Warranty Service Form Instruction Page

Warranty Process Overview – Please read the following important information

• Entire warranty process can take from 4 – 6 weeks to be completed. Please cooperate

with us so to avoid process delays. • The warranty only covers the product; it does not include transportation and/or inspection fees associated with this warranty. • A $25 inspector transportation fee applies, prior to the inspection, as well as a local delivery fee if the warranty is approved. (Delivery fees start at $59) • An independent 3rd party inspection company approved by the manufacturer will perform the warranty inspection of your mattress/foundation. The procedure used by the inspection company is the finite way, approved by the manufactures, to inspect your sleep set. The inspection company will contact you to set up an appointment to inspect the product in your home. The inspector’s responsibility is to gather information and they will not make any decisions. • Results are communicated by mail only. They will be mailed out within 10 business days after the inspection.

a. If your product is covered, an approval letter including instructions on how to complete your exchange process will be mailed out.

b. If your product is not covered, a denial letter including the reason for the decision will be mailed out.

• Body indentations are normal. Warranty covers indentation of 1 ½” or greater. For further details please look in the additional warranty questions link. Please fax, email or mail the completed Warranty Service Claim Form with a copy of your receipt. Pictures are optional, please only email or mail them. Casa Bella Furniture is here to assist you with your manufacturer warranty service. Due to manufacturer warranty

terms and procedures, we will only honor warranties that fall within the manufactures guidelines on products

purchased from CB Furniture. Please take the time to thoroughly answer the following questions. These answers

will aid your Warranty Liaisons as we present your warranty claim on your behalf to the manufacturer. An

incomplete form may lead to a denial of your claim. Therefore the more information you can provide, the better

your Warranty Liaison can represent you in your case. It is extremely helpful to provide photos of the defect.

If the manufacturer needs further information to process your claim, we may contact you to schedule a visit by a third-party inspector. Once a final determination has been made, we will contact you in writing. The entire warranty process can take from 4-6 weeks. Please also be aware of the following:

A copy of the original sales receipt must be submitted with the Warranty Service Claim Form. Your warranty will not be processed without proof of purchase. Customer is responsible for providing copy of sales receipt.

Stains void the warranty based on to the manufacturer’s warranty guidelines.

Warranties only cover products; they do not include transportation and/or inspection fees.
