason for your hands and feet to become ice cold in such situations.

• The salivary glands usually secrete saliva both to lubricate the food down the throat and also to di-gest the carbohydrates, which starts right in the mouth. But at the time of stress there is no role of this salivation. That’s why your mouth becomes dry and also sometimes you feel a block in your throat.

So, in short there is a continuous gush of highly oxygenated, glucose rich blood through the dilat-ed, hardened blood vessels, under high pressure. Perfect!!! by this automated mechanism of the body you can easily combat stress and come out of it successfully.

THE TWIST IN THE STORY…So, this was ‘Acute Stress’. Now just imagine, if this stress response mechanism gets activated quite of-ten, say day after day, all the above changes take place in the body, such that there is very little or rather, no time for the body to come back to the normal mode, for the body to repair and replenish itself.

Isn’t it this, which is happening these days? In this latest era, the era of ‘Pandemic’, the entire human-kind on this planet is panic stricken. There is noth-ing other than pain, distress, disease, helplessness and death in the air. Yes, ‘Death’, something that is feared by anything living on this earth, called the ul-timate fear of the living beings. Death, either of self, or of the loved ones or of the fellow beings, which is so commonly witnessed by the common man these days, thanks to the advancements in the information and broadcasting industry.

So, today man on this earth is in continuous stress, irrespective of age and class. So, here the story takes a turn from the ‘Protective Acute Stress Response’ to the ‘Destructive Chronic Stress Sequel’.

WHEN THE DISASTER MANAGER BECOMES THE CONSTANT COMPANION…Now let’s briefly check, what exactly is the effect of this chronic stress on the human body? The blood vessels carry constantly high volumes of blood to the needy places.• In due course under this constant dilatation and

pressure, the blood vessels slowly become hard and also there are structural changes in the vessel walls leading to accumulation of plaque.

• The result is nothing but ‘Hypertension’, which is the high blood pressure.

• Various vascular diseases or the ‘Diseases of the Blood Vessels’.

• An array of ‘Heart diseases’ as the heart is strenu-ously pumping at above normal rates, day in and day out.

• Because of the continuously circulating sugar in blood, much of which is unused as, no tiger is running behind us, in the long run insulin re-sistance develops leading to ‘Hyper Glycemia’ or simply Diabetes.

• Chronic stress weakens the immune system, making us prone to various spectrum of diseases and infections.

• Various studies prove that, chronic stress is capa-ble enough to reduce the response of the individ-ual towards prophylactic vaccinations.

• Another adverse affect is delayed wound heal-ing.

• The memory power is affected badly leading to short term memory loss.

• Every case of chronic stress is generally pre-sented with a history of some or other kind of a chronic pain.

• Stress induced sexual dysfunctions are not un-common today, both in males and females.

• Frequent mood swings, which affect the validity of the relationships.

• Long standing stress response has even been found to have certain structural changes in the very crucial structures in the body.

• Shortening of the telomeres at the ends of the DNA is observed. Also there is shrinkage in the size of the hippocampus, which is a structure in the brain responsible for memory.

A GAP IN THE FULLY AUTOMATED MECHA-NISMSo, taking all the things together we ultimately real-ize that this state of chronic stress finally reduces you merely into a ‘Bag of Diseases.’

Now we know that the situation is grave, but what is the possible remedy? The greatest challenge here is that the external stimulus which triggers this en-tire chain of reactions is obviously out of our control. Along with that, adding fuel to the fire, the reactions of the body towards this kind of stimuli is a self run mechanism of the body controlled by the Autono-mous Nervous System, which is not at all in the con-scious control of the body.

So, in that case are we totally helpless in such a sit-uation? Entangled in the complex circuits of the na-tures creations? If asked so, the answer is NO, we are not helpless. If God has sent you to the Earth with a peculiar lock in one hand, then definitely he has given a key in the other too. The only thing is that you have to recognize the right key and use it most appropriately.

As I told before, all the ‘Fight and Flight’ responses are controlled by the Sympathetic Nervous System, which in turn is a part of the Autonomous Nervous System. Till this point we are in the problem, but what if I say that this information is incomplete and there is little more to say.

