Dementia Ambient Care - Multi-sensing Monitoring for Intelligent Remote

Management and Decision Support Dementia, a leading cause of disability in the elderly, currently affects nearly 10 million people in Europe and over 35 million worldwide.

Dem@Care, aims to provide novel solutions for the holistic management of dementia that exploits both medical knowledge and the latest advances in information and communication technologies by developing a closed-loop management solution (see Figure 1) for People with Dementia (PwD) through multi-parametric remote monitoring and the personalised analysis of measurements. Specifically, it implements:

• A loop for people with dementia and their informal caregivers that: i) monitors and assesses their cognitive and behavioral status by integrating a multiplicity of wearable and in-situ sensors, ii) enables time evolving context-sensitive profiling to support reactive and proactive care, iii) provides personalized and adaptive support. • A professional loop that: i) provides objective observations regarding the health progression of the person with dementia and medication effectiveness, ii) warns about trends closely related to dementia (e.g. apathy), iii) supports preventative care decision making and adjustment of treatment recommendations.

CITARD ACTIVE Citard Active is a product supporting older persons to carry out an inward and outward active life. It can be seen as a social network stimulator in which people are connected with each other, share activities and information among themselves, with caretakers and family members. Being active enables the elderly to feel better and remain fit for longer. By using an app in a tablet the elderly can enlarge and maintain their social network, become aware of all the activities that will take place and easily register to participate, and share their agendas with their family or friends. With this app, elderly are encouraged to take part in activities and be more active. The proposed activities are selected by taking into consideration their ageing related cognitive and physical behaviour changes.

Participants, in four care centres of Orbis Medisch en Zorgconcern in the Netherlands, have experienced Citard Active as easy to use and described it as a nice way to support their social lives.

Citard Active is an SME with 10 years of experience, active in the ICT sector and concentrating in the eHealth and Ambient Assisted Living sectors.

LORAN Ltd.

The ‘Azienda Ospedaliero-Universitaria Consorziale Policlinico di Bari’ (Polyclinic of Bari) is located in Bari, Apulia, and is the biggest general hospital unit in south Italy. It counts more than 60 hospital wards and 1550 hospital beds. Therefore, every day it has thousands visitors, but it’s hard to get informations and right directions to hospital wards needed. To resolve this discomfort, Loran Ltd has developed a free mobile app, BARI HEALTH, available for the most used mobile devices such as Apple, Android and Windows Phone. Very easy to use, this app consists in a interactive map of the whole hospital area, where to find easily the hospital ward that you look for. Moreover, for each hospital ward, you can read all information, as telephone number, email address, time visit, time talks with doctors, chief physician’s name, primary nurse’s name, medical equipe, and more. Bari Health is also able to search doctors and hospital wards by name, and it is always in updating. In a couple of months it has collected very positive reviews. Bari Health was thought for a new idea of support for citizens and all visitors of this very big hospital unit.

INTERACTIVE PLATFORM SUPPORTS PARKINSON’S PATIENTS AND CAREGIVERS TO BETTER COPE AND MONITOR THE

PROGRESS OF THE DISEASE

The LiveWell platform was designed to support both recently diagnosed patients and new caregivers in managing the progress of the disease and to facilitate the monitoring of the most common symptoms and understanding some practical methods and tips to better cope with the disease and improve their wellbeing. Resulting from a cooperation work and funded by the European Commission, the LiveWell platform is now fully available for use after validation sessions performed by several experts in the medical field, including the Parkinson’s Patients Associations from the participating countries.

This innovative and interactive platform offers a set of features, completely free-of-charge, that serve the sole purpose of responding directly to the needs of the recently diagnosed Parkinson’s Patients and their caregivers, including families, colleagues and friends. More importantly however is that while helping these end-users through the facilitation of useful contents, activities and exercises that help them to better cope and monitor the progress of the disease, this platform also connects the patients and the caregivers with the Health and Medical professionals.

Benefits for the Parkinson’s Associations are numerous and include the unique opportunity of strengthening their role with the provision of distance care services for its members with mobility constrains, and specialized informational packages that can contribute to improve recently diagnosed patients and caregivers wellbeing and turning the monitoring of the disease between medical appointments a more easy and motivational task.

The LiveWell Project, supported by the European Commission under the Lifelong Learning Programme, is coordinated by INOVA+ (Portugal) in partnership with University Plymouth (United Kingdom), BitMedia (Austria), Parkinson Association of Madrid (Spain), Ana Aslan International Foundation (Romania); Samvil ehf Fjarkennsla.com (Iceland), and Brežice General Hospital (Slovenia).

SocialRobot Project develops an ICT-based service providing Robot, flexible and adaptable to meet elderly requirements as they change. The implemented framework is based on the principles of a Service Oriented Architecture. The robot is equipped with navigation, decision making and perception state of the art algorithms that are exposed as functionalities. These are combined differently to provide innovative and personalized services, tailored to meet its owner needs and expectations. Services are highly adaptable to dynamic parameters and benefit from the knowledge of end-users preferences, abilities, disabilities and personal information that are provided by one of the system’s components, the Social Care Community Network (SoCoNet). It enables the effective administration and coordination of independent user profiles and Virtual Care Teams (VCTs) around the elderly person along time. The robot and services are being developed in closed loop with end-users so as to maximize their acceptability and usability. Initial laboratory experiments with three different test scenarios demonstrated that the robot’s hardware and software are ready to start the pre-trials with a small sample of real users, in late November, 2014.

European cities and regions take

a step forward to better meet the

needs of their older residents

On the occasion of the International Day of Older Persons, AFE-INNOVNET, a thematic network on innovation for age-friendly environments, officially applied to become the EU affiliated programme of the WHO’s Global Network of Age-Friendly Cities and Communities.

“The International Day of Older Persons is the right moment to recall the importance of promoting and implementing innovative solutions to address the challenges posed by population ageing and to underline the need to create a society which is fair and sustainable for all generations”, said Anne-Sophie Parent, Secretary General of AGE Platform Europe and coordinator of AFE-INNOVNET network.

Supported by EU funding, AFE-INNOVNET is a Thematic Network aimed at mobilising local and regional authorities and other stakeholders to support the scaling-up of innovative solutions to support active and healthy ageing across Europe. It follows the WHO approach on age-friendly environments which seeks to support local actors to adapt their community environment and services to the needs of their ageing population in the areas of housing, transport, social participation, access to services, built environment, information, respect and social inclusion, civic participation and employment.

With over 160 members, the AFE INNOVNET network looks forward to continuing working closely together with WHO to scale-up and implement innovative solutions for active and healthy ageing.
