Welcome to myHealthRecord

Hello,

Welcome to myHealthRecord – Women’s College Hospital’s (WCH) new online patient medical record. myHealthRecord provides patients with access to their medical information online through the portal (www.womenscollegehospital.ca/myhealthrecord) and through the “MyChart” app for iPhone, Android and tablet devices.

This guide provides an overview of many of myHealthRecord's features and how to use them. By registering for myHealthRecord, you can:

Review summaries of your previous appointments, including issues addressed during each visit, your vital signs, and tests or referrals that were ordered.*

View your medications, including dosage information and instructions.* View new lab results, as well as trends over time. For example, you can view results for your past

cholesterol tests on a graph to see if you're close to meeting your target number.* Contact your care team and/or provider through a medical advice request.* Access your family members' medical records. For example, you can view your child's immunization

history and upcoming appointments** *Functionality may not be supported by all clinics.

**Functionality not yet available

Please note: Test results ordered during a visit at the Family Practice Health Centre (FPHC – located at 77 Grenville St.) or at the Crossroads Clinic will be visible in myHealthRecord, as long as the tests were performed at Women’s College Hospital. However, any health information captured at FPHC or at the Crossroads Clinic will not be included in your myHealthRecord account, and you will not be able to update your general information such as medications and allergies for either clinic’s team. At this time, you will also not be able to request, cancel or view your FPHC or Crossroads Clinic appointments.

Sign up for myHealthRecord

Use your activation code to sign up To register for myHealthRecord you’ll need an activation code specific to your account. The activation code can be obtained from the hospital administrative staff at Check-In (found on your registration letter) or from your provider (found on your After Visit Summary). After you have obtained an activation code, you can use it to complete the myHealthRecord registration process.

1. From the myHealthRecord login page, click Sign Up Now in the New User? section. 2. Enter your activation code and other personal verification items, such as your Medical Record Number

(MRN) and your date of birth. Click Next. 3. On the next page, choose the following:

myHealthRecord username- This should be something that others wouldn't be likely to guess but easy for you to remember. Once you have created a username, it cannot be changed.

Password- This should be a unique combination of numbers and letters, using both uppercase and lowercase letters. Your password must be at least six characters and must be different from your myHealthRecord username.

Security question- This question will be used to verify your identity if you forget your myHealthRecord password. Choose a security question from the list and enter your answer. Your answer cannot include your myHealthRecord password.

4. On the next page, you can choose whether you want to receive a notification message in your personal email when there is new information available in your myHealthRecord account. If you opt to receive email alerts, enter your email address.

Log in to myHealthRecord 1. In your web browser, enter www.womenscollegehospital.ca/myhealthrecord and access the login page. 2. Take some time to look around and familiarize yourself with the home page. If you have any questions

Messaging

View messages from your clinic You can read any messages sent by your provider or other clinic staff by going to your Inbox (Messaging > Inbox). Click the message you want to read. From here, you can reply to the message or delete it. You’ll be able to review:

Sent Messages

If you're looking for a specific message, you can enter key words in the Search field on the Inbox

page to locate them.

Receive email notifications when new myHealthRecord messages are available You can choose to receive notification by email to let you know when a new message is available in your myHealthRecord Inbox.

1. Go to Preferences > Notifications. 2. Select the E-mail check box next to New Messages. 3. Update your email address and mobile phone number if needed to ensure that you receive the

notifications. 4. Click Save Changes.

Ask your clinical department/provider for medical advice If you have a non-urgent medical question, you can send a message to your WCH department. This message is secure, meaning your information stays private as it is sent over the Internet. The message is then reviewed by clinic staff and triaged accordingly.

Note: If you are not comfortable with clinic staff members seeing your message, please make an appointment to speak directly to your clinician.

Examples of when you could use the Get Medical Advice feature include messaging your provider if you're not sure whether you should come in for an appointment, if you need clarification on the dosage of one of your medications or about something that was discussed in a recent visit or if you just want advice about a common illness.

1. Go to Messaging > Get Medical Advice. 2. Click Visit Follow-Up Question. 3. Select a recipient from the list. You will only be able to send a medical advice request to clinics and

providers to whom you have had a visit with previously (within 365 days). 4. Select a subject for your message and enter your question. 5. When you are finished, click Send.

Someone at your clinic should respond to you within three business days (72 hours). If you have opted to receive email or text notification for new messages in your myHealthRecord account, you will receive a message letting you know that the clinic has responded to your request.