THE SILVER LINING IN DARK CLOUDS…The above mentioned Autonomous Nervous System has two parts, one is the Sympathetic Nervous Sys-tem, which we discussed so far. But there is another part called the ‘Para Sympathetic Nervous System’, which is entirely opposite to that of the Sympathetic Nervous System.

So, this Para Sympathetic Nervous System, otherwise known as the ‘REST AND DIGEST’ mechanism, also has an army of hormones to back itself. These hormones which are collectively called the ‘Happy Hormones’ include Dopamine, Oxytocin, Serotonin and Endorphines. These hormones act opposite to the Adrenaline and Noradrenaline.

There are so many studies carried out and plenty of

research papers published which prove that phys-ical exercises done on a regular basis increases the amount of endorphins secreted in the body. In the same way various mental relaxation techniques like meditation and practicing mindfulness are set to re-lease hormones like Serotonin and Dopamine. Like that showing love and empathy towards fellow beings and maintaining good, healthy social relationships are said to be the key of releasing the hormones like Oxytocin and Serotonin. Thus, it is possible to bathe the body in a cascade of these happy hormones.

THE BIGGEST CHALLENGE OF ALL TIMES…Man is said to rule the planet for millions of years now. This position is definitely attained by him be-cause he has proved himself superior to all other cre-ations, not by his size or the power, but by his mental abilities. But these mental abilities have to be cau-tiously applied very precisely in peculiar situations with utmost attention and care, and man has been doing this for ages and proving himself day after day.

You may be wondering, all of a sudden why have I made an attempt to count the superior abilities of the Human Being. It’s because human psychology says that, ‘The man when motivated, can move the mountains.’ And here the point is that for the above mentioned ‘Rest and Digest’ system to roll into ac-tion the body must be in a state of rest or relaxation.

Hence, that itself is the greatest hurdle, I am sure at this point you may be thinking that, ‘How imprac-tical! How could someone in a state of stress be re-laxed? Here I have a very minute but crucial check that could be experimented easily.

Say in a situation of stress, either you yourself or a loved one of yours asks you to calm down for a sec-ond and stay at one place and take three to five long deep breaths. What do you notice? You can see that your breathing rate , which was much higher has come down to normal, your heart that was beating like anything has calmed down, you are in a state of little relaxation and you are at least in a state to think, what to do next?

So, there is the point. This is the gap where we can make an intervention. When this self run mecha-nism of increased respiratory rate and heart rate

could be pulled back to normal merely by few con-sciously initiated deep breaths then we are at a point of success for sure.

So, now we know that there is an enemy, and we know that it can be defeated, we also know how it could be defeated and we have the weapon. So what is the delay? We just have to fight back. We just have to replace the Fight or Flight responses with the Rest and Digest mechanism. But all this is possible only when we take the initiatives ourselves. The human will-power is the only key for him to conquer stress and rule over his life.

WHAT IS THE REMEDY?• Regular physical exercise.• Practicing proper, balanced, wholesome diet.• Mental relaxation techniques like yoga, medita-

tion, mindfulness exercises.• Yoga asanas provide both mental and physical

relaxation and health.• Movement meditation techniques like Tai Chi,

and other forms of martial arts concentrating on the breath control and energy relocation.

• Guided imagery and guided meditation, espe-cially for the little children and elderly.

THE LITTLE GRAIN OF GOLD IN THE SAND… All of us may agree that as there are two sides to a coin, there are also two ways to view a thing. The very famous example of the glass which is half full or half empty is inevitable to be mentioned here. In the light of the present scenario, I cannot stop myself from saying that even though it is a state of global distress, there is a golden opportunity in between these moments, that could be harnessed in the best ever way. That is the period of quarantine, the period of isolation, where you are either alone or with your immediate family. In both the cases, I believe this is the greatest ever chance to actually reinvent your, physical, mental and spiritual self.

So, view this period of quarantine as the opportunity to completely fill your half filled glass of health with positive vibes and invest in at least one new habit, which stays with you for a lifetime. My personal advice is nothing else but Yoga, which in itself is so wholesome and so complete.

Thus, I wish each one of you to come out of this pan-demic as a warrior and much more than that to stand up in this life as a winner. So that we, living in this era could leave our foot prints on the sands of time. Be-cause ultimately man has to conquer this pandemic.

