BLACK DATA PROCESSING ASSOCIATES Information Technology THOUGHT LEADERS STATEMENT OF WAIVER OF LIABILITY During August 12, 1998 through August 16, 1998 in Orlando, Florida , the National BDPA Information Technology THOUGHT LEADERS, 1111 14 th Street NW, Washington, D. C., 20005, will sponsor and conduct 13 th Annual High School Computer Competition (hereinafter “Competition“), is a registered participant for the Competition. We, (hereinafter “Participant”), and legal guardian of Participant, of , hereby waive our rights under all laws and entitlements providing such rights, to bring a suit for damages on our behalf against the National BDPA Information Technology THOUGHT LEADERS so that may participate in the competition and all related events. In signing this waiver, we jointly and severally, extinguish our right to bring suit against the National BDPA Information Technology THOUGHT LEADERS for any injury of any kind that Participant sustains while traveling to, attending, and participating in the aforementioned competition. We waiver our right to bring suit whether the injury be accidental or intentional in nature, whether the injury is immediately discovered or discovered at some future time, and regardless of the identity of the party inflicting injury. We understand that by waiving our right to bring suit against the National BDPA Information Technology THOUGHT LEADERS, we are from this day forward prohibited from suing the National BDPA Information Technology THOUGHT LEADERS and any of its employees, representatives, and agents while fulfilling their duty to the organization, for any action or non-action as it relates to any injury sustained Participant while traveling to and from or during the competition. We have read and understand the above provision. We agree to the above provisions voluntarily, intelligently, willingly and intentionally. We understand that we cannot withdraw this waiver without the consent of