If you want to view a message after you've sent it, you can do so by going to Messaging > Sent

Messages. Messages that appear in bold text have not yet been read by clinic staff.

Visits

View your upcoming appointments You can view your future appointments in the Upcoming Appointments feature (Visits > Upcoming Appointments). Select a scheduled appointment to see information such as:

the date, time, and location of the visit any pre-visit instructions from the clinic directions to your clinic

eCheck-In With eCheck-In (Visits > Upcoming Appointments), you can use myHealthRecrod to complete the following tasks from anywhere so that you don't need to do them when you arrive at the clinic:

verify or update coverage and demographics information

verify or update medications and allergies

answer appointment-related questionnaires

Confirmation When confirming an appointment (Visits > Upcoming Appointments), you can use myHealthRecrod to indicate that you acknowledge the upcoming appointment to prevent WCH from providing reminder emails and phone calls for that upcoming appointment.

View your After Visit Summary for past appointments or

admission To access an After Visit Summary for a previous appointment in myHealthRecord, go to Visits > Visit Summaries and select the appointment you want to review. This feature might be helpful if you've lost the summary or patient instructions from your last visit, or if you want to review the procedures that were performed.

View your provider’s notes from the visit If your provider has shared their notes from the visit with you, you can view them in the After Visit Summary by selecting the Notes tab.

Request an appointment To request an appointment, go to Visits > Request an Appt. You will be asked to choose the provider you want to see, the reason for the visit, and preferred dates and times. You can also enter comments regarding why you are requesting the appointment. After you submit your request, someone from the clinic will contact you to verify an appointment date and time.

Cancel an appointment Depending on the date and time of your next appointment, you might be able to cancel it through myHealthRecord.

Note: If it is too close to your appointment date or time, you will need to call the clinic to cancel your appointment (the days/time restrictions are TBD by WCH).

To cancel an appointment:

1. Go to Visits > Cancel Appts, and select the appointment from the list. Click Continue. 2. Enter cancellation comments. 3. Click Confirm Cancellation.

Fast Pass Depending on the clinic and your appointment’s date, time and provider, you may be eligible for Fast Pass. Fast Pass automatically sends you an email message to notify you of a wait list appointment offering. Upon receiving the message, you can then log in to myHealthRecord and claim the offer if it is still available or decline the offer to keep the original appointment and wait for another offer. If you are eligible for a Fast Pass offer, your clinic will automatically add you to the wait list.

Note: Fast Pass is not supported by all clinics at WCH. If you are unsure if you clinic is participating please contact [email protected] for assistance.

To accept or decline a Fast Pass appointment offer:

Via myHealthRecord Website

1. Log in to myHealthRecord (www.womenscollegehospital.ca/myhealthrecord) 2. On the home page, you will be prompted with a notification under “You Might Want To…” indicating

that you have an opportunity to move your Appointment to an earlier time.

3. Simply select Accept New Appointment or Keep Existing Appointment. By accepting a new appointment your previous appointment, date will automatically be canceled and rescheduled to the new proposed date/time. Should you choose to keep your existing appointment, you will still be considered for earlier appointment opportunities.

Appointment offers are on a first come first serve basis. Should another eligible patient accept the offer, you’ll be offered the next earliest appointment offer. You have 5 days to accept or decline the appointment offer. If either of the above scenarios occur, you’ll see the following notification on the myHealthRecord home screen.

Via myHealthRecord mobile app (i.e. MyChart)

1. Open the MyChart mobile app and log in.

2. Navigate to Appointments.

3. Navigate to Offers and decide to Accept the appointment offer or Keep your existing appointment.

My Medical Record

View your test results With myHealthRecord, you can view test results as soon as they become available, rather than waiting for a phone call or letter from your clinician. To view test results, go to My Medical Record > Test Results. Select a test to see more information about it, such as:

the standard range for the result any additional comments your provider entered about the result

Receive email or text messages when new results are available 1. Go to Preferences > Notifications. 2. Select the E-mail or Text check box next to New Test Results. 3. Click Save Changes.

Manage your medications

View your current medications Go to My Medical Record > Medications to see all of your current medications in one place. You can see details for each medication, including the prescribed dosage, instructions, and the clinician who prescribed the medication. You can view additional information about a medication, such as precautions to consider when taking the medication and potential side effects by clicking the About This Medication link.