They have been observed to have conduct problems, externalizing problems, Peer problems and general psychosocial distress when compared with children who did not exercise. Children with physical activity had lower hyperactivity, inattention and less proso-cial behaviour problems. They have a lot of things going around in their head besides the emotion-al perspective. The biggest worry of a little child is whether or not he will be able to meet his friends, teachers in the school and in teenagers, worry will be, will they or the family gets sick.

In this crisis situation children can perceive anxiety which can cause them to feel stressed, while parents and guardians are looking for ways to safeguard their kids from viruses. It's also important to help them cope with their pandemic stress.

Some may become more attached, some may be-come anxious, angry, hyperactivity, some may even start wetting the bed, and some may become silent. The way of expressing the psychosocial distress is different in each child and they behave differently depending on their age.

What can we do??Always bear in mind, as this is also a difficult time for them so they need more love care and attention now1. Listen to their concerns2. Show them your support 3. Be there to reassure them

As this is a challenging time for them it will be dif-ficult to understand what they are going through, what they are hearing from elder people, what they are seeing online, what they are seeing on TV, so this will make them more vulnerable leads to anxiety, stress and sadness.

An open discussion with your children in a support-ive way according to their age helps them to cope up with the situation. Keeping a child busy during this pandemic is a big task for parents. In such a con-dition, we can introduce new activities like reading, dancing, Yoga, meditation etc.

(1) TALK TO THEMStress, sadness, anxiety and depression can be re-duced by parents to some extent by talking about what is happening and providing clear information. Always remember to talk with your child in such a way that they can understand as per their age. If your child is a teenager, then talk him about something they like eg:-sports, TV- shows, friends. Explain them clearly what will happen if he/she affected the covid-19 and let them know they may require med-ical attention or hospitalization to make sure they recover in a very friendly way. If separation is nec-essary due to hospitalization, quarantine, isolation parents should ensure them you will be with them in a regular contact via video call or voice call or any other means.

(2) LISTEN TO THEM. Parents should listen to them calmly about what they are feeling, their concerns about the current situa-tion, their opinion and ideas so we can help them to solve their problems and temper tantrums.

(3) PLAY WITH THEM. Encourage children to play indoor game and activi-ties like chess, carroms, dumb charades, antakshari, balancing beam, scrabbles etc.

If your child is a baby or toddler, sing a song for him, make music using pots and spoons, copy their fa-cial expression, copy their sound, tell a story, read a book, and show pictures.

If your child is around 6-10 year old then play dance and freeze, do chores together -make cleaning and cooking a game.

If your child is a teenager then go for a walk around the home or do a work out together.

(4) BE WITH THEM. For parents keeping the home atmosphere positive is like playing with fire nowadays, when the little one & teenagers are driving you crazy it will end up in saying 'Stop doing that /Shut up'.

Children are much more likely to copy what we do so give them a positive vibe and praise them for what

they do right and scold them calmly and make them understand if they do something wrong. Assist them in their online classes and give short breaks between the session so they can feel fresh and energetic. Al-ways remember to praise a child if he behaves well.

(5) SPENDING TIME WITH THE WHOLE FAM-ILY.'Switch off the TV and mobile this is virus free (fam-ily) time'.

Try to keep the children close to parents, grand-parents and other family members. It is a chance to strengthen relationships with your children as the schools are shut down; it helps them to feel loved, se-cured and show them they are important. Praise the child frequently for their strength such as showing compassion, helpfulness and courage.

Using simple tools like telling stories, singing with them, playing games, cooking and an interactive ses-sion with parents and grandparents to spend their time productively.

(6) PLAN FOR A VIRTUAL PLAYDATE.As they are missing school, fun classes, playground, friends group, caring teachers, we can encourage them to socialize with friends and classmates via vir-tual platform under parent’s supervision.

(7) MAKE A HEALTHY ROUTINE FOR THEM.Maintain a healthy routine for your child as it might hamper their cognitive growth.

First establish a flexible and consistent daily routine in the house and stick to it as much as possible. Cre-ate a routine in such a way that they can participate in household activities, online classes, exercise, med-itation and also provide enough time to play and rest.

Ask your children to make a school time table their own, children usually follow it better if they make it.

A healthy routine should have a minimized screen time so they can express their feelings and creative ideas which help to make them feel relaxed and re-lieve stress and frustration.

