Notice of Default in Complying with the Corporations Tax Act

The Director has been notified by the Minister of Finance that the following corporations are in default in complying with the Corporations Tax Act.

NOTICE IS HEREBY GIVEN under subsection 241(1) of the Business Corporations Act, that unless the corporations listed hereunder comply with the requirements of the Corporations Tax Act within 90 days of this notice, orders will be made dissolving the defaulting corporations. All enquiries concerning this notice are to be directed to Ministry of Finance, Corporations Tax, 33 King Street West, Oshawa, Ontario L1H 8H6.

Periodically, temporary applications are filed with the Board. Details of these applications can be made available at anytime to any interested parties by calling (416) 326-6732.

The following are applications for extra-provincial and public vehicle operating licenses filed under the Motor Vehicle Transport Act, 1987, and the Public Vehicles Act. All information pertaining to the applicant i.e. business plan, supporting evidence, etc. is on file at the Board and is available upon request.

Any interested person who has an economic interest in the outcome of these applications may serve and file an objection within 29 days of this publication. The objector shall:

1. complete a Notice of Objection Form,

2. serve the applicant with the objection,

3. file a copy of the objection and provide proof of service of the objection on the applicant with the Board,

4. pay the appropriate fee.

Serving and filing an objection may be effected by hand delivery, mail, courier or facsimile. Serving means the date received by a party and filing means the date received by the Board.

Pursuant to delegated authority and in accordance with subsection 1(1) of the Foreign Cultural Objects Immunity from Seizure Act, R.S.O. 1990, c.F-23, the works of art or objects of cultural significance listed in Schedule “A” attached hereto, which works or objects are to be on temporary exhibit during the John Ruskin: Artist and Observer exhibition at the National Gallery of Canada in Ottawa, Ontario, pursuant to loan agreements between the National Gallery of Canada and the lender(s) listed in the attached Schedule “A”, are hereby determined to be of cultural significance and the temporary exhibition of these works or objects in Ontario is in the interest of the people of Ontario.

Basic occupational health and safety awareness training — workers

1. (1) An employer shall ensure that a worker who performs work for the employer completes a basic occupational health and safety awareness training program that meets the requirements set out in subsection (3) as soon as practicable. (2) Subsection (1) does not apply if, (a) the worker previously completed a basic occupational health and safety awareness training program and provides the

employer with proof of completion of the training; and (b) the employer verifies that the previous training meets the requirements set out in subsection (3). (3) A basic occupational health and safety awareness training program for workers must include instruction on the following:

1. The duties and rights of workers under the Act. 2. The duties of employers and supervisors under the Act. 3. The roles of health and safety representatives and joint health and safety committees under the Act. 4. The roles of the Ministry, the Workplace Safety and Insurance Board and entities designated under section 22.5 of the

Act with respect to occupational health and safety. 5. Common workplace hazards. 6. The requirements set out in Regulation 860 (Workplace Hazardous Materials Information System (WHMIS)) with

respect to information and instruction on controlled products. 7. Occupational illness, including latency. Basic occupational health and safety awareness training — supervisors

2. (1) An employer shall ensure that a supervisor who performs work for the employer completes a basic occupational health and safety awareness training program that meets the requirements set out in subsection (3) within one week of performing work as a supervisor. (2) Subsection (1) does not apply if, (a) the supervisor previously completed a basic occupational health and safety awareness training program and provides

the employer with proof of completion of the training; and (b) the employer verifies that the previous training meets the requirements set out in subsection (3). (3) A basic occupational health and safety awareness training program for supervisors must include instruction on the following:

1. The duties and rights of workers under the Act. 2. The duties of employers and supervisors under the Act. 3. The roles of health and safety representatives and joint health and safety committees under the Act. 4. The roles of the Ministry, the Workplace Safety and Insurance Board and entities designated under section 22.5 of the

