2019 My Benefits at Yale Managerial and Professional staff

Medical InsuranceYale University ofers two options for medical insurance, Yale

Health and Aetna.

Yale Health, Yale’s fagship plan, is a not-for-proft, physician-

led health insurance option exclusively for members of the

Yale community. Located in a state-of-the-art medical center

on campus, Yale Health ofers a wide variety of health care

services on-site including primary care, specialty care, 24/

acute care, radiology, lab, and pharmacy. When your condition

requires more specialized care or a hospitalization, there’s an

extensive network of specialists drawn largely from the faculty

of Yale School of Medicine. With Yale Health, emergency care

is covered anywhere in the world.

Alternately, you may choose to enroll in Aetna Smart Care

Plan or Aetna Choice (with Health Savings Account and

Personal Accident) plan through our Aetna program.

If you participate in the Aetna Smart Care Plan and elect an

HSA, the university will provide a one-time deposit, of $500,

$ 50, or $1,000 depending on your coverage election, into

your account. In addition, you can also fund the account with

pre-tax payroll contributions (subject to annual IRS limits).

Colonial Life Accident Coverage will also be provided, free

of charge, for you and your covered dependents. Accident

Coverage provides lump-sum benefts for covered accidents

and some hospital confnements.

Vision InsuranceEyeMed ofers two options; the Basic plan which provides

allowances for frames and lenses, and the Enhanced plan,

which also covers eye exams and more frequent hardware

allowances as well as other valuable enhancements.

Dental InsuranceDelta Dental is the exclusive dental carrier for Yale University

Tobacco-Free YaleTobacco-Free Yale takes a holistic approach to supporting Yale

students, faculty, and staf by ofering various programs and

resources geared toward helping you and your loved ones quit

tobacco use and qualify for lower contributions on healthcare.

Disability ProtectionShort-Term DisabilityThe Short-Term Disability plan, in conjunction with Sick Pay,

provides salary protection for up to 26 weeks for extended

absence due to a non-work related illness or disability. This

coverage is provided by the university at no cost to you.

Long-Term DisabilityYou are automatically enrolled in Long-Term Disability

coverage. In the event of an approved disability, the beneft

pays 60% of base monthly earnings. Coverage is provided at

no cost on salaries up to $150,000. Supplemental coverage, for

those earning over $150,000 is contributory.

Life InsuranceYou are provided with $50,000 of basic term life insurance

at no cost, as well as accidental death and dismemberment

coverage.

Supplemental Life Insurance:Employee Supplemental Life You also have the option of pur-

chasing Supplemental Life Insurance of up to six times your

annual salary to a maximum of $2,000,000.

Spouse Life You have the option of purchasing an additional

$25,000 or $50,000 spousal coverage

Dependent Life You have the option of purchasing $5,000 or

$10,000 in child life coverage.

3 The amount you must pay for services, afer the deductible has been paid.

4 The maximum amount you have to pay toward the cost of your medical care in the course of one year, including co-insurance and deductible.

5 If eligible, the HSA employer contribution will be prorated for non calendar year efective dates. The employer contribution requires HSA eligibility, account set up, and is contingent on enrollment in the Aetna Smart Care plan.

6 If a Preferred drug is available and an Alternative or Non-preferred drug is dispensed, you will be charged the applicable co-pay plus the diference in cost between the Alternative or Non-preferred drug and Preferred drug cost even if your provider requests the Alternative drug. In most cases, the Preferred drug is the same chemical entity/active ingredient as the Non-preferred drug.

In-Vitro Fertilization & Advanced Reproductive Technology (ART) covers four (4) cycles of ovulation induction and artifcial insemination. This is a University lifetime limit per contract unit regardless of carrier chosen.

Do you have other insurance?For members covered by more than one insurance plan (such as your employer plan and your spouse’s employer plan)

Coordination of Benefts (COB) is the method used by Yale Health to determine which plan pays frst, which pays second, and the

amount paid by each plan. Yale Health will use the COB method to ensure that members covered by more than one insurance plan

will receive the benefts they are entitled to while avoiding overpayment by either plan.

If you are enrolled in an insurance plan in addition to Yale Health, you must provide information about that plan to Yale Health on

the Insurance Information Update (COB) form, which can be found at http://yalehealth.yale.edu/resources/forms.

Failure to disclose this information may afect the terms of your coverage or denial of claims. Please visit http://yalehealth.yale.edu/

coverage for more information about coordination of benefts.