You can also click Submit or Review Updates to request changes to your medication list.

Remove a medication you're no longer taking by clicking .

Manage your allergies

View your current allergies Go to My Medical Record > Allergies to see all of your currently listed allergies in one place. You can see your reaction details for each allergy, view additional information about an allergy and review your personal notes about your allergies.

You can also click Submit or Review Updates to request changes to your allergies list.

Remove a allergen that is no longer applicable by clicking .

View a summary of your health information To get a summary of your medical record, go to my Medical Record > Health Summary. This summary includes:

Respond to questionnaires from your clinic Your clinic might make questionnaires available in myHealthRecord so you can complete them online instead of filling out a form when you get to the clinic.

You might be able to respond to questionnaires in three different places:

Open generally available questionnaires from my Medical Record > Questionnaires. If your provider wants you to complete a questionnaire for an upcoming appointment, go to Visits >

Upcoming Appointments. Select the appointment and open the questionnaire by clicking its name in the Questionnaires section of the appointment details.

If your provider sends you a myHealthRecord message with an attached questionnaire, open it by clicking the questionnaire link near the top of the message.

If you need to close a questionnaire before you finish it, click Save and Quit to save your progress. You can come back to the questionnaire later to finish and submit it.

Coverage

Review and update your coverage information To review the coverage information your clinic has on file, go to my Medical Record > Coverage

To update your insurance information, make any of the following changes:

Edit an existing coverage by clicking Request a Change. Remove a coverage by clicking Remove. Add a new coverage by clicking Add a coverage. New coverage additions are submitted for verification

when you log out of MyHealthRecord.

Preferences and Administrative Features

Personalize myHealthRecord There are three ways you can personalize how myHealthRecord appears for you. For each account you have access to, you can:

specify the color scheme change the name that appears on that person's tab add or change the photo

Note that photos you upload through myHealthRecord are visible to medical staff, so you should only use a photo that shows each person's face.

1. Go to Preferences > Personalize or click the wrench icon below the proxy tabs. 2. Click Edit. 3. Make any of the following changes:

To change the nickname for a family member, edit the name in the Nickname field. To specify a different color scheme, select a color from the list.

4. Click Save.

Note: Editing your profile photo feature is not currently enabled.

Update your personal information You can update your address, phone number or email address at any time so that your clinic always has the most up-to-date information in your record.

1. Go to Preferences > Demographics. 2. Enter the new information and click Accept Changes. 3. Confirm that your updated information is correct, and then click Submit Changes.

Your updated information is sent to your clinic and will be applied to your patient health record in real time.

Customize your notification preferences myHealthRecord can send you notifications by email or text message when there is new information available in your account. You can specify your preferences for different types of notifications, including new messages, test results, billing statements and letters, prescriptions, appointment updates, and more.

1. Go to Preferences > Notifications. 2. For each notification you want to receive, select the E-mail or Text check box. 3. Update your email address and mobile phone number if needed to ensure that you receive the

notifications. 4. Click Save Changes.

Linked Apps and Devices Track and manage the devices linked to your myHealthRecord account. You can review permissions and remove access to myHealthRecord. Additionally, you can also unlink devices that have been added as trusted devices from the MyChat mobile app.

1. Go to Preferences > Linked Apps and Devices. 2. Click View Permissions, Remove Access or Unlink.

Change your myHealthRecord password or update your

security question and answer To ensure that your medical information stays protected, consider changing your myHealthRecord password periodically. To do so, go to Preferences > Change Password. From this page, you can also update the security question and answer that is used when you forget your myHealthRecord username or password.

Mobile Apps You can also access your myHealthRecord account through the “MyChart” app, available for download for iPhone, Android and tablet devices. The mobile app contains many of the same features as the myHealthRecord website, allowing you to do all of the following, and more:

view test results send and receive messages request appointments and confirm upcoming appointments and view After Visit

Summaries for past appointments view your health summary, including allergies, immunizations, and medications update your health coverage access family members' charts

If you have an iOS device, you can pair it with Apple Watch™. With myHealthRecord for Apple Watch, you can:

see alerts for new information in myHealthRecord view upcoming appointments read messages review their medication list

Download the myHealthRecord App To install the myHealthRecord app, go to the App Store or Google Play Store and search for "MyChart” or visit www.womenscollegehospital.ca/myhealthrecord.
