Superstructure replacement for four bridges in Herkimer (BIN1002720), Schoharie (BINs 1007890 & 1007900), & Oneida (BIN 2206680) counties Removal of the existing superstructure for all of the bridges; Construction of new superstructure and approach slabs; Approach sidewalk reconstruction at West Main Street (BIN
1002720); Modifications to each of the substructures as necessary to
accommodate each new superstructure. Full Bridge Replacement for one bridge in Oneida County
(BIN 1051810) Removal of existing substructure and superstructure; Replacement of the existing bridge; including new
foundations, substructure, superstructure and approach slabs

The Department owns all Right-of-Way for the existing structures. Right of Way has been purchased for BIN 2206680
(Chenango Rd/Sauquoit Creek) Acquisition maps have been posted as reference
documents. No additional Right-of-Way acquisitions or
temporary easements are anticipated to be required for this project.

In accordance with Part 4 – Utility Requirements, Design-Builder is responsible for:
Verifying utility facility locations; Identifying & resolving any conflicts not yet
documented; Coordinating with utility owners for utility
relocations; Providing stakeout and final layout of utility
relocations; Protecting existing utilities; Cost to repair damages to utilities caused by
Design-Builder; Informing the Department of all
meetings/correspondence with utility owners.

BIN 1002720 - Village of Ilion, West Main Street Over Steele Creek
Village of Ilion (Electric): Village of Ilion (Electric) shall relocate utility poles, overhead
lines and any facilities necessary so that the superstructure can be removed and replaced, including any necessary tree pruning.
Northland Communications, Time Warner Cable, Verizon: Telecommunication companies shall relocate overhead lines
and any facilities necessary so that the superstructure can be removed and replaced, including splice notifications to customers (usually 30 to 45 days notice).
National Grid (Gas): National Grid shall perform all work necessary to relocate
the underground gas main and any facilities necessary so that the superstructure can be removed and replaced.

BIN 2206680 - City of Utica, Chenango Road over Sauquoit Creek
National Grid (Electric): No impacts to National Grid’s Sub-T tower will be allowed.
National Grid shall perform all work necessary to relocate utility poles, risers, overhead lines and any other impacted facilities so that the superstructure can be removed and replaced.

BIN 1002720 - Village of Ilion, West Main Street Over Steele Creek
Village of Ilion (Water): The Design-Builder shall relocate water line to avoid impact with the
removal and replacement of the superstructure. All work shall be performed in accordance with the standards and requirements of the Village of Ilion.
Village of Ilion (Sanitary): Prior to final pavement placement, the Design-Builder shall coordinate
an inspection of all structures within the limits of the project with the Village, and if the Village then determines that repairs and/or replacements of manholes, sewer mains, or sanitary facilities are required, the Design-Builder may contract separately with the Village to perform the work.
However, any sanitary facilities damaged as a result of the Design-Builder shall be repaired and/or replaced in accordance with the standards and requirements of the Village of Ilion at no cost to the Village or the Dept.
In addition, the Design-Builder shall adjust the rim elevations of any manholes located within the paving limits to meet finished grade. Payment for the adjustment of manholes to finished grade shall be included in the Lump Sum price bid for BIN 1002720.

BIN 2206680 - City of Utica, Chenango Road over Sauquoit Creek
Mohawk Valley Water Authority (Water) : The Design-Builder shall relocate the existing water line to avoid
impact with the removal and replacement of the superstructure. All work shall be performed in accordance with the standards and requirements of the Mohawk Valley Water Authority.

BIN 1051810 - Town of Marcy, Route 291 over Nine Mile Creek Mohawk Valley Water Authority (Water) :
There is an existing underground water line approximately 20’ east of the bridge. Any work regarding this utility shall be performed in accordance with the standards and requirements of the Mohawk Valley Water Authority.
Town of Marcy (Sanitary): Prior to final pavement placement, the Design-Builder shall
coordinate an inspection of all structures within the limits of the project with the Town, and if the Town then determines that repairs and/or replacements of manholes, sewer mains, or sanitary facilities are required, the Design-Builder may contract separately with the Town of Marcy to perform the work.

Three Way Coordination – DB, Utility & State Coordination meeting will be held between the
Department, all short-list firms, and the utility companies before or shortly after issuance of Final RFP.
DB Responsibilities DB Team responsible for holding utility coordination
meetings; DB Team to provide supporting documents for utility
design, including: plans, Table of Conflicts & Special Note of Utility Coordination;
Table of Conflicts to include existing & proposed locations;
Special Note of Utility Coordination must include work description, proposed locations and time frames.

Superstructure and Bridge Replacements in Regions 2 & 9Design-Build Project (PIN 2805.65; D900022) Contract Information Lump Sum Design-Build Contract; Best Value Selection Process (50% Price, 50% Quality); Project Completion Date shall be the earlier of:
November 30, 2016; or The number of days past NTP proposed by the successful
Proposer on Form SCD (ITP Appendix E) Liquidated Damages will be imposed for not meeting
the Project Completion date and for Impacts to Traffic in excess of the number of calendar days indicated on Form SCD;
Early Completion Bonus if Project Completion is achieved ahead of the Project Completion Date;
Escrow Submission of Bid Documents for Successful Proposer.

Alternate Technical Concepts (ATCs) are proposals from the Proposer to design and construct the Project in a manner that is different than the Contract Requirements and which: are submitted to the Department for review,
consideration and approval prior to Proposal submittal; have significant savings in time, and/or reduction of
traffic impacts, and/or community disruption, and/or project costs, and/or environmental impacts, and/or other beneficial impacts to Project parameters;
in the Department’s judgment offers an equal or superior Project.

Proposers may submit, on a confidential basis, up to five (5) ATCs, using Form ATC, for the Department’s pre-approval;
The proposed end product of each alternative must, in the Department’s judgment, be equal to or better than the Work product described in the Contract Documents;
Department may provide ‘Conditional Approval’ of an ATC. Proposers must respond to Conditional Approvals by the established date;
The Design-Builder is solely responsible for obtaining approvals from any local municipality, public agency or permit issuing agency for ATC concepts which would affect their facilities, operations or issued permit.

If One-On-One Meetings are held, at least one meeting will be held with each Proposer;
One-on-One Meetings will be recorded; Department reserves the right to disclose to all
Proposers any issues raised during the meetings that may materially affect the project;
Department will not disclose information pertaining to an individual Proposer’s technical concepts, proposal or ATCs;
Additional One-on-One Meetings may be held if required to review ATCs or if the Department believes it is warranted.

QC is Design-Builder’s Responsibility Design-Builder shall appoint a QC Manager responsible for
QC Plan for both Design and Construction Design QC shall be the Design-Builder’s responsibility and
shall be performed in accordance with the QC Plan Construction Inspection shall be performed by an
Independent Engineering Firm (Hired by the Design-Builder) Materials Testing shall be performed by an Independent