Eyes are the window, through which we see the

world. Hence for creating a better vision, we need to care, protect and strengthen our eyes. The present covid situation urges us to spent more time in digital environment and this create stress and burden in our eyes.

Due to faulty lifestyle, increased screen time, artifi-cial lighting, and pollution, vision problems are in-creasing day by day. Eye strain, reduced vision and Computer vision syndrome are the after effects of this digital era.

Role of yoga in improving vision:Yoga is a science which helps in improving our phys-ical as well as mental wellbeing. Yogasanas, pranaya-ma and meditation, as a whole, plays an important role in improving eye health. But this article, unfolds a specific yogic exercise called TRATAKA.

Trataka:The literal meaning of Sanskrit word ‘trataka’ is gaze steadily. Trataka is a meditation tech-nique which involves focusing eyes through intent, but relaxed gazing. The technique involves gazing or fixing the eyes on one point, object or candle flame. Trataka is in-cluded under shad karma(6 cleansing tech-niques).Hath yoga pradipika mentions that practice of trataka eradicates all eye diseas-es, eye fatigue and removes lethargy. Due to its immense benefits, it is mentioned in Ha-thayoga pradipika, to keep trataka carefully as a secret like a golden casket.

Trataka is of two forms:• Bahiranga trataka • Antaranga trataka

Bahiranga Trataka:This is external trataka or external concentrated gaz-ing. Focus on a single object without blinking until eyes pain or begin to water. The object slowly be-comes imprinted in mind.

What should we focus? — Candle flame — Flower — Statue of god — By gazing at the tip of nose — Black dot in white wall

You may select any single object or a dot and concen-trate your vision on to it.

Candle Gazing:It is the most common and simple form of trataka.

Vision with Yoga

Steps-:1. Select a dark room. Light a candle and place it at

eye level2. You may sit in a comfortable meditative posture.

Keep your spine straight.3. Steadily gaze at the middle part or upper part of

the flame without blinking. (The tip of the flame may flicker, hence middle part is better)

4. This can be discontinued when tears start flow-ing or when eye fatigue is felt.

5. When eyes get tired, slowly close the eyes and in-ternal trataka can be performed.

Note: Time duration will be different in each person. Stop the procedure when you feel fatigue or when eyes are filled with tears. Do not over strain your eyes

Antaranga Trataka:This is internal gazing. It involves clear and stable in-ner visualization of an object. After bahiranga trata-ka, close your eyes. Then gaze at the afterimage of the candle flame (or any object) in your mind.

Who cannot do?Persons with glaucoma, astigmatism and those who have just undergone eye surgeries can avoid trataka with candle flame. They may choose any other ob-ject.

Post procedure:After the trataka procedure, small level of eye strain is felt. To reduce that, a post procedure is necessary.1. Palming2. Eye wash

Palming/Cupping:Gently rub the palms together to produce heat and place the palms on the eyes in the form of a cup.Duration-15 seconds

Benefits:Helps in relaxing the eyes

Eye wash:Take 2 eyecups. Pour clean filtered water into it. Slowly dip both eyes into the eyecups and start blink-ing. You may do it for 30-40 seconds and then slowly close your eyes. Keep your eyes closed for about a minute.

What are the benefits? — Cleans, heals and invigorate eyes — Cleanses the tear glands — Improve vision — Reduce eye strain and eye fatigue — Delay age related eye problems — Strengthen eye muscles — Improve concentration and relieve nervous tension — Relax and rejuvenate eyes.

For liquid components – we can use any plant-based items such as coconut milk, coconut water, and fruits like watermelon, cucumber, or even drinking water.

For base – we can use any vegetables like spinach, or carrot or beetroot which provides a nice color. Even

fruits like oranges, Papaya, Pear, apple, banana, or any seasonal fruit can also be used.

For Sweetener - we can usually add Bananas as a base as it is naturally very sweet fruit otherwise also can add mangoes.

For flavour - we can add Herbs, mint, cinnamon, Ginger, etc...

from the fridge instead of ice cubes) • Soaked oats - 1 cup • Honey - 3 tablespoon• Cardamom powder -half tablespoon • Basil leaves 3 to 4• Water - Qs • Chia seeds ( soaked) 4 tablespoon ( for toppings) • 1/2 cup cooking oats or rolled oats soaked in 1

cup of water. Mix well. Cover the bowl and keep in the fridge overnight.

