Peninsula Community Health Services and Valley View Health Center in Chehalis were two of five non-profit community health centers in the Washington, Wyoming, Alaska, Mon-tana, and Idaho region, and the only two in Washington State,

to receive grant awards from the John A. Hartford Foundation. The $1.94 million grants are to improve depression care and will help the five clinics implement Project IMPACT (Improving Mood-Promoting Access to Col-laborative Treatment). IMPACT is an evidence-based depression care model offered by the University of Washington (AIMS Center). The awards are made possible with funding from a two-year Social Innovation Fund

grant provided by the Corporation for National and Community Service to the Hartford Foundation in 2012.

The funds were awarded to nonprofit community primary care organizations in rural areas which are either medically underserved or face a health profes-sional shortage. PCHS received the grant in October 2013.

The Margaret A. Cargill Foundation is providing the matching funds for PCHS for the duration of the grant of the SIF Grant.

Through these resources the PCHS Behavioral Health Program has doubled in size for clinic-based

behavioral health professionals as well as incorporated the services of a half-time psychiatrist and a full-time psychiatric nurse practitioner. We know the mind cannot be separated from the body---so we continue on our quest for the full integration of behavioral health into primary care services.¨

The PCHS Quality Program moved the needle throughout 2012 and 2013 as we created, reviewed and updated all the elements of our Quality Program in preparation for accreditation.

In May, 2013, we welcomed the site review team from AAAHC (The Accreditation Association for Ambula-tory Health Care) and following a comprehensive re-view we became accredited as primary care providers

and for the Patient Centered Medical Home. Quality studies, measures and outcomes became the norm for providers and patients alike.

Some might ask---how do you take a good idea and make it happen? Leadership is the answer! PCHS wants to salute the volunteers who comprise our Board of Directors. A majority of the PCHS Board are patients who are served in PCHS clinics.

We saw 23,069 unduplicated patients; this compares

to 22,934 in 2012. We had a total of 77,186 encounters which com-

pares to 75,598 in 2012. The good news is a reduction in our unin-

sured patients. The PCHS Behavioral Health program saw the greatest

increase in the number of patient visits.

The Affordable Care Act, ObamaCare --- you choose the

name, PCHS Enrollment, Eligibility and Outreach spe-

cialists were hard at work making it happen here in Kitsap

County. We saw the largest enrollment in the Medicaid

Expansion “Washington Apple Health”. PCHS patients

and staff will work hard in 2014 to be sure this new insured experience,

for many of our patients, is a successful one. We continued to enroll

new enrollees through March, 2014 and will continue throughout the

year to enroll patients and community members in Washington Apple

Health! Please call for information or assistance.

PCHS Services The PCHS

medical team provides compre-

hensive health care for every

age and community referrals

for specialty care. Services in-

clude, but are not limited to,

acute and chronic care, health

maintenance checks, walk-in

care (at the Wheaton Way site),

immunizations, behavioral

health counseling, and 24-hour

on-call care. Both medical and

dental PCHS patients have the

advantage of pharmacy services

at each clinic location.

The PCHS dental team provides

a wide range of dental services

for children and, thanks, to the

Affordable Care Act, many

adults now have access to

dental services under the Wash-

ington Apple Health program.

PCHS still provides limited

emergency dental care for

adults. Community specialty

referrals are also available.

THANK YOU to our many donors who help in-need

individuals access medical and dental health care

through contributions to our Dr. Tegenfeldt Patient

Assistance Fund and Willow’s Fund and help children

improve their reading skills through the Reach Out

and Read program. Dr. Tegenfeldt, now retired, was

known for giving patients money from his own pocket

to obtain additional care services. Willow’s Fund was

established to help the homeless in

honor of author Richard LeMieux,

former PCHS board member.

Getting signed up for my medical insurance.

It’s so confusing, but actually thankful I will

finally have medical insurance. I know it’s a

touchy subject for many. But many of us have

been unable to afford any insurance yet didn’t

qualify for any type of help. I’m not here to

start a debate. But thrilled I will finally be able

to afford to go see a doctor after 3 years.

If anyone in Kitsap county is having a rough

time getting their medical stuff figured out call

peninsula health and make an appt with one of

their peeps there. Anyone can go! No pressure

to decide on a plan, but they help you with all

the overwhelming information. I was so

impressed! -Lelaine

Pharmacy

PCHS is very proud of our integrated pro-

gram approach to care---of course our first

commitment is to “accessible, affordable,

compassionate, quality health care ser-

vices for our communities”. The PCHS

Clinical Pharmacy demonstrates a com-

mitment to integrated care in every aspect

of their work. Newly accredited as a Diabetes Education Program---the

addition of comprehensive diabetes care will be added to an already im-

pressive body of work. Of course, we fill prescriptions, counsel patients,

and provide support to providers. The Pharmacy program also handles

most of the requested refills, supports patients in accessing care when

needed, and multiple education/training

programs with a special focus on “anti-

coagulation” training which they provide

one on one for patients taking these drugs.

Dental

The PCHS Dental program met every

program goal established for 2013.

We expanded our adult programs

providing access to care for up to six

adults on most clinic days. As of

January, 2014, dental coverage

returned to Medicaid and we expect

to see these numbers grow even

more. Dr. Michael Insani leads this

program to excellence. Of course, the central reason for the creation of

the PCHS dental program was to provide access for Medicaid enrolled

children. We join a growing number of dentists who will serve this

population. We hope, over time, we will see the number of dentists grow

who are willing to serve adults!

PHARMACY AND DENTAL PROGRAM UPDATES

I’m so grateful for all the

people that work here.

Everyone is so good to

me. Thank you. I realize

now how important my

health is and wished I

would have paid more

attention when I was

younger.

I come by choice. Since

Peninsula began it has

come light years in its

great providers and

care. I love it & I never

thought I would.