Ways to SaveFlexible Spending AccountsWith a Flexible Spending Account, you can reduce your taxable

earnings and receive reimbursement of eligible out-of-pocket

expenses. You may contribute up to the IRS limit to a Health

Care and/or Dependent Care account.

Note: If you contribute to an HSA, your use of a Healthcare

Flexible Spending Account will be limited to certain dental and

vision expenses

College Savings Plan (529)You can save for the cost of a college education by opening a

529 college savings plan for yourself or family members.

RetirementYale University Retirement Account Plan (YURAP)University ContributionYou are automatically enrolled to save 5% of your eligible pay

as a pre-tax contribution to your YURAP 403(b) account and

the university will match 100% of your contributions, up to

5%. The university automatically contributes a Core contribu-

tion of 5% of eligible earnings below and .5% above the Social

Security Wage Base (SSWB). You will receive the university

Core contribution whether or not you contribute to the Plan.

Flexibility and ChoiceYou may increase, decrease, or stop your contribution to

YURAP at any time, and can elect to contribute a fat dollar

amount, a percentage of your salary, or the IRS annual

maximum amount. You may choose to make all pre-tax

contributions, all Roth 403(b) afer-tax contributions, or a

combination of both.

Pre-tax employee contributions and university match are

sheltered from federal and state taxes; contributions and

earnings are not taxed until withdrawn. The Roth 403(b)

afer-tax contribution option provides for tax-free withdrawal

of earnings with a qualifed withdrawal. Each July, if you are

saving less than 10%, YURAP’s annual automatic-escalation

feature will adjust your savings rate. If your employee

contribution rate is below 5%, it will increase to 5%; and, if

your contribution rate is above 5%, but less than 10%, it will

increase by 1%.

Yale University 457(b) Deferred Compensation PlanThis plan allows eligible participants to defer additional

pre-tax dollars and increase their retirement savings up to the

current IRS limit, if already contributing the maximum to a

403(b) plan. Eligible participants include tenured professor,

professor on continuing appointment in the School of

Medicine, or faculty or staf with base salary at or above 1.5

times the Social Security Wage Base. Log in to www.tiaa.org

Medical InsuranceYale University ofers two options for medical insurance, Yale

Health and Aetna.

Yale Health, Yale’s fagship plan, is a not-for-proft, physician-

led health insurance option exclusively for members of the

Yale community. Located in a state-of-the-art medical center

on campus, Yale Health ofers a wide variety of health care

services on-site including primary care, specialty care, 24/

acute care, radiology, lab, and pharmacy. When your condition

requires more specialized care or a hospitalization, there’s an

extensive network of specialists drawn largely from the faculty

of Yale School of Medicine. With Yale Health, emergency care

is covered anywhere in the world.

Alternately, you may choose to enroll in Aetna Smart Care

Plan or Aetna Choice (with Health Savings Account and

Personal Accident) plan through our Aetna program.

If you participate in the Aetna Smart Care Plan and elect an

HSA, the university will provide a one-time deposit, of $500,

$ 50, or $1,000 depending on your coverage election, into

your account. In addition, you can also fund the account with

pre-tax payroll contributions (subject to annual IRS limits).

Colonial Life Accident Coverage will also be provided, free

of charge, for you and your covered dependents. Accident

Coverage provides lump-sum benefts for covered accidents

and some hospital confnements.

Vision InsuranceEyeMed ofers two options; the Basic plan which provides

allowances for frames and lenses, and the Enhanced plan,

which also covers eye exams and more frequent hardware

allowances as well as other valuable enhancements.

Dental InsuranceDelta Dental is the exclusive dental carrier for Yale University

Disability ProtectionShort-Term DisabilityThe Short-Term Disability plan, in conjunction with Sick Pay,

provides salary protection for up to 26 weeks for extended

absence due to a non-work related illness or disability. This

coverage is provided by the university at no cost to you.

Long-Term DisabilityYou are automatically enrolled in Long-Term Disability

coverage. In the event of an approved disability, the beneft

pays 60% of base monthly earnings. Coverage is provided at

no cost on salaries up to $150,000. Supplemental coverage, for

those earning over $150,000 is contributory.

Life InsuranceYou are provided with $50,000 of basic term life insurance

at no cost, as well as accidental death and dismemberment

coverage.

Supplemental Life Insurance:Employee Supplemental Life You also have the option of pur-

chasing Supplemental Life Insurance of up to six times your

annual salary to a maximum of $2,000,000.

Spouse Life You have the option of purchasing an additional

$25,000 or $50,000 spousal coverage

Dependent Life You have the option of purchasing $5,000 or

$10,000 in child life coverage.