Method of Preparation• First, rinse and peal 2 small to medium sized

mangoes.• Chop the mango flesh into the small cubes.• Add chopped cubes to a blender or mixer jar. • Add 1 cup of soaked oats and also add 3 table-

spoons of honey and sprinkle 1/2 tablespoon of cardamom powder into the blender jar.

• Add some water in it and blend until smooth and creamy

JOINT PAINS - A SILENT KILLER

Gout is a common type of arthritis caused by excess uric acid in the bloodstream which results in intense pain, swelling, and stiffness in joints particularly it affects the joint in the big toe. The symptoms of gout are due to the formation of uric acid crystals in the joints and the body’s response to them. Gout attacks often occur without warning in the middle of the night.

Most gout cases are treated with specific medica-tions.

SymptomsThe signs and symptoms of gout almost always occur suddenly, and often at night. Other commonly af-fected joints include the ankles, knees, elbows, wrists and fingers. The pain is likely to be most severe with-in the first four to 12 hours after it begins. • Lingering discomfort-After the most severe

pain subsides, some joint discomfort may last from a few days to a few weeks. Later attacks are likely to last longer and affect more joints.

• Inflammation and redness-The affected joint or joints become swollen, tender, warm and red.

• Limited range of motion-As gout progresses, you may not be able to move your joints normal-ly.

CausesGout occurs when urate crystals accumulate in your joint, causing the inflammation and intense pain of a gout attack. Urate crystals can form when you have high levels of uric acid in your blood. Your body produces uric acid when it breaks down purines — substances that are found naturally in your body. Purines are also found in certain foods, including red meat and organ meats, such as liver, seafoods in-cluding anchovies, sardines, mussels, scallops, trout and tuna. Normally, uric acid dissolves in your blood and passes through your kidneys into your urine. But sometimes either your body produces too much uric acid or your kidneys excrete too little uric acid. When this happens, uric acid can build up, forming sharp, needle like urate crystals in a joint or sur-rounding tissue that cause pain, inflammation and swelling.

Uric acid is a heterocyclic compound of carbon, nitrogen, oxygen, and hydrogen with the formula C5H4N4O3. It forms ions and salts known as urates and acid urates, such as ammonium acid urate. Uric acid is a product of the metabolic breakdown of pu-rine nucleotides, and it is a normal component of urine.

Risk factorsYou're more likely to develop gout if you have high levels of uric acid in your body. Factors that increase the uric acid level in your body include:Diet: Eating a diet rich in red meat and shellfish and drinking beverages sweetened with fruit sugar (fructose) increase levels of uric acid, which increase your risk of gout. Alcohol consumption, especially of beer, also increases the risk of gout.

Weight: If you're overweight, your body produces more uric acid and your kidneys will take more diffi-cult time to eliminate uric acid.

Medical Conditions: Certain diseases and condi-tions increase your risk of gout. These include un-treated high blood pressure and chronic conditions such as diabetes, obesity, metabolic syndrome, and heart and kidney diseases.

Certain Medications: Low-dose aspirin and some medications used to control hypertension — in-cluding thiazide diuretics, angiotensin-converting enzyme (ACE) inhibitors and beta blockers — also can increase uric acid levels. Using of anti-rejection drugs prescribed for people who have undergone or-gan transplant can also triggers gout.

Family history: If other members of your family have had gout, you're more likely to develop the dis-ease.

Age and Sex: Men are also more likely to develop gout earlier — usually between the ages of 30 and 50 — whereas women tend to have lower uric acid levels but after menopause.

Recent Surgery or Trauma: Experiencing recent surgery or trauma can sometimes trigger a gout at-tack. In some people, receiving a vaccination can trigger a gout flare.

ComplicationsPeople with gout can develop more-severe condi-tions, such as:

Recurrent Gout: Some people may never experience gout signs and symptoms again. Others may experi-ence gout several times each year. Medications may

help prevent gout attacks in people with recurrent gout. If left untreated, gout can cause erosion and de-struction of a joint.