Exemptions

3. (1) The requirements set out in section 1 do not apply to an employer with respect to a supervisor if, (a) before this Regulation came into force, the supervisor was performing work as a supervisor for the employer; and (b) the employer verifies that, before this Regulation came into force, the supervisor completed a basic occupational

health and safety awareness training program that meets the requirements set out in subsection 2 (3). (2) The requirements set out in section 1 do not apply to an employer with respect to a worker or supervisor if, (a) another employer was exempt with respect to the worker or supervisor under subsection (1); and (b) the worker or supervisor provides the employer with proof of the exemption. Record of training

4. (1) An employer shall maintain a record of the basic occupational health and safety awareness training required by sections 1 and 2 that is completed by workers and supervisors who perform work for the employer. (2) An employer shall maintain a record of workers and supervisors who perform work for the employer in respect of whom the employer is exempt under section 3. (3) If a worker or supervisor completes a training program under subsection 1 (1) or 2 (1), the employer shall, at the request of the worker or supervisor, provide the worker or supervisor with written proof of completion of the training. (4) If an employer is exempt with respect to a supervisor under subsection 3 (1), the employer shall, at the request of the supervisor, provide the supervisor with written proof of the exemption. (5) If, within six months of a worker or supervisor no longer performing work for an employer, the worker or supervisor requests a written proof described in subsection (3) or (4), the employer shall provide the worker or supervisor with the requested written proof.

5. (1) An employer shall carry out the training programs necessary to enable a committee member to become a certified member, and the programs must be selected in accordance with the training and other requirements established by the Chief Prevention Officer under section 7.6 of the Act. Formation élémentaire de sensibilisation à la santé et à la sécurité au travail : travailleurs

1. (1) L’employeur veille à ce que le travailleur qui exécute du travail pour lui termine un programme de formation élémentaire de sensibilisation à la santé et à la sécurité au travail qui répond aux exigences du paragraphe (3) aussitôt que possible. (2) Le paragraphe (1) ne s’applique pas si les conditions suivantes sont remplies : a) le travailleur a déjà terminé un programme de formation élémentaire de sensibilisation à la santé et à la sécurité au

travail et fournit à l’employeur une preuve d’achèvement de la formation; b) l’employeur vérifie que la formation antérieure répond aux exigences du paragraphe (3). (3) Tout programme de formation élémentaire de sensibilisation à la santé et à la sécurité au travail s’adressant aux travailleurs comprend un enseignement théorique sur les sujets suivants : 1. Les devoirs et les droits des travailleurs selon la Loi. 2. Les devoirs des employeurs et des superviseurs selon la Loi. 3. Les rôles des délégués à la santé et à la sécurité et des comités mixtes sur la santé et la sécurité selon la Loi. 4. Les rôles du ministère, de la Commission de la sécurité professionnelle et de l’assurance contre les accidents du travail

et des entités désignées en vertu de l’article 22.5 de la Loi en ce qui concerne la santé et la sécurité au travail. 5. Les risques courants dans les lieux de travail. 6. Les exigences du Règlement 860 (Système d’information sur les matériaux dangereux utilisés au travail (SIMDUT))

à l’égard des renseignements et de l’instruction sur les produits contrôlés. 7. Les maladies professionnelles, y compris leur latence.Formation élémentaire de sensibilisation à la santé et à la sécurité au travail : superviseurs

(1) L’employeur veille à ce que le superviseur qui exécute du travail pour lui termine un programme de formation élémentaire de sensibilisation à la santé et à la sécurité au travail qui répond aux exigences du paragraphe (3) au plus tard une semaine après avoir exécuté du travail à titre de superviseur. (2) Le paragraphe (1) ne s’applique pas si les conditions suivantes sont remplies : a) le superviseur a déjà terminé un programme de formation élémentaire de sensibilisation à la santé et à la sécurité au

travail et fournit à l’employeur une preuve d’achèvement de la formation; b) l’employeur vérifie que la formation antérieure répond aux exigences du paragraphe (3). Exemptions

3. (1) Les exigences de l’article 1 ne s’appliquent pas à l’employeur en ce qui concerne un superviseur si les conditions suivantes sont remplies : a) le superviseur exécutait du travail à ce titre pour l’employeur avant l’entrée en vigueur du présent règlement; b) l’employeur vérifie que le superviseur a terminé, avant l’entrée en vigueur du présent règlement, un programme de

formation élémentaire de sensibilisation à la santé et à la sécurité au travail qui répond aux exigences du paragraphe 2 (3).

