HMSA’s Care Connection ProgramHMSA’s Care Connection program gives you the support you need to manage your medical condition. Support is available for patients who have the following chronic diseases:

• Asthma and COPD: Helps you understand your condition, triggers and symptoms better. For all HMSA members age 2 and older who have asthma, or members age 18 and older who have chronic obstructive pulmonary disease (COPD).

• Behavioral: Helps you manage and overcome stress, anxiety, depression, substance abuse and any other behavioral health issues. The program refers you to the appropriate provider(s), care coordination facilities, and/or case management services depending on your needs.

• Heart Disease: Helps you manage your condition, medications and diet. For members 18 and older who have coronary artery disease (CAD) or heart failure.

• Diabetes: Helps you manage and control your diabetes. You can have fewer symptoms and avoid long-term problems with careful management. Diabetes education classes are available in the community. For members age 2 and older.

• Kidney Disease: Helps you manage your condition and prevent complications. For members 18 and older who have chronic kidney disease. CC

Q: What if I don’t want to participate in the program?

A: If you choose not to participate, simply call the appropriate toll-free Care Connection number listed on the back cover.

How the Program WorksPhone calls: Trained nurses and dietitians will call you to explain how the program works. They will:

• Provide information and answer your questions about medications, diet, exercise and any other health concerns you may have.

• Reinforce your health care goals.

• Discuss ways you can better manage your health.

• Help you find the right community resources.

• Offer a toll-free number for you to call when you need assistance.

Commonly Asked Questions

Q: How do I join the program?A: HMSA and Health Plan Hawaii members who

have any of the Care Connection program conditions are automatically enrolled at no cost. If you have not received any calls or mailings, call the toll-free number, and a clinician will enroll you.

Q: Should I tell my doctor that I’m in the program?

A: Yes, your doctor knows that this program is available to you, and it’s important to share information with your doctor. HMSA’s Care Connection program can be a valuable part of managing your disease. Your doctor will receive regular Care Connection reports on how you are doing.

Q: What can I expect from this program?A: People who participate the Care Connection

program can expect to live more active, normal lives. They will also understand their conditions better and learn the best ways to manage them.