Advanced gout: Untreated gout may cause deposits of urate crystals to form under the skin in nodules called tophi (TOE-fie). Tophi can develop in sev-eral areas, such as your fingers, hands, feet, elbows or Achilles tendons along the backs of your ankles. Tophi usually aren't painful, but they can become swollen and tender during gout attacks.

Kidney Stones: Urate crystals may collect in the urinary tracts of people with gout, causing kidney stones. Medications can help reduce the risk of kid-ney stones.

DiagnosisThe most reliable test for gout is finding uric acid crystals in a sample of the joint fluid obtained by joint aspiration. Blood uria is the commonest blood test to estimate the level of Uric acid. Other blood tests commonly performed are white blood cell count, electrolytes, renal function, and erythrocyte sedimentation rate (ESR).

Ayurvedic Outlook to GoutVata Rakta or Aadya Vata is a classical description of Gout in Ayurveda. Vata dosha, Pitta dosha and Rakta Dhatu are specially vitiated to cause Vatarakta. Sev-eral causes are listed in Ayurveda, which causes Va-tarakta are excessive intake of Lavana (salty), Amla (Sour), Katu (pungent), Tikta (bitter), Kshara (alka-line), Snigdha (Too oily), and Ushna (Too hot) foods, eating dried & preserved or spoiled fish/meat, Ulava-lu (Horsegram), Blackgram, Nishpava (anumulu, a bean variety), sour curd or buttermilk, incompatible foods like fish with milk products, drinking alcohol, daytime sleep, staying awake in the night, anger etc.

Based on the seat of the disease, Vatarakta is classi-fied into 2 types.

Uttana (superficial) – This is located in the twak (skin) & mamsa (muscle).Uttana Vatarakta presents with itching, burning, ache, pain on extension, prick-ing pain, throbbing pain, contraction and associated with brownish black, red or coppery discolouration of Skin.

Gambhira (Deep) – This is located in the deeper dhatus and more severe in nature. Tophi or nodule formation occurs. Gambhira Vatarakta present with oedema, stiffness, hardness, excruciating pain in the interior of the body, blackish brown, coppery disco-louration of skin, burning sensation, pricking pain, twitching sensation and suppuration of joints.

Ayurvedic treatment for gout comprises of herbal medicines that help the body to eliminate uric acid, thereby reducing the inflammation. In Ayurveda a detailed description of Vatarakta chikitsa is dis-cussed in all texts along with lines of treatment, being Shodhana (internal purification), Shaman (palliative medicine) and Bahyachikitsa (external treatments on body), which is a very effective treatment and is eventually the need of the hour.

Kokilaksha – (Asteracantha Longiflia) is one such herb which is very effective in providing relief to pa-tients suffering from Gouty arthritis. The Ayurvedic therapies that are effective in relieving inflammation are Dhanyamla dhara (pouring fermented liquid over the inflamed joint), Podikkizhi (herbal powder massage), Ilakkizhi (Herbal leaves massage), and after the inflammation is relieved procedures like Tailadhara ( Pouring medicated oil) on the affected part and Nhavarakkizhi (Medicated rice massage) are advocated along with Ksheeravasthi (Medicated milk enema). All these treatments are very effective in protecting the joints affected by Gouty arthritis.

Foods to Avoid if You Have Gout• Skip foods and drinks that are high in purines to

help lower your chances of an attack.• Beer and grain liquors (like vodka and whiskey)• Red meat, lamb, and pork• Organ meats, such as liver, kidneys, and glandu-

lar meats like the thymus or pancreas (you may hear them called sweetbreads)

• Seafood, especially shellfish like shrimp, lobster, mussels, anchovies, and sardines

• High-fructose products like soda and some juic-es, cereal, ice cream, candy, and fast food

Best Foods for a Gout Diet• Low-fat and non dairy fat products, such as yo-

gurt and skimmed milk

• Fresh fruits and vegetables• Nuts, peanut butter, and grains• Fat and oil• Potatoes, rice, bread, and pasta• Eggs (in moderation)• Meats like fish, chicken, and red meat are fine in

moderation (around 4 to 6 ounces per day).• Vegetables: veggies like spinach and asparagus.

What Can You Drink if You Have Gout?Foods aren’t the only thing that can affect uric acid. What you drink matters, too.

