get referrals to appropriate treatment resources. Other responsibilities shall include
participating in joint treatment planning with criminal justice entities in order to provide
supportive transition from incarceration to the community. The COC CM will provide
rehabilitative functions such as skills training either individually or in a group and case
management as required. Works under limited supervision with considerable latitude for