(2) Les exigences de l’article 1 ne s’appliquent pas à l’employeur en ce qui concerne un travailleur ou un superviseur si les conditions suivantes sont remplies : a) un autre employeur était exempté à l’égard du travailleur ou du superviseur en vertu du paragraphe (1); b) le travailleur ou le superviseur fournit à l’employeur une preuve de l’exemption.Registre de formation

4. (1) L’employeur tient un registre de la formation élémentaire de sensibilisation à la santé et à la sécurité au travail exigée par les articles 1 et 2 qu’ont terminée les travailleurs et les superviseurs qui exécutent du travail pour lui. (2) L’employeur tient un registre des travailleurs et des superviseurs qui exécutent du travail pour lui et à l’égard desquels il est exempté en vertu de l’article 3. (3) À la demande du travailleur ou du superviseur qui a terminé un programme de formation en application du paragraphe 1 (1) ou 2 (1), l’employeur lui fournit une preuve écrite d’achèvement de la formation. (4) L’employeur qui est exempté à l’égard d’un superviseur en vertu du paragraphe 3 (1) lui fournit, à sa demande, une preuve écrite de l’exemption. (5) L’employeur fournit la preuve écrite visée au paragraphe (3) ou (4) au travailleur ou au superviseur qui la lui demande dans les six mois où il cesse d’exécuter du travail pour lui.

5. (1) L’employeur dispense les programmes de formation nécessaires pour permettre aux membres des comités de devenir des membres agréés, ces programmes devant être choisis conformément aux conditions, notamment en matière de formation, que le directeur général de la prévention établit en vertu de l’article 7.6 de la Loi. (2) Il est entendu que la définition qui suit s’applique au paragraphe (1).«dispenser» S’entend en outre du fait de payer le coût de la formation.

6. Le Règlement de l’Ontario 780/94 est abrogé.Entrée en vigueur

7. (1) Sous réserve du paragraphe (2), le présent règlement entre en vigueur le jour de son dépôt. (2) Les articles 1 à 4 entrent en vigueur le dernier en date du 1er juillet 2014 et du jour du dépôt du présent règlement.

Un nouvel abonné peut commander des copies d’éditions précédentes de la Gazette au coût d’une copie individuelle si l’inventaire le permet.

The Ontario Gazette is published every Saturday. Advertisements/notices must be received no later than 3 pm on Thursday, 9 days before publication of the issue in which they should appear. For weeks including Easter Monday, November 11th or a statutory holiday allow an extra day. For the Christmas/New Year holiday schedule please contact the Gazette at (416) 326-5310 or by email at [email protected]

Advertising rates and submission formats:

1) Please submit all notices in a Word.doc format to: [email protected]

2) For a first insertion electronically submitted the basic rate is $75 up to ¼ page.

3) For subsequent insertions of the same notice ordered at the same time the rate is $40 each.

4) Clients may confirm publication of a notice by visiting The Ontario Gazette web site at: www.ontariogazette.gov.on.ca or by viewing a printed copy at a local library.

Subscriptions:

The annual subscription rate is $126.50 + H.S.T. for 52 or 53 weekly issues beginning the first Saturday in January, payable in advance. In-year new subscriptions will be pro-rated for the first year. A new subscriber may order back issues of the Gazette at the single-copy rate as inventory permits.

Refunds for cancelled subscriptions will be pro-rated from 50% or less depending upon date. For subscription information/orders please call (416) 326-5306 during normal business hours.

Single Copies:

Individual Gazette copies may be ordered on-line through the website at www.serviceontario.ca/publications or by phone at 1-800-668-9938.