DosIt’s a good idea to drink lots of fluids -- 8 to 16 cups a day. At least half of what you drink should be water. Vitamin C (think orange juice) also can help lower uric acid, but studies also show that the high fructose in OJ may boost uric acid levels, so drink it in mod-eration. GARBHASANSKARAIMPLANTING A SEED TO LIFE

According to World Health Organisation, Precon-ception care is the provision of biomedical, be-havioural and social health interventions to women and couples before conception. It aims at improving their health status and reducing behaviours and in-dividual environmental factors that contribute to poor maternal and child health outcomes.

Ayurveda, the science of life explained every aspect of life from daily regimen, seasonal, regimen, men-strual regimen, do’s and don’ts in different diseases, prevention, causative factors and treatment of dis-eases. Along with these, Ayurveda also throw light on pre- conception care at different contexts.

In present era, the incidence of congenital anoma-lies is increasing day by day. The first cohort study in India revealed that the congenital anomaly rates were high, affecting 1 in 44 births in the cohort. This highlights their public health importance.

By following the Ayurvedic regimens before concep-tion, will surely help to prevent such problems to a great extent by improving the parental health there by improving the quality of sperm and ovum and formation of a health embryo. Ayurveda explains pre- conception care starting from the age prefera-ble for pregnancy, factors responsible for pregnancy, how to perform the act of coitus to get sreshta praja etc.

Rajaswalacharya( Mode of living during menstru-ation )During the first 3 days of periods, women should re-main isolated. She should avoid sleeping during day time, application of kajal, crying, bathing, oil anoint-ing, massaging, cutting of nails, fast racing, laugh-ing, talking too much, listening to too many topics, combing, fast wind as well as excessive exercises.

She should sleep on bed of darbha (A type of scared

grass ) spread over the ground, eat havisya (meal made of ghee, shali rice and milk) or yavaka (meal made of barley and milk).

By following these women can adopt the physiolog-ical changes during menstruation and prevent men-strual abnormalities.

Age and Criteria for Marriage According to Acharya Susruta, the age prescribed for marriage is 25 for male and 16 for female.

Acharya Vagbhata said it as 21 yrs and 12 yrs respec-tively and added some special characteristics for that girl. That she should belong to the same gotra of the man, nor in blood relation, not having contagious diseases, who is beautiful, modest, possess good qualities and who are free from sins, praise worthy and the marriage should be performed with auspi-cious method. The age said by Acharya Vagbhata is the age of marriage and not of conception. In India, during the time of Vagbhata there was prevalence of child marriage, but the couple were allowed to live together some years. The qualities for selection of bride can be taken for both partners. Recent studies also proved that consanguineous marriage (marriage in b/w blood relation) carry recessive traits.

Appropriate age for conception Acharya Susrutha said that the males get sexual maturity only by age of 25 and female by age of 16. Acharya Charka says that male can get sexual matu-rity at the age of 16. So that is the appropriate time of conception when both partners are of full vigour and valor at this age and the born child also posses these qualities

Factors responsible for conception The factors responsible for conception according to modern gynaecological texts includes:1. Healthy spermatozoa2. Capacitation and awesome reaction and acquire

motility. 3. Motile spermatozoa 4. Ovulation5. Patency of fallopian tube and oozyte should be

picked up by fimbriae6. Fertilisation of oozyte by spermatozoa at ampul-

la of tube

7. Embryo should reach the uterine cavity after 3-4 days of fertilization

8. Endometrium should be receptive and corpus luteum should function adequately

Ayurveda explains these all factors in four major terms1. Ritu ( Time of ovulation )2. Kshetra ( The Uterus or the reproductive tract )3. Ambu ( Nourishment including hormones )4. Beeja ( Healthy sperm and ovum )

Ritu – means kala or time, it explains the importance of ovulation such as when the conception chances are high and also ritu describes about the seasons Aadana Kala and Visarga Kala. Among these vis-arga kala (winter period), gives strength to the hu-man body. So conception during this period gives a healthy progeny compared to that during adana kala.

Kshetra – It devotes a healthy uterus. Uterus is hav-ing different parts, i.e. cervix, fundus and body. It is attached with fallopian tube and the tube has fim-briated end which opens to the peritoneal cavity and also after ovulation the ovum is collected and transported to the sperm in the isthmus of tube. The cervix and its secretions are responsible for the ca-pacitation and acrosmal reactions of the sperm that enter the cervix during the act of coitus. This help in the transportation of sperm to the isthmus of tube. The inner lining of uterus is called as endometrium. After the union of sperm and ovum in the fallopian tube the embryo should travel to the uterus within 3 – 4 days and should be implanted in the endome-trium. For that the endometrium should be recep-tive. Ayurvedic Acharyas are explaining this whole science in a word Kshetra which is very relevant.

Ambu - Ambu means water. Water is the nourishing factors for plants. In this context Acharya says about all the nourishing factors should be normal. There can be the hormones and the nutrition to the fetus from the mothers. According to Acharya susruta, the fetal nourishment before vyaktibhava is by upa-snehana and upasvedana nyaya( i.e. from the nearby tissue of uterus ). After the formation of umblilical cord (i.e. Nabhinadi ) through the nabhinadi which is directly connected to the hridaynadi of mother.

BeejaBeeja refers to healthy spermatozoa and ovum. When the beeja is deposited in the vagina it is trav-elling high to the fallopian tube for this, the beeja should posses all required qualities like spatikapham (clear) , dravam (liquid - gives idea about viscosity) snigdham (unctuous), madhuram ( sweet due to its fructose content) and madhugandhi (smell of hon-ey).

Ovum is produced only in ritukala. So for ovulation there should be normal menstruation and for getting a healthy ovum rajaswala charya has been explained.

Ayurvedic Panchkarma and preconceptional care Panchkarma is a versatile and effective therapy in all types of problems. It includes 5 types of therapies namely vaman (Emesis), virechana (Purgation), Vas-ti (Enema), nasya (nasal administration of medicine) and raktamokshan (bloodletting). In preconception care, after marriage it will be beneficial for the couple to take a panchakarma therapy to get blessed with a healthy baby.

Ayurvedic preconception care, starts from the menopause, by the regimens followed by the girl during menstrual period including diet and other. For getting a proper child, “Soumanasya”(Happi-ness) is considered to be as the best therapy. Mental-ly prepared mind and body favours the development of child. Along with that, they can start to have an Ayurvedic regimen with medicines and diet for one or two months as a part of preconceptional care and after that can try for a baby with happy mind and without any stress. After all, Life will be given from the hands of God. As per Ayurveda, sukra can mix up with ovum but atma (life) will be generated from God only. So, mind filled with prayers and a well nourished body can only reproduces a healthy child.

Nimba or the Neem tree (Azadirachta indica Juss.) is one of the most versatile medicinal drug that has gained worldwide acceptance due to its medicinal as well as insecticide properties. Neem was identified by WHO/UNEP 1989 as an environmentally powerful natural pesticide and is considered as one of the most potential drug of 21st century for its pest management, environment protection and medicinal properties. Usually neem trees grow in sub arid and sub humid areas of tropical and subtropical regions and it is very common in all parts of India. In Ayurvedic medicine, Nimba (Neem) is used traditionally from a very ancient period for varieties of ailments like skin disorders, fever, wound healing, diabetes, bleeding piles, boils, eye diseases, urinary stones, jaundice etc. The drug possess antimalarial, antipyretic, anti-inflamma-tory, anti-helminitic, antiviral properties. Therapeutically all the basic part of neem are made use of like the leaves, bark, fruits, flowers, seeds which proves to be effective in different types of ailments.

Skin Disorders: In skin disorders, the juice extracted from the leaves of neem if taken for a month will help to cure the disease within one month if the person abstains from unwholesome diet.

Wound: If the wounded parts of skin are eaten by the maggots, it is cleaned and a paste made from the powder of neem bark and Embelia ribes fruit which is mixed in cow’s urine and applied externally.

Diabetes: Decoction of neem is efficacious in treating early stages of diabetes.

Urticaria: By the regular use of the leaves of neem and amla mixed with ghee will alleviate urticaria and skin disorders.

Jaundice: Person suffering from jaundice should take cooled decoction of neem along with honey in morning.

Vaginal Disorders: Washing frequently with decoction of nimba and also fumigating with the bark will make vagina free from foul smell and sliminess.
